A Study on Participation in Clinical Decision Making by Home Healthcare Nurses (가정전문간호사의 임상적 의사결정 참여도에 관한 연구)

  • Purpose: This study was done to identify participation by home healthcare nurses in clinical decision making and factors influencing clinical decision making. Methods: A descriptive survey was used to collect data from 68 home healthcare nurses in 22 hospital-based home healthcare services in Korea. To investigate participation, the researcher developed 3 scenarios through interviews with 5 home healthcare nurses. A self-report questionnaire composed of tools for characteristics, factors of clinical decision making, and participation was used. Results: Participation was relatively high, but significantly lower in the design phase (F=3.51, p=.032). Competency in clinical decision making (r=.45, p<.001), perception of the decision maker role (r=.47, p<.001), and perception of the utility of clinical practice guidelines (r=.25, p=.043) were significantly correlated with participation. Competency in clinical decision making (Odds ratio [OR]=41.79, p=.007) and perception of the decision maker role (OR=15.09, p=.007) were significant factors predicting participation in clinical decision making by home healthcare nurses. Conclusion: In order to encourage participation in clinical decision making, education programs should be provided to home healthcare nurses. Official clinical practice guidelines should be used to support home healthcare nurses’ participation in clinical decision making in cases where they can identify and solve the patient health problems.

A Study for Perception and Preference on Flexible Working Pattern of Nurses in Hospital (병원간호사의 탄력적 근무형태적용에 대한 인식 및 선호하는 근무형태 조사)

  • Purpose: The Purpose of this study was to identify for perception and preference on Flexible working pattern of Nurses in hospital. Method: The subjects were 260 nurses enrolled in 11 hospitals located in S and P city. Questionnaire method was utilized for data collection. Data was analyzed by SPSS statistical program. Results: 86.5% of the nurses were working 3 shifts. 70.8% of nurses agreed that they decided to give up nurses because of very irregular working pattern. They want to work 40hrs/weeks, and agree to applying to Flexible working pattern. There was statistically significant between career(F=2.839, p=.039), working place(F=3.086, p=.047), the mind of change of occupation(F=.479, p=.002) and expected life pattern change after applicating for Flexible working pattern. Conclusion: We need to Various Flexible working pattern model. As well as we need to public information about nurses' Flexible working pattern.

A Correlation Study on Nurses′Subjective Perception and Their Effectiveness (간호원의 주관적 지각과 직무효율성간의 상관연구)

  • The present study in the view of humanistic psychology was attempted to seek the purpose of study, the relationship between nurses′working efficiency that measured by an observational working efficiency scale and their subjective perception. "Perceptual orientation scale"(Choy, Chung Hoon 1971) was administered to 100 nurses during the Period of May to July 1972, and the relationship with their ratings by their supervising head nurses. Educational background, ages, and their lengths of caller were compared with their subjective perceptions. All the relationship were calculated by the method of chi-squares and correlational coefficients. The results of this study are summarized as follow: 1. There is no significant relationship between nurses′subjective perceptions and their working ratings. 2. There is significant relationship between nurses′subjective perceptions and their educational backgrounds. 3. There is no significant relationship between nurses′subjective perceptions and their lengths of career. 4. There is no significant relationship between nurses′subjective perceptions and their ages. The results of the present study are interpreted to the sampled research group of this study and are not to be extended to any other groups.

Attitude toward Health behaviors in Student Nurses (간호대학생의 건강행위에 대한 태도 조사연구)

  • Health perception is a very important issue for student nurses to be future health professionals. The concept of health has changed with the emphasis of 'health promotion', and thus nurses now have a demanded role as a health promoter. According to health promotion theories, health-promoting behaviors are affected by cognitive variables, and human behaviors are related to thoughts and perception. This study was conducted to recognize the relationships of attitude toward health behaviors to other health related variables. The data was collected using a self administered survey. The instrument used in this study was attitude of health behavior scales that were developed by Okayama medical school. The subjects were 512 student nurses recruited from 1 college and 3 universities. The data was analyzed using SPSS pc program with mean, correlation. and multiple regression technique. The results were as follows: 1. The mean score of attitude of health behaviors of the student nurses was 8.11, showing a relatively high level. A high score of health belief was reported in 'self-responsible type' with the score of 4.80, and a high score of 'think for health or disease' was reported in passive type with the score of 3.12. 2. Specifically, there were significant positive correlations between the level of 'attitude of health behaviors' and other health related variables such as health perception, think for health or disease, and age. 3. Stepwise multiple regression analysis revealed that active thought, leisure, self-responsible perception and depression accounted for 13%of variance of attitude toward health behaviors. In conclusion, although student nurses certainly perceived the performance of health behavior, they need to learn in more systematical way in order to be a better health promoter, one of the role of nurses. The results of the study suggest that further studies need to be done on attitude toward health behaviors and performance of health behavior by student nurses.

The Perception Degree of Teaching Effectiveness of Nurses and Nursing Students on Clinical Nursing Practicum (임상실습에 대한 간호사와 간호학생의 교수효율성 인지정도)

  • Purpose: This study was conducted to use as a preliminary data to improve the quality of clinical nursing education by comparing the perception of teaching effectiveness in nurses and nursing students. Method: The participants were 165 nurses and 289 nursing students. Data was collected from June 2 to 16, 2008. The research instruments used an effective clinical instructors which was developed by Reeve(1994). Result: The perception of teaching effectiveness in nurses and nursing students were 3.45 and 3.19 respectively and it showed that nurses' perception of teaching effectiveness are higher than that of nursing students. Nurses had strong points in the factors of "interpersonal relationship/ communication", "availability as a supporter", "teaching method and evaluation".In additions, the teaching effectiveness had significant differences to age, years of work, position, educational background and job satisfaction in nurses, otherwise, sex, differences of educational level, satisfaction of clinical practicum and major in students. Conclusion: It is needed a further study as below: to develop an education system which is to improve an interpersonal relationship and to upgrade the quality of educator; to develop a teaching method program of clinical nursing education to improve teaching effectiveness.

The Difference of the Nurses' Perception on the Importance and Performance of Rehabilitation Nursing Service (산업장 재활간호서비스의 중요도에 대한 간호사의 지각과 수행의 차이)

  • Purpose: The purpose of this study was to compare the rehabilitation nursing services what nurses recognize to be important and what they do in practice of the industrial workers. Method: The subjects were 150 nurses who were working at the industrial hospital. The questionnaire was made up with 43 items about the perception on importance and performance of rehabilitation services. Result: The highest services in the perception on importance and performance subscale were physical and therapeutic services. The highest difference between perception on importance and performance was the social and occupational services. The perception on importance of rehabilitation services had positive correlation with performance. The group take care of general patients had a higher level of perception on importance than the other group, and level of performance had significant difference with age and position. Conclusion: We need to develop the strategy for promoting the performance of services that recognize it will be important services, but not to do. Further study on system for promoting the nurse practitioner who responsible for rehabilitation nursing.

Perception and Practice of Hospital Infection Control of Nursing Staff depending on the Supplementation of Nurses in Long-term Care Hospitals (간호사 확보에 따른 노인요양병원 간호인력의 병원감염관리 인지도 및 실천도)

  • Purpose: The purpose of this study was to describe the perception and practice of hospital infection control of nursing staff in long-term care hospitals by the level of supplementation of nurses. Methods: The participants were 212 nurses and nurse assistants in 13 long-term care hospitals in a metropolitan city and the data were gathered by self-reported questionnaires during August 2011 and analyzed by SPSS/WIN program. Results: The beds per a nurse were 15, and the proportion of nurses among nursing staff in long-term care hospitals was about 33%. In general, the level of infection control in practice was lower than that of perception. The highest perception and practice domain was 'Management of disinfection/contamination', and the lower level domains were 'Personal hygiene' and 'Hand washing' There were statistically significant differences in the hospital infection control of perception and practice depending on age, education, career in long-term care hospital, job position, the quantity of beds, nurse, and nurse assistant, beds per a nurse and proportion of nurses in hospitals. Conclusion: According to these results, the systematic and continual education on hospital infection control of the nursing staff in long-term hospitals should be carried out. In addition, the policy to add more nurses into long-term care hospitals must be implemented.

Relationships of Nurses' Perception, Nursing Performance, Job Stress, and Burnout in Relation to the Joint Commission International Hospital Accreditation (간호사의 국제의료기관평가에 대한 인식과 간호업무성과, 평가 관련 직무 스트레스 및 소진과의 관계)

  • Purpose: The purpose of this study was to ascertain current status of nurses' perception, nursing performance, job stress, and burnout in relation to the Joint Commission International (JCI) hospital accreditation and to verify the relationships among these variables. Methods: This cross-sectional study was performed using questionnaires. Data were collected from 220 nurses who worked at one hospital from April 5 to May 4, 2013. Results: The scores for perception, nursing performance, job stress and burnout in relation to the JCI hospital accreditation were(on a five-point Likert scale) 3.23, 4.01, 3.56 and 3.40 respectively. A positive correlation was observed between perception and nursing performance. Burnout was negatively correlated with perception and nursing performance, and positively correlated with job stress. Conclusion: These results indicate that developing positive perception and reduced burnout in relation to the JCI hospital accreditation can produce good nursing performance. These findings can be utilized to develop strategies for reducing job stress and burnout in relation to the JCI hospital accreditation.

Perception of Clinical Ladder System, Professional Self-Concept, Self-Efficacy, and Organizational Commitment by the Level of Clinical ladder among Intensive Care Unit Nurses (중환자실 간호사의 경력개발제도에 따른 경력개발제도의 인식, 전문직 자아개념, 자기효능감, 조직몰입)

  • Purpose : This study aimed to verify the effectiveness of the clinical ladder system (CLS) by identifying the perception of the CLS, professional self-concept, self-efficacy, and organizational commitment among intensive care unit (ICU) nurses. Methods : Data were collected through a questionnaire survey of 173 nurses working in 11 ICUs at a tertiary hospital in Seoul. Results : The perception of the CLS, self-efficacy, and organizational commitment showed significant difference according to the CLS level. As a result of the partial Spearman correlation analysis after adjusting for demographic difference according to CLS (age, marital status, education, and clinical career), CLS positively correlated with the perception of the CLS (r=.16, p=.045), professional self-concept (r=.24, p=.001), and self-efficacy (r=.21, p=.007). On the contrary, organizational commitment negatively correlated with the perception of the CLS (r=-.43, p<.001) and self-efficacy (r=-.32, p<.001). Conclusion : The CLS is effective for professional self-concept and self-efficacy. However, specialists 2 nurses, who had high professional self-concept and self-efficacy, showed low level of organizational commitment. Thus, it is necessary to examine the factors of work burden and career plateau that are imposed after promotion and improve experience and planning according to the growth needs. Moreover, regular evaluation of the CLS will require hospital support to ensure that ICU nurses appreciate and actively participate in the support.

Perception and Sexual Attitude Unwed Mother in Nursing Students and Obstetrics and Pediatric Nurses (간호대학생과 산과 및 아동 간호사의 미혼모에 대한 인식과 성태도)

  • This study was undertaken to identify the perception and sexual attitude of nursing students and obstetrics and pediatric nurses towards unwed mothers, and to identify differences in the general characteristics. Data were collected as a self-reported questionnaire survey from September, 2018 to January, 2019. Subjects included 94 nursing students enrolled at 2 nursing colleges, and 101 nurses of 8 hospitals located in the B & C Metropolitan. The data collected was analyzed by descriptive statistics, frequencies, percentages, means and standard deviations, independent t-test, and one-way ANOVA. Comparing results obtained from nursing students and nurses revealed a statistically significant difference in their overall perception of an unwed mother and sexual attitude. The perception of an unwed mother showed significant differences considering the grade, age and sibling status of nursing students, whereas nurses revealed significantly different perceptions according to their age, occupational position, clinical career, career of current work department, marital status, and children status. Sexual attitude was significantly different for nursing students according to their experience contact with unwed mother, whereas perception of nurses differed significantly for sexual attitude according to experience contact with unwed mother, types of hospital, and marital status. We propose that data obtained from this research can be used as basic data for research and education related to the perception of unwed mothers and sexual attitude of nursing students and nurses.
