Differences in the expression rate and genotype of Porphyromonas gingivalis according to smoking status (흡연행태에 따른 Porphyromonas gingivalis의 발현율과 유전형 차이)

  • Purpose: The purpose of this study was to differences in the expression rate of Porphyromonas gingivalis according to smoking status, smoking amount and period of smoking. Methods: At the time of investigation, 30 smokers and non-smokers were recruited among patients with periodontitis with a probing pocket depth(PPD) of 4 mm or more. General information was collected using a self-questionnaire, and the average value was used by a dentist to measure the probing pocket depth of three times each for the first or second molar. Plaque collection and analysis were performed by collecting only subgingival plaque using a conventional method, and the expression rate of P. gingivalis was confirmed using polymerase chain reaction (PCR). For statistical analysis, the SPSS Ver 25.0 program was used. Results: Smoking did not have a significant effect on the expression of P. gingivalis, but it did affect the expression of more type II genotypes (p<0.05). In addition, smokers had more slight periodontal pocket, and the amount and duration of smoking did not affect the expression of P. gingivalis. Conclusions: In the future, it is necessary to reinforce the group of smokers and non-smokers with healthy oral conditions, and to investigate the quantitative difference in the expression rate and genotype of P. gingivalis over time of harmful substances in smoking.

  • Post-tensioning the full-depth precast slab longitudinally is to eliminate the tensile stresses in the transverse joints and to prevent any leakage through the joints. When the prestressing is applied to full-depth precast slab which does not composite steel girder, stress concentration occurs at the corners of shear pocket, and compressive stress is not uniformly distributed in the section of precast slab. In this paper, full-depth precast slabs using four different shapes of shear pockets are analyzed by commercial finite element program. Round type of shear pockets is superior to reduction in stress concentration.

Utilization and Out-of-pocket Expenditure of Complementary and Alternative Medicine in Low-income Patients with Osteoarthritis in a City (일개 시지역 저소득 골관절염 환자의 보완대체요법 이용실태 및 비용 -의료급여 및 건강보험하위 20% 대상자를 중심으로-)

  • Objective: The use of complementary and alternative medicine (CAM) is common especially among patients with osteoarthritis The aim of this study was to investigate the utilization rate and expenditures of patients who use CAM. Method: Two hundred seventy four patients with osteoarthritis were interviewed by a telephone survey. A structured questionnaire about sociodemographic features and type, cost, satisfaction and reason of CAM utilization was used Results: Among 274 patients with osteoarthritis, 251 patients(91.6%) had used at least one type of CAM during six months. There was a significant difference in sex (female), age (70 years), medical security (insurance), educational level between the user and non-user of CAM. Hyperthermia was the most use. The average cost for CAM utilization was 120 thousands won/person during six months and there was no difference in sociodemographic features among the out-of-pocket cost of users. The scores of satisfaction for CAM use were ranged between 60-70. Conclusions: CAM became a popular source of health care because of elderly and lay referral system. And Korean spent a substantial amount of out-of-pocket money on CAM without benefit. Health care system and professionals should pay more attention to CAM, make a evidence for CAM.

  • In this study, a series of column test as a way in order to make up for the weakness point of the conventional acceleration method were conducted to both propose the suction board drain method and grapes the specific improvement character of this method as a result of a sort of plastic drain board and a phase of vacuum pressure conditions. On this occasion, the study focused on computing the effective factors of the fittest Suction board drain method affected by each condition through confirming the settlement generated during the test, the water content reduction and stress increase effect occurred arising from the test, and the ratio of consolidation related to the improvement period. In accordance with the shape of core and that whether the core is attached to the filter(pocket or adhesion), the castle type of adhesion and the column type of pocket are more efficient than the others as a consequence of the test to find out the improvement effect depending on each drainage such as a castle type, coil type, harmonica type, column type of pocket and a castle of the adhesion. In case of the step suction pressure, the shorter the period of $-0.8\;kg/cm^2$ as a final step of the suction pressure is, the better the improvement is. In addition, the correlation between degree of consolidation per each suction pressure level and duration of application was drawn as a curve and the point of inflection on this curve was provided to determine the duration period to maximize the consolidation.

Elucidation of the Inhibitory Effect of Phytochemicals with Kir6.2 Wild-Type and Mutant Models Associated in Type-1 Diabetes through Molecular Docking Approach

  • Among all serious diseases globally, diabetes (type 1 and type 2) still poses a major challenge to the world population. Several target proteins have been identified, and the etiology causing diabetes has been reasonably well studied. But, there is still a gap in deciding on the choice of a drug, especially when the target is mutated. Mutations in the KCNJ11 gene, encoding the kir6.2 channel, are reported to be associated with congenital hyperinsulinism, having a major impact in causing type 1 diabetes, and due to the lack of its 3D structure, an attempt has been made to predict the structure of kir6.2, applying fold recognition methods. The current work is intended to investigate the affinity of four phytochemicals namely, curcumin (Curcuma longa), genistein (Genista tinctoria), piperine (Piper nigrum), and pterostilbene (Vitis vinifera) in a normal as well as in a mutant kir6.2 model by adopting a molecular docking methodology. The phytochemicals were docked in both wild and mutated kir6.2 models in two rounds: blind docking followed by ATP-binding pocket-specific docking. From the binding pockets, the common interacting amino acid residues participating strongly within the binding pocket were identified and compared. From the study, we conclude that these phytochemicals have strong affinity in both the normal and mutant kir6.2 model. This work would be helpful for further study of the phytochemicals above for the treatment of type 1 diabetes by targeting the kir6.2 channel.

How Much should the Poor Pay for their Health Care Services under the National Health Insurance System? (계층간 진료비 본인부담의 형평성에 관한 연구)

  • Although National Health Insurance(NHI) in the South Korea has guaranteed access to health insurance coverage to virtually all the people, a significant portion of out-of-pocket spending can create substantial financial burdens for some beneficiaries, particularly those with low incomes. Previous studies have estimated the magnitude of out-of-pocket spending by types of chronic illness or in- and out-patients. Prior estimates, however, have not given a complete picture of the impact of health care costs on lower-income populations. The result from this study shows that 20 percent of beneficiaries in the lowest-income quintile spent more than twelve percent of their household equivalent income out-of-pocket health services, whether they were enrolled in a Health care services or not. In comparison, the beneficiaries in the highest-income quintile level spent only 2 percent of their income out-of-pocket on health care. Also, the regression analysis suggests that age, household income, number of chronic illness, type of hospital in addition to the number of usage may affect the size of out-of-pocket spending.

  • In the high pressure state, the leakage flow rate of hydraulics is one of serious problems and the great reason to decrease the volume efficiency. In this paper, I tried to clarify tribology characteristics for the slipper hydrostatic bearing in swash plate type axial piston pumps and motors by means of experiment . I measured the leakage flow rate between swash plate and piston shoe with change the supply pressure and oil temperature at a swash plate angle of 0。 . And I also investigated the slipper pocket pressure and calculated oil film thickness for theoretical method. So. 1 have analyzed the tribology characteristics of hydrostatic bearing for leakage flow rate and oil film thickness with oil temperature and supply pressure.

  • Two type pistons which had different configuration were made to find out the effects on combustion and flame propagation. Flame propagation speed was obtained by use of the cylinder head gasket ionization probe. Ionization Probe voltage output and flame propagation speed were increased according to the air fuel mixture ratio increase. Exhaust direction flame propagation speed was fastest in combustion chamber and next was front direction, rear direction and intake direction cause of tumbling motion in cylinder. In case of remove the valve pocket in piston, average flame propagation speed changed slowly and spark timing was advanced. Also emission was decreased.

Influence of periodontal treatment on the glucose and lipid metabolism in Korean type 2 diabetic patients (한국인 제2형 당뇨병환자에서 치주치료가 당질 및 지질대사에 미치는 영향)

  • The aim of this study was to appraise the influence of conventional periodontal treatment on metabolic control in Korean type 2 diabetic patients. In addition, their periodontal change was compared with non-diabetic patients. Before and after treatment, it was performed to measure periodontal and metabolic indices in thirteen type 2 diabetic patients. Periodontal indices included plaque index, gingival index, bleeding on probing, probing pocket depth, gingival recession, and clinical attachment level. Metabolic indices included glycated hemoglobin(HbA1c), fasting plasma glucose, fasting plasma insulin, total cholesterol, triglyceride, and HDL-cholesterol. Plaque index, gingival index, bleeding on probing, probing pocket depth, and gingival recession showed significant improvements in the statistics. Diabetic patients showed no statistically significant differences in the changes of periodontal indices compared with non-diabetic patients. HbA1c values decreased in five of the thirteen subjects and fasting plasma glucose levels were reduced in four of the seven subjects after periodontal treatment. All five subjects whom HOMA values were calculated in showed the increases of insulin secretions. The results of this study ascertained the possibility of the better glycemic contol after conventional periodontal treatment in Korean type 2 diabetic patients and diabetes were well healed of their periodontal diseases after the treatment.
