PURPOSE: The purpose of this study was to determine the effect of shoulder external rotation on muscle activities of the scapular upward rotators during arm elevation. METHODS: Nineteen healthy subjects with no medical history of shoulder pain or upper extremity disorders were recruited for this study. Electromyography (EMG) was used to measure the muscle activities of the serratus anterior (SA), upper trapezius (UP), lower trapezius (LT) and infraspinatus (IS) muscles during arm elevation. The EMG activities were recorded while the subjects performed $90^{\circ}$ arm elevation with three different arm positions; palm down (PD), neutral position (NP), and palm up (PU). While seated in a chair, the subject was asked to raise the upper extremity in the sagittal plane in random order. Subjects performed $90^{\circ}$ arm elevations in three trials at each arm position. The mean EMG activity normalized by the maximal voluntary isometric contraction was analyzed across three arm positions. Repeated measures one-way ANOVA and the post hoc Bonferroni tests were used to determine the differences in muscle activities among the three arm positions. RESULTS: The EMG activities of the SA and IS were significantly greater in the PU condition than in the other conditions during arm elevation. No significant difference was noted between the NP and PD conditions during arm elevation. CONCLUSION: These results suggest that shoulder external rotation (palm up position) can be used to activate the SA. Therefore, we recommend a scapular protraction exercise in the palm up position for strengthening the SA.
Background: The characteristics of lateral epicondylitis (LE) are muscle strength weakness and increased common extensor tendon (CET) thickness. Ultrasonography has recently been used to evaluate tendinopathy. Diamond taping (DT) is commonly used to manage patients with LE. However, no previous studies have investigated the effects of DT on CET thickness. Objects: The aim of this study was to investigate the effects of DT applied around the lateral elbow on CET thickness, grip strength, and wrist extension force in healthy subjects. Methods: The subjects were 26 adults (13 male) in their twenties. First, the CET thickness was measured at rest. The CET thickness was measured by using ultrasonography at two points. The subjects were then instructed to perform maximal grip activities or maximal wrist extension activities before and after DT around the lateral elbow. The DT technique was applied using non-elastic tape. While the subjects performed maximal grip activities, the investigator measured the maximum grip strength (MGS) and CET thickness. Likewise, while the subjects performed maximal wrist extension activities, the investigator measured the maximum wrist extension force (MWEF) and CET thickness. Results: The MGS showed a statistically significant improvement after DT taping application in men (p < 0.05). The MWEF showed a statistically significant improvement after DT application in male (p < 0.01) and female (p < 0.05). When performing the activities, the CET thickness increased compared to that at rest. However, CET thickness didn't show a statistically significant improvement before and after DT. Conclusion: This study shows that DT applied around the lateral elbow is effective in improving MGS and MWEF. However, it does not affect CET thickness.
The purpose of the survey was to find out the prevalence rate and related factors for low back pain of nurses and to develop educational program for prevention of the low back pain. The subjects of the survey were 593 nurses of general hospitals in Tae-gu City. The survey was conducted from June 10, 1994 to October 3, 1994. The questionnaire included 34 question items concerning general characteristics, factors related to low back pain, physical nursing activities, and characteristics of low back pain. The results were as follows : 1. Among 593 of subjects, the prevalence rate of low back pain showed 62.1%. 2. In relation of health related life activities and low back pain prevalence, self-reported health state was highly significant(P=.000). 3. In relation of work environmental factors and low back pain prevalence, job satisfaction (P=.026), job stress(P=.020), and workload(P=.002) were significant. 4. In relation of physical nursing activities and low back pain prevalence, bending (P=.000), trunk twisting(P=.003), stretching(P=.006), and pulling and pushing(P=.046) were significant. 5. Physical nursing activities inducing back stress was varicant according to wards. The results of this study pointed out that the subjects' low back pain prevalence was related to the work-related physiologic and psychologic factors. Therefore, for the effective prevention of low back pain, both practicing the body mechanics and raising the morale of the nurses are needed in educational program.
This study aimed to investigate the effect of differing heel heights on the electromyographic (EMG) activity in vastus medialis (VM) and vastus lateralis (VL) during stair ascending and descending activities. A total of 26 healthy women volunteered to perform stair-ascending and stair-descending tasks with 3 heel heights: barefoot, 3 cm, and 7 cm. The EMG activities of the VM and VL were recorded during the tasks. During the stair ascending and descending tasks, the EMG activities of both VM and VL significantly changed with differing the heel heights (p<.05). Moreover, the EMG activities of VM and VL during the stair ascending task were significantly higher than the corresponding values during the stair-descending task (p<.05). However, there were no significant differences between the VM:VL EMG ratios for the 3 heel heights (p>.05). The VM:VL EMG ratios between the 2 tasks differed significantly in the 7 cm high heel condition (p<.05). Despite an increase in the EMG activities in both VM and VL during stair ascending and descending tasks, there was no change in the relative EMG intensities of VM and VL, which was measured by calculating the VM:VL ratio this result indicates that no VM:VL imbalances were elicited. The relative EMG intensities of VM and VL during stair descent were lower than the corresponding values during the ascent, suggesting that VM and VL may show an imbalance in the eccentric activation during the weight-acceptance phase. This study provides useful information that will facilitate future research on how heel height affects muscle activity around the knee joint.
PURPOSE: This study examined the effects of the chopping pattern of proprioceptive neuromuscular facilitation (PNF) on the neglect, balance ability, and activities of daily living in stroke patients with hemi-spatial neglect. METHODS: Twenty stroke patients with neglect volunteered for this study. The subjects were assigned randomly to either the experimental group (EG, n=10) or control group (CG, n=10). The chopping pattern of PNF and exercise therapy were applied in the EG and CG, respectively, for four weeks. Neglect, balance, and activities of daily living were evaluated at the pre-and post-intervention. The assessment tools included a line-bisection test, balance test using Space balance 3D, and modified Barthel index. The pre-and post-intervention values were compared in each group using a paired t-test and the between-group differences were assessed using an independent t-test. Statistical significance was set to ${\alpha}=.05$ for all variables. RESULTS: Significant differences were observed between the EG and CG in terms of neglect, balance, and activities of daily living (p<.05). Both groups showed significant improvement in neglect, balance, and activities of daily living (p<.05). CONCLUSION: These results suggest that the chopping pattern of PNF is effective in improving the hemi-spatial neglect, balance, and activities of daily living in stroke patients with neglect.
Background: Improvement of the lumbo-pelvic stability can reduce the compensatory action of the erector spinae (ES) during prone hip extension (PHE). Furthermore, the application of abdominal drawing-in (ADI) maneuver increases the action of gluteus maximus (GM) and decreases the action of ES during PHE by improving the lumbo-pelvic stability. However, the post-ADI exercise effects on PHE, not the real-time application of ADI maneuver, has not been studied. Objects: This study is aimed at investigating the post-ADI exercise effects on the muscle activities of GM and ES during PHE. Methods: A total of 24 female adults participated in the study, and they were divided into two groups: Those with normal abdominal muscles ($n_1=12$) and those with weak abdominal muscles (WA) ($n_2=12$). Before the intervention, the subjects' GM and ES muscle activities during PHE were measured. Subsequently, the two groups were asked to perform the ADI exercise for 10 minutes. After the ADI exercise, the GM and ES activities were equally measured during PHE. Results: The comparison result of the ES muscle activities before intervention shows a significant difference between the two groups (p<.05); the WA group showed higher muscle activities than the normal group. For the within-group comparison, the muscle activities of the ES in the WA group significantly decreased after the ADI exercise (p<.05). For the GM muscle activity, no significant difference was observed in all comparisons (p>.05). For the changes in muscle activities before and after the ADI exercise, a significant difference exists between the two groups only for the changes in ES activities (p<.05); WA group exhibits higher changes than the normal group. By contrast, no significant difference exists between the two groups for the changes in GM activities (p>.05). Conclusion: After the ADI exercise, the compensatory action of ES in the female adults with WC is implied to decrease during PHE.
Purpose : This study compared the cost-effectiveness ratio of physical therapy in health centers and home physical therapy, two physical therapy methods for home-bound stroke patients, and clarified the economic validity regarding the effect of home physical therapy. Methods : To measure and compare the cost and effectiveness of the two physical therapy methods for stroke patients, subjects were recruited based on in-hospital and home physical therapy. Among the entire data collected, 82 and 90 participants were selected for in-hospital and home physical therapy, respectively. To measure costs, regarding both in-hospital and home physical therapy, direct cost and indirect cost for patients, family, medical institutes, and the government were measured. In addition, activities of daily living were measured in both methods to measure their effectiveness. Through collected data, the cost-effectiveness and incremental cost-effectiveness ratios were analyzed. Results : Based on the analysis of cost-effectiveness, home physical therapy showed lower cost-effectiveness than in-hospital physical therapy. Furthermore, the incremental cost-effectiveness ratio also showed a difference, which implied home physical therapy could have high effectiveness compared to cost. Conclusion : Based on these results, home physical therapy could be considered as an alternativeto other methods of physical therapy, for home-bound stroke patients. In addition, the result of thisstudy contribute by providing evidence that home physical therapy offers economic benefits and canbe more effective in treating home-bound patients when policy decisions are made to establish a home physical therapy system.
Objectives : The purpose of this study was to compare differences in muscle activities according to distance changes, and success or failure in relation to approaches during a round of golf in order to obtain basic data on golf swings. Methods : To achieve our research goal, we asked eight professional golfers playing for the Korea Professional Golfers' Association (height: $1.76{\pm}0.05m$, weight: $73.87{\pm}9.21kg$, career duration: $12.87{\pm}4.48yr$) to perform approach swings at distances of 30, 50, and 70 m. Results : No differences were observed in the muscle activity of the extensor carpi radialis that were caused by the distance changes. In addition, we found that the wrist extensors seemed unaffected by the increase in approach distance. Also, we found that the powers of the approach shots were driven by efficient movements rather than by the strength of the arms. We confirmed that when the distance of the approach increased, the golfers should perform their back-swing tops and follow-through right from the right to the left pelvic limb. To achieve successful approach swings despite distance changes, golfers should first work on the activity of the erector spinae to prepare for rotatory power in the P1 section. Moreover, golfers should increase the activity of the erector spinae on the left when they need to deal with the distance improvements in the P2 and P3 sections. Conclusion : In the light of the discussion above, we may infer that despite approach distance changes during a round of golf, ideal swings can be realized by consistent activities of the wrist extensor muscles and improved performances of the pelvic limb muscles. Furthermore, this study suggests that golfers should improve the consistency of muscle activities in all the other body parts to achieve the ideal swing.
Purpose: The talk test (TT) is an alternative, self-reported method for prescribing and guiding exercise training in both healthy adults and patients with cardiovascular and pulmonary diseases. This study examined whether the TT is a valid tool for evaluating the exercise intensity during two different types of aerobic activity on a treadmill or stationary bicycle in a healthy population. Methods: A total of ten subjects (six males and four females) who had no medical history related to musculoskeletal, cardiovascular, and pulmonary disorders were enrolled in this study. They were evaluated using the TT, which consisted of three-level of difficulties demanding cardiac loads while performing aerobic activities on a treadmill and bicycle ergometer across two consecutive days in a counterbalanced manner. During the activities, the psychophysiological response markers were collected in terms of the heart rate, oxygen saturation, rating of perceived exertion, and metabolic equivalents. Results: Statistical analyses revealed a significant difference in the between-subject variance regarding the TT level effect (p<0.05). On the other hand, no significant findings were detected on the between-group variance(p>0.05) and the TT level×group interaction (p>0.05). The independent t-test indicated no significant differences in heart rate, oxygen saturation, rating of perceived exertion, and metabolic equivalents at any levels of the TT in the two groups. The TT showed a strong correlation with the rating of perceived exertion. Conclusion: This study showed that the TT is a valid and alternative tool for evaluating the aerobic exercise intensity in a healthy population. In addition, differences in the psychophysiological response markers between two aerobic activities, treadmill and bicycle ergometer, were detected in the same exercise intensity scaled with the TT. The TT can be used to evaluate and prescribe the exercise intensity of aerobic activity in cardiovascular and pulmonary physical therapy.

Background: Muscle weakness and impaired trunk muscle control are common in stroke patients. The bridging exercise (BE) is generally used for trunk stabilization and improving the overall function of stroke patients. The effectiveness of the BE with hip adductor contraction (BEHA) in facilitating trunk muscle activation has been well studied in healthy adults. However, the impact of BEHA in sub-acute stroke patients has not yet been investigated. Objects: The purpose of this study was to determine the effects of BEHA on the electromyography (EMG) activities and the asymmetry of the rectus abdominis (RA), external oblique (EO) and internal oblique (IO) abdominal muscles. Methods: Twenty participants with sub-acute stroke (11 males and 9 females) were recruited. Each participant was asked to perform bridging exercises for five seconds under three different conditions: BE in a neutral position (BEN), BEHA with a large ball (BEHAL) and BEHA with a small ball (BEHAS). The EMG amplitudes of the bilateral RA, EO and IO and the asymmetry of the EMG activity between the sound and affected sides were compared among the conditions. The significance level was set at ${\alpha}=.05$. Results: The EMG activities of RA, EO and IO were significantly greater during BEHAL and BEHAS than during BEN (p<.05); the asymmetry of the RA, EO and IO decreased significantly during BEHAL and BEHAS compared to BEN (p<.05). However, no measured variables showed any significant differences between BEHAL and BEHAS (p>.05). Conclusion: This study compared the EMG activities of the RA, EO and IO on both sides and the asymmetry of the RA, EO and IO during BEN, BEHAL and BEHAS. Our findings suggest that BEHA was more effective for individuals with hemiplegic stroke at facilitating and normalizing abdominal muscle control than BEN.
