  • Although the efficient formation of urban space structure is a key factor in energy saving and environmentally-friendly aspect, the maintenance of the center and sub-center of the city that are key factors has been becoming increasingly difficult due to the variability and complexity of urban activities. In the case of Busan, amid the expansion of urban scale due to rapid economic development and overpopulation, systematic approaches to professional diagnosis and maintenance have been significantly insufficient - other than the city basic plan which has been conducted at an interval of 20 years. For the effective management of urban central area, systematic monitoring of the CBD through demand forecast and blight forecast at a city level must first be implemented. In order to fulfill this goal, this study is to figure out the current state of the CBD through the diagnosis on blight of the urban central area in the viewpoint of rehabilitation of the CBD and to propose the measures for practical utilization of the information about space for the further management of the central area of the city. For analysis, the study looks into the present state in terms of physical index, economic index, and social index. And then as a micro-approach by utilizing economic index, the study has thoroughly examined the economic blight of the Seomyun urban central area of Busan. The outcome of the analysis shows that in terms of population distribution and land utilization the area is in the stage of inefficient dispersion after having gone through the stage of suburbanization. It is expected that this study, as the material that proves the necessity of enhancing the function of the CBD, can propose the direction for the management of the urban center of Busan through blight prediction and management of the urban center and can provide the basic data for the long-term urban development that aims at the efficient strengthening of functions of the CBD.

  • The purpose of this study was to suggest the effective squat exercise position to strengthening vastus medialis oblique, and vastus lateralis in quadriceps. Subjects were twenty patients with patellofemoral pain syndrome (PFPS) volunteered to participate in this study. All subjects were applied to static squat convergence exercise with knee flexed $45^{\circ}$, $60^{\circ}$, and $90^{\circ}$ for 30 seconds total 5 times. Measurement variables were maximal voluntary isometric contraction (MVIC) of the quadriceps, Q angle and length of thigh. Those were measured before and after the squat exercise on knee joint angle, change rate of which were used for statistical analysis. As a result, squat exercise with knee flexed $90^{\circ}$ increase significantly among knee joint angle in the MVIC change rates of quadriceps (p<.05), however the rates of Q-angle and length change of thigh showed no significant difference. Therefore, this findings suggest that squat exercise with knee flexed $90^{\circ}$ strengthen quadriceps effectively in patients with PFPS.

  • The bond strength is one of the most important factor in establishing long-term success of esthetic restorative dentistry. So, various restorative materials have been introduced to improve the esthetic and physical properties. Ormocer (organically modified ceramic) was developed as a result of such efforts. This study was performed to compare the shear bond strength of ormocer based adhesive with that of existing dentin adhesive. In this study $Admira^{(R)}$ and $Admira^{(R)}$ bond of the ormocer system are grouped together for ADM, Single $Bond^{(R)}$ which is an one-bottle adhesive and Z-250TM which is hybrid composite resin of BIS-GMA system for SIN, and $Definite^{(R)}$ of ormocer and Etch & $Prime^{(R)}$ 3.0 which is a self etching priming/ bonding agent for ETC. The results of this study were as follows. : (1) In the comparison of shear bond strength according to different adhesive system, shear bond strength was increased in the order of ETC group, SIN group, ADM group. There was no significant difference between ADM group and SIN group. However, there was a significant level of difference between ADM and ETC groups as well as SIN and ETC groups( p<0.05). (2) Examination by a scanning electron microscope showed a well established hybrid layer and resin tag in both ADM group and SIN group, while ETC group showed a minimal formation of the hybrid layer when compared with ADM and SIN groups. From the above results, it may be reasonable to start the clinical application of ormocer system, and it is recommended that ormocer system should be used along with an ormocer based adhesive because ormocer system showed the lower shear bond strength when it used with other existing self etching priming/bonding agent. The self etching priming/bonding agent showed relatively low shear bond strength, and it is considered that the further study should be needed.

  • Purpose: Direct splinting material should have high flexural strength to withstand force during mastication and low modulus of elasticity to provide some movement while force applied for relief of stress. The purpose of this study was to compare flexural strength and modulus of elasticity of several resinous splinting materials. Materials and Methods: Four materials; Super-Bond C&B, G-FIX, G-aenial Universal Flo, FiltekTM Z350 XT; were used in this study. Fifteen rectangular bar specimens of each material were prepared. Three-point bending test were performed to determine physical properties. Maximum load at fracture was recorded and flexural strength and modulus of elasticity were calculated. One-way analysis of variance (ANOVA) and Scheffe's tests at a 0.05 level of significance were conducted on all test results. Results: Statistical analysis reveals that Super-Bond C&B had significant low mean value for flexible strength and the other three materials showed no significant difference. For modulus of elasticity, Super-Bond C&B exhibited statistically lower modulus of elasticity. G-FIX presented intermediate result, showing statistically higher modulus of elasticity than Super-Bond C&B but lower than G-aenial Universal Flo and FiltekTM Z350 XT. There was no significant difference on modulus of elasticity between G-aenial Universal Flo and FiltekTM Z350 XT. Conclusion: Using a G-FIX, the newly commercially available splinting material, which shows higher fracture resistance properties comparable to flowable and restorative composite resin and a relatively flexible nature might be a beneficial for stabilizing teeth mobility.

  • Self-injurious behavior is often showed in mental retardation, especially in autism. Self-injurious behavior has been regarded as a symptom cluster rather than a disease but it is an emergent clinical situation that can directly affect mortality. This case is about a refractory autistic patient who showed a self-injurious behavior of hitting the head repetitively. He was hospitalized and was treated by pharmacotherapy and behavior therapy and for this reason this clinical experience is reported with literature review. The patient is a 7-year old boy who was ward admitted from 1999 April 20 till July 10 into OO hospital OO ward because of self-injurious behavior. During the 12 weeks he had admission treatment. As for the pharmacotherapy, haloperidol was dosed up from 0.5mg to 1.0mg from the 4th week and combination drug therapy was done during the admission with naltrexone 25-50mg. As for the behavioral therapy, Differential Reinforcement of Other behavior was used and regular play therapy was done. To remove the physical restraint, headgear and hard sleeve was used. Currently, OPD follow up treatment is being done and haloperidol 0.5mg and naltrexone 50mg is maintained. The patient’s mother is educated and play therapy is done an hour daily at home. When the patient was released form the hospital, self-injurious behavior was decreased more than the moderate state and remission state is still being maintained at the outpatient clinic.

  • Objectives and methods : The Standards for Reporting Interventions in Clinical Trials of Acupuncture (STRICTA) were published in five journals in 2001 and 2002. These guidelines, in the form of a checklist and explanations for use by authors and journal editors, were designed to improve reporting of acupuncture trials, particularly the interventions, thereby facilitating their interpretation and replication. Subsequent reviews of the application and impact of STRICTA have highlighted the value of STRICTA as well as scope for improvements and revision. To manage the revision process a collaboration between the STRICTA Group, the CONSORT Group, and the Chinese Cochrane Centre was developed in 2008. An expert panel with 47 participants was convened that provided electronic feedback on a revised draft of the checklist. At a subsequent face-to-face meeting in Freiburg, a group of 21 participants further revised the STRICTA checklist and planned dissemination. Results : The new STRICTA checklist, which is an official extension of CONSORT, includes six items and 17 sub-items. These set out reporting guidelines for the acupuncture rationale, the details of needling, the treatment regimen, other components of treatment, the practitioner background, and the control or comparator interventions. In addition, and as part of this revision process, the explanations for each item have been elaborated, and examples of good reporting for each item are provided. In addition, the word "controlled" in STRICTA is replaced by "clinical", to indicate that STRICTA is applicable to a broad range of clinical evaluation designs, including uncontrolled outcome studies and case reports. Conclusions : It is intended that the revised STRICTA, in conjunction with both the main CONSORT Statement and extension for nonpharmacologic treatment, will raise the quality of reporting of clinical trials of acupuncture.

  • The $Na^+/H^+$ exchanger is responsible for maintaining the acidic tumor microenvironment through its promotion of the reabsorption of extracellular $Na^+$ and the extrusion of intracellular $H^+$. The resultant increase in the extracellular acidity contributes to the chemoresistance of malignant tumors. In this study, the chemosensitizing effects of cariporide, a potent $Na^+/H^+-exchange$ inhibitor, were evaluated in human malignant mesothelioma H-2452 cells preadapted with lactic acid. A higher basal level of phosphorylated (p)-AKT protein was found in the acid-tolerable H-2452AcT cells compared with their parental acid-sensitive H-2452 cells. When introduced in H-2452AcT cells with a concentration that shows only a slight toxicity in H-2452 cells, cariporide exhibited growth-suppressive and apoptosis-promoting activities, as demonstrated by an increase in the cells with pyknotic and fragmented nuclei, annexin V-PE(+) staining, a $sub-G_0/G_1$ peak, and a $G_2/M$ phase-transition delay in the cell cycle. Preceding these changes, a cariporide-induced p-AKT down-regulation, a p53 up-regulation, an ROS accumulation, and the depolarization of the mitochondrial-membrane potential were observed. A pretreatment with the phosphatidylinositol-3-kinase (PI3K) inhibitor LY294002 markedly augmented the DNA damage caused by the cariporide, as indicated by a much greater extent of comet tails and a tail moment with increased levels of the p-histone H2A.X, $p-ATM^{Ser1981}$, $p-ATR^{Ser428}$, $p-CHK1^{Ser345}$, and $p-CHK2^{Thr68}$, as well as a series of pro-apoptotic events. The data suggest that an inhibition of the PI3K/AKT signaling is necessary to enhance the cytotoxicity toward the acidtolerable H-2452AcT cells, and it underlines the significance of proton-pump targeting as a potential therapeutic strategy to overcome the acidic-microenvironment-associated chemotherapeutic resistance.
