The Ross Procedure in Pediatric Patients: A 20-Year Experience of Ross Procedure in a Single Institution

  • Background: The Ross/Ross-Konno procedure is considered a good option for irreparable aortic valve disease in pediatric patients because of its hemodynamic performance and potential for growth of the pulmonary autograft. This study is a review of the long-term results of our 20-year experience with the Ross and Ross-Konno operations in a single institution. Methods: Between June 1995 and January 2016, 16 consecutive patients (mean age, $6.0{\pm}5.9years$; range, 16 days to 17.4 years) underwent either a Ross operation (n=9) or a Ross-Konno operation (n=7). The study included 12 males and 4 females, with a median follow-up period of 47 months (range, 6 to 256 months). Results: There were no cases of in-hospital or late mortality. Six reoperations were performed in 5 patients. Four patients underwent right ventricular-pulmonary artery (RV-PA) conduit replacement. Two patients underwent concomitant replacement of the pulmonary autograft and RV-PA conduit 10 years and 8 years after the Ross operation, respectively. The rate of freedom from adverse outcomes of the pulmonary autograft was 88% and 70% at 5 and 10 years, respectively. The rate of freedom from valve-related reoperations was 79% and 63% at 5 and 10 years, respectively. Conclusion: Pulmonary autografts demonstrated good durability with low mortality. The Ross/Ross-Konno procedure is a good option that can be performed safely in pediatric patients with aortic valve disease, even in a small-volume center.

Development of Durable Reliability Assessment Methods for Heavy Duty Coatings

  • Heavy duty coating are required to have minimum durable period of 15 years under average usage environment because these paints are coated with purpose of anti-corrosion, antifouling, plastering etc. Onto steel structures constructed upon land and sea and other ferrous structures of electric power generation plants, electricity transmission towers, large structures of various plants, etc. Therefore we tried to estimate heavy duty coating longevity through reliability evaluation method and used combined cyclic anti-conrrosion test method composed of drying, moisturizing and salt spray as for accelerated life test to estimate longevity. Accelerated life test hours to heavy duty coating of first grade (with longevity not less than 15 years) specification may be obtained from troubleless test hours $t_n=\frac{B_p}{n^{1/\beta}}\left[\frac{1n(1-CL)}{1n(1-p)} \right]^{1/\beta}=19.671$ (yr) where shape parameter $\beta=1.1$, confidence level CL=80 %, warranty life $B_{10}=15$ yr and sampling size n=10 (2 sets). Because acceleration factor {AF} found by accelerated test is 41.7, accelerated life test hours required may be represented about 4,132 hr so that if this amount of hours is converted to number of cycles(6 hr/cycle) of complex cycle corrosion resistance test then the amount is tantamount about 690 cycles. That means if there does not occur trouble failure (with defect factor sum not more than 20) during when there is performed 690 cycles of combined cyclic anti-corrosion test to heavy duty coating specimen then it signifies that there can be warranted longevity $B_{10}$ of 15 yr under condition of confidence level CL=80 %.

이종심조직판막기능부전에 대한 외과적 요법 (Surgical experiences of tissue valve failure)

  • Results of reoperation for tissue valve failure were presented with review of current knowledge. Through January 1986, 19 patients required reoperation: 18 had undergone mitral, 2 aortic, and 1 tricuspid valve. Primary tissue failure was the main cause of tissue valve failure: it occurred in 18 valves [15 mitral, 2 aortic, and 1 tricuspid] at a mean postoperative interval of 54-25 months [range 29-120]. During the same period, 2 patients required reoperation for prosthetic valve endocarditis, 1 for paravalvular leakage. The types of valves failed were 12 lonescu-Shiley valves, 5 Hancock valves, and 1 Carpentier-Edwards valve. All 6 patients younger than 15 years of age at first operation had been implanted with lonescu-Shiley valves and failed after a mean interval of 35 months. In contrast, 11 patients older than 15 years had been implanted with 5 Hancock, 6 lonescu-Shiley, and 1 Carpentier-Edwards valve initially, and eventually failed after mean intervals of 81, 53, 47 months each other. The kinds of valves used at reoperation were 8 lonescu-Shiley, 4 Bjork-Shiley, and 6 St. Jude Medical valves for primary tissue failure cases and 3 lonescu-Shiley valves for the other 3 cases. Overall mortality at reoperation was 10%: 5.6% for primary tissue failure, 50% for prosthetic valve endocarditis. In 15 cases [all mitral] primary tissue failure were caused by calcification associated with or without leaflet destruction or fibrous ingrowth, and in 2 cases [all aortic] caused by cusp perforation and tearing without any evidence of calcification. In conclusion: 1 primary tissue failure is the main cause of reoperation in patients with tissue valve failure; 2. the majority of the failed valves is in mitral position; 3. leaflet calcification is the leading pathology of primary tissue failure; 4, reoperation for tissue valve failure may be a major concern, although the mortality is low; 5. the limited durability of tissue valve suggests its use be restricted to selected cases.

Bileaflet Mechanical Valve의 임상적 고찰 (Clinical Analysis of Bileaflet Mechanical Valve Replacement)

  • Experience with bileaflet mechanical valve replacement at the Inha Hospital in 192 patients, operated on from June 1986 until April 1993. Two hundred fourty-one prostheses [51 Duromedics, 79 St.Jude Medical, and 111 CafboMedics]were implanted during the total 195 operations. Mitral valve replacment[MVR]was done in 113 cases, aortic valve replacement[AVR]in 34, tricuspid valve replacement[TVR]in 2, and double valve replacement[DVR]in 46 cases.Of the total patients, 63.0% were women and 37.0% were men. The mean age of the patients was 40.8 years, ranged from 14 to 67years. Overall early mortality was 9.2\ulcorner%[18 out of 195]; 9.7%[11 out of 113]for MVR, 14,7% [5 out of 34]for AVR, and 4.3%[2 out of 46]for DVR. All of the operative survors were followed over a period of one to 83 months with a mean of 37 months, for total 543 patient-years. So far, eleven patients[6.7% of the long-term survivors]were lost to follov-up after a mean postoperative follow-up of 22.8 months. There were nine late deaths; three deaths due to prostetic valve endocarditis, two due to persistent heart failure, one due to cerebral hemorrhage, one due to aortic dissection after Bentall oreration, and two sudden deaths. Actuarial survival rate at 6.9 years was 94.8%, There were seventeen valve-related complications; three prosthetic valve thromboses, three thrombembolisms, three instances of prosthetic valve endocarditis, two paravalvular leakages, and six hemorrhagic complications related to anticoagulation. The actuarial rate of freedom from all valve-related complications at 6.9years was 91.3%. There were significant decreases in the heart size postoperatively that can be demonstrated by comparison of cardio-thoracic ratios on simple chest X-ray and left ventricle dimensions on echocardiography. We conclude that this midterm follow-up shows good results in terms of hemodynamics and durability although further long-term evaluations are mandatory.

도막의 기계적 성질과 포수방법이 단청의 내후성에 미치는 영향 (Influence of Mechanical Properties of Painting Layers and Priming Methods to Weathering Resistance of Danchung)

  • 전통목조건축물의 단청에서 발생하는 도막의 박리현상을 개선시키기 위해, 목재의 친수성으로 인해 목재와 도막간의 팽윤수축률의 차이에 의해 발생되는 응력이 도막의 내후성에 미치는 영향을, 도막의 기계적 성질(인장성질과 응력완화)과 교착제(아교, 아크릴에멀젼(Primal AC-3444), 아크릴수지 (Paraloid B-72))에 의한 포수방법으로부터 평가하였다. 아크릴에멀젼 도막은, 아크릴에멀젼의 유리전이온도가 상온 이하$(7^{\circ}C)$로써 외력에 대한 응력완화가 큰 성질을 가지고 있기 때문에, 열화촉진실험과 옥외폭로실험에서 박리가 발생하지 않았으며, 다른 교착제를 사용한 도막에 비해 내후성이 가장 뛰어났다. 또 목재에 교착제를 저농도에서 고농도까지 중복칠하는 포수방법이 다른 포수방법들보다 도막의 내후성을 향상시키는 효과가 있었다.

인공심장판막의 개발 및 동물실 (Development and Animal Tests of Artificial Heart Valves)

  • A heart supplies bloods of about 15, 000 liters to each human organ in a day. A normal function of heart valves is necessary to this act of heart. The disease of heart valve develops to a narrowness of a closure, resulting in an abnormal circulation of bloods. In an attempt to eliminate the affliction of heart valves, the operation method to repair with artificial heart valves has been developed and saved numerous patients over past 30 years. This replacement operation has been performed since early 1960`s in Korea, but all the artificial heart valves used are imported from abroad with very high costs until recent years. The artificial heart valve using pyrolytic carbon has been developed at KAIST, which was proved to be stable in the mechanical performance and durability. Therefore, the in viva performance of this valve was examined through animal tests. The artificial heart valves used in this study are tilting disc type valves, in which the disc were made of graphite coated with pyrolytic carbon and the cages were made of titanium. In viva testings of these valves were performed in 12 dogs, in which right ventriculo-pulmonary arterial [Croup I] or inter-aortic [Croup IV] valved conduit was implanted using polytetrafluoroethylene conduits containing KAIST valve and aortic valve [Group II] or pulmonary valve [Croup III] was replaced by a KAIST valve with a 21mm or 19mm tissue annulus diameter. In group I and II, pre-and post-operative transvalvular pressure gradient was measured and compared with other prosthetic valves. During post operative period laboratory examination was performed including hemoglobin, hematocrit, red cell count, white cell, lactic acid dehydrogenase and platelet. The eight surviving dogs were sacrificed and autopsy was performed at 2, 6, and 8 weeks. KAIST valve has low transvalvular gradient and relatively high orifice area. Average ventriculo-aortic peak systolic transvalvular gradient was 14 mmHg in 21 mm valve and 19 mmHg in 19 mm valve. The valve has slight intravascular hemolysis effect. Thrombogenic effect of low polishing quality and eddy currents around small orifice is high. The valve has vulnerability of disc movement. These animal tests suggest that the improvement of the heart valve design, surface polishing state and prescription methods.

복합열화촉진실험 및 장기현장폭로실험에 의한 RC구조물 보수공법의 보수성능평가 (Performance Evaluation of Repair Methods for RC structures by Accelerating Test in Combined Deterioration Chamber and Long-Term Field Exposure Test)

  • At present, the selecting system and analytic estimation criterion on repair materials and methods of the deteriorated RC structures have not yet been set up in domestic. Under these circumstances, deterioration such as shrinkage crack, corrosion of rebar has been often occurred after repair, and this finally results in too frequent repairs. In this study, three types of repair methods were experimentally investigated by the accelerating test in a combined deterioration chamber and long-term field exposure test. Three types of repair methods applied in this study belong to a group of polymer cement mortar, which is commonly used in repair works. According to the results of this study, durability of repair mortar layers and corrosion properties of recovered rebar could be investigated in short period by the accelerating test in a combined deterioration chamber, which can simulate the condition of repeated high-and-low temperature and repeated dry-and-wet environment, spraying chloride solution and emitting $CO_2$ gas. After 36 month long-term filed exposure test in the coastal area, harmful macro-cracks are observed in the polymer cement mortar layer of some repair methods. These crack are considered to result from drying shrinkage of polymer cement mortar. Also, after 36 month exposure, amount of corrosion area and weight loss of rebar are found to be different according to the types of repair methods.

  • In-Wheel System은 고성능의 전기모터를 Wheel에 직접 장착하여 파워트레인 요소를 모두 제거함으로써 차량시스템의 효율을 높인 시스템이다. 추후 친환경 차량에 적용할 수 있는 신개념의 플랫폼을 제공할 수 있는 고효율, 고성능 차량 시스템이기에 다양한 분야에 적용되고 있다. 이러한 시스템의 급증은 우리 생활의 안전과 직결되므로 개발단계에서 부품의 신뢰성을 확인하고 인증하여야 한다. 신뢰성이란 "부품이나 시스템이 주어진 환경에서 고장 없이 일정기간 동안 요구기능을 수행하는 특성"이다. 따라서 본 논문에서는 In-Wheel System의 핵심 부품인 In-Wheel Motor를 신뢰성 평가 중 하나인 내구 수명에 대한 검증 방안을 In-Wheel Motor의 운전 조건으로부터 가감속 토크 및 실효토크를 계산해서 이를 바탕으로 가속수명시험을 위한 속도 패턴을 제안하여 시험시간을 단축하고, 또한 In-Wheel Motor의 수명 신뢰성을 검증한다.

열화억제형 방수공법에 대한 검토 (A Study of the Deterioration Restraining Agent Drainage method of Construction)

  • 철도교량의 경우 교면방수 시공 후 방진매트, 궤도자갈 및 침목부설, 레일 설치 등의 후속공정이 뒤따르고, 시공 및 경제적인 여건으로 인해 위의 공정이 연속적으로 시행되지 못하고 일정 구간 단위로 후속공정이 이루어지는 특수성을 갖고 있다. 이로 인해 교면방수 시공 이후 표면의 노출이 장기화 되는 것이 불가피한 실정으로, 시간 경과에 따른 열화현상으로 인해 발수재의 분해 및 용출이 발생하여 방수시공 초기의 침투깊이를 유지하지 못하고 있는 경우가 많다. 이처럼, 철도교량이 갖고 있는 시공적인 특수성과 궤도자갈이 부설되지 않고 영구적으로 표면이 노출되는 부분을 고려한다면 표면의 장기적인 노출에도 방수성능이 저하되지 않는 내구성을 갖고 있는 방수공법을 선택하는 것이 무엇보다 중요하다고 할 수 있다. 본 연구의 목적은 장기간 표면노출에 따른 열화현상을 방지하는 열화억제형 방수공법(DRA방수공법)에 대하여 시험시공을 통한 객관적인 분석 결과를 도출하고, DRA 방수 공법의 KNOW-HOW를 습득하는 것으로, 이를 통하여 철도교량의 교면방수 열화현상으로 인해 발생하는 하자를 방지하여 잦은 보수와 보강으로 인한 경제적 손실을 최소화하고 현장에서의 시공관계자들이 방수공법을 선정하는데 따른 합리적인 판단 기준을 제공하는데 있다.

현장 그라운드 앵커 장기거동 분석을 통한 잔존긴장력 평가 (Evaluation of Residual Tensile Load of Field Ground Anchors Based on Long-Term Measurement)

  • 비탈면 보강 용도로 사용되는 영구앵커의 경우 사용기간 동안 지지력 및 내구성이 확보되어야 한다. 그러나 최근 국내·외 연구 결과에 따르면 앵커의 장기거동에 따라 긴장재 파단, 정착구 파손, 비탈면 변형 및 손상, 시간에 따른 잔존긴장력 감소 등의 현상들이 보고되고 있다. 이러한 문제는 앞으로 증가할 것으로 보이며, 이로 인한 유지보수 비용의 증가, 시설물 붕괴 사고 등의 문제가 불가피할 것으로 보인다. 이에 본 연구에서는 그라운드 앵커의 유지관리 절차 및 방법을 현실적으로 보완하기 위해 문헌연구를 수행하여 국내 유지관리 기법의 문제점 및 한계를 제시하였다. 이후 실제 현장 그라운드 앵커에 설치되어 있는 하중계 계측자료를 분석하여 앵커들의 경과일에 따른 잔존긴장력의 변화를 파악하였고, 최종 분석 결과를 통해 앵커의 시공조건, 구성암반 등이 긴장력의 증감에 미치는 영향을 파악하였다.
