  • Purpose: The basic vascular anatomy and versatility of the anterolateral thigh flap was reported firstly by Song in 1984 and then by Zhang who introduced the reverse flow pattern of this flap. In this case, the authors reviewed various articles and their experiences with the distally based anterolateral thigh flap and applied it for coverage of bone-exposed wound occurred at the distal of the disarticulated knee stump. We consequently reported the reliability and resourcefulness of this flap in the difficult and limited situation. Methods: A 67-year-old-man who had suffered from arteriosclerotic obliterans inevitably underwent the disarticulation at knee joint due to clinical deterioration. He presented to our clinic with soft tissue necrosis and bone exposure at the stump. We debrided the wound and conducted the distally based anterolateral thigh island flap by transecting proximal portion of descending branch of the lateral circumflex femoral artery and the $14{\times}10cm$ sized flap was transferred to cover the defect. The pedicle measured 14 cm in length with pivot point 7 cm above the patella. Results: The postoperative course was mainly uneventful except early venous congestion for 4 days and subsequent partial skin loss. The wound was healed by secondary intension and no other sequelae had been observed during follow-up period of 12 months. Conclusion: Despite the presence of various reconstructive choices, the distally based anterolateral thigh island flap can be designed to repair soft tissue defects around the knee region, providing its reliable blood supply and long pedicle length, especially in the challenging cases.

  • In religious symbology, an emblem is a symbolic mark, which differentiates a religion from other groups. In addition, it holds a symbolic or conceptual character that enables viewers to recognize a certain religion. Daesoon Jinrihoe, a representative order among Korean religious traditions, also has a symbolic mark; however, it has not been designated with an official name as symbol despite its usage dating back to May 11, 1978. At the time, that mark has served as an emblem. Afterwards, the emblem was printed and has been officially used as a flag (unofficially known as the Dao Flag, the Fellowship Flag, etc.) since October 20th of that same year. The emblem of Daesoon Jinrihoe which is not only printed in the flag but has been utilized as a symbolic mark representing the order. Nevertheless, it is hard to find research related to this symbol. Consequently, this study aims to apprehend the existing materials about the flag's emblem and its meaning, as well as attempt to interpret its various implications. Indeed, this work will suggest another point of view about the emblem given that it embraces ambiguity. This research suggests that the emblem symbolically depicts the Daesoon (Great Itineration), Samwon (三圓, Three Circles), Sadae (四大, Four Dae), and the Center, and that, all together, this can imply more profound meanings than were expressed in previously posited explanations. As such, this study draws further significance from Daesoon Thought and find: first, the circle in the center of the emblem signifies the pivot of Daesoon; not just the earthly circle (地圓) or the human circle (人圓). This opens up the possibility that the circle symbolizes Mugeuk (Limitlessness) and Taegeuk (Great Ultimate), which include the pivot of Heaven, Earth, and Humanity. Secondly, the symbol of soil (土) in the center is separated from the human circle and reveals the symbol of harmony and creation as the rod shape of Four Dae. Thirdly, the protuberances in the circle point to specific directions and this allows for additional layers of meaning.

  • The purpose of this research is to show how to use the patternist's CAD system in order to make design patterns efficiantly. Using the PAD System Technologies company's PAD System Module, which is very officiant in designing patterns, the basic pattern or design pattern can be designed and applyed to the Sample Module, thus, then it could manufacture, retouch or choose a model, making it posible to go to the next level which is seam patterns for industrial use. By understanding the change of the design that is chosen through the design pattern making and Sample Module process', this study is presentated a plan on how to utilize the patternist's working adaption ability by the CAD system, and also considered the problems in using it. Ability of Design Confrontation of CAD System: 1) The result of an operation can be saved and retouched. On a saved list the patterns can be chosen and changed, thus, making it posible to complete a design very quickly. 2) When sewing every designed pattern in Sample Module, the model can be operated merely by selecting each sewing position, thus creating the same effect of a designer's muslin sample process in a much lesser time. Point at issue in the utilization CAD system: 1) When combining two darts into one place the amounts of darts that are already these have to be erasers, and then, the two darts points are moved that place, thus, containing them, which is very conplicated. 2) When making a gathering, because system can only be recognized in a opend line, the pattern should be separated in order to put a gathering inside a pattern. 3) In order to readjust the amounte of dart. The dart is erased first and then made new dart again. 4) When making tucks in a Sample Module, the form that gets narrow towards the pivot point can not be recognized so the sewing lines of the tucks is adjusted into parallel lines. 5) The Sample Module 3D operation can not attach the bodice to the shirts, instead they become fixed into each other. In the case of shoulder line tucks the amounts of pleats can not be expanded draperary naturally, thus, making it hard to showing the silhouette accurately. If these problems are solved, especially if the silhouette of a model can be created to have natural drapery in the 3D Module, models can be shown on the screen in possible to compare and examine even a change of a detail, by designing this 3D Module Production sample process will be more effective, thus, making the CAD system more useful in sewing design process.

  • The purpose of this study was to find the characteristics of the frontal natural head position(NHP) of patients with facial asymmetry, and to contribute to the diagnosis of facial asymmetry in the clinical examination of orthodontic patients. Twenty adult patients who had apparent facial asymmetry and no severe sagittal skeletal discrepancy were selected as the asymmetry group, and 21 young adults who had symmetric faces were selected as the symmetry group. Frontal cephalograms were obtained in the state of NHP using a pivot-mounted fluid level device. The degree of the menton deviation was defined as the angle between the line drawn through crista galli and anterior nasal spine and the line drawn through crista galli and menton. The following angles were measured and each of them was compared with the degree of the menton deviation one is the angle between the true vertical line and the supra-orbital line which is a tangent line to the extreme cranial point on the supra-orbital margin, and the other is the angle between the true vertical line and the cervical line drawn through the midpoint of atlas and the 4th cervical vertebra. Through the statistical analysis, following results were obtained. 1. The angle between the supra-orbital line and the true vertical line was much mote deviated from the right angle in the asymmetry group than in the symmetry group. 2. The angle between the cervical line and the true vertical line in the asymmetry group showed greater tendency than in the symmetry group, but the difference was not statistically significant. 3. In the asymmetry group, the degree of the menton deviation was positively correlated with the angle between the supraorbital line and the true vertical line. The above results suggest that racial asymmetry patients show the tendency to have the tilted NHP to compensate the deviation of menton position.

  • Purpose: Maximizing human comfort in design of medical environments depends immensely on specialized architects particularly critical care design; the study proposes Evidence-Based Design as an apparent analog to Evidence-Based Medicine. Healthcare facility designs are substantially based on the findings of study in an effort to design environments that augment care by improving patient safety and being therapeutic. On SPSS (Statistical Package for Social Science) t-test is applied to simulate two independent variables of PDR (Pre Design-Research) and POE (Post- Occupancy Evaluation). PDR is conducted on relatively new hospital Hallym University Dongtan Sacred Heart Hospital to analyse visibility from researchers' point of view, here the ICU is arranged in I-Shape. POE is applied on Dongguk University Ilsan Hospital to simulate walking on LogWare where two NS are designed based on L- Shape and Seoul St. Mary's Hospital, The Catholic University of Korea where five NS are functional for ICU Intensive Care Unit, Surgical Intensive Care Unit (SICU), Medical Intensive Care Unit (MICU), Critical Care Unit (CCU), Korean Oriental Medical Care Unit which are mostly arranged in U-Shape, and walking pattern is recognized to be in a zigzag path. Method: T-Test is applied on two dependent communication variables: walkability and visibility, with confidence interval of 95%. This study systematically analyses the Nurse Station (NS) typo-morphology, and simulates nurse horizontal circulation, by computing round route visits to patient's bed, then estimating minimum round route on LogWare stop sequence software. The visual connectivity is measured on depth map graphs. Hence the aim is to reduce staff stress and fatigue for better patients care by minimizing staff horizontal travel time and to facilitate nurse walk path and support space distribution by increasing effectiveness in delivering care. Result: Applying visibility graph and isovist field on space syntax on I- Shape, L- Shape and U- Shape ICU (SICU, MICU and CCU) configuration, I-shape facilitated 20% more patients in linear view as they stir to rise from their beds from nurse station compared to U-shape. In conclusion, it was proved that U-Shape supply minimum walking and maximum visibility; and L shape provides just visibility as the nurse is at pivot. I shape provides panoramic view from the Nurse Station but very rigorous walking.

  • The purposes of this study are to analyze/model the mode choice behavior of the regional traveler by express bus/express train and to offer useful source in deciding the public transportation policy. The data analyzed were trips of both modes from March, 1980 to November, 1981, between Seoul and other nineteen cities; the data were grouped as five groups according to the change of service variables. Service variables were travel time(unit: minute), cost(:won), average allocation time(:won), service hour(:hour), and dummy variables by mode. As model Logit Model with linear or log utility function were postulated. As the result of this study, some reseanable models were constructed at Model Type I(eq. 2. of this paper) based on the above data except the dummy. It was judged that the parameters calibrated by Group III and Group IV data in table 4, were optimal. Among the parameters, the parameter of travel cost was most reliable. There was a tendency preferring express bus to train in October and November. With the constructed model and Pivot-Point Method. the demand change of express train caused by the service variables' change could be forecasted over 99%.

  • It is to use effectively for stream channel and watershed management as the prediction and the analysis of bed changes characteristics in the Seomjin river downstream. The necessary data (section, bed composition material, pivot point water elevation, coefficient of roughness) with regard to analysis of the bed changes characteristics were based upon the survey data and analysis results in the Seomjin river maintenance basic plan. The prediction of bed changes was also completed with HEC-6 model. The study results were summarized as follows: The main factor of bed changes in the Seomjin river downstream can be decided by extreme extraction of bed aggregate rather than the change of hydrological data. According to the analysis of bed stability based on the relation between friction velocity and representative grain size, and the relation between dimensionless tractive force and representative grain size, the Seomjin river downstream appears to be increased overall. The bed composition material in the stream channel of the Seomjin river of 2003 year shows higher composition rate of gravel and lower composition rate of sand as compared to those of 1989 year. According to result that the prediction of bed changes, it is estimated that the bed will be risen approximately 1.5 m to the place up to 9 km from the estuary, have been repetitively risen and fallen up to 1 m to the place between $9{\sim}21\;km$ section, and fallen about 0.5m to the place between $22{\sim}25\;km$ section. As a result, the bed of the Seomjin river downstream can be decided to be risen gradually. However, since the prediction of this study is based on the assumption that there will be no forced aggregate picking, the bed changes can be much greater than expected when there is a massive aggregate picking as it had happened before.
