경피적 척추 성형술 후 조기에 발생한 새로운 척추 압박골절 (Early Onset Subsequent Vertebral Compression Fracture after Percutaneous Verteroplasty)

  • 목적: 골다공증성 척추 압박 골절에 대한 경피적 척추 성형술 후 조기에 발생하였던 새로운 연속 골절의 특성과 위험인자에 대하여 알아보고자 하였다. 대상 및 방법: 2013년 1월부터 2015년 12월까지 골다공증성 척추 압박 골절로 경피적 척추 성형술을 시행받은 환자 중 인접 추체에 새로운 골절이 발생한 44예를 대상으로 하였으며 골절 발생 시기에 따라 2군으로 나누었다. 3개월 내에 골절이 발생한 경우가 22예(A군)였고 3개월 이후에 발생한 경우가 22예(B군)였다. 두 군 간의 나이, 성별, 골밀도, 체질량 지수, 기존 척추 골절의 존재, 초기 골절 위치, 추간판 내 시멘트 누출, 시멘트 주입량, 추체의 높이 회복 정도, 후만각 교정 정도의 차이에 대하여 후향적으로 비교 분석하였다. 결과: 두 군 간의 나이, 성별, 골밀도, 체질량 지수, 기존 척추 골절의 존재, 초기 골절 위치, 추간판 내 시멘트 누출, 후만각 교정 정도에는 유의한 차이는 없었다. 시멘트 주입량이 많은 경우와 추체 높이 회복 정도가 큰 경우에서 3개월 내에 골절이 발생한 빈도가 유의하게 높았다. 결론: 경피적 척추 성형술 시 시멘트 양과 추체 높이 회복 정도는 3개월 이내에 발생하는 조기 인접 척추 골절의 위험인자로 판단되며 추시 과정에서 주의가 필요할 것으로 판단된다.

Chronic Spinal Epidural Hematoma Related to Kummell's Disease

  • Chronic spinal epidural hematoma related to Kummell's disease is extremely rare. An 82-year-old woman who had been managed conservatively for seven weeks with the diagnosis of a multi-level osteoporotic compression fracture was transferred to our institute. Lumbar spine magnetic resonance images revealed vertebral body collapse with the formation of a cavitary lesion at L1, and a chronic spinal epidural hematoma extending from L1 to L3. Because of intractable back pain, a percutaneous vertebroplasty was performed. The pain improved dramatically and follow-up magnetic resonance imaging obtained three days after the procedure showed a nearly complete resolution of the hematoma. Here, we present the rare case of a chronic spinal epidural hematoma associated with Kummell's disease and discuss the possible mechanism.

The Retrial of Percutaneous Vertebroplasty for the Treatment of Vertebral Compression Fracture

  • Objective : For the treatment of osteoporotic vertebral compression fracture, percutaneous vertebroplasty (PVP) is currently widely used as an effective and relatively safe procedure. However, some patients do not experience pain relief after PVP. We performed several additional PVP procedures in those patients who did not have any improvement of pain after their initial PVP and we obtained good results. Our purpose is to demonstrate the effective results of an additional PVP procedure at the same previously treated level. Methods : We reviewed the medical records and the radiologic data of the PVP procedures that were performed at our hospital from November 2005 to May 2008 to determine the patients who had undergone additional PVP. We identified ten patients and we measured the clinical outcomes according to the visual analogue scale (VAS) score and the radiologic parameters, including the anterior body height and the kyphotic angulation. Results : The mean volume of polymethylmethacrylate injected into each vertebrae was 4.3 mL (range: 2-8 mL). The mean VAS score was reduced from 8 to 2.32. The anterior body height was increased from 1.7 cm to 2.32 cm. The kyphotic angulation was restored from 10.14 degrees to 2.32 degrees. There were no complications noted. Conclusion : The clinical and radiologic outcomes suggest that additional PVP is effective for relieving pain and restoring the vertebral body in patients who have unrelieved pain after their initial PVP. Our study demonstrates that additional PVP performed at the previously-treated vertebral levels could provide therapeutic benefit.

골다공증성 척추체 압박골절에 대한 경피적 척추성형술시 자기공명영상과 골 주사 검사의 의의 (The Value of Preoperative MRI and Bone Scan in Percutaneous Vertebroplasty for Osteoporotic Vertebral Compression Fractures)

  • Objective : Percutaneous vertebroplasty is often complicated by the presence of multiple fractures or non-localizing pain in the patients with osteoporotic vertebral fractures. The purpose of this study is to estimate the value of preoperative radiologic studies in the localization of symptomatic vertebrae and to determine the factors which can influence on the clinical results. Materials and Methods : We retrospectively reviewed the clinical and radiologic data of 57 vertebrae in 30 patients underwent percutaneous vertebroplasty for osteoporotic vertebral compression fractures. Inclusion criteria was severe pain(McGill-Melzack score 3, 4 or 5) associated with the acute vertebral fractures and absence of spinal nerve root or cord compression sign. Acute symptomatic vertebral fracture was determined by the presence of signal change on MR images or increased uptake on whole body bone scan. Results : Pain improvement was obtained immediately in all patients and favorable result was sustained in 26 patients(86.7%) during the mean follow-up duration of 4.7 months(5 complete pain relief, 21 marked pain relief). Those who underwent vertebroplasty for all acute symptomatic vertebrae had significantly better clinical result than those who did not. Further vertebral collapse and eventual bursting fracture occurred in 1 vertebra which showed intradiskal leakage of bone cement and disruption of cortical endplate on postoperative CT scan. Conclusion : Preoperative MR imaging and whole body bone scan are very useful in determining the symptomatic vertebrae, especially in the patients with multiple osteoporotic vertebral fractures. To obtain favorable clinical result, the careful radiologic evaluation as well as clinical assessment is required. Control of PMMA volume seems to be the most critical point for avoiding complications.

Percutaneous Vertebroplasty Following Postural Reduction in Unstable Vertebra Plana; Is it a Contraindication?

  • Objective : Severe vertebral body collapse [vertebra plana] is considered a contraindication to vertebroplasty by most authors. The purpose of this study is to determine the efficacy of vertebroplasty in treating severe compression fracture patients with osteoporosis. Methods : 16 patients underwent 18 vertebroplasties following postural reduction for vertebra plana. The fractures were defined vertebrae that have collapsed to more than 75% of their original height. Imaging and clinical features were analyzed, including involved vertebrae level, vertebral height after postural reduction for 2 days, injected cement volume, clinical outcome and complications. Results : Involved veretebra were located from level T7 to L4. Vertebral body collapse averaged 79% [range $12{\sim}25%$] of the original height. After pillow reduction for 2 days, vertebral body height increased 35% of the original height [range $15{\sim}45%$]. The kyphotic wedge was $12^{\circ}$ before procedure and was decreased $7.0^{\circ}$ after vertebroplasty. The mean injected cement volume was 3.8ml [range $2.0{\sim}4.9ml$]. After the procedure, surgical outcome was excellent in 8 [50%] of 16 patients, good in 7 [42%] and unchanged in one [8%]. The mean pain score [VAS score] prior to vertebroplasty was 8.3 and it changed 3.2 after the procure. Cement leakage to the adjacent disc [5 cases] and paravertebral soft tissues [4 cases] developed but there were no major complications. Conclusion : We propose that vertebra plana due to osteoporosis is not a contraindication to vertebroplasty. Vertebroplasty following postural reduction for severe compression fracture is safe and effective treatment.

Vertebroplasty for the Treatment of Compression Fractures in the Upper and Middle Thoracic Spine

  • Background: Vertebroplasty that is performed in the upper and middle thoracic spine presents technical challenges that are different from those in the lower thoracic or lumbar region due to the small pedicle size and angular severity for thoracic kyphosis. We report the results of percutaneous vertebroplasty and review its effectiveness in treating intractable osteoporotic compression fractures in the upper and middle thoracic spine. Methods: Patients who underwent vertebroplasty due to painful osteoporotic compression fractures at T3 T8 were retrospectively analyzed. The compression rate, volume of injected cement, clinical outcome (VAS score) and complications were analyzed. Results: Forty-three vertebral bodies from 41 patients (32 females and 9 males, age from 64 to 78 years old) underwent vertebroplasty. The mean compression rate improved from 35% to 17%. Bipedicular injections of bone cement were performed at 3 levels of 2 patients, and unipedicular injections were performed in 40 levels of 39 patients. The mean VAS score prior to surgery was 7.7, which improved to 2.4 within 48 hours after surgery, and the mean VAS score after 6 months was 1.5, which was significantly lower. All patients recovered uneventfully, and the neurological examination revealed no deficits. Cement leakage to the adjacent disc (9 levels) and paravertebral soft tissues (10 levels) developed. However, there were no significant complications related to the procedure such as a pneumothorax or pulmonary embolism. Conclusions: Transpedicular vertebroplasty is a safe and effective treatment for the upper and middle thoracic regions, and has a low complication rate.

Effects of Percutaneous Sacroplasty on Pain and Mobility in Sacral Insufficiency Fracture

  • Objective : Sacral insufficiency fracture (SIF) contributes to severe low back pain. Prolonged immobilization resulting from SIF can cause significant complications in the elderly. Sacroplasty, a treatment similar to vertebroplasty, has recently been introduced for providing pain relief in SIF. The purpose of this study is to investigate the clinical short-term effects of percutaneous sacroplasty on pain and mobility in SIF. Methods : This study is conducted prospectively with data collection. Sixteen patients (3 men and 13 women) with a mean age of 77.5 years (58 to 91) underwent sacroplasty. Patients reported visual analogue scale (VAS; 0-10) and Oswestry disability index (ODI; 0-100%) scores. VAS and ODI scores were collected preoperatively and again at one day, one month, and three months postoperatively. Questionnaires measuring six activities of daily living (ADLs) including ambulating, performing housework, dressing, bathing, transferring from chair, and transferring from bed were collected. Ability to perform ADLs were reported preoperatively and again at three months postoperatively. Results : The mean preoperative VAS score ($mean{\pm}SD$) of $7.5{\pm}0.8$ was significantly reduced to $4.1{\pm}1.6$, $3.3{\pm}1.0$, and $3.2{\pm}1.2$ postoperatively at one day, one month, and three months, respectively (p<0.01). The mean ODI score (%) also significantly improved from $59{\pm}14$ preoperatively to $15.5{\pm}8.2$ postoperatively at one month and $14.8{\pm}8.8$ at three months (p<0.01). All ADL scores significantly improved at three months postoperatively (p<0.01). Conclusion : Percutaneous sacroplasty alleviates pain quickly and improves mobility and quality of life in patients treated for SIF.

척추성형술을 위한 의료 영상 시스템의 설계 및 개발 (Design and Implementation of A Medical Image Guided System for Vertebroplasty)

  • 척추에 대한 외과수술은 신경조직을 손상하지 않아야 하기 때문에 수술 절차가 신중하게 계획되고 고도의 정밀을 요구한다. 척추성형술(verte-broplasty)은 골다공증으로 인한 척추의 골절을 치료하기 위해 제안된 비교적 새로운 외과적인 수술방법으로 최소침습적인 수술 기법으로 피부를 통하여 골절된 척추부위에 구멍을 내고 그곳에 PMMA(polymethylmethacrylate)라 불리는 골시멘트(bone cement)를 주입하여 치료하는 방법이다. 최근의 연구결과, 수술 후 즉각적인 고통의 경감을 가져오며, 골다공증으로 인한 골절부위에 강도가 강한 골시멘트를 주입하게 됨으로써 골다공증 척추체에 구조적인 안정성을 제공해 주는 것으로 알려져 있다. 수술 후 발생하는 합병증의 대부분은 과도한 골시멘트의 주입으로 인한 것으로 알려져 있으며, 따라서 척추성형술의 치료에 가장 중요한 요인은 주입한 골시멘트의 양을 조절하는 것이다. 이에 본 연구에서는 환자의 컴퓨터단층촬영(computer tomography)을 통하여 획득한 영상을 토대로 척추성형술 시술시 필요한 의료 영상 시스템을 설계 및 개발하고, 텍스처 분석을 통하여 개개인의 환자에게 필요한 골시멘트의 양을 계산하는 알고리즘을 개발하여 이를 실제 환자의 시술시에 적용해 보았다. 결과적으로 본 시스템은 척추성형술시에 최적의 골시멘트의 양을 예측할 수 있었으며, 정형외과의 다른 부위의 수술시에도 적용가능성을 확인할 수 있었다.

159명의 골다공증성 흉추 및 요추부 골다공증성 추체 압박골절 환자에 대한 경피적 척추성형술 후 치료결과 (Percutaneous Polymethylmethacrylate Vertebroplasty in the Treatment of Osteoporotic Thoracic and Lumbar Vertebral Body Compression Fractures : Outcome of 159 Patients)

  • Purpose : To assess therapeutic effects of percutaneous polymethylmethacrylate(PMMA) vertebroplasty on the pain caused by osteoporotic thoracic and lumbar vertebral body compression fractures in a large scale of a prospective clinical design, and to determine clinical factors influencing its therapeutic effects. Methods : A prospective clinical study was carried out in 349 vertebral levels of 159 patients between April 1998 and July 1999. The compression fractures were confirmed with bone scan and spine CT, and bone marrow density was measured. Visual analogue scale(VAS) score was used for pre- and post-operative assessments of the pain. All 159 patients were assessed immediately after surgery, and 140 patients of them were followed-up for about 6 months in average. Results : Partial and complete pain relief was sustained immediately after operation in 73%, through follow-up period in 88% of the patients. Pain relief was not proportional to the amount of PMMA or the rate of increase in the height of the compressed vertebral body. It appears that 3 to 6cc of PMMA was proper enough to sustain pain relief. Better clinical improvement was achieved in the patients treated within 6 months after occurrence of vertebral body fracture. The most frequent surgical complication was epidural leakage of PMMA, and the most serious complication was extravertebral leakage into the paravertebral muscles, which appeared to exert the worst influence on the outcome. However, surgery was not required in these patients. Conclusion : Therapeutic effects of PMMA percutaneous vertebroplasty on osteoporotic vertebral body compression fractures were confirmed in a relatively large scale of prospective clinical study. It appears that good outcome can be achieved in patients treated within 6 months after fracture, treated each level with 3 to 6cc of PMMA in amount. without serious complications.

경피적 척추성형 후 발생한 우심방 내 골 시멘트 색전의 도관경유제거술: 시술 중 두 동강나 우폐동맥으로 이동한 색전 (Transcatheter Removal of Bone Cement Embolism in the Right Atrium after Percutaneous Vertebroplasty: The Embolus Broke in Half and Migrated to the Right Pulmonary Artery Intraoperatively)

  • 골 시멘트 색전은 경피적 척추성형 후 드물지 않게 발생하는 합병증이다. 이는 보통 작은 폐동맥 색전이고 대부분 무증상이기 때문에 치료를 필요로 하지 않는다. 하지만 심장 내에 생긴 골시멘트 색전은 매우 드물며, 치명적인 결과를 야기할 수 있다. 이전 보고된 증례들은 심장 내 골시멘트 색전을 주로 개심수술을 통해 제거하였으며, 중재시술로 제거한 것은 3개 증례만 보고되어 있었다. 따라서 저자들은 경피적 척추성형 후 발생한 우심방 내 6 cm 크기의 골 시멘트 색전을 도관경유로 안전하게 제거한 1예를 보고하고자 한다. 색전이 시술 중에 두 동강나 우폐동맥으로 이동하였으나 두 개의 스네어와 하나의 필터 제거 장치를 이용하여 서로 반대 방향에서 접근시켜 우폐동맥 내의 색전 조각들을 조심스럽게 잡아 끌어낸 후 쉬스와 일렬로 만들어 제거하였으며, 시술 후 합병증은 발생하지 않았다.
