Exacerbation Prevention and Management of Bronchiectasis

  • Bronchiectasis, which is characterized by irreversibly damaged and dilated bronchi, causes significant symptoms, poor quality of life, and increased economic burden and mortality rates. Despite its increasing prevalence and clinical significance, bronchiectasis was previously regarded as an orphan disease, and ideal treatment of this disease has been poorly understood. The European Respiratory Society and British Thoracic Society have recently published guidelines to assist physicians in the clinical field. Guidelines and reports suggest comprehensive management that includes both non-pharmacological and pharmacological treatment. Physiotherapy and pulmonary rehabilitation are two of the most important non-pharmacologic therapies in bronchiectasis patients; long-term inhaled antibiotics and macrolide therapy have gained significant evidence in reducing exacerbation risk in frequent exacerbators. In this review, we summarize recent updates on bronchiectasis treatment to prevent exacerbation and manage clinical deterioration.

Scapular muscle endurance, shoulder pain, and functionality in patients with rotator-cuff-related shoulder pain: a matched, case-control study

  • Background: Deficiency in scapular muscle endurance (SME) is a risk factor for rotator-cuff-related shoulder pain (RCRSP). However, the exact relationship among SME, pain, and functionality remains unclear. This study aims to compare SME, pain, and functionality in RCRSP patients to those in age-sex-matched healthy controls. Methods: Twenty-three patients with RCRSP and 23 age-sex matched healthy controls were included in the study. SME was measured using a 1-kg dynamometer. Self-reported pain level was assessed using a visual analog scale. The Functional Impairment Test-Hand, Neck, Shoulder, and Arm (FIT-HaNSA) was also used to assess functional impairment. Results: The control group had higher SME and total FIT-HaNSA scores than the patient group (P<0.05). There was a statistically significant and positive correlation between SME and FIT-HaNSA scores in both groups (P<0.05). Conclusions: SME was affected by RCRSP. Pain and functional impairment were correlated with low SME.

A Literatural Study on the Evidence of Using Thermotherapy and Cryotherapy of Cutaneous and Muscle Meridian in Korean Medical Physiotherapy (한방이학요법 중 경피경근온냉요법에 대한 문헌적 고찰)

  • Objectives : The purpose of this study is to establish literatural evidence about thermotherapy and cryotherapy for Korean medicine through literatural review. Methods : Applicable paragraphs which were related to the thermotherapy and cryotherapy of cutaneous and muscle meridian were phrased from in "Yibujicheng(醫部集成) and "Dongyibaojian(東醫寶鑑)" where were archiving of Oriental or Korean medicine literatures. Searched paragraphs were analysed for establishing historical and theoretical bases of thermotherapy and cryotherapy in Korean medicine. Results : Thermotherapy of cutaneous and muscle meridian(經皮經筋溫熱療法) such as hot pack, warm water therapy, paraffin bath, ultrasound is originated from yu(慰) warm water(溫水) hot water(熱水). Matching indications are various pain conditions(caused by coldness(寒), hard-work(僗若), extravasated blood(瘀血), inflammatory skin disease, frostbite and several internal diseases. It also treats gynecological diseases and facial palsy. Diathermic therapy on acupuncture points(穴位照射溫熱療法) such as infra-red, microwave, shortwave is originated from huolu(火爐), wenlu(溫爐), xianglu(香爐), lamp light(燈火). Its objective is to improve the effects of herb medicine by aiding sweating or to treat the residual symptoms of fever disease or to care skin disease and pain from bone fracture, contusion. Cryotherapy of cutaneous and muscle meridian(經皮經筋溫寒冷療法) such as ice pack, ice spray, iced whirpool, cool water bath is originated from lengfu(冷敷), lengtie(冷貼), lengshiyu(冷石熨). Matching indications are contusions, animal bite injury, corn(肉刺) and (淋病), eye disease, nasal bleeding, hemorrhoid, inflammatory skin disease and chicken pox. Conclusions : Thermotherapy and cryotherapy of cutaneous and muscle meridian(經皮經筋溫冷療法) are the treatments which were used in Korean medicine from the ancient Korean medicine. As scientific equipments were originated from yu(慰), huolu(火爐), wenlu(溫爐), xianglu(香爐), lamp light(燈火). lengfu(冷敷), lengtie(冷貼), lengshiyu(冷石熨). It can be said that these are elements of Korean medicine. More rigorous studies are needed to establish clinical evidence about not only thermotherapy and cryotherapy but also the other physiotherapy of Korean medicine.

The Effect of Chest Physiotherapy on the Amount of Tracheal Secretion and $PaO_2$ (흉부물리요법이 기관 분비물량과 동맥혈 산소분압에 미치는 영향)

  • The purpose of this research was to find out the effect of chest physiotherapy on the amount of tracheal secretion and $PaO_2$. After changing position of the neurosurgical patients who had tracheostomy cannula, experimental treatments were applied as bellows, manual chest percussion on groupI, chest percussion and manual chest vibration on groupII, chest percussion and mechanical chest vibration on groupIII were done. After these trials, we have analyzed the efficacy of each procedures comparing the group differences in the quantity of tracheal secretion and $PaO_2$. Target samples were sixty patients aged between 20 to 60 who have tracheostomy state and decreased consciousness status that were admitted in NICU of a university hospital from June 1 to August 31, 1999. They assigned randomly into three experimental groups. To compare the effect of each interventions, tracheal secretion quantify was measured and $PaO_2$ was analyzed via arterial blood gas analyzer. The data were analyzed by ANCOVA of 5% significance level using SPSS P/C program. The results were as bellows. 1) The first hypothesis 'There is a difference In the quantify of the secretion among GroupI, GroupII and GroupIII' was accepted.(F=29.27, p=0.00) 2) The second hypothesis 'There is a difference in $PaO_2$ among GroupI, GroupII and GroupIII' was rejected.(F=1.71, p=0.19) From this study results, positional change and manual chest vibration including chest percussion were the most effective treatment to get maximum amount of tracheal secretion and it was confirmed that mechanical chest vibration also made much better effect than sole chest percussion method. Therefore, we concluded that the mechanical or manual chest vibration with chest percussion is more effective respiratory care method than the sole chest percussion.

Analysis of Research Trends Using G-power in Physiotherapy Research in Korea: Systematic Review

  • Purpose: This study examined the research trends using G-power centered on major Korean registered sites and presents the research trends of studies with a high level of evidence. Method: This study selected three research journals listed in the Korea Research Foundation among physical therapy journals in the field of physiotherapy. The selected papers were classified according to the following: 1) study type, 2) study level, and 3) quality of the study. Result: Sixty-nine articles were selected and classified according to the study type. The musculoskeletal system, central nervous system, and others comprised 73.91% (n=51), 17.39% (n=12), and 8.7% (n=6), respectively. The results were classified into 62 experimental studies, four observational analytic studies, and three descriptive studies. The experimental studies included 22 randomized controlled trials (level 2) and 40 non-randomized trials (level 3). Sixteen randomized controlled trials were classified for a qualitative evaluation, and 12 papers with a score of six or more (good) were found. Conclusion: Many high-level studies were found in G-power, and the quality of the study was also good. Therefore, the effective number of samples obtained using G-power is one of the ways to enable high-quality research.

Cervical Range of Motion Associations with Sub-clinical Neck Pain (경미한 경부 증상이 있는 대상자의 경추 관절 운동 범위 연구)

  • 목적: 병원치료가 필요하지 않을 만큼의 경부통증/증상과 관절가동범위, 경흉추의 다각적 면에서 관계를 조사하기 위해서이다. 방법: 연령은 19세에서 42세(평균연령 28세로 실험에 참여하기를 원하는 건강한 성인 40명을 대상으로 하였다. 경흉추의 척추 자세, 경추 능동 관절 가동범위, 경부 분절 길이 등의 다각적인 면을 측정하였다. 모든 측정들은 동일시에 각 대상자에게 서로 다른 측정자에 의해 두 번 실시되었다. 경부 근 지구력은 수정된 Biering-Sorensen 검사법에 의해 측정되었다. 마지막으로 대상자들에게 경부 통증/증상의 재발에 대한 질문을 하였다. 결과: 14명의 대상자들은 경미한 재발성 경부 통증/증상을 보고하였다. 경부 근 지구력 시간(F(1,38)=6.75, p=0.01)과 좌측 회전 가동 범위(F(1,38)=4.56, p=0.04)가 경부 통증을 가진 대상자들에서 유의하게 감소하였다. 신전 가동범위가 재 측정에서 특정군 변화 즉, 정상군 증가, 경부 통증/증상군 감소(F(1,38)=4.67, p=0.04)가 보였다. 경부 통증/증상군은 정상군과 비교 시 후인 가동 범위의 증가하였다(F(1,38) =4.56, p=0.04). 통증 유무에 관계없이 모든 대상자들에서 우측 회전보다 좌측 회전에서 가동범위가 더 크게 나타났고(F(1,38) =4.34, p=0.04), 반복 측정에서 좌측 측방굴곡 (F(1,38) =5.10, p=0.03)과 우측 측방 굴곡(F(1,38) = 5.27, p=0.03)의 감소가 나타났다. 결론: 경미한 증상의 경부통증 대상자군과 정상적인 대상자군을 비교할 때 그룹 간 차이는 경부 근 지구력 시간의 감소, 좌측 회전 가동 범위 감소와 특히, 두 번 째 측정에서 신전범위 감소가 나타났으나 후인의 가동범위는 증가로 관찰되었다. 이러한 결과들은 경부 통증의 발생과 관련된 초기 가동범위 변화를 제안한다.

Burden of Neck Pain and Associated Factors Among Sewing Machine Operators of Garment Factories in Mekelle City, Northern Part of Ethiopia, 2018, A Cross-Sectional Study

  • Background: Neck pain is a major public health problem among sewing machine operators working in textile factories. Even though the textile industries are growing in number in Ethiopia, but there is a dearth of published studies on the prevalence of neck pain. Therefore, this study was aimed to assess the prevalence and associated factors of neck pain among sewing machine operators of garment factories in Mekelle city. Method: An institutional-based cross-sectional study design was implemented among 297 sewing machine operators' working in garment factories in Mekelle city. A systematic random sampling technique was used. Data were collected through interviews and analyzed using Statistical Package for Social Science version 23. Finally, variables with 95% confidence interval (CI): p < 0.05 in the multivariate analysis were significantly declared. Results: Two hundred ninety-seven sewing machine operators were enrolled, with 98.7% response rates. In this study, the 12-month prevalence rate of neck pain was found to be 42.3% (95% CI: 36.6%-47.9%), and variables like such as break time [adjusted odds ratio (AOR): 5.888, 95% CI: (2.775-12.493)], working hours per day [AOR: 6.495, 95% CI: (2.216-19.038)], static posture [AOR: 4.487, 95% CI (1.640-12.275)], and repetitive activity [AOR: 4.519, 95% CI:(2.057-9.924)] were associated with neck pain. Conclusion: In this study, neck pain is a major public health problem. Continuous work without break time, working greater than 8 hours per day, sitting in the same position for greater than 2 hours, and high repetitive activities were found significantly associated with neck pain. Owners and governmental bodies need to focus on developing preventive strategies and safety guidelines.

Role of suprascapular nerve block in idiopathic frozen shoulder treatment: a clinical trial survey

  • Background: Several therapeutic methods have been proposed for frozen shoulder syndrome. These include suprascapular nerve block, a simple and cost-effective technique that eliminates the need for nonsteroidal anti-inflammatory drug therapy. Methods: This was a clinical trial that included patients with unilateral shoulder joint stiffness. Patients were divided into three groups: those treated with isolated physiotherapy for 12 weeks (PT group), those treated with a single dose intra-articular injection of corticosteroid together with physiotherapy (IACI group), and those treated with a suprascapular nerve block performed with a single indirect injection of 8-mL lidocaine HCL 1% and 2 mL (80 mg) methylprednisolone acetate together with physiotherapy (SSNB group). The variables assessed were age, sex, side of involvement, dominant limb, presence of diabetes, physical examination findings including erythema, swelling, and muscle wasting; palpation and movement findings; shoulder pain and disability index (SPADI) score; and the visual analog scale (VAS) score pre-intervention and at 2-, 4-, 6-, and 12-week post-intervention. Results: Ninety-seven patients were included in this survey (34 cases in the PT group, 32 cases in the IACI group, and 31 cases in the SSNB group). Mean age was 48.55±11.06 years. Fifty-seven cases were female (58.8%) and 40 were male (41.2%). Sixty-eight patients had a history of diabetes (70.1%). VAS and SPADI scores and range of mototion degrees dramatically improved in all cases (p<0.001). Results were best in the SSNB group (p<0.001), and the IACI group showed better results than the PT group (p<0.001). Conclusions: Suprascapular nerve block is an effective therapy with long-term pain relief and increased mobility of the shoulder joint in patients with adhesive capsulitis.

Targeting motor and cognitive networks with multichannel transcranial direct current stimulation along with peripheral stimulation in a subacute stroke survivor: single case study

  • Objective: Reacquisition of motor functions following stroke depends on interhemispheric neural connections. The intervention highlighted in the present case is an insight for augmenting motor recovery by stimulating the lesioned area and adjacent areas governing the motor behaviour of an individual. The purpose of this study was to determine the changes in the motor and cognitive outcomes through multi target stimulation of cortical areas by application of multichannel transcranial direct current stimulation (M-tDCS) in a stroke survivor. Design: A case report. Methods: The patient was a participant of a trial registered with the clinical trial registry of India (CTRI/2020/01/022998). The patient was intervened with M-tDCS over the left primary motor cortex i.e. C3 point and left dorsolateral prefrontal cortex i.e. F3 point with 0.5-2 mA intensity for the period of 20 minutes. SaeboFlex-assisted task-oriented training, functional electrical stimulation over the lower extremity (LE) to elicit dorsiflexion at the ankle and eversion of the foot, and conventional physiotherapy rehabilitation including a tailored exercise program were performed. Outcome assessment was done using the Fugl-Meyer assessment scale (FMA) for the upper and lower extremity (UE and LE), Montreal Cognitive Assessment (MOCA), Wisconsin Gait Scale (WGS) and the Stroke Specific Quality of Life (SSQOL) measures. Assessment was taken at Day 0, 15 and 30 post intervention. Results: Improvement was observed in all the outcome measures i.e FMA (UE and LE), MOCA, SSQOL and WGS across the span of 4 weeks. Conclusions: M-tDCS induced improvement in motor functions of the UE and LE, gait parameters and cognitive functions of the patient.
