Hypoplagia of Right Clavicle Associated with Neurofibromatosis (A Case Report) (쇄골의 저형성을 동반한 신경섬유종증(증례보고))

  • Neurofibromatosis is characterized by its skeletal involvement and cutaneous manifestation. Some patients develop focal area of hypoplasia and atrophy at limbs. We present the case of a 13-year-old boy with a fracture of the hypoplastic clavicle associated with neurofibromatosis and brownish cutaneous macules. The patient received open reduction and internal fixation with a semitubular plate and biopsy was done in order to clarify the pathology.

Late-onset Brachial Artery Occlusion caused by Subclavian Artery Stenosis after Clavicular Fracture: A Case Report

  • We report a rare case of late-onset brachial artery occlusion caused by subclavian artery stenosis with excessive scar tissue after open reduction and plate fixation for clavicular fracture. When he referred to us, the right hand were pale and the radial and ulnar pulses at the wrist were absent. CT-angiogram showed compression of subclavian artery by excessive scar tissue beneath the fracture site and angiography revealed stenosis of subclavian artery with thrombus and complete obstruction of blood flow in the brachial artery with emboli. Therefore, we performed embolectomy. 2 years after operation, patient was essentially asymptomatic except mild pain after long standing elevation of arm. We recommend that minimal soft tissue dissection should be needed in the operative treatment of clavicular fracture, especially soft tissue beneath the clavicle should be protected maximally.

Management of Traumatic C6-7 Spondyloptosis with Cord Compression

  • A case of total spondyloptosis of the cervical spine at C6-7 level with cord compression is described in a 51-year-old male. Because the bodies of C6 and 7 were tightly locked together, cervical traction failed. Then the patient was operated on by a posterior approach. Posterior stabilization and fusion were performed by C4-5 lateral mass and C7-T1 pedicle screw fixation and rod instrumentation with bridging both C4-5's rods to the C7-T1's extended ones. After C6 total laminectomy and foraminotomy, the C6 body was returned to its proper position. Secondly, anterior stabilization and fusion were performed by C6-7 discectomy with a screw-plate system. A postoperative lateral plain radiograph showed good realignment. In this case, we report the clinical presentation and discuss the surgical modalities of C6-7 total spondyloptosis and the failed close reduction.

  • Background: Clinical and radiological results based on fracture types and associated injuries after the treatment of tibial plateau fracture were evaluated for analyzing prognostic factors. Materials and Methods: From June 1997 to June 2002, 50 cases were followed for at least 1 year. Mean age was 47.4 years, and mean follow period was 30.0 months. Fracture classification was performed by the Schatzker method. Clinical and radiological evaluation were performed by the Porter and Rasmussen method. Evaluation was based on degree of reduction and associated injuries, etc. Results: The most common cause of injury was traffic accident (37 cases, 74%), The common fracture types by Schatzker classification were type II (14 cases) and VI (12 cases). Methods of treatment were screw fixation (15 cases), plate and screw (21 cases), external fixator (5 cases), and conservative treatment (9 cases). The most common associated injuries were ipsilateral fibular fracture (18 cases) and MCL (medial collateral ligament) injury (8 cases). Conclusion: Acceptable results after treatment of tibial plateau fracture were obtained from the anatomical reduction group, non-associated injury group, the young age group, and the early ROM (range of motion) beginning group.

  • Severe maxilla1y deficiency can be caused by cleft lip and palate(CLP), other craniofacial deformities, atrophy in the edentulous maxilla, and trauma. Patients with maxillary deficiency present a difficult treatment challenge. Traditionally, this skeletal deformity has been treated by Le Fort osteotomy, skeletal repositioning, and fixation with mini-plates and screws. The drawbacks of this method include a limited amount of anterior maxillary advancement often requiring simultaneous mandibular setback, the inability to create new bone, and minimal soft tissue adaptation to the new position, all of which increase the potential of relapse in case of large advancement. The alternative method of maxillary distraction osteogenesis offers promising results for successfully treatment of these patients while potentially minimizing the risk of relapse.

Chest Wall Reconstruction with a Transverse Rectus Abdominis Musculocutaneous Flap in an Extremely Oversized Heart Transplantation

  • An 8-year-old girl diagnosed with dilated cardiomyopathy and Russell-Silver syndrome was admitted to our pediatric intensive care unit due to low cardiac output and multiple-organ dysfunction. The patient was placed on the heart transplant waiting list and extracorporeal membrane oxygenation was performed as a bridge to transplantation. After 17 days, heart transplantation was performed. The donor was a 46-year-old female (weight, 50 kg; height, 150 cm). The donor:recipient weight ratio was 3.37:1. Because the dimension and volume of the recipient's thoracic cage were insufficient, the sternum could not be closed. Nine days after transplantation, the patient underwent delayed sternal closure. To obtain adequate space, we left the sternum 4.5 cm apart from each margin using four transverse titanium plates. A transverse rectus abdominis musculocutaneous flap was chosen to cover the wound. Due to the shortage of donors, a size-mismatched pediatric heart transplantation is sometimes unavoidable. Closure of the opened sternum of a transplant recipient can be challenging. Sternal reconstruction after an extremely oversized heart transplantation with transverse titanium plate fixation and a musculocutaneous flap can effectively achieve sternal closure and stability.

Outcome of Open Reduction Via Retromandibular Approach for Mandibular Subcondyle Fracture (하악골 과두경부 골절에 있어서 후하악부 절개법을 이용한 관혈적 정복술의 유용성)

  • Subcondylar fractures have generally been treated by intermaxillary fixation except in cases where there is an absolute indication for open reduction. The reason behind a less aggressive surgical approach lies on the inherent difficulties in manipulating fragments in such a small area at the risk of damaging facial nerves or vessels such as the internal maxillary artery. However, long-term follow-up studies showed that conservative treatment of subcondylar fractures results in disturbances of occlusal function, deviation of the mandible, internal derangement of the temporomandibular joint, and ankylosis of the joint. We carried out open reduction of dislocated subcondylar fractures in 14 patients from 2000 to 2004 by a retromandibular approach. After the reduction of fractured bone, two H-shaped miniplates with 6 holes were fixed with screws at the anterior and posterior surfaces of the subcondyle. The retromandibular approach allowed good access and easy manipulation of the subcondyle. Immediate relief from malocclusion and correction of mandibular midline shifting were observed in all patients. Late temporomandibular dysfunction and ankylosis were not observed. Open reduction with plate osteosynthesis made it possible to avoid IMF in 7 of the 14 patients. The present study shows that open reduction through this retromandibular approach can produce good outcome in adult patients with subcondylar fracture.

Comaprison of Six Serological Methods for the Diagnesis of Bovine Brucellosis (브루셀라병(病) 검색(檢索)에 사용(使用)되는 여러가지 혈청진단법(血淸診斷法)의 비교연구(比較硏究))

  • Results obtained from six secological tests for diagnosing bovine brucellosis-standard plate agglutination test (SPT), standard tube agglutination test(STT), complement fixation test(CFT), Rivanol test (RT), agar gel precipitation test (AGP) and counterimmunoelectrophoresis(CIEP) were compared using 38 sera from brucella reactors and 222 sera from dairy and beef cattle in field. The SPT gave 1.6% apparent false negative reactions and 15.4% apparent false positive reactions when compared with STT which is an official test for bovine brucellosis in this country. The distribution of antibody titers determined by STT showed that 37.5% of 38 reactors had antibody titers ranging from 100 to 200, and the remaining 62.5% had antibody titers of 400 or higher. when 38 reactor judged by STT were tested by CFT and RT, 32 cattle(82.4%) were positive by CFT and 33 cattle (86.8%) were positive by RT, respectively. This results suggest that RT is comparable to CFT in the diagnosis of bovine brucellosis. The results also indicated that both AGP and CIEP were insensitive to detect brucella infection in cattle.

COMPLICATIONS ASSOCIATED WITH OPEN REDUCTION OF MANDIBULAR FRACTURES (관혈적 정복술로 치료한 하악골 골절에서의 합병증)

  • Purpose: The aim of this study was to evaluate the complications of open reduction and internal fixation of mandibular fracture with miniplates. Patients and Methods: A total of 134 patients who presented with 196 fractures were analyzed retrospectively. Complications were evaluated for factors such as age, the site of fracture, the severity of fracture, delayed operation, preoperative wound contamination, the site and disposition of teeth in the fracture line and midfacial fractures. Statistical analysis was used to compare complications to risk factors. Results: Of the 134 patients, 20 patients had some form of postoperative complications and complication rate was 14.9%. These complications included infection, plate fracture, malocclusion, wound dehiscence, nerve injury and nonunion. There was a significant correlation between complication rate and the severity of fracture, preoperative wound contamination and the site of the fracture and disposition of teeth(p<0.05). Conclusion: The occurrence of postoperative complications in the treatment of mandibular fractures was related to the severity of fracture, preoperative wound contamination and the site of the fracture and disposition of teeth.

Monoclonal antibody-based enzyme immuno-slide assay (EISA) in the rapid diagnosis of Peste des petits ruminants of goats

  • Monoclonal antibody (mAb)-based enzyme immune-slide assay (EISA) was used for the detection of Peste des petits ruminants (PPR) virus from field samples collected from a natural outbreak. The clinicopathological study was undertaken to diagnose the case primarily of PPR. Antigen was detected from discharges and faeces of infected goats and swabs of postmortem lesions prepared on glass slide or glass plate using acetone fixation. Nasal discharge collected at the early stage of disease course or lung is an appropriate ante- or postmortem sample for this technique, respectively. Convalescent polyclonal sera collected from recovered animals which were diagnosed as PPR by EISA showed high antibody titer against PPR by C-ELISA, demonstrating the satisfactory specificity of the test. Therefore, EISA is a sensitive and specific assay to confirm PPR infection both in field and laboratory conditions and especially suitable for developing country.
