Objectives : Albizzia Julibrissin was formulated to contain various natural products known to cure erectile dysfunction. This study was aimed to investigate the effects of Albizzia Julibrissin on the nitric oxide synthase (NOS) activity, nitrite level, antioxidation and erectile responses induced by ethanol in corpus cavernosum penis of rats. Methods : The crushed Albizzia Julibrissin was extracted 3 times, each time with 3 volumes of methyl alcohol at $60^{\circ}C$ for 24 h. The extract was filtered and evaporated under a reduced pressure using a rotary evaporator to yield 45.3 g. Albizzia Julibrissin extract was oral-administered 100 mg per 1 kg of body weight for 20 days, while the normal group was administered only with a saline. The efficacy of Albizzia Julibrissin against erectile function was examined as described in the text. Results : The level of urethral NOS activity and nitrite were increased by Albizzia Julibrissin. The level of lipid peroxide was decreased by Albizzia Julibrissin. The level of urethral lipid peroxide in the ethanol-Albizzia Julibrissin double administered rats was decreased as low as in the norma! group, while the one in the ethanol-treated group was increased. The level of urethral nitrite, NOS activity, glutathione and serum testosterone in the ethanol-Albizzia Julibrissin double administered rats were as high as in the normal group, while the one in the ethanol-treated group was decreased. The erectile response to cavernous nerve stimulation in the ethanol-Albizzia Julibrissin double administered rats increased as high as in the normal group while the one in the ethanol-treated group decreased. Conclusions : Albizzia Julibrissin was shown to be effective for the treatment of erectile dysfunction induced by ethanol in rats.

Plastibell is one of the three most common devices used for neonatal circumcision in the United States, with a complication rate as low as 1.8%. The Plastibell circumcision device is commonly used under local anesthesia for religious circumcision in male neonates, because of cosmetic reasons and ease of use. Occasionally, instead of falling off, the device may get buried under the skin along the shaft of the penis, thereby obstructing the normal flow of urine. Furthermore, the foreskin of neonates is highly vascularized, and hence, hemorrhage and infection are possible when the skin is cut. Necrosis of penile skin, followed by urethral obstruction and renal failure, is a serious surgical mishap requiring immediate corrective surgery and medical attention. We report a case of fulminant urosepsis, acute renal failure, and pyelonephritis in a 4-day-old male neonate secondary to impaction of a Plastibell circumcision device. Immediate medical management was initiated with fluid resuscitation and mechanical ventilation; thereby correcting life threatening complications. Pediatricians and Emergency Department physicians should be cognizant of the complications from Plastibell circumcision device in order to institute appropriate and timely management in neonates.
Congenital adrenal hyperplasia(CAH) is a recognized cause of precocious pseudopuberty. Some children with CAH also develop true precocious puberty with early maturation of the hypothalamic-pituitary-gonadal axis. We review a case of CAH who eventually developed central precocious puberty nine months after initial treatment with corticosteroid. A 3-year-old boy visited complaining of rapid growth, a large penis and frequent penile erections. This patient was diagnosed with CAH with elevated 17-OH progesterone and cortical hypertrophy of adrenal gland on CT scan. His gonadotropin levels were within the normal prepubertal range. Even on treatment with corticosteroid he grew rapidly and had testicular enlargement, pubic hair development and rapid bone maturation. At second admission, his gonadotropin levels were elevated both basally and in response to LHRH stimulation, suggesting that the CAH led to early activation of pubertal gonadotropin secretion(true precocious puberty). He was treated with monthly depot injections of a LHRH analog in addition to the hydrocortisone. His second sexual characteristics regressed gradually and rate of linear growth and bone maturation decreased.
Objectives : These present study was designed to investigate the relaxation effects of Alpiniae Oxyphyllae Fructus(AOF) on isolated corpus cavernosum smooth muscle. Methods : Rabbit corpus cavernous tissues were prepared in strip. Then relaxation responses of AOF at 0.01-3 ㎎/㎖ in contracted strips induced by phenylephrine(PE) were measured. To evaluate mechanisms, indomethacin(IM) tetraethylammonium chloride(TEA), Nω-nitro-L-arginine(L-NNA), methylene blue(MB) were treated before AOF extract(0.1-3 ㎎/㎖) infused into precontracted strips induced by PE. And 1 mM Ca2+was infused into precontracted strips after pretreatment of AOF extract(3 ㎎/㎖) in Ca2+-free krebs-ringer solution. NO concentration was measured by Griess reagent system. Ratio of smooth muscles to collagen fibers and eNOS positive reaction were measured by histocheminal and immunohistochemical process. Results : The cavernous strips were significantly relaxed by AOF extract 0.1, 0.3, 1, 3 ㎎/㎖ and the pretreatment with IM 10 μM,L-NNA 100 μM, MB 10 μM inhibited relaxation of AOF compared to non-pretreatment, but the pretreatment with TEA 100 μM didn't affect relaxation of AOF. In a Ca2+-free solution, pretreatment with AOF reduced increase on contraction of strips by Ca2+supply than non-pretreatment. On HUVEC, NO concentration was increased. On corpus cavernosum of penis in Spontaneous Hypertensive Rat, ratio of smooth muscles to collagen fibers and eNOS positive reaction in AOF group were increased compared to PE groupConclusions : Taken this results, we can suggest that AOF extract exerts a relaxation effects on rabbit corpus cavernosum smooth muscle in part by suppressing influx of extracellular Ca2+throughout prostacyclin, the NO-cGMP system.
As a part of estimating quantitative radiation treatment doses, we produced a mathematical phantom based on the standard Korean male. Then, with the prostate as the source organ, we calculated the absorbed dose in the prostate and surrounding organs forecasted to occur during brachytherapy for prostate cancer. To simulate the procedure, we selected $^{25}I$ and $^{103}Pd$ useful in brachytherapy of the prostate as the radionucleids and made an assumption that 1 Ci of initial radioactivity is administered. As a result, we found that the prostate, as the source organ, indicated 101 Gy/Ci and 7.24 Gy/Ci, respectively, in case of $^{125}I$ and $^{103}Pd$. With the exception of the prostate, organs with high absorbed doses were found to be in the order of the penis and scrotum, sigmoid colon, testicles and the urinary bladder, which are relatively close to the prostate.

GX-12 is a naked DNA vaccine developed by the DongA Pharmaceutical Co. Ltd. and Genexine for the treatment of HIV infection. This study was peformed to evaluate the biodistribution and expression of GX-12 mRNA in gonadal tissues, and to investigate the histopathological changes after the repeated intramuscular injection. GX-12 (400 $\mu\textrm{g}$/head) was injected into the left anterior tibialis once a week for four weeks. On day 1, 5, 15, 30 and 45 after the final administration, gonadal tissues (testes, epididymis, seminal vesicles, penis, prostate glands, ovaries, vagina, uterus) and the injection site (muscle) were harvested and examined for the expression of mRNA by RT-PCR. In addition, histopathological examination was peformed at each time point. At the injection site, mRNA expression of GX-12 was detected only at early time points (1 ~ 15 days after injection) but not thereafter. However, in gonadal tissues, mRNA expression was not identified at all time points both in male and female rats. There were no histopathological changes in all reproductive organs and muscle. Based on these results, it is unlikely that the plasmid DNAs of GX-12 was distributed to- and expressed in gonadal tissues, suggesting that the chance of germline integration and transmission is negligible.
The surgical treatment of extensive urethral strictures remains a controversial topic; although techniques have evolved, there is still no definite method of choice. Since 1968, when Orandi presented an original technique for one-stage urethroplasty using a penile skin flap, the Orandi technique has become the most prevalently used one-stage procedure for anterior urethral strictures. We present a 20-year follow-up experience with one-stage reconstruction of long urethral strictures using a longitudinal ventral tubed flap of penile skin, with some important technical changes to Orandi's original technique to overcome the deficient vascularity caused by periurethral scar tissue. In 1997, a 55-year-old male patient complained of severe voiding difficulty and a weak urinary stream because of transurethral resection of the prostate due to benign prostatic hyperplasia. Another 47-year-old male patient had the same problem due to self-removal of a Foley catheter in 2002. In both patients, a urethrogram demonstrated extensive strictures involving the long segment of the anterior urethra. A rectangular skin flap on the ventral surface of the penis was used considering the appropriate length, diameter, and depth of the neourethra. The modified Orandi flap provided a pedicled strip of penile skin measuring an average of 8 cm. The mean duration of follow-up was 20.5 years. A long-term evaluation revealed stable performance characteristics without any complications.
This study was designed to investigate the effects KH-204 on erectile dysfunction in hyperlipidemic rat. KH-204 has been evaluated antihyperlipidemic and antierectile dysfunction effects on experimental hyperlipidemic rats induced by high fat diet. After oral administration of the water extract KH-204 (50, 100, 200, 300 mg/kg) to hyperlipidemic rats for 8 weeks, the variables including body weight, total cholesterol, HDL and LDL levels in serum, the expression of eNOS and nNOS in penis were measured. And erectile function was determined by measurement of intracavernosal pressure (ICP) and maximal arterial pressure (MAP) after electrical stimulation of the cavernosal nerve. Oral administration of KH-204 significantly inhibited the increases of serum total cholesterol and LDL-cholesterol levels and the decreased of serum HDL-cholesterol levels in hyperlipidemic rats induced by high fat diet. The penile expression level of the two enzyme (eNOS, nNOS) were increased significantly after oral administration of the KH-204 50 mg/kg. Erectile function after 10 volts stimulation was significantly decreased in the hyperlipidemic rat compared with the normal rat, but increased in KH-204 group compared with hyperlipidemic group. These results suggest that KH-204 is effective for erectile dysfunction in hyperlipidemia.
Phenoxy compounds, 2,4-Dichlorophenol acetoxy acid (2,4-D) and 2,4-dichlorophenol (DCP), are widely used as a hormonal herbicide and intermediate for pesticide manufacturing, respectively. In order to assess the potential of these compounds as endocrine disruptors, we studied the androgenicity of them wing in vivo and in vitro androgenicity assay system. Administration of 2,4-D (50 mg/kg/day, p.o.) or DCP (100 mg/kg/day, p.o.) to rats caused an increase in the tissue weight of ventral prostate, Cowpers gland and glands penis. These increase of androgen-dependent tissues were additively potentiated when rats were simultaneously treated with low dose of testosterone (1 g/kg, s.c.). 2,4-D increased about 350% of the luciferase activity in the PC cells transiently cotransfected phAR and pMMTV-Luc at concentration of $10^{-9}$ M. In 2,4-D or DCP-treated castrated rats, testosterone 6$\beta$-hydroxylase activity was not significantly modulated even when rats were co-treated with testosterone. In vitro incubation of 2,4-D and DCP with microsomes at 50 $\mu$M inhibited testosterone 6$\beta$-hydroxylase activity about 27% and 66% in rat liver microsomes, about 44% and 54% in human liver microsomes and about 50% and 45% in recombinant CYP3A4 system, respectively. The amounts of total testosterone metabolites were reduced about 33% and 75% in rat liver microsomes, 69% and 73% in human liver microsomes and 54% and 64% in recombinant CYP3A4 by 2,4-D or DCP, respectively. Therefore, the additive androgenic effect of 2,4-D or DCP by the co-administration of the low dose of testosterone may be due to the increased plasma level of testosterone by inhibiting the cytochrome P450-mediated metabolism of testosterone. These results collectively suggested that 2,4-D and DCP may act as androgenic endocrine disrupter by binding to the androgen receptor as well as by inhibiting the metabolism of testosterone.

Hypospadias is a rare congenital malformation of the urethra reported in cattle. The urethral lumen of male indigenous Korean calf is open along the ventral aspect of the penis in the perineal region. Renal abscess and renal stone formation causing urinary tact infection has not been reported in hypospadia calves. The objective of this study was investigation for renal abscess and renal stone formation through autopsy. Histopathological examination and laboratory tests were performed. At autopsy, the pustules were formed on the right renal cortex, and the renal medulla abscess were formed on right and left part of the renal pelvis. Histopathological finding, this case was diagnosed as severe acute suppurative and necrotizing pyelonephritis, and severe chronic interstitial nephritis with fibrosis and moderate multifocal acute cystitis with edema. Milky exudate of the kidney has been identified as Actinomyces meyeri using the VITEK-2 system for identification of bacteria, and the stone has been identified as carbonate apatite using FT-IR system for quantification analysis. This case report describe the hypospadias complicated with urinary tract infection due to carbonate apatite stones and Actinomyces meyeri.
