Comparison of the Formula of PSA, Age, Prostate Volume and Race Versus PSA Density and the Detection of Primary Malignant Circulating Prostate Cells in Predicting a Positive Initial Prostate Biopsy in Chilean Men with Suspicion of Prostate Cancer

  • Background: Combining risk factors for prostate cancer into a predictive tool may improve the detection of prostate cancer while decreasing the number of benign biopsies. We compare one such tool, age multiplied by prostate volume divided by total serum PSA (PSA-AV) with PSA density and detection of primary malignant circulating prostate cells (CPCs) in a Chilean prostate cancer screening program. The objectives were not only to determine the predictive values of each, but to determine the number of clinically significant cancers that would have been detected or missed. Materials and Methods: A prospective study was conducted of all men undergoing 12 core ultrasound guided prostate biopsy for suspicion of cancer attending the Hospital DIPRECA and Hospital de Carabineros de Chile. Total serum PSA was registered, prostate volumecalculated at the moment of biopsy, and an 8ml blood simple taken immediately before the biopsy procedure. Mononuclear cells were obtained from the blood simple using differential gel centrifugation and CPCs identified using immunocytchemistry with anti-PSA and anti-P504S. Biopsy results were classed as positive or negative for cancer and if positive the Gleason score, number of positive cores and percent infiltration recorded. Results: A total of 664 men participated, of whom 234 (35.2%) had cancer detected. They were older, had higher mean PSA, PSA density and lower PSA-AV. Detection of CPCs had high predictive score, sensitivity, sensibility and positive and negative predictive values, PSA-AV was not significantly different from PSA density in this population. The use of CPC detection avoided more biopsies and missed fewer significant cancers.Conclusions: In this screening population the use of CPC detection predicted the presence of clinically significant prostate cancer better than the other parameters. The high negative predictive value would allow men CPC negative to avoid biopsy but remain in follow up. The formula PSA-AV did not add to the predictive performance using PSA density.

Effect of Different Spray Dried Plasmas on Growth, Ileal Digestibility, Nutrient Deposition, Immunity and Health of Early-Weaned Pigs Challenged with E. coli K88

  • A total of 96 piglets were weaned at 19 and 13 days in Exp. 1 and 2, respectively, and allotted to one of four diets: three with different spray dried plasmas (SPs) and one with hydrolysed casein (HC). SPs were from pigs (SPP), mixed origin (SMP), and mixed origin with standardized level of immunoglobulins (SMPIG). All the diets contained 1.7% total lysine, 25% of the test protein source, 45% corn starch, 15% lactose, 2% sucrose, 7% soybean oil. At d 4 and d 2 in Exp. 1 and 2, respectively, piglets were perorally challenged with $10^{10}$ CFU E. coli K88. Growth performance, immunity, and health condition were measured for 15 days and 14 days in Exp. 1 and 2, respectively. To investigate apparent ileal digestibility and nutrient deposition, all piglets were sacrificed at d 14 in Exp. 2. In 1. 3 piglets died in HC diet and 1 in SPP diet. HC diet showed higher mortality (p<0.01) than other diets. In Exp. 2, no clinical sign of infection was detected, no difference for the content of E. coli K88 was found in feces at 4 and 6 days after the infection, and no E. coli K88 was found in the jejunum at the end of experiment. In both experiments, feed intake was lower for HC diet and ADG was 96, 106, 122 and 155 for HC, SPP, SMP and SMPIG diet, respectively (HC vs others, p<0.05; SMPIG vs other SP, p<0.01). Heal apparent digestibility of nitrogen in sacrificed piglets was higher for HC diet (p<0.05). After the challenge, K88-specific titers in saliva (Exp. 1) and in plasma (Exp. 2) were reduced in SMP and SMPIG. The piglets positive to the adhesion of the used E. coli strain to the intestinal brush borders had a significantly reduced growth (p<0.01) and a higher K88-specific IgA titer in plasma, in comparison with negative ones. This effect was independent of the diet. The data show the relevance of spray dried plasma sources and particularly of SP with standardized level of immunoglobulins for the feeding of early-weaned at the risk of infection by enterotoxigenic bacteria.

Aflatoxin Contamination of Red Chili Pepper From Bolivia and Peru, Countries with High Gallbladder Cancer Incidence Rates

  • Chilean red chili peppers contaminated with aflatoxins were reported in a previous study. If the development of gallbladder cancer (GBC) in Chile is associated with a high level of consumption of aflatoxin-contaminated red chili peppers, such peppers from other countries having a high GBC incidence rate may also be contaminated with aflatoxins. We aimed to determine whether this might be the case for red chili peppers from Bolivia and Peru. A total of 7 samples (3 from Bolivia, 4 from Peru) and 3 controls (2 from China, 1 from Japan) were evaluated. Aflatoxins were extracted with acetonitrile:water (9:1, v/v) and eluted through an immuno-affinity column. The concentrations of aflatoxins B1, B2, G1, and G2 were measured using high-performance liquid chromatography (HPLC), and then the detected aflatoxins were identified using HPLC-mass spectrometry. In some but not all of the samples from Bolivia and Peru, aflatoxin B1 or aflatoxins B1 and B2 were detected. In particular, aflatoxin B1 or total aflatoxin concentrations in a Bolivian samples were above the maximum levels for aflatoxins in spices proposed by the European Commission. Red chili peppers from Bolivia and Peru consumed by populations having high GBC incidence rates would appear to be contaminated with aflatoxins. These data suggest the possibility that a high level of consumption of aflatoxin-contaminated red chili peppers is related to the development of GBC, and the association between the two should be confirmed by a case-control study.

Lack of an Apparent Association between Mycotoxin Concentrations in Red Chili Peppers and Incidence of Gallbladder Cancer in India : an Ecological Study

  • Our recent studies conducted in South America have shown that mycotoxin contamination of red chili peppers (RCPs) may be associated with an increased risk of gallbladder cancer (GBC). Whether this relationship exists in India, a country with a high incidence of GBC and high consumption of RCPs, is unclear. We therefore measured concentrations of aflatoxins (AFs) and ochratoxin A (OTA) in RCPs from areas of low, medium, and high incidence of GBC in India, and compared these concentrations with GBC incidence in each area. Twenty-one RCP samples were collected from nine cities (eight from a low-incidence area, five from a medium-incidence area, and eight from a high-incidence area). Concentrations of AFs and OTA were measured using high-performance liquid chromatography. No significant differences in mean concentrations of AFs and OTA were found in the three areas. AFB1 levels in the low-incidence area ($10.81{\mu}g/kg$) and high-incidence area ($12.00{\mu}g/kg$) were more than 2.2 and 2.4 times higher compared with the maximum permitted level of AFB1 in spices ($5.0{\mu}g/kg$) set by the Commission of the European Communities, or that ($4.4{\mu}g/kg$) obtained in our previous study in Chile. Our results show that the mean concentrations of mycotoxins in RCPs are similar among the three areas in India with different incidences of GBC. Further studies with human subjects are needed to evaluate any association between AFB1 and GBC.

Development of Water Hammer Simulation Model for Safety Assessment of Hydroelectric Power Plant (수력발전설비의 안전도 평가를 위한 수충격 해석 모형 개발)

  • Sustainable growth of hydroelectric power plants is expected in consideration of climate change and energy security. However, hydroelectric power plants always have a risk of water hammer damage, and safety assurance is very important. The water hammer phenomenon commonly occurs during operations such as rapid opening and closing of the valves and pump/turbine shutdown in pipe systems, which is more common in cases of emergency shutdown. In this study, a computational numerical model was developed using the MOC-FDM scheme to reflect the mechanism of water hammer occurrence. The proposed model was implemented in boundary conditions such as reservoir, pipeline, valve, and pump/turbine conditions and then applied to simulate hypothetical case studies. The analysis results of the model were verified using the analysis results at the main points of the pipe systems. The model produced reasonably good performance and was validated by comparison with the results of the SIMSEN package model. The model could be used as an efficient tool for the safety assessment of hydroelectric power plants based on accurate prediction of transient behavior in the operation of hydropower facilities.

A Latent Class Analysis and Predictors of Chronic Diseases -Based on 2014 Korea National Health and Nutrition Examination Survey- (만성질환에 관한 잠재계층분석과 예측요인 -2014 국민건강영양조사를 중심으로-)

  • The aim of this study was to investigate the latent classes and predictors of chronic diseases such as hypertension, dyslipidemia, arthritis, thyroid disease, depression, atopy, allergy, and diabetes. The subjects of this study were Korean citizens who participated in the Korea National Health and Nutrition Examination Survey in 2014. Stratified cluster sampling method was used with a sample size of 7,550. Latent hierarchy analysis was applied to this data. Four classes were identified. Class 1 consisted of participants with hypertension and diabetes. Class 2 consisted of participants with atopy and allergies. Class 3 consisted of participants with dyslipidemia, arthritis, thyroid disease, and depression. Class 4 consisted of participants without any chronic diseases. In comparing Class 1 to Class 4, age, physical activity, self-management, obesity, and presence of high cholesterol were found to be significant. In comparing Class 2 to Class 4, gender, age, and education level were significant. When Class 3 was compared to Class 4, gender, age, pain and discomfort, as well as high cholesterol were found to be significant. Diabetes and hypertension should be treated as comorbid conditions, applying integrated treatments involving effective drug treatment, diet, and physical activity programs. Atopy was found to be strongly correlated with allergies. Thyroid disease was found to coexist with dyslipidemia and arthritis, along with having a strong correlation to depression. Age-appropriate preventive measures can help reduce the risk of chronic diseases.

The Effects of Job Stress and Mental Health of Care Service Workers on Self-diagnosed Oral Symptom in Seoul (서울시 돌봄 서비스 노동자의 직무스트레스와 정신건강이 주관적 구강 증상에 미치는 영향)

  • This study examined the effect of job stress and mental health factors on the oral symptoms of caring service workers in Seoul. For the research method, a survey was conducted from July 24 to August 6, targeting 150 caring service workers. The survey items were job stress, mental health (physical symptoms, anxiety, and insomnia), and self-diagnosed oral symptoms. The results of the study were as follows. Job stress (p=0.001) had a significant effect on the oral symptoms. In addition, physical symptoms (p<0.001), anxiety symptoms (p<0.001), and insomnia symptoms (p<0.001) all had significant effects on the oral symptoms. Among the risk factors, physical symptoms had the greatest influence on oral symptoms. In addition, according to the correlation result of job stress, anxiety symptoms, insomnia symptoms, oral symptoms, as job stress increases, physical symptoms (p<0.001), anxiety symptoms (p<0.001), insomnia symptoms (p<0.001), oral symptoms (p<0.01) showed an increasing positive correlation and a statistically significant difference. Physical and psychological labor power is greatly exhausted by care service workers performing tasks that require physical and mental care for those who are being cared for. Therefore, there is a need for systematic expansion measures for the mental health of workers and the introduction of customized oral health education programs to improve oral health because improved health and oral health conditions can increase work performance.

Historical Review for the Government Contractor Defense (Government Contractor Defense(정부계약자항변)에 대한 연혁적 고찰)

  • A significant rise in product-liability cost is expected due to the newly passed product liability amendment Bill approved during the assembly plenary session on March 30, 2017. Korean government legal service(KGLS) filed a damage suit against Korea aerospace industries, Ltd.(KAI) and Hanwha Techwin Co., Ltd., the manufactures of the KUH-1 Surion helicopter crashed. KGLS alleged claims under the product liability Act, the warrant liability Act and the non-performance of contract act. The accountability limits of military aircraft manufacturers was a highly divisive issue among related scholars and legal practitioners. The bottom line was that military aircraft manufacturers had no product-liability insurance available. The United States courts have, therefore, developed the government contractor defense(GCD) and it was recognized by the U.S. Supreme Court in Boyle v. United Technologies corporation(1988). product liability insurances for military aircraft manufacturers are excessively expensive and it cannot be added onto the military procurement cost accounting. However, having an aircraft accident without one can be ruinously expensive. Therefore, the manufacturers should promptly set up appropriate risk management measures. This thesis will first review the advance GCD theory, and then find a way to either reform government contract related regulations.

A Study on the Development of a Preventive Index Based on the Statistical Data of Ship Accidents (사고 통계기반 선박사고 예방지수 개발에 관한 연구)

  • Despite the facts that navigation technology and educational programs were developed, and that maritime law has been enforced strictly, the number of maritime accidents has not dropped significantly. Accident prevention is emphasized because of the extreme life, property, and environmental losses that accompany maritime accidents. Even though there are a variety of preventive activities, their performance is not satisfactory. In order to promote preventive activities and to organize safety management, a Preventive Index of Ship Accident(PISA) and a Preventive Level of Ship Accident(PLSA) which are abstracted from the statistical data of ship accidents (2009~2013) are proposed. The survey module by which these variables can be calculated is composed of preventive domains such as education, engineering, enforcement and information, and is applied for the preventive indexes measurement of cargo, passenger, tanker, tugboat and fishery ships. We compared and analyzed measured result with statistical accident rates and cases to confirm the index utilities. It was found that there is some correlation between the index value and the ship accident rate. It is expected that this index will become useful to evaluate a preventive status for the prevention of ship accidents and for the maritime safety culture.

First Clinical Experience about RapidArc Treatment with Prostate Cancer in Ajou University Hospital (아주대학교병원에서의 전립선암에 대한 래피드아크 치료)

  • In this study, the patient with localized prostate cancer who had previously been treated at Ajou University Hospital was randomly selected since March, 2009. we performed IMRT and 2RA plans and the same dose objectives were used for CTVs, PTVs, rectum, bladder, and femoral head of the respective plans. Arc optimizations and dose calculations were performed using Eclipse versions 8.6. In this paper, we evaluated the performance of IMRT and RA plans to investigate the clinical effect of RA for prostate cancer case. In our comparison of treatment techniques, RA was found to be superior to IMRT being better dose conformity of target volume. As for the rectum and bladder, RA was better than IMRT at decreasing the volume irradiated. RA has the ability to avoid critical organs selectively through applied same dose constraints while maximally treating the target dose. Therefore, this result suggests that there should be less rectal toxicity with RA compared with IMRT, with no compromise in tumor margin. These findings, which show more favorable rectal, bladder, and femoral head DVHs with RA, imply that should not result in excess risk of toxicity when this technique is used. Many experiences with RA have shown not only dosimetric advantage, but also improved clinical toxicity when comparing with IMRT. The main drawbacks of RA are the more complex and time-consuming treatment planning process and the need for more exact physics quality assurance (QA).
