Purpose : The purpose of this study was to investigate the effect of tensor fasciae latae length on the rotation of pelvis during one leg stance. Methods : 41 healthy adults participated in this study. The movement of the pelvis and trunk was measured using 3-dimensional motion analyzer, during one leg stance. The movement of the pelvis and trunk was collected lateral shift, rotation, side bending, and flexion-extension. Tensor fasciae latae length of subjects was measured in sidelying positon with neutral position of hip joint and flexion $90^{\circ}$ of knee. Also, the range of motion of hip exteral and interal rotaion were measured in prone position wih lexion $90^{\circ}$ of knee. The subjects were separated 2 groups that more pelvic rotation group(n=15) and less pelvic rotation group(n=15) according to the degree of pelvic rotation. Results : The more pelvic rotation group was showed significantly higher in the ROM of hip external rotation than less pelvic rotation group(p<0.05). The difference of tensor fasciae latae length not showed significant difference between groups. During one leg stance, The movement of the shifting and flexion-extension of trunk and pelvis were not showed significant difference. But the side bending and the rotation of pelvis and trunk showed significant difference between groups. Conclusion : The difference of tensor fasciae latae length not showed significantly in more pelvic rotation group and less pelvic rotation group. But, this study suggests that the pelvis instability brings the instability of the trunk during one leg stance.
To evaluate the diagnostic usefulness and significance of quantitative sacroiliac joint scintigraphy in the assessment of sacroiliitis, we measured Sacroiliac Joint/Sacrum Uptake Ratio(SIS Ratio) by region of interest(ROI) method using $^{99m}Tc-methylene$ diphosphonate. The observed results were as follows: 1. Using ROI method, the SIS ratios for the control group of 65 persons were $1.05{\pm}0.08$(left) and $1.06{\pm}0.07$(right) which were narrower in range than those of slice method $(mean{\pm}S.D.)$. 2. The effects of age, gender and laterality on SIS ratio were not significant. 3. In left side, one of 6 patients with rheumatoid arthritis had SIS ratio in excess of 2 standard deviation of normal control group, and remainder had SIS ratios within normal limit. In right side, 3 patients had SIS ratios in excess of 2 standard deviation of normal control group, and remainder, within normal limit. 4. In both sacroiliac joint, 2 of 3 patients having sacroiliitis clinically with Reiter's syndrome whose pelvis A-P X-ray findings showed normal had high SIS ratios (left/right; 1.31/1.69, 1.90/1.80), but SIS ratio of one patient who had no evidence of sacroiliitis clinically was within normal limit. 5. In 6 patients with ankylosing spondylitis in both sacroiliac joints, 4 whose pelvis A-P Xray findings showed severe sclerotic change of sacroiliac joints had SIS ratio within normal limit or below that of normal control group, and SIS ratios of 2 patients whose pelvis A-P X-ray findings showed were increased. 6. 4 of 5 patients with low back pain of which cause could not be evaluated clinically and radiologically had SIS ratios in excess of that of normal control group. It would be concluded that quantitative sacroiliac joint scintigraphy is useful and sensitive screening method in the diagnosis as well as in the assessment clinical activity of sacroiliitis.
In substitution of beam-nonlinear spring model for real-car, it may have errors due to complicated characteristics of joint and overestimation of joints stiffness. In this research, a method for the joint modeling was suggested by nonlinear static and dynamic analyses of beam and shell joint models and verified by the application of accomplished joint modeling method to simplified full car model. In consequence, modified simplified full car model was improved in local acceleration and rigid wall force. Finally, the frontal crash analyses with the dummy were established and the accelerations of accelerations of head, chest and pelvis had good agreements with those of shell model.
Purpose: The purpose of this study was to investigate changes in movement strategies of lower limb joints depending on the type of heel during sit-to-stand. Methods: Twenty healthy females participated in this study. All subjects performed sit-to-stand three times each with three different types of heels - bare feet, 9 cm high-heeled shoes, and unstable shoes. Trails were conducted in random order. Three-dimensional motion analysis systems were used for collection and analysis of the kinematic data of lower limb movements. Results: Results of this study showed kinematic differences in pelvis, hip joints, knee joints, and ankle joints during sit-to-stand based on the type of heels. At the initial sit-to-stand, hip joint flexion, knee joint flexion, ankle joint flexion, and ankle joint inversion showed significant differences. The maximal angles of hip joint flexion, hip joint adduction, knee joint flexion, ankle joint flexion, and ankle joint inversion were significantly different, while hip joint adduction, pelvic forward tilt, hip joint rotation, knee joint flexion, ankle joint flexion, and ankle joint inversion differed significantly during the terminal of sit-to-stand. Conclusion: Therefore, the type of heel played an important role in selection of lower limb movements during sit-to-stand which were essential parts of daily life movements.
Dislocation after a total hip arthroplasty occurs in approximately 1% of patients; however, the frequency is much higher after revision surgery. To prevent dislocation, use of a larger femoral head is recommended, and a dual mobility femoral head has been introduced. However, reducing the dual mobility femoral head to the acetabular component is difficult in cases involving contracture in the soft tissue around the joint. A 72-year-old male patient who developed a periprosthetic joint infection underwent two-stage revision surgery using MUTARS®. Two months after the revision, the hip joint became dislocated and manual reduction was attempted; however, dislocation occurred again. During another revision using a dual mobility bearing, the soft tissue around the hip joint was too tight to reduce. The problem was overcome by first repositioning the dual mobility head into the acetabular socket, followed by assembly of the diaphyseal portion of the implant.
New USNCAP has been carried out by NHTSA including front and side crash from MY2011. In this paper, test results for USNCAP Side crash were reviewed by statistical analysis. This review focused on side crash test results to investigate the effect of changes from new USNCAP side crash test protocol among 30 passenger cars. These results were summarized as followings. Total number of 5 star vehicles on the front seat dummy (16 vehicles, 53.3%) was slightly smaller than the rear seat's (17 vehicles, 56.7%) in MDB test. For the ES-2re dummy, chest injury, ie maximum rib deflection contributed to 66% in the mean value of $P_{joint}$. Pelvis injury was highly dependent upon performance up to 87% in the SID-IIs dummy cited on the rear seat in average $P_{joint}$. For Pole test, pelvis injury made contribution to the average performance to 83%. For standard deviation, it showed the largest value in the same body region as the mean value for each dummy. Overall front seat performance showed 14 vehicles, 44.6% with 5 star vehicles less than each MDB or Pole test result. This result showed that performances in MDB test were different pattern to Pole test on driver position. Number of 5star vehicles for overall side NCAP performance are 18 passenger cars (60%). Curtain airbag and driver thorax airbag were equipped in all test vehicles. One vehicle is equipped with thorax airbag in the rear seat. Results from two side tests considered as reliability problem, ie the cause for large standard deviation in side crash test. Consequently, the countermeasure for new USNCAP side crash test is essential to design the effective side structures for side collision and to control well dummy kinematics with curtain and thorax airbag in order to reduce chest and pelvis injuries.
Purpose: Hip microinstability is defined as hip pain with a snapping and/or blocking sensation accompanied by fine anatomical anomalies. Arthroscopic capsular plication has been proposed as a treatment modality for patients without major anatomic anomalies and after failure of properly administered conservative treatment. The purpose of this study was to determine the efficacy of this procedure and to evaluate potential predictors of poor outcome. Materials and Methods: A review of 26 capsular plications in 25 patients was conducted. The mean postoperative follow-up period for the remaining patients was 29 months. Analysis of data included demographic, radiological, and interventional data. Calculation of pre- and postoperative WOMAC (Western Ontario and McMaster Universities Osteoarthritis) index was performed. Pre- and postoperative sports activities and satisfaction were also documented. A P<0.05 was considered significant. Results: No major complications were identified in this series. The mean pre- and postoperative WOMAC scores were 62.6 and 24.2, respectively. The WOMAC index showed statistically significant postoperative improvement (P=0.0009). The mean satisfaction rate was 7.7/10. Four patients with persistent pain underwent a periacetabular osteotomy. A lateral center edge angle ≤21° was detected in all hips at presentation. We were not able to demonstrate any difference in postoperative evolution with regard to the presence of hip dysplasia (P>0.05), probably because the sample size was too small. Conclusion: Capsular plication can result in significant clinical and functional improvement in carefully selected cases of hip microinstability.
Objective: The purposes of this study were to compare the kinematics of a two-arm kettlebell swing between experts and beginners and to identify the correct postures and biomechanical key points in an attempt to prevent sports injuries induced by a kettlebell swing. Methods: Four experts (height, $169.7{\pm}1.5cm$; weight, $70.5{\pm}1.8kg$; age, $32.0{\pm}1.0years$) licensed to teach kettlebell exercises and three beginners (height, $173.7{\pm}4.1cm$; weight, $78.3{\pm}3.8kg$; age, $30.0{\pm}1.4years$) with no kettlebell exercise experience participated in this study. Each participant performed 15 repetitions of a two-arm kettlebell swing using a 16-kg weight. Joint angles, angular velocities, and peak angular velocity sequences were calculated and compared between the two groups. Results: Large ranges of motion (ROM) of the pelvic angle and hip joints were detected in the experts, while beginners showed greater ROM of the shoulder joint. Peak angular velocity magnitudes and sequences were significantly different between the two groups. Experts lifted the kettlebell upward using the hip joints, pelvis, and shoulder joints (proximal to distal order) sequentially and lowered it using the reverse order of peak angular velocities from the shoulder to hip joints. Conclusion: Mobility of the pelvic segment and hip joint are required, while stability of the other joints is needed to produce appropriate two-arm kettlebell swings. The activation and coordination of the gluteal and hamstring muscles are key points in kettlebell exercises.
In this research, a reach posture prediction based on a two-segment trunk model was made. Recently, reach posture prediction models have used inverse kinematics to provide a single posture that a person naturally takes, with a single segment trunk model that had some shortcomings. A two-segment trunk model was first developed with two links; pelvis link and lumbar-thoracic link. The former refers to the link from the hip joint to L5/S1 joint while the latter does the link from L5/S1 to the shoulder joint. Second, a reach prediction model was developed using the two-segment trunk model. As a result, more reliable equations for two-segment trunk motion were obtained, and the lean direction which refers to the movement direction of the trunk was not found to have a significant effect on the two-segment trunk motion. The results also showed that the hip joint is more preferred over L5/S1 to serve as a reference point for trunk models and the reach prediction model being developed predicted the real posture accurately.

Knowledge of the relationship between the hip and spine is essential in the effort to minimize instability and improve outcomes following total hip arthroplasty (THA). A detailed yet straightforward preoperative imaging workup can provide valuable information on pelvic positioning, which may be helpful for optimum placement of the acetabular cup. For a streamlined preoperative assessment of THA candidates, classification systems with a capacity for providing a more personalized approach to performance of THA have been introduced. Familiarity with these systems and their clinical application is important in the effort to optimize component placement and reduce the risk of instability. Looking ahead, the principles of the hip-spine relationship are being integrated using emerging innovative technologies, promising further streamlining of the evaluation process.
