보육교사에서 구강건강행동 및 구강자각증상과 BMI의 연관성 (Relationship between BMI, oral health behavior and perceived oral symptoms among child care teachers)

  • Objectives : The aim of this study was to analyze relationship between body mass index(BMI), oral health behavior, and perceived oral symptoms among child care teachers. Methods : The subjects were 310 child care teachers who worked in child care centers in Jeollabukdo Province, Korea. A cross tab analysis and a multiple regression analysis were conducted to find a possible relationship between oral health behavior, oral symptoms and BMI. Results : BMI in child care teachers was 35.1% and male and married teachers had higher BMI. Higher BMI revealed perceived periodontal disease in smokers. Higher BMI was closely related to perceived xerostomia and halitosis. Conclusions : BMI is a very important index to develop health improvement and obesity control.

노인의 스트레스 인지가 전반적인 건강상태 및 구강건강상태에 미치는 영향 (Effect of perceived stress on general health and oral health status in elderly: results from the Korea national health and nutrition examination survey 2014)

  • Objectives: This study was conducted to identify the rates of perceived stress of elderly people over 65 years old and to confirm the influence of stress on general health and oral health status. Methods: Using data from the National Health and Nutrition Survey for 2014, 1,472 people over 65 years of age were selected as final subjects. Stress was used as an independent variable and dependent variables were included physical health (perceived health status), mental health (depression), and oral health (perceived oral health status). The following confounding variables were adjusted for the current study: demographic characteristics (gender, age, education level, house income) and health - related characteristics (drinking, smoking, exercise, frequency of tooth brushing, using oral care product, dental exam, comorbidity, restrict activity). Complex sampling analysis was applied and logistic regression was performed to determine the effects of stress on physical health, mental health and oral health status. Odds ratio (OR) and 95% confidence interval (95% Confidence Interval, 95% CI) were calculated. Results: Logistic regression indicated that stress was significantly associated with low physical health (OR=2.18, 95%CI: 1.49-3.20), low mental health (OR=8.68, 95%CI: 4.98-15.11), low oral health (OR=1.53, 95%CI: 1.06-2.21) after adjusting for confounding variables. Conclusions: The perceived stress of the elderly was found to be related to the general health and oral health status. Therefore, it is necessary to evaluate stress as a predictor of health risk for the health promotion of the elderly on multidisciplinary assessment and continuous evaluation. In addition, health support policies should be provided to achieve good health status for elderly.

고등학생들의 구강건강행위에 영향을 미치는 요인 (Factors influencing oral health behaviors in high school students)

  • Objectives : This study was to infectify variable the factors influencing oral health behaviors in high school students. Methods : Data were collected from 241 high school students Busan and Kyungnam Province for four weeks in July 2012. The SPSS 18.0 program was used for data analysis, which included t-test, correlation analysis, multiple logistic regression analysis. Results : Positive correlation in the order of social support(r=.381), self-efficiency(r=.309) with perceived benefit of oral health behavior and social support with total heal index(r=.298) are shown. In the contrary, the relation between social support(r=-.400), subjective oral health status(r=-.222) with impairments of oral health behavior have negative one. Experience in three or more tooth brushing has an effect on social support and subjective oral health status(p<.05). Perceived benefit(p<.001) of oral health life and subjective oral health status(p<.05) are affected by recognition of correct tooth brushing. Intake of harmful food on tooth has an effects on perceived benefit, self-efficiency, total health index and subjective oral health status(p<.05). Conclusions : In conclusion, systematic activation of oral health education in school and correct self-activity of oral health will promote oral health of youth.

노인의 본인인지 구강건강상태와 우울감 (Association of self-perceived oral health and depression in the elderly: the sixth Korea national health and nutrition examination survey(the 6th KNHANES))

  • Objectives: The aim of the study was to investigate the association of self-perceived oral health and depression in the Korean elderly. Methods: The subjects were 1,329 elderly in Korea from the sixth Korea National Health and Nutrition Examination Survey (the 6th KNHANES). The dependent variable was depression that continued more than two weeks. Model I for the impacts of depression on self-reported oral health, Model II for the impacts of depression on chewing problem, Model III for the impacts of depression on speaking problem was evaluated. PASW Statistics 18.0 version was used for frequency analysis, chi-square test and logistic regression analysis. Results: Female elderly were much more prone to depression than male. Female had lower monthly compensation, less medicaid, chewing problem, speaking problem, and less education. For these reasons, they tended to have more depression than male (p<0.001). Self-perceived oral health impacts on depression included poor self-reported oral health(p<0.01), poor chewing problem(p<0.01) and poor speaking problem(p<0.05). On the other hand, male did not show a statistically significant association. Conclusions: The study showed the self-perceived oral health related quality of life had a significant influence on depression in the elderly. The continuing lifelong oral health care can prevent depression in the later life because oral health care improvement can enhance the self-perceived oral health status.

교정치료의 의료서비스 만족도가 구강건강관련 삶의 질(OHIP-14)에 미치는 영향 (The effect of medical service satisfaction of orthodontic treatment on life quality related to oral health(OHIP-14))

  • Objectives: The purpose of this study is to investigate the relationship between perceived oral health, medical service satisfaction, and oral health impact profile (OHIP-14) and the factors affecting OHIP for orthodontic patients, and to provide basic data to improve the quality of life of orthodontic patients in accordance with oral health. Methods: The study explained the purpose and purport of this study to orthodontic patients who visit dental clinics located in Daejeon and Chungcheong province for about two months from July 01, 2019, and conducted a self‐reported questionnaire survey for 220 participants who agreed to participate in this study. of the collected questionnaires, data of 197 subjects were used for the final analysis except 23 that were inadequate. Results: As for orthodontic treatment satisfaction according to perceived oral health and OHIP, there was statistically significant difference between respondents who were 'very healthy' and 'not healthy' (p<0.05). The higher the supplementary service satisfaction, the higher the orthodontic treatment satisfaction, and the higher the perceived oral health, the OHIP was found to be the higher. The factors affecting OHIP were found to be significantly associated with orthodontic treatment satisfaction and perceived oral health. The explanatory power was 15.6%. Conclusions: Active and ongoing oral health education should be provided to dental hygienists to ensure that the patient's oral cavity is healthy, and although additional services need to be improved, it is more important to have management strategies for patients to improve orthodontic treatment satisfaction.

한국 노인의 본인인지 구강건강상태와 치주질환 관련성 (A study on the correlation between self-perceived oral health status and periodontal diseases in elderly Koreans)

  • Objectives: The objective of the study was to review the correlation between self-perceived oral health status and periodontal diseases in elderly Koreans, using data from the $6^{th}$ (2nd year) Korea National Health and Nutrition Examination Survey (2014). Methods: The subjects for this study were a total of 1,454 elderly people aged 65 years or older who responded to the health questionnaires of the $6^{th}$ (2nd year) Korea National Health and Nutrition Examination Survey conducted in 2014. Their general characteristics were analyzed using frequency analysis, while a cross-tabulation analysis (${\chi}^2-test$) was performed to understand the correlation with periodontal diseases. To clarify any effect of self-perceived oral health status on periodontal diseases, the selected variables were controlled and subsequently analyzed according to the logistic regression analysis. Results: In terms of the difference between elderly people with periodontal disease and those without periodontal disease, higher prevalence rates of periodontal diseases were found in women, those of younger age, those with lower educational and income levels, those with poorer subjective oral health status, those in the presence of chewing discomfort, those who had a toothbrushing frequency of twice per day, and/or those who had received no oral examination over the previous one year. Regarding the effect of self-perceived oral health status on periodontal diseases, 1.78-fold and 1.74-fold higher prevalence rates of periodontal diseases were shown with poorer subjective oral health status and in the presence of chewing discomfort, respectively. Conclusions: Based upon the results above, it is considered that a better understanding of self-perceived oral health status is necessary for a healthy life of the elderly. Furthermore, constant relevant studies and effective prevention programs intended to moderate the progress of or prevent periodontal diseases in the elderly in communities should be performed and implemented for the sake of better quality of life and oral health.

일부 군인의 구강건강상태인식과 구강증상의 관련성 (Relationship between Perceived Oral Health Status and Oral Health Related Symptoms among the Korean Soldiers)

  • 본 연구는 일부 군인의 구강건강상태인식과 구강증상의 관련성을 파악하여 추후 군인들의 구강건강향상과 삶의 질 증진에 기초자료로 활용하고자 하였다. 연구의 취지를 설명하여 참여를 희망하는 군인 263명을 대상으로 자기기입식 설문조사를 수행하였다. 수집된 자료는 연구대상자의 일반적인 특성에 대해 빈도분석을 실시하였고, 일반적인 특성과 구강건강상태인식을 교차분석 하였으며, 구강증상이 구강건강상태인식에 미치는 영향을 파악하기 위해 회귀분석을 실시하였다. 그 결과, '건강하지 않음'은 29.7%, '보통'은 52.9%, '건강함'은 17.5%로 나타났다. 구강건강상태인식에 영향을 주는 증상으로는 '치아가 아프다(${\beta}=0.262$)', '잇몸에서 피가 난다(${\beta}=0.170$)'인 것으로 나타났다. 앞으로 군인들에게 지속적인 구강보건교육을 실시하여 군인들이 구강건강에 관심을 가질 동기부여의 기회를 제공하고, 또한 구강건강에 대한 긍정적인 태도를 갖게 함으로써 구강건강에 대한 지식과 인식도를 높여야 할 것이다.

50대 이상 장노년층의 구강건강 관련 삶의 질과 구강 건강 결과의 관련성 (Association between Oral Health Related Quality of Life(OHQoL) and Oral Health Outcome in Adults over 50 Years Old)

  • 50대 이상 장 노년층 성인들의 구강건강 관련 삶의 질(Oral-health related quality of life, OHRQoL)과 건강 결과(health outcome)의 관련성을 평가하기 위해 148명을 편의추출하여 구강검사와 면접조사를 통해 OHIP-14(Oral health impact profile-14)와 인지 건강 및 구강건강 상태를 측정하고, 독립표본 t 검정, 일원분산분석, 교차분석, 상관분석 등을 실시한 결과 다음과 같은 결론을 얻었다. 1. OHIP-14의 모든 문항에 대해 조사 대상자 중 9.5%가 전혀 문제나 불편감을 느끼지 않는다고 응답하였으며, 가장 문제를 많이 느끼는 차원은 '신체적 통증'(69,8%)과 '심리적 불편감'(63.5%)으로 나타났다. 2. 평가 구강건강 상태에 따른 구강건강 관련 삶의 질 수준을 비교한 결과 우식경험치아수, 잔존치아수에 따라 유의한 차이가 나타났으나(p=0.045), 치주상태와 틀니사용여부에 따른 차이는 나타나지 않았다. 3. 조사 대상자의 43.9%(N=65)가 자신이 '건강하지 않다'고 느끼고 있으며, 43.9%(N=65)가 자신의 구강건강상태가 '나쁘다'고 생각하고 있고, 54.7%(N=81)가 자신의 구강건강상태에 대해 '만족하지 않는다'고 응답했다. 4. 조사 대상자가 느끼는 인지 건강상태에 따른 구강건강 관련 삶의 질을 비교한 결과 인지 건강상태, 인지 구강건강상태, 구강건강 만족도에 따라 유의한 차이가 나타났다(p<0.001). 5. 조사 대상자의 인지 건강상태와 OHIP-14 점수는 유의한 상관관계가 있으나(r=0.326, p<0.01), 인지 건강상태와 우식경험치아수, 잔존치아수의 상관관계는 통계적으로 유의하지 않았다.

한국 노인의 주관적 구강건강평가와 삶의 질 관련성 연구 (Association between Perceived Oral Health and Quality of Life in Korea Older Population)

  • 본 연구는 한국 노인의 주관적 구강평가와 일반건강평가, 삶의 질의 관련성 연구를 위하여 국민건강영양조사 제 7기 1차년도 자료 중 60세 이상 노인 1,866명(69.5세)의 자료를 SPSS 23.0으로 위계적 로지스틱 회귀분석하였다. 대상자의 87.7%가 구강상태를 보통 이하로 인식하였으며, 여성, 낮은 교육수준 일 경우 구강상태를 부정적으로 평가하였다(p<.05). 일반건강평가가 긍정적일수록 구강평가도 긍정적이었으며(F=19.04, p<.001), 공변량 통제 후 로지스틱 회귀분석에서 일반건강평가가 부정적일수록(OR: 2.86, 95% CI: 1.5-5.5), 영구치 우식이 있을수록(OR: 2.68, 95% CI: 1.67-4.32), 우울 문제가 있을수록(OR: 1.45, 95% CI: 1.42-2.57) 구강평가는 부정적이었다. 따라서, 고령자의 주관적 구강평가와 일반건강평가, 삶의 질 사이에는 유의한 관계가 있으므로 삶의 질 향상을 위하여 통합적 정책이 이루어져야 할 것으로 사료된다.

노인의 주관적 건강상태와 건강관련 삶의 질의 관련성 : 구강건강수준의 조절 효과 중심으로 (Relationship between perceived health status and health-related quality of life in the elderly: A focus on moderating effects of oral health)

  • Objectives: This study aimed to validate the moderating effect of oral health on the relationship between perceived health status and health-related quality of life in the elderly and to use this information as primary data to suggest oral health policies for the aged society. Methods: This study included 3,707 subjects aged over 65 years who answered all the variables used in the study model and completed the health questionnaire and screening survey based on the sixth Korea National Health and Nutrition Examination Survey. Baron & Kenny's linear regression analysis using SPSS 22.0 and SPSS Macro Version 3.1 programs was performed to confirm the moderating effect of the number of remaining natural teeth, chewing difficulty, and cardiovascular disease on the relationship between perceived health status and healthrelated quality of life in the elderly. Results: The number of remaining natural teeth, chewing difficulties, and cardiovascular diseases affected the perceived health status of the elderly. As the number of remaining natural teeth increased, the effect of perceived health status on the quality of life in the elderly was buffered. The effect of perceived health status on the quality of life increased with chewing difficulties and the number of cardiovascular diseases. In particular, it was confirmed that chewing difficulties, rather than the number of cardiovascular diseases. had a greater effect on the quality of life in the elderly. Conclusions: Oral health policies and projects are required to ensure oral rehabilitation with dentures and implants and restore chewing function to improve the quality of life of the elderly in Korea.
