A Study on Home Healthcare Convergence for IEEE 11073 Standard (IEEE 11073 표준을 위한 홈 헬스케어 융합에 대한 연구)

  • Medical paradigm shift has been based on disease treatment into wellness care so that changes need more IT-based smart medical services. In addition, individual based smart devices are more focused on healthcare services and can provide access to personal medical information, health conditions and social welfare managed by users. In this paper, IEEE11073 PHD (Personal Health Devices) and HL7 (Health Level 7) standards of legacy healthcare devices are developed for communicating with each individual based smart device and providing healthcare service in smart TV environment through a unified home healthcare gateway.

  • In this paper, the complexity analysis for implementation of the PM-store is performed in terms of the number of instruction cycles which is executed by CPU in a personal health device(PHD) in order to transfer the large amount of the periodically generated measurement data using the PM-store concept defined in ISO/IEEE 11073 PHD standards. We propose an analytic model that is focused on the number of instruction cycles executed by CPU depending on the PM-store hierarchy.

  • As telecommunication technologies in telemedicine services are developed, the expeditious development of wireless and mobile networks has stimulated wide applications of mobile electronic healthcare systems. However, security is an essential system requirement since many patients have privacy concerns when it comes to releasing their personal information over the open wireless channels. Due to the invisible feature of mobile signals, hackers have easier access to hospital networks than wired network systems. This may result in several security incidents unless security protocols are well prepared. In this paper, we analyzed authentication and authorization procedures for healthcare system architecture to apply secure M-health systems in the hospital environment. From the analyses, we estimate optimal requirements as a countermeasure to its vulnerabilities.

  • Recently, research about ESS(Emergency Support System) has been actively carried out to provide a variety of medical services using smart devices and wearable devices. Smart healthcare provides a personalized health care service using various types of bio-signal measuring sensors and smart devices. For the smart healthcare using a smart device, it is need to research about personal health monitoring using a smart wearable devices, and also need to research on service methods for first aid measures after an emergency. In this paper, we proposed about group management based emergency support system, that is monitoring about personal bio signal using smart devices and wearable devices to protect patient's life. The system notices to the medical volunteers based on the position information when an emergency situation. In addition, we have designed and implemented an emergency support system providing the information of the patient on the display when transmitting a picture of a patient using a smart device to the server.

  • Several sensor devices have been developed for monitoring individual's health and status information and services which visualize customized information by associating applications running on smartphones with sensor devices also have been emerged. Though these applications provide similar information to a user, each sensor device has its own application caused by non-standardized packet formats. In this paper, we propose a sensor device identification for dynamic interworking between a smartphone and personal sensor devices. In the proposed scheme, we can use the same application which plays role of a client on the smartphone as changing sensor devices because server stores packet information of sensor devices.

  • E-health services have provided interoperability between personal health devices in personal area network, based the ISO/IEEE 11073 standard. In the healthcare system, the manager handles most agents concurrently through wireless communication. However, due to the distance limitation and the increased number of agents, it may be difficult to provide continuous connectivity. Recently, body area devices have been equipped with various applicable agents, which can even handle agents on behalf of the manager. A BAD may act as an intermediary device to increase system efficiency and performance. In this study, a device called "proxy", which can be installed as software on BAD devices, is proposed. The data measured by an agent can be sent to the proxy first, and subsequently be sent to the manager again. Agents and the manager are not aware of the proxy existence and work normally without the proxy. Furthermore, a new smart proxy and modified manager are proposed. The smart proxy acts as one agent handling measurement data from several agents, which can transmit a significant amount of data at once. The proxy and smart proxy maintain compatibility with existing devices that conform to the 20601 standard. The proposed schemes are verified and the complexities of devices are analyzed. The analysis shows no significant difference among the proxy, smart proxy, and manager. Simulations exhibit that the proposed schemes can improve the system performance.

Exploratory Study on Acceptance Intention of Mobile Devices and Applications for Healthcare Services (헬스케어 서비스를 위한 모바일 디바이스 및 어플리케이션 수용의도에 관한 탐색적 연구)

  • This study aims to examine the preferred device types, the difference of device properties by service types, and acceptance intention for applications and willingness to pay for healthcare services. To attain this goal, this research conducted a survey targeted to the subjects of those concerned in hospitals and medical institutions, the aged and patients with chronic disease, and consumers having interest in health maintenance. According to the result of the analysis, acceptance intention for mobile devices for healthcare services was high in general. And patients with chronic disease and those concerned in hospitals showed high acceptance intention. Smart phones were preferred the most in the support of healthcare services. And there was the difference of device properties by service types, and in health maintenance, easiness and playfulness were somewhat high. Acceptance intention for mobile applications was rather high in personal use, and regarding the properties, personal information protection was valued highly.

  • It is essential to guarantee a smooth communication and data exchange in a PHD(Personal Healthcare Device) network for applications providing U-health services. In spite of that, most of PHDs are heterogeneous, so the heterogeneity of their protocols makes it difficult to develop an integrated gateway sending sensed healthcare data to U-health service providers. To solve this problem, we suggest the design and implementation of a device adapter model based on dynamic managed module in this paper. Our model were implemented to work on the OSGi-based gateway middleware and to have interoperability in connection with the HL7 system that is the standard of the Healthcare Information systems. In addition, our model has an architecture supporting a communication based on the object serialization in order to provide extensibility in the functional aspect of applications. Through the experiment on a test-bed which is an implementation of the device adapter module for electrocardiogram and blood-pressure/blood-sugar device having one channel, we have confirmed the accuracy of sensing and sending data.

Development of Realtime ECG Analysis and Monitoring System (실시간 심전도 분석 및 모니터링 시스템 개발)

  • ECG is used on purpose to keep good health or monitor cardiac function of aged person as well as on purpose to diagnose the disease of heart patients. The ambulatory ECG monitoring system under guarantee of safety and accuracy is very efficient to prevent the progress of heart disease and sudden death. These systems can detect the temporary change of ECG that is very significant to diagnose heart disease such as myocardial ischemia, arrhyamia and cardiac infarction. In this paper, we describe the ECG signal analysis algorithm and measurement device for ECG monitoring. The authors designed a small-size portable ECG device that consisted of instrumentation amplifier, micro-controller, filter and RF module. The device measures ECG with four electrodes on the body and detects QRS complex and ST level change in realtime. Also it transmits the measured signals to the personal computer. The developed software for ECG analysis in personal computer has the function to detect the feature points and ST level changes.

  • 최근 급성 질환으로 인한 사망률은 꾸준히 증가하고 있다. 이러한 급성 질환은 초기 증상 발생 시 올바른 인지와 신속한 대처가 요구된다. 그러나 유지 관리비용 면에서 모든 개인의 응급상황을 관리할 수 있는 의료시스템은 구축하기 어렵다. 본 논문에서는 언급한 문제점을 해결하기 위해 웨어러블 디바이스와 개인건강기록 시스템을 제안한다. 웨어러블 디바이스에서 측정한 심박 체온의 생체신호로 응급 상황을 판별해 지정된 보호자에게 알린다. 또한, 응급버튼을 통해 곧바로 응급상황을 알린다. 개인이나 가족과 관련된 건강정보를 관리할 수 있는 개인건강기록(Personal Health Record)을 제공한다. 본 시스템을 통해 사용자의 응급상황에 신속하게 대처하여 생명을 보호할 수 있을 것으로 기대한다.
