Oral Bioadhesive Gels of Recombinant Human Epidermal Growth Factor(rhEGF) for the Healing of Gastric Ulcers (재조합 상피세포성장인자를 함유한 경구 점착성 겔제의 위궤양 치유효과)

  • The objective of this study was to develop effective oral formulations of rhEGF for gastric ulcer healing using polycarbophil. hydroxypropylcellulose(HPC) and sucralfate as its bioadhesive bases. Cytoprotective effects of rhEGF, cell proliferation and differentiation. on the ulcers induced by ethanol or acetic acid in rats were studied. rhEGF release from HPC formulation was much faster than that from polycarbophil formulation. HPC formulation combined with small amount of sucralfate showed much slower release of rhEGF than only HPC base only. rhEGF preparations with bioadhesive polymers showed better effects on the healing of gastric ulcers than EGF solution when administered orally. When rhEGF preparations were administered at once and the animals were under starvation, polycarbophil formulation showed better effect on gastric ulcers than HPC formulation. Otherwise, when rhEGF preparations were given more than three times and the rats were fed normally, HPC formulation showed good healing efficacy of ulcers compared to polycarbophil formulation. rhEGF showed dose-dependent effect on the healing of both chronic and acute ulcers.

  • 1. 위장관통과 실험결과 polycarbphil 흑은 carbomer로 코팅한 생체막점착성 마이크로캅셀 모두가 Eudragit RS/RL 마이크로캅셀보다 위 및 소장상부에 다량 존재하는 것으로 보아 Polymer의 생체막점착성에 의해 제제의 위내용배출이 억제되었음을 알 수 있었다. 2. Polycarbophil 흑은 carbomer로 코팅한 생체점착성 마이크로캅셀의 AUC는 Eudragit RS/RL 마이크로캅셀의 결과에 비해 현저히 증가되었다. Polycarbophil로 코팅한 생체막점착성 마이크로캅셀은 carbomer로 코팅한 제제에 비해 Cm의 증가뿐아니라 전체적인 약물흡수의 증가와 높은 혈중농도의 지속을 보여주어 생체막점착성 제제의 이상적인 약물흡수패턴을 보여주었다. 3. 흡수속도효율 (ARE)면에서 생체막점착성 마이크로캅셀이 코팅전의 마이크로캅셀에 비해 약간 떨어졌으나, polycarbophil 코팅제제의 경우 전체적인 흡수양상이 마이크로캅셀에 비해 우수하고 또한 제어방출효율 (CRE)이 현저하게 증가되어 생체막점착성 제제로서의 유용성을 확인할 수 있었다. 4. 위내용배출의 지연효과, AUC, CRE 등을 종합해 볼때, Polycarbophil 로 코팅한 생체막점착성 마이크로캅셀은 투여횟수의 감소 및 생체이용율을 향상시키는 유용한 제제로 평가되었다.

  • Bioadhesive microcapsules of cephalexin, using Eudragit RS/RL coated with polycarbophil or carbopol, were evaluated biopharmaceutically. The GI transit of microcapsules in rats was studied. Bioadhesive microcapsules coated with polycarbophil or carbopol were shown to have substantially longer GI transit time than Eudragit RS/RL microcapsule. The delay in transit time was due to bioadhesion of the polymer to the mucin-epithelial cell surface which was clearly observable on animal autopsy. Plasma drug levels in rabbits showed that bioadhesive microcapsules resulted in a longer duration of action and greater bioavailability than other microcapsule or drug powder. Thus, the principle of bioadhesion can significantly improve therapy, due to a reduced rate of gastric emptying, an increase in contact time, and the intimacy of contact of the drug with the absorbing membrane.

  • The objective of this study was to find a rational dosage form for vaginal mucosal delivery of LHRH. Vaginal absorption of LHRH was estimated by measuring its ovulation inducting effect in rat and in vitro vaginal membrane permeation study in rabbit. THe effects of different hydrogel bases, such as Polycarbophil and Pemulen compared with solutions on vaginal membrane permeation of LHRH were investigated. Sodium laurate, disodium ethylenediamine brane permeaiton of LHRH were investigated. Sodium ethylenediamine tetraacetate (EDTA) and sodium tauro-24, 25-dihydrofusidate (STDHF), which are effective peptidase inhibitors were chosen as additives to a LHRH hydrogel delivery system and LHRH solutions. A Polycarbophil compared with a solution formulation 3.4 times increase in LHRH vaginal membrane permeability compared with a solution formulation. Vaginal membrane permeability from the Polycarbophil was greater than that from Pemulen hydrogels. This may be due to the larger bioadhesive values. LHRH solution with EDTA(2%), STDHF(1%) and sodlaurate(0.5%) showed 4.1 times, 4.8 times and 6.0 times of ovulation inducing activity compared with control. These results suggest that enzyme inhibition effect of EDTA, STDHF and sod, laurate may be result in substantial enhancement of vaginal absorption. By administraiton of Polycarbophil hydrogels containing LHRH the ovulation inducing activity was 3.3 times greater than the solutions. This result indicates the bioadhesive hydrogels as well as peptidase in hibition significantly improved absorption of LHRH. By coadministration with these inhibitors the ovulation inducing activity of Polycarbophi hydrogel containing LHRH was comparable with subcutaneous administration in ovulation inducing activity.

  • The mucosal route of administration(nasal, buccal, conjunctival and vaginal) has recently been considered as an alternative to parenteral delivery for many peptide drugs because enzymatic degradation of these agents may be partly avoided. The objective of these study was to establish the optimal mucosal administration dosage form of $LHRH/[D-Ala^6]LHRH$, based on presystemic metabolism. We reported previously the peptidase inhibition effect of medium chain fatty acid salts(sodium caprylate, soadium caprate and sodium laurate), EDTA and STDHF on the proteolysis of $LHRH/[D-Ala^6]LHRH$ in rabbit mucosal homgenates. We also reported that EDTA, STDHF and sodium laurate markedly increased the potency of $LHRH/[D-Ala^6]LHRH$ solution applied vaginally. In the present study, by administration of polycarbophil hydrogel containing LHRH the ovulation inducing activity was 3.3 times greater than solution. These results indicate not only peptidase inhibitor but also polycarbophil hydrogel significantly improved the absorption of this drug. The results of this study would provide the feasibility as a rational dosage form for improving bioavailability and self administration of this hydrogel by the vaginal application.

  • To develop the buccal delivery system of rhEGF for the treatment of buccal mucosal ulcer, polymer films and hydrogels were investigated. Methods: Hydrogels for thermosenstive sol/gel systems were prepared by the cold method (Schmolka, 1972). And mucoadhesive films were prepared by mixing sod. alginate/polycarbophil 974p. To find an optimum buccal mucosal adhesive gel or film, the gel strength of the poloxamer and sod. alginate/polycarbophil 974p hydrogels were determined by the Simple Rheology Method and their mucoadhesiveness were measured by the Instron (M 4400, Instron Co., U.S.A.) method.   • We have studied the effect of rhEGF on the buccal mucosal ulcer healing. rhEGF was rapidly degraded upon incubation with the hamster buccal mucosal homogenates; The degradation of rhEGF was significantly inhibited by sodium lauryl sulfate (SLS). Eudispert hv hydrogel and Polycarbophil 974P hydrogel were prepared for rhEGF delivery and their mucoadhesiveness was measured by the $Instron^R$ method. The mucoadhesive force of Eudispert hv was significantly greater than that of Polycarbophil 974P. Moreover, rhEGF in Eudispert hv hydrogel remained stable for about 2 months. To evaluate the ulcer healing effect of rhEGF, the buccal mucosal ulcer was induced in golden hamsters using acetic acid. At 24 h after administration of rhEGF/Eudispert hv hydrogel, the ulcerous area was decreased compared with rhEGF solution and, as a result, the curative ratio was $36.8\pm5.68$%. By the addition of SLS (0.5%) to Eudispert hv hydrogel, the curative ratio increased 1.5 times. The mechanism of the action was probably due to a combination of protection of the drug against proteases present in mucosa and prolongation of the release of rhEGF from the formulation at the site of action.

  • The purpose of this study is to investigate the effect of mucosal vaccine delivery vehicles and adjuvants on the local and systemic antibody responses following mucosal immunization of mice with hepatitis B surface antigen (HBsAg). Mice were immunized on days 0 and 21 by administration of hepatitis B surface antigen B (HBsAg) into the vagina.   • A novel polymeric tablet of tinidazole (TD) was formulated to treat Helicobacter pylori and Giardia lambria more efficiently with reduced hepatotoxicity by controlling the release of TD after oral administration. TD tablets containing various concentrations of either xanthan gum (XG, viscosity enhancer) and/or polycarbophil (PC, mucoadhesive) were prepared by the wet granulation method. In vitro release of TD into pH 2.0 and pH 5.0 buffer solutions was observed at 37°C by using an USP dissolution tester and an UV (313 nm) spectrophotometer. In vivo absorption of TD tablets was investigated in rabbits by measuring the blood concentration of TD after oral administration using a HPLC. Compared to a commercial TD tablet, in vitro release of TD in both pH 2.0 and pH 5.0 buffer solutions significantly decreased as the concentration: of XG or PC in the tablet increased up to 30%. However, when XG and PC was added in combination, TD was completely released in a pH 5.0 buffer solution within 8 hours, whereas the release of TD in pH 2.0 buffer solution significantly decreased. TD in a commercial tablet was rapidly absorbed after oral administration in rabbits. After oral administration of the polymeric tablets that contain both XG and PC, plasma concentration of TD dramatically decreased. Since the oral absorption of TD significantly decreased by the addition of XG and PC in the tablets while TD completely released in a pH 5.0 buffer solution, it was speculated that more TD was retained in the gastrointestinal tract. Thus, it was possible to control the release of TD by changing the content of XG and/or PC in the tablet, thereby manipulating the release rate and the gastrointestinal retention of TD after oral administration in rabbits.
