관절염 환자의 운동행위 결정요인 (Determinant factors of Exercise behaviors in Patients with Arthritis)

  • The aims of this study were to understand and to predict the determinant factors affecting the exercise behaviors and physical fitness by testing the Ponder's health promotion model, and to help the patients with rheumatoid arthritis and osteoarthritis perform the continuous exercise program, and to help them maximize the physical effect such as muscle strength. endurance, and fuctional status and mental effects including self efficacy and quality of life, and improve the physical and mental wellbeing, and to provide a basis for the nursing intervention strategies. We analyzed the clinical records of 208 patients with rheumatoid arthritis and degenerative arthritis who visited the outpatient clinics at H university hospital in Seoul between October 5, 1999 and October 24, 1999. Data were composed of self reported questionnaire and good of fitness score which were obtained by pedalling the ergometer of bicycle for 9 minutes. SPSS Win 8.0 and Window LISREL 8.12a were used for statistical analysis. 24 Of 54 hypothetical paths were supported in modified model, which was considered as a proper model with improved fit index. The physical fitness was directly influenced by exercise participation behavior and education level, and indirectly by physical fitness, while fatigue, physical disability, pastexercise behavior, life-style, self-efficacy, which explained 20% of physical fitness. The exercise participation were directly influenced by perceived benefits and self-efficacy, and indirectly influenced by life-style, fatigue and physical disability, and directly and indirectly by past exercise behavior, which explained 53% of exercise participation. Exercise score were directly affected by perceived health status, perceived benefits, self efficacy, and past exercise behavior, and were indirectly affected by fatigue, physical disability, and life-style, which explained 50%. Perceived health status were directly influeced by level of education, depression, sleep disorder, and physical disability, which explained 34% of perceived health status. Perceived benefit was directly influenced by fatigue, sleep disorder, physical disability, and life-style, which explained 45%. Perceived barriers was directly influenced by fatigue, sleep disorder, and lifestyle, which explained 9%. Self- efficacy was directly influenced by fatigue, physical disability, past exercise behavior, and level of education, which explained 61%. In conclusion, important variables for physical fitness were exercise participation and level of education, and variables affecting exercise participation were perceived self-efficacy, benefits, and past exercise behavior. Perceived self-efficacy of exercise was a significant predictor of exercise participation. Life-style, fatigue, and physical disability showed direct effects on perceived benefit, perceived barriers, and self-efficacy, and indirect effects on exercise behavior. Therefore, disease related factor should be minimized for physical performance and well being in nursing intervention for patients with rheumatoid arthritis, and plans to promote and continue exercise should be soaked to reduce disability. In addition, Exercise program should be planned and performed by the exact evaluation of exercise according to the ability of the patients and the contents to improve the importance of exercise and self efficacy in self control program, dedicated educational program should be involved.

노인의 계획된 운동이 자아효능감, 우울, 생활만족도에 미치는 영향의 메타분석 (A Meta-analysis of the Effect of Planned Exercise on their Self Efficacy, Depression, Life Satisfaction)

  • 본 연구는 남여 65세 이상 계획된 운동이 자아효능감, 우울, 생활만족도에 어떤 영향의 크기가 있는지 2004-2014년 국내외 석사 및 박사 학위논문 관련 14문헌을 토대로 메타분석법을 통하여 신뢰성 높은 통합적인 자료 제시에 있다. 노인피험자 대상은 계획된 운동 경험이 없는 문헌을 선정하였다. 또한 운동기간은 12주-24주 사이이며, 매주 3회씩 한회 50분정도의 운동량이었다. 운동 강도는 피험자가 평소 하던 유산소성 운동이었다. 노인의 계획된 운동은 운동전에 비하여 후에 자아효능감의 평균효과크기는 0.793(26.1%증가), 우울은 -0.91(31.9%감소), 생활만족도는 1.506(43.4%증가)로 큰 영향이 있는 크기이었다. 이는 운동 전의 심리적 변인이 계획된 운동 후에 우울감은 낮추고 자아효능감과 생활만족도에 긍정적인 큰 영향을 미친다는 의미이다. 노년기에 건강하고 행복한 삶을 영위하기 위해서는 원활한 움직임이 지속가능하도록 구체적인 자료 제공과 적극적인 독려가 병행되어야 하겠다. 이 결과는 노인의 일상생활에 적절한 운동에 최선을 다하도록 동기부여 자료가 될 것이며, 여성 나아가 연령별로 구체적인 자료가 더 제공되어 예방의학적 건강한 삶을 영위할 수 있도록 실질적인 자료가 많이 확충하여야 하겠다.

계획행동이론을 적용한 대학생의 생활체육 참여의식 연구 : COVID 19 팬데믹 이후를 중심으로 (Causal relationship between exercise commitment and exercise continuation intention according to the use of mobile home training : Changes in fitness after Covid-19)

  • 본 연구는 계획행동이론(Theory of Planned Behavior ; TPB)을 바탕으로 코로나-19 환경에서 대학생이 인식하는 생활체육과 참여의사 및 참여행동에 관하여 인과 관계(SEM)를 적용하여 대학생이 인식하는 생활체육을 탐색하고, 생활체육 종목을 예측하기 위한 기초자료를 제공하고자 하였다. 이를 위해 2020년 12월 1일 ~ 2021년 5월 1일까지 표본계획에 의해 모바일 프로그램인"서베이 몽키"를 사용하여 총 267명 분석에 사용했다. 연구결과 계획행동이론는 생활체육 참여의사와 통계적으로 유의미한 정(+)의 상관관계가 있는 것으로 나타났다. TPB는 생활체육 참여행동에 통계적으로 유의미한 정(+)의 상관관계가 있는 것으로 나타났으며, 생활체육 참여의사는 생활체육 참여행동과 통계적으로 유의미한 정(+)의 상관관계가 있는 것으로 나타났다.

에어로빅 운동이 여대생의 체내지질 및 Ca 대사에 미치는 영향 (The Effect of Aerobic Dancing on Lipid and Ca Metabolism in College Women)

  • This study was designed to find out the effects of aerobic exercise on lipid and Ca metabolism in seven healthy college women, aged 20 to 22 years. Metabolic studies were conducted before and after a 10-weeks exercise period, during which subjects participated in the planned aerobic dance program every day except Sunday. The initial mean time engaged in the exercise was 28 minutes and it was gradually increased up to 45 minutes around the middle point of the exercise period. During both of metabolic study periods, the subjects ate experimental diets which supplied about 120g of protein and 600mg of Ca daily and during the rest of the experimental period they ate their usual diets. The use of alcoholic beverages and drugs were prohibited. The results were summarized as follows. 1) The effect of exercise on lipid metabolism. (1) Mean body weight decreased significantly after exercise(p<0.01) and it did not show a significant decline in skinfold thickness and total body fat contents. (2) Serum total cholesterol level decreased significantly after exercise(p<0.05) and TG level also tended to be lower than that of pre-exercise period. Exercise did not exert any influence on the level of serum HDL-cholesterol in this study. (3) Exercise did not alter total lipid content in feces and apparent lipid absorption rate. 2) The effect of exercise on Ca and P metabolism. (1) After exercise, focal Ca excretion was slightly reduced, however, urinary Ca excretion was not significantly changed. In the results, a slight increase was shown in body Ca retention after exercise. (2) Exercise tended to increase urinary P excretion, but neither P balance nor fecal excretion was significantly changed after exercise. (3) Bone mineral content was not affected by exercise. In summary, aerobic exercise decreased total cholesterol and TG level in serum and tended to increase body Ca retention. With the results, it can be concluded that the additional physical activities beyond the normal daily life in college women might prevent some degenerative diseases-suchas atherosclerosis and osteoporosis.

대학생들의 운동의도와 여가시간 신체활동 간의 관계에서 극복계획과 실행계획의 매개역할 (Relationship Between Collegiate Student Exercise Intention and Leisure-Time Physical Activity: The Mediating Role of Action and Coping Planning)

  • 이 연구는 여가시간 신체활동 의도와 운동행동 간의 관계에서의 틈(gap)을 극복하고 이 관계를 잇는 매개 변인들의 역할을 검증하고, 기존의 계획행동이론의 확장을 도모하는데 목적을 두었다. 이에 여가시간 신체활동에 대한 의도와 운동행동 간의 관계에서 실천계획-1)극복계획과 2)실행계획-의 매개효과를 검증하였다. 연구의 목적을 달성하기 위하여 편의표집법을 활용하여 강원도 내 위치한 세 곳의 대학에서 교양체육에 참여하고 있는 대학생 253명(남학생=118; 여학생=105)을 대상으로 설문조사를 하였다. 수집된 자료는 SPSS와 Amos프로그램을 활용하여 분석되었으며, 기술통계, 신뢰도, 상관분석, 그리고 구조 관계를 검증하기 위해 구조방정식 모형을 적용하여 의도, 극복계획 및 실행계획, 그리고 여가시간 신체활동 및 운동 간의 관계를 검증하였다. 특히, 의도와 행동 간의 관계에서 극복계획과 실행계획의 매개효과를 검증하기 매개 분석을 실시하였다. 그 결과, 신체활동 의도는 실행계획과 운동행동에 직접적으로 영향을 미쳤으며, 실행계획은 의도에 비해 실제 운동행동을 더욱더 잘 예측하는 것으로 나타났다. 따라서 매개변인인 극복계획과 실행계획은 신체활동에 대한 참여 의도가 실제 운동행동에 미치는 영향에서 부분적으로 모두 매개역할을 하는 것으로 나타났다. 이 연구를 통해 나타난 결과를 종합해 보면, 극복계획과 실행계획의 개념화와 모형검증은 의도-행동의 틈을 극복하고 실제 현장에서 실용적으로 활용될 수 있다는 사실을 알 수 있었다.

  • The aims of this study were to understand and to predict the determinent factors affecting the exercise behaviors and physical fitness by testing the Pender's revised health promotion model, and to help the patients with rheumatoid arthritis and osteoarthritis perform the continous exercise program, and to help them maximize the physical effect such as muscle strength, endurance, and functional status and mental effects including self efficacy and quality of life, and improve the physical and mental well being, and to provide a basis for the nursing intervention strategies. Of the selected variables in this study, the endogenous variables included the physical fitness, exercise score, exercise participation, perceived benefits of action, perceived barriers of action to exercise, activity-related affect(depression) and perceived self-efficacy, interpersonal influences(family support), situational factors(duration of arthritis, fatigue) and the exogenous variables included personal sociocultural factor(education level), personal biologic factor(body mass index), personal psychologic factor(perceived health status) and prior related behavior factors(previous participation in exercise, life-style). We analyzed the clinical records of 208 patients with rheumatoid arthritis and degenerative arthritis who visited the outpatient clinics at H university hospital in Seoul. Of 75 hypothetical paths that influence on physical fitness, exercise participation, exercise score, perceived benefits of action, perceived barriers of action to exercise, activity-related affect(depression) and perceived self-efficacy, interpersonal influences(family support), situational factors(duration of arthritis, fatigue), 40 were supported. The physical fitness was directly influenced by life-style, perceived health status, education level, family support, fatigue, which explained 12% of physical fitness. The exercise participation were directly influenced by life-style, education level, past exercise behavior, perceived benefits of action, perceived barriers of action, depression and duration of arthritis, which explained 47% of exercise participation. Exercise score were directly affected by perceived self efficacy. BMI, life-style, past exercise behavior, perceived benefits of action, family support, perceived health status. perceived barriers of action, and fatigue, which explained 70%. Perceived benefits of action was directly influenced by BMI, life-style, which explained 39%. Perceived barriers of action were directly influeced by past exercise behavior, perceived health status, which explained 7%. Perceived self efficacy were directly influeced by level of education, perceived health status, life-style, which explained 57%. Depression were directly influeced by past exercise behavior, BMI, life-style, which explained 27%. Family support were directly influeced by life-style, perceived health status, which explained 29%. Fatigue were directly influeced by BMI, life-style, perceived health status. which explained 41%. Duration of arthritis were directly influeced by life-style, past exercise behavior, BMI, which explained 6%. In conclusion, important variables for physical fitness were life-style, and variable affecting exercise participation were life-style. Perceived self-efficacy of exercise was a significant predictor of exercise score. BMI, Life-style, perceived benefits of action, family support, past exercise behavior showed direct effects on perceived self-efficacy. Therefore, disease related factor should be minimized for physical performance and well being in nursing intervention for patients with rheumatoid arthritis, and plans to promote and continue exercise should be seeked to reduce disability. In addition, Exercise program should be planned and performed by the exact evaluation of exercise according to the ability of the patients and the contents to improve the importance of exercise and self efficacy in self control program, dedicated educational program should be involved. This study suggest that the methods to reduce the disease related factors, the importance of daily life-style, recognition of benefit of exercise, and educational program to promote self efficacy should be considered in the exercise behavior promotion and nursing intervention for continous performance. The significance of this study is also thought to provide patients with chronic arthritis the specific data for maximal physical and mental well being through exercise, chronic therapeutic procedure, daily adaptation and confrontation in nursing intervention.

건강한 삶을 위한 여성 고령자들의 근력운동이 건강관련 요인에 미치는 영향 (Effect of Strength Exercise Program on Health-Related Factors of the Aged for Healthy Life)

  • 본 논문은 노인요양시설에서 생활하는 고령자를 대상으로 12주간 실시한 근력운동 프로그램(주 3회, 회당 60분)이 노인들의 건강관련 체력 및 대사증후군 요인에 미치는 영향을 살펴보기 위하여 실시되었다. 총 27명의 대상자(운동군 15명, 대조군 12명)가 참여하였으며 연구의 결과는 다음과 같다. 12주간의 근력운동 프로그램은 노인의 건강관련 체력요인을 유의하게 증가시켰으며, 특히 근력 및 평형성, 그리고 유연성의 증가가 보행능력을 크게 향상시켜 낙상을 예방하는데 도움이 될 수 있을 것으로 생각된다. 그리고 노인요양시설에서 근력운동이 노인들의 혈압과 혈당을 유의하게 감소시켜 대사증후군의 위험인자를 긍정적으로 개선시킴으로써 건강수준을 증가시키는 것으로 나타났다.

에어로빅 훈련이 체지방, 체중, 혈압, 심박수 및 스트레스 반응에 미치는 영향 (An Effect of Aerobic Exercise on Blood Pressure, Pulse Rate Body Fat, Body Weight, Symptoms of Stress Response -With special reference to the adult woman-)

  • The purpose of this study were two fold : first, to determine the effect of aerobic exercise on BP, pulse rate, body fat, body weight, symptoms of sterss response: and secondly, to apply the aerobic exercise on nursing practice in the healthy or ill subjects. In an attempt to investigate the physiological and psychological effect of exercise, a quasi -experiment, non-equivalent control group pre-test & post-test design was planned. Experiments were carried out from July 4, through August 30, 1994 with 37 subjects conveniently sampled from K & Y sports center located in Taejon. The 37 research subjects were assigned to experimental(16 subjects) and control(21 subjects) groups. Aerobic exercise was carried out for experimental group from three times to five times a week for 6 weeks in aerobic center. Data were analyzed with t-test, mean, per centage of change using SAS program. Results were obtained as follows: 1) Systolic blood pressure (M=123.75mmHg, % of change=1.12%) and diastolic blood pressure (M=74.38mmHg, % of change=5.79%) was increased on that of experimental group after 6 weeks experiment. But there was no significant difference between experimental group and control group. 2) Body weight was decreased on that of experimental group(M=56.54Kg, % of change=-1.60%) and that of control group(M=52.05, % of change=-0.99%) after 6 weeks. But there was no significant difference between experimental group and control group. 3) Body fat was decreased on that of experimental group(M=30.53%, % of change=-3.60%) and that of control group (M=28.75%, % of change=-3.52%) after 6 weeks. There was no significant difference between experimental group and control group. 4) Pulse rate was decreased on that of experimental group(M=69.19회/min, % of change -8.43%) and that of control group(M=76.0회/min, % change -0.07%) after 6weeks. There was significant difference(t=-2.621, P<0.05) between experimental group and control group. 5) Symptoms of stress response were decreased on that of experimental group(M=0.97) and that of control group(M=1.15) after 6 weeks. There was no significant difference between experimental group and control group. Therefore, adherence to aerobic exercise for 6 week seem to be affected mainly pulse rate of subjects. A futher study is necessary to determine the difference in the effect of variety exercise, programs, to study over 6 weeks, to produce professional educational program for exercise speciality nurses.

계획된 행위이론을 적용한 실습중 사고예방 교육 프로그램 개발 - 치기공학과 학생을 중심으로 - (Development of an Accident Prevention Education Program on an application of the Theory of Planned Behavior during Exercise (Focusing on Students Majoring in Dental Technicians))

  • Purpose: The purposes of this study were to provide information about safe actions during the practice of dental technology, promote belief in safe actions among the students of dental technology, and reinforce their knowledge about how to act safety. Methods: Open questions were asked to 120 sophomores majoring dental technology to understand the target group's belief about safe actions. Following the instructions by Ajzen & Fishbein, the collected data were grouped according to similar beliefs. Those beliefs of the highest frequency were given a priority and got to represent the belief level of the target group. Even with belief changes, however, they would not be able to practice safe actions if they had no knowledge about how to do safe actions. For desirable behavioral changes among the students, the students were then asked to discuss the current state of safety accidents and issues focusing on the materials and processes used in the practice and give presentations regarding the subjects to identify risk factors. Then the identified risk factors were used to devise the program contents so that the students could practice safe actions through accurate behavioral approaches and belief changes by educating them about how to act safely by the practice processes and the involved materials and devices. Results: Not only do the students of Dept. of Dental Technicians have difficulties with the complex practice processes while practicing to make dental prosthesis, but they are exposed to the risk of accidents as they're poor at dealing with the materials and devices. Although there is a definite need for accident prevention education programs before practice, the reality is that the school doesn't the schoosuch education. Based on the judgment that an education program designed to reinforce belief for behavioral changes would generate effgrams results, an education program was developed by applying the Theory of Planned Behavior(TPB). Along with their beliefs, the current state of accidents n. Basepractice, demand for safety education, and practice level of safe actions were used to device the contents of accident prevention education and to develop an accident prevention education program for dental technology. Conclusion: The study presents an accident prevention education program developed to reinforce dental technology students' beliefs and knowledge and to help them do safe actions during practice.

산업장 건강증진 프로그램 개발에 관한 연구 -건강행위의 변화의도 예측요인 및 대상의 유형 분류를 중심으로- (Planning Health Promotion Program in Workplace)

  • In recent years, interests in health promotion have been stimulated by the epidemiological transition from infectious to chronic diseases as lead ing causes of death, the aging of the population, rapidly escalating health care costs, and epidemiologic findings linking individual risk to morbidity and mortality. It is not surprising that the workplace has been targeted as a promising setting for health promotion. In Korea, national attention to the opportunities for workplace health promotion began in the first 1990s. But there is no in depth study to identify the relating factors to the health promotion program in the workplace. The objective of this study is behavioral and physical characteristics to find that in crease the person's risk for a range of health problems and to analysis other characteristics to influence the degree of his/her intention to change health behavior. In addition, this study is purposed to present the process of planning health promotion program in the workplace. To accomplish these objectives, one workplaces was selected. And 363 employees in those workplaces were served as subjects for the study. Major findings in this study are as follows. (1) They have many risk factors such as smoking, drinking, lack of sleep, law rate of regular exercise, irregular eating, stress. (2) Some of the health risk factors such as smoking, drinking, and stress have the negative correlation to the intention to change. (3) Among cognitive and socio-environmental factors, significant predictors to the intention are attitude and social support. (4) In the cluster analysis to segment the target population in to homogeneous unit, three clusters of lifestyle are specified. (5) Smoking cessation and exercise program are planned for the risk group to change their behavior.
