싱가포르 보건의료체계의 약사 양성교육과 약사 직능에 대한 체계적 고찰 (A Systematic Review on the Vocational Pharmacy Education and Pharmacists' Role in the Singapore's Healthcare System)

  • Background: Singapore has the stable healthcare system with utilizing pharmacist manpower in proper positions by demand of populations' health among Asian countries. Objective: This study aims to systematically review (1) the pharmacists' role and (2) the pharmacy education system of Singapore in comparison with Korea. Method: We searched for information about academic, medical and governmental institutions related to professional pharmacists' practice in Singapore by primarily using database such as DBpia, KISS, Google Scholar and ProQuest and the official website of the Singapore Ministry of Health. We contacted and arranged the visit schedules with National University of Singapore, National Health Group's polyclinics, Agency for Integrated Care, National University Hospital, and community chain pharmacies. During onsite visits, we interviewed pharmacists working in each institution and obtained additional documents and materials relevant to this manuscript work. Results: To become a registered pharmacist in Singapore, the pharmacy curriculum requires four full-time academic years and six additional months allotted for pre-registration training. Pharm.D. course is offered for pharmacy graduate students with additional two full-time years of study. Team teaching and inter professional education program seem the most significant method in pharmacy education. Pharmacists working at hospitals, polyclinics, and community pharmacies in Singapore take broader roles and offer more cognitive services such as smoking cessation program and medication reconciliation. Especially, pharmacists in Agency for Integrated Care fill the role of primary care providers for the continuing care of the community through the governmental support toward the patients-centered integrated care. Conclusion: Singaporean pharmacists take significant and active roles in collaboration with other healthcare providers. Efforts such as interprofessional pharmacy education and governmental endorsement of the systematic and interactive care between pharmacists and other medical providers in Singapore are needed to be urgently applied to Korea healthcare system for the promotion of population health.

Ginsenoside Rh2 reduces depression in offspring of mice with maternal toxoplasma infection during pregnancy by inhibiting microglial activation via the HMGB1/TLR4/NF-κB signaling pathway

  • Background: Maternal Toxoplasma gondii (T. gondii) infection during pregnancy has been associated with various mental illnesses in the offspring. Ginsenoside Rh2 (GRh2) is a major bioactive compound obtained from ginseng that has an anti-T. gondii effect and attenuates microglial activation through toll-like receptor 4 (TLR4)/nuclear factor-kappa B (NF-κB) signaling pathway. GRh2 also alleviated tumor-associated or lipopolysaccharide-induced depression. However, the effects and potential mechanisms of GRh2 on depression-like behavior in mouse offspring caused by maternal T. gondii infection during pregnancy have not been investigated. Methods: We examined GRh2 effects on the depression-like behavior in mouse offspring, caused by maternal T. gondii infection during pregnancy, by measuring depression-like behaviors and assaying parameters at the neuronal and molecular level. Results: We showed that GRh2 significantly improved behavioral measures: sucrose consumption, forced swim time and tail suspended immobility time of their offspring. These corresponded with increased tissue concentrations of 5-hydroxytryptamine and dopamine, and attenuated indoleamine 2,3-dioxygenase or enhanced tyrosine hydroxylase expression in the prefrontal cortex. GRh2 ameliorated neuronal damage in the prefrontal cortex. Molecular docking results revealed that GRh2 binds strongly to both TLR4 and high mobility group box 1 (HMGB1). Conclusion: This study demonstrated that GRh2 ameliorated the depression-like behavior in mouse offspring of maternal T. gondii infection during pregnancy by attenuating the excessive activation of microglia and neuroinflammation through the HMGB1/TLR4/NF-κB signaling pathway. It suggests that GRh2 could be considered a potential therapy in preventing and treating psychiatric disorders in the offspring mice of mothers with prenatal exposure to T. gondii infection.

일개 약학대학 실무실습교육 후 학생의 실무교육기관 및 프리셉터 평가 (The Students' Evaluation of Practice Sites and Preceptors in Pharmacy Experiential Education)

  • Objective: This study was conducted to assess the current status of experiential education, by analyzing pharmacy students' satisfaction and evaluation for practice sites and preceptors for 3 years from 2014 through 2016. Methods: Students evaluated the practice sites and preceptors using a 5-point and a 4-point Likert scale, respectively. Analysis was performed based on the types of curriculums and practice sites. In addition, sub-analysis was conducted based on the types of health-system pharmacy and the community pharmacy. Results: Total 203 students responded to the survey questionnaires. The 3-year average score of students' satisfaction with practice sites was 4.25. In the sub-analysis conducted based on the types of health-system pharmacy, the score was higher in the tertiary hospital pharmacies than that of the general hospital pharmacies (4.49 vs. 4.06, P<0.001). For community pharmacy, the difference in the score of the 3-year analysis was not significant, depending on the types (4.51 vs. 4.33, P=0.054). The average score of students' evaluation on the preceptors was 3.37 in the 3-year analysis. The score was significantly higher for the preceptors of tertiary hospital pharmacies than those of general hospital pharmacies (3.61 vs. 3.25, P<0.001). For community pharmacy, no significant differences were found, depending on the types of pharmacy (3.53 vs. 3.43, P=0.309). Conclusion: The students' satisfaction with practice sites and preceptors in pharmacy experiential education was high. However, it varied greatly depending on the types of educational institutions. Mutual effort between university and educational institutions is required, to narrow the gap in the degree of students' satisfaction.

Effectiveness of Tobacco Education for Pharmacy Students in Indonesia

  • Background: Smoking remains the major preventable cause of death worldwide, especially cancer-related death. Evidence clearly indicates that tobacco-related morbidity and mortality is reduced by smoking cessation. Pharmacists are well-positioned to provide tobacco cessation services an involvement of pharmacists in smoking cessation is encouraged by several organizations. While Indonesia's prevalence of smoking is in the first rank in Asian countries, none of the pharmacy schools in Indonesia are currently offering tobacco-related courses in their existing curricula at present. Our study aimed to develop and to evaluate the effectiveness of tobacco education (TE) for pharmacy students in Indonesia. Materials and Methods: A 6-hour TE was developed and evaluated using pre-test/post-test with control group design. A total of 137 fifth-year pharmacy students at Gadjah Mada University (GMU), Yogyakarta, were chosen as an intervention group while a total of 105 fifth-year students of Islamic University of Indonesia, (UII) served as the control group. Knowledge, perceived-role, self-efficacy, and ability to perform counseling using the 5A's framework were evaluated. Results: A significant improvement (P < 0.001) in knowledge, perceived-role, and self-efficacy was found in the intervention group but not in the control group. In addition, we revealed that 89.7% of the intervention group were able to perform counseling using 5A's. Conclusions: The developed TE significantly improved student knowledge, perceived-rolse, self-efficacy, and created an ability to perform cessation counseling. Integration of TE education in curricula of Indonesian pharmacy schools nation-wide should be encouraged.

다직종 암환자 교육·상담에 대한 설문 개발과 환자 만족도 조사 (The Development of Multidisciplinary Cancer Patient Education·Counseling Questionnaire and Satisfaction Survey)

  • Background: Presently, a multidisciplinary team of doctors, pharmacists, nurses, and dietitians provides patient education to impart information on chemotherapy. However, studies on multidisciplinary education satisfaction are inadequate. In this study, we aimed to contribute to the improvement of quality of multidisciplinary education counseling for patients with cancer by developing a satisfaction questionnaire and analyzing the satisfaction survey. Methods: A questionnaire was developed by an expert group, and the responses were recorded using the 5-point Likert scale. After conducting a pre-test, factor analysis was performed to evaluate validity. The reliability of the questionnaire was measured by Cronbach's alpha coefficient. A satisfaction survey was conducted by self-administration method. Results: Based on the results of factor analysis, factors can be divided into two parts: "overall education" and "each team member's education" (total 14 questions). The construct validity and reliability of the questionnaire are sufficiently high. Fifty-one patients took the survey between January 2, 2018 and January 20, 2018. Twenty-six (51%) patients responded that they were "very satisfied" and 22 (43.1%) patients responded that they were "satisfied". Conclusion: By developing a questionnaire on multidisciplinary education counseling for patients with cancer, it is possible to perform evaluation and research of cancer patient education. This study will contribute to the management and improvement of quality of multidisciplinary education.

국내 임상약학 교과과정 현황 및 미국 교과과정과의 비교 (Examination of Clinical Pharmacy Curriculum in Korea and Its Comparison to the U.S. Curriculum)

  • Objective: The objective of this study was to evaluate the 6-year clinical pharmacy curriculum in Korea among 35 schools of pharmacy and to compare the pharmacy practice experience curriculum with the U.S. Methods: Data on the 6-year clinical pharmacy curriculum was collected and analyzed from 35 schools of pharmacy in Korea. Data were collected from each school's website, or through professors in clinical pharmacy or the administrative office, when not available online. Guidance for U.S. clinical pharmacy curriculum was referenced from the Accreditation Council for Pharmacy Education (ACPE) Accreditation Standards and Guidelines. Results: Pharmacotherapy was the only course that was offered in every school of pharmacy with average of $11.5{\pm}2.8$ credit hours offered. Only six subjects were offered in more than half of the schools. Average pharmacy practice experience credit hours in Korea were $1.8{\pm}0.6$, $7.8{\pm}1.5$, $4.9{\pm}1.2$, $3.5{\pm}1.1$, $11.8{\pm}1.2$ in introductory, hospital, community, pharmaceutical industry and administration, and intensified pharmacy practice experience, respectively. While the U.S. required introductory pharmacy practice experience (IPPE) to be conducted in the real pharmacy setting, the IPPE in Korea was conducted as an in-class simulation. The total required hours of IPPEs and APPEs were 1400 hours in Korea and 1740 (300+1440) hours in the U.S. Conclusion: Clinical pharmacy curriculum in Korea is offered through a variety of courses and the pharmacy practice experience curriculum has been adopted by every school of pharmacy. A guidance outlining the major required contents of clinical pharmacy curriculum could help standardize and advance the clinical pharmacy education in Korea.

약대 프리셉터 양성교육 후 프리셉터 역량의 중요도-수행도 분석연구 (Importance-Performance Analysis of Pharmacy Preceptors' Competencies after a Preceptor Education Program)

  • According to the changed '2+4'-year pharmacy school curriculum in Korea, pharmacy preceptors play a highly crucial role in educating student pharmacists through practice-based learning. Whether the new curriculum in a Korean pharmacy school will become firmly established or not depends on the competency of a preceptor. Before the preceptors with high competencies are anticipated, the effective education programs for them should be provided. However, there have been little studies regarding the competencies of pharmacy preceptors in Korea. This study was to analyze the degree of differences between importance and performance of preceptors' competencies. The self-reported questionnaire was utilized to collect data from pharmacists in the preceptor education program of a university. Total 122 pharmacists participated in the program, and 78 responded to the survey questionnaires. The average of importance for overall evaluation items was 4.32 whereas that of performance for them was 3.51 by using a 5-point Likert scale. The evaluation items with relatively marked differences between importance and performance were 'sufficient time allowance for students', 'teaching research results regarding newly approved drugs', 'clear understanding of goals and learning contents about pharmacy practice experiences', and 'plan for systemic experience educations'. Further follow-up researches on changes of the differences in importance- performance of preceptors' competencies after precepting student pharmacists should be implemented. Furthermore, the effective and appropriate methods for the preceptor education programs based on the follow-up studies should be developed to reduce the difference between importance and performance.

북한의 약사교육 시스템과 시험제도(국가면허) 연구 (Pharmacist Education and National License Examination System in North Korea)

  • Unity, understanding our pharmacist pharmacy education and training and their ecosystems rather then overwrite the system will get you ready to integrate and balance searching for contacts. Since 2015 we started the 6-year pharmacy education in the North were already carried out (the current 5.5 years). This chemist (or pharmacist) educate university education and vocational school and health officials made in the training school in jimyeo. Duration of each is to the University of 5.5 years, college three years, health officer training school two years and various types of training methods (weekly, communication, special) is applied is positive. License test system is also in favor of the state graduation test graduation test committee of professors in the university college diploma than the national notification system, and pharmacists 'qualifications' - are licensed is granted. The North Korean education system pharmacists and pharmacist review and analysis test system for the future reunification of Korea oriented education system pharmacists and pharmacists in health care personnel office systems through correct understanding and awareness-will be a useful resource integration plan designed to pharmacists.

약사직능수행 향상을 위한 약사국사시험 개선에 관한 연구 (Development of National Liscensing Examinaion for Improvement of Pharmacist)

  • To maximize the services of phatmacists, we studied the best national liscensing examination for improvement of pharmacist. Among 650 Pharmacist who are posed the questions, 282 pharmacist such as professor in college of pharmacy(46), pharmacists in drug store and hospital (136), administrators in The Ministry of The Health and Welfare(48) and researchers and phamacists in pharmaceutical company (52) replied it. By analyzing these questionares, the results were as follows: In the Korean national liscensing examination for medial care team, pharmacist has been examed over two times the subjects as compared with another national examinations for medical and public health liseneses and 6-year curriculum of medical school. Now, the national examination for pharmacy liscence have been tested the basic subjects such as pharmaceutical analytical chem-istry, medicinal chemistry, biochemistry, etc., but almost demander, pharmacist who has been engaged about over 80% in the drug stores and hospital pharmacy, want to have exam pharmacy practics which are clinical pharmacy, pathohysiology and pharma-cotherapy of diseases like The United State of America, but not basic chemisty and biology. If the Korean liscensing examination for pharmacy will be changed like America, pharmacy education in the college of phar-macy will be progressed in detail. There are no clerkships and internships in the pharmaceutical education program in Korea, and so most of pharmacist want to have 6-year curriculum for pharmacy education and to test the pharmacy practice for Doctor of Pharmacy(Pharm. D.) but not basic science.
