Post-traumatic pseudoaneurysms of internal maxillary artery are rare, but may be life-threatening. When arterial damage leads to pseudoaneurysm formation, delayed intractable epistaxis can occur. We report our experience with the diagnosis and management of a ruptured internal maxillary arterial pseudoaneurysm that was discovered preoperatively in a patient with a zygomaticomaxillary complex (ZMC) fracture. He presented to the emergency room with epistaxis, which ceased shortly, and sinus hemorrhage was observed with a fracture of the posterior maxillary wall. The patient was scheduled for open reduction and internal fixation (ORIF) of the ZMC fracture. However, immediately before surgery, uncontrolled epistaxis of unknown origin was observed. Angiography indicated a pseudoaneurysm of the posterior superior alveolar artery. Selective endovascular embolization was performed, and hemostasis was achieved. After radiologic intervention, ORIF was successfully implemented without complications. Our case shows that in patients with a posterior maxillary wall fracture, there is a risk of uncontrolled bleeding in the perioperative period that could be caused by pseudoaneurysms, which should be considered even in the absence of typical symptoms.
The three hundred and sixty nine patients who underwent either isolated or concomitant coronary artery bypass graft surgery since May, 1977 till December, 1993 at the Yonsei University Cardiovascular center were studied with respects to the incidence of operative risk factors, surgical methodology and consequent results. The patients were classified into two periods, according to the time of the surgery in relation to the date of the opening of the Yonsei cardiovascular center. Period I[1977 to 1990 , consisting of the patients who underwent surgery prior to the opening date, harboured a total of 189 patients with the mean age of 55 years, and the second, Period II[1991 to 1993 , those who underwent after the opening, of 180 patients with the mean age of 60 years. The Period II patients were involved in more operative risk factors, compared to the ones in Period I. The anatomy of the coronary arteries of the patients of Period II were more likely to have multilesional and left main disease. The patients in Period I were older, had more prominent left ventricular dysfunction and were more likely to be exposed to the risk factors. The number of implanted grafts were greater period II[average of 2.5 grafts per patient in Period I VS 3.2 in Period II and the frequency which the used left internal mammary artery was also significantly higher in Period II[49 and 104 cases in Period I and Period II . The incidence of perioperative myocardial infarction was 20 patients[10.6% in Period I, 14 patients[7.8% in period II. And the operative mortality was 20 patients[10.6% in period I, 8 patients[4.4% in period II. In conclusion we think that the operative results have improved in Period II, compared to that of Period I, in spite of the higher risks, due to accumulation of surgical experiences, improved surgical techniques and myocardial protection, specialized teamwork, application of the intraoperative TEE and appropriate pharmacological interventions by anesthesiologist.
이 연구는 관상동맥질환 수술기주위의 결과예측에 있어 수술 전 백혈구 및 D-dimer 수치가 유용한 인자가 될 수 있는지를 규명하기 위해 회고적으로 실시하였다. 수술 전 백혈구 수는 수술 전 및 후의 심장표지자인 troponin-I, creatine kinase-MB, C-반응단백 각각의 농도와 정의 상관성이 있었다. 수술 전 D-dimer 농도 역시 수술기주위의 심장표지자들과 정의 상관관계를 보였다. 수술 전 백혈구 수는 간표지자인 aspartate aminotransferase 및 alanine aminotransferase의 수술 전후 농도와 정의 상관관계를 보였는데 비해, 수술 전 D-dimer 농도는 수술 전후 빌리루빈 농도, 수술 전 글루코스 농도, 수술 후 크레아티닌 농도와 각각 정 또는 역의 상관관계가 있었다. 수술 전 백혈구 및 D-dimer 수치 둘 다 수술 전 고밀도콜레스테롤 농도 및 좌심실구출계수와 각각 역의 상관관계가 있었다. 수술전 백혈구 수는 수술시간 및 수술 후 기계보조 호흡시간과 각 정의 상관관계를 보였고, 수술 전 D-dimer 농도는 수술 후 기계보조 호흡시간, 중환자실 치료기간, 재원기간과 각 정의 상관관계가 있었다. 이 회고적 조사의 결과는 수술 전 백혈구 및 D-dimer 수치가 관상동맥수술 환자들의 수술기주위 결과예측에 있어 임상적으로 유용한 인자들이 될 수 있음을 시사하고 있다.
Forty-eight patients with significant left main coronary artery obstruction underwent coronary artery bypass graft between September,1989 and September,1993. They consisted of 29 males and 19 females with the mean age of 58 [range 33 - 78 . Seventeen patients[35% had stenosis in left main coronary artery only and twenty-nine[61% had associated lesions in other coronary artery systems as well.Two[4% had isolated left coronary ostial lesion. The operative mortality was 4.17% [2/48 which is comparable to the mortality of overall coronary artery bypass surgery cases during the same period[p >0.05 . Perioperative myocardial infarction was occurred in two patients[4.17% . And the infarction rates between the two groups also showed no difference[p >0.05 . The forty-six survivors showed significant improvement in exercise tolerance as well as symptomatology. We conclude that surgery for left main coronary artery disease can be done with no increased risk through careful anesthesia,surgery, and aggressive postoperative care in the critical period.
To elucidate a potential contribution of endotheline-1[ET-1] to the genesis of pulmonary hypertension and postoperative pulmonary hypertensive crisis in the patients with heart disease, we measured plasma levels of the ET-1 during perioperative period of open heart surgery. In addition, we examined changes of ET-1 during perioperative period and correlations between ET-1 levels and hemodynamic variables. 12 patients including 5 acquired heart disease and 7 congenital heart disease patients were selected randomly as a study group, Group A and B, respectively. 6 patients proved not having heart or hemodynamic problem were selected as a control, Group C. 110 blood samples from pulmonary artery[ET-P] and radial artery[ET-S] were taken and assayed by Sep-pak extraction and RIA. ET-1 levels of Group A were ET-P, 3.94$\pm$5.31pg /ml, ET-S, 3.10$\pm$2.90pg/ml[p>0.05], Group B were ET-P, 1.63$\pm$0.62pg/ml, ET-S, 1.99$\pm$2.45pg/ml[p>0.05], Group C were ET-P, 1.97$\pm$2.02pg/ml, ET-S, 1.72$\pm$0.77pg/ml[p>0.05]. There were no statistically significant differences of ET-1 levels among the Group A, B, C[p>0.05]. There was no correlation between pulmonary artery pressure[PAP] and ET-1 level[p>0.05], and ET-1 levels were not increased even in the cases of pulmonary hypertensive criwis or low cardiac output syndrome, whereas significant correlation between ET-S and pulmonary vascular res-istance[Rp] [r=0.36, p<0.05], and negative correlation between ET-S and OS saturation of pulmonary artery[OS-P][r= -0.49, p<0.01] were identified. Another significant finding was peak increase of ET-1 levels in the postoperative period 1 hour[p<0.05] and then gra-dualy decrease through the postoperative period. In conclusion, ET-1 has no correlation with PAP, whereas correlation with Rp, and inverse correlation with OS-P. It is suggested that ET-1 is neither the direct causative substance of pulmonary hypertension nor pulmonary vasospasm but there must be increased production of ET-1 in chronic pulmonary hypertensive state. Counter-regulatory mechanism to ET-1 is speculated during the pulmonary vasospasm.
Few topics in operative and perioperative patient management generate more controversy than that of appropriate fluid and electrolyte therapy. especially, controversy has swirled around colloid vs crystalloid therapy and the composition of administered fluids, agreement among clinicians as to what fluid therapy is appropriate, and in what amount, is rare. This controversy likely will be enhanced by Arieff' s provocative article. He described 11 adults and 2 pediatric patients. All developed fatal postoperative pulmonary edema, seemingly caused solely by excessive postoperative fluid administration. From January 1999 to December 1999, we investigated 24 patients, which were operated by orthognathic surgery, about the intraoperative fluid therapy and the associated effect in orthognathic surgery, which is regarded as one of the major surgery of oral and maxillofacial surgery. First, They were devided into two groups, that is one-jaw surgery and two-jaw surgery, and each groups were devided by intraoperative fluid volume of 8ml/kg/hr. Subjective assesment was collected through use of a series of 3 questionnaries. In each questionnaire, a 5-point Liekert scale was used far assessment of following parameters of recovery from anesthesia: headache, dizziness, drowsiness, nausea/vomiting, thirst. The patient completed questionnaire 1 at 4 hour after surgery, questionnaire 2 was completed at 24 hours after surgery, and questionnaire 3 was completed at 48 hours after surgery. This study demonstrated that appropriated perioperative rehydration decreases postoperative adverse outcomes and improved the patient's perception of the postoperative period.

Background: We examined perioperative predictors of sustained sinus rhythm (SR) in patients undergoing the Cox maze operation and concomitant cardiac surgery for structural heart disease. Materials and Methods: From October 1999 to December 2008, 90 patients with atrial fibrillation (AF) underwent the Cox maze operation and other concomitant cardiac surgery. Eighty-nine patients, all except for one postoperative death, were followed-up with serial electrocardiographic studies, 24-hour Holter monitoring tests, and regular echocardiographic studies. Results: Eighty-nine patients undergoing the maze operation were divided into two groups according to the presence of SR. At the time of last follow-up (mean follow-up period, $51.0{\pm}30.8$ months), 79 patients (88.8%) showed SR (SR group) and 10 patients (11.2%) had recurrent AF (AF group). Factors predictive of sustained SR were the immediate postoperative conversion to SR (odds ratio, 97.2; p=0.001) and the presence of SR at the 6th month postoperatively (odds ratio, 155.7; p=0.002). Duration of AF, mitral valve surgery, number of valves undergoing surgery, left atrial dimension, and perioperative left ventricular dimensions and ejection fractions were not predictors of postoperative maintenance of SR. Conclusion: Immediate postoperative SR conversion and the presence of SR at the 6th postoperative month were independent predictors of sustained SR after the maze operation.
The purpose of this study was to elucidate the effects of musical listening on anxiety in patients undergoing abdominal hysterectomy. On the basis of the research criterias, 46 patients were recruited from Kang-Nam St. Mary's Hospital in Seoul from October 1 to December 31, 1995. 20 patients of them were participated as the experimental group who received musical listening session with self-selected music tapes, while 26 patients as the control group who didn't receive that session. The musical listening sessions composed of 3 periods, the pre-operation evening, just before sleeping, the operation morning. There were no differences between two groups, in terms of age education, religion, the experience of operation, the perception of uterus, and the weight(%). The State-Trait Anxiety Inventory(STAI) was used to measure anxiety on all patients. And serum cortisol levels, fasting blood sugar levels, systolic and diastolic blood pressure were collected at the day before surgery and the operation day. The results were as follows : 1. There were no significant differences in State-Anxiety between the two groups. 2. The differences of serum cortisol level between the day before surgery and the operative day were significantly lower in experimental group than in control group(P=0.03). 3. The level of fasting blood sugar was significantly decreased in experimental group(P=0.01). 4. Systolic blood pressure level was significantly decreased in experimental group(P=0.02). While Diastolic blood pressure level was not significant between two groups. It is suggested imply that the musical listening during the perioperative period may alleviate the perioperative anxiety levels in patients undergoing abdominal hysterectomy.
In the field of orofacial surgery, a red blood cell transfusion (RBCT) is occasionally required during double jaw and oral cancer surgery. However, the question remains whether the effect of RBCT during the perioperative period is beneficial or harmful. The answer to this question remains challenging. In the field of orofacial surgery, transfusion is performed for the purpose of oxygen transfer to hypoxic tissues and plasma volume expansion when there is bleeding. However, there are various risks, such as infectious complications (viral and bacterial), transfusion-related acute lung injury, ABO and non-ABO associated hemolytic transfusion reactions, febrile non-hemolytic transfusion reactions, transfusion associated graft-versus-host disease, transfusion associated circulatory overload, and hypersensitivity transfusion reaction including anaphylaxis and transfusion-related immune-modulation. Many studies and guidelines have suggested RBCT is considered when hemoglobin levels recorded are 7 g/dL for general patients and 8-9 g/dL for patients with cardiovascular disease or hemodynamically unstable patients. However, RBCT is occasionally an essential treatment during surgeries and it is often required in emergency cases. We need to comprehensively consider postoperative bleeding, different clinical situations, the level of intra- and postoperative patient monitoring, and various problems that may arise from a transfusion, in the perspective of patient safety. Since orofacial surgery has an especially high risk of bleeding due to the complex structures involved and the extensive vascular distribution, measures to prevent bleeding should be taken and the conditions for a transfusion should be optimized and appropriate in order to promote patient safety.
Background: We aimed to evaluate the incidence, predictive factors, and impact of acute kidney injury (AKI) after thoracic endovascular aortic repair (TEVAR). Methods: A total of 53 patients who underwent 57 TEVAR operations between 2008 and 2015 were reviewed for the incidence of AKI as defined by the RIFLE (risk, injury, failure, loss, and end-stage kidney disease risk) consensus criteria. The estimated glomerular filtration rate was determined in the perioperative period. Comorbidities and postoperative outcomes were retrospectively reviewed. Results: Underlying aortic pathologies included 21 degenerative aortic aneurysms, 20 blunt traumatic aortic injuries, six type B aortic dissections, five type B intramural hematomas, three endoleaks and two miscellaneous diseases. The mean age of the patients was $61.2{\pm}17.5years$ (range, 15 to 85 years). AKI was identified in 13 (22.8%) of 57 patients. There was an association of preoperative stroke and postoperative paraparesis and paraplegia with AKI. The average intensive care unit (ICU) stay in patients with AKI was significantly longer than in patients without AKI (5.3 vs. 12.7 days, p=0.017). The 30-day mortality rate in patients with AKI was significantly higher than patients without AKI (23.1% vs. 4.5%, p=0.038); however, AKI did not impact long-term survival. Conclusion: Preoperative stroke and postoperative paraparesis and paraplegia were identified as predictors for AKI. Patients with AKI experienced longer average ICU stays and greater 30-day mortality than those without AKI. Perioperative identification of high-risk patients, as well as nephroprotective strategies to reduce the incidence of AKI, should be considered as important aspects of a successful TEVAR procedure.
