골반압박벨트 착용이 한발서기 시 여성 천장관절통증 환자의 체간과 고관절 신전근 활성 양상에 미치는 영향 (Effects of Applying the Pelvic Compression Belt on the Trunk and Hip Extensor Electromyography Pattern in Female Patients With Sacroiliac Joint Pain During the One-Leg Standing)

  • The pelvic compression belt (PCB) contributes to improving sacroiliac joint stability, and it has been used as an additional therapeutic option for patients with sacroiliac joint pain (SIJP). This study aimed to investigate whether the muscle activation patterns of the supporting leg was different between asymptomatic subjects and subjects with SIJP during one-leg standing, and how it changes with the PCB. 15 subjects with SIJP and 10 asymptomatic subjects volunteered to participate in this study. Surface electromyography (EMG) data (reaction time [RT] and muscle activation) were collected from the internal oblique, lumbar multifidius, gluteus maximus and biceps femoris muscles during one-leg standing with and without the PCB. Without the PCB condition, in the SIJP group, the biceps femoris muscle showed the fastest RT among all muscles (p<.05), whereas in the asymptomatic group, the RT of the internal oblique muscle was the most rapid (p<.05). In condition without the PCB, the biceps femoris EMG amplitudes in the SIJP group were significantly greater than that in the asymptomatic group (p<.05). After the application of the PCB, the RT of the biceps femoris muscle was significantly increased only in the SIJP group (p<.05). Moreover, the biceps femoris EMG amplitudes significantly decreased and the gluteus maximus EMG amplitudes significant increased only in the SIJP group by applying the PCB (p<.05). However, this had no such effect on the gluteus maximus and biceps femoris EMG patterns in the asymptomatic group (p>.05). Thus, this study supports the applying the PCB to patients with SIJP can be used as a helpful option to modify the activation patterns of the gluteus maximus and biceps femoris muscle.

Characterization of Acetylcholine-induced Currents in Male Rat Pelvic Ganglion Neurons

  • The pelvic ganglia provide autonomic innervations to the various urogenital organs, such as the urinary bladder, prostate, and penis. It is well established that both sympathetic and parasympathetic synaptic transmissions in autonomic ganglia are mediated mainly by acetylcholine (ACh). Until now, however, the properties of ACh-induced currents and its receptors in pelvic ganglia have not clearly been elucidated. In the present study, biophysical characteristics and molecular nature of nicotinic acetylcholine receptors (nAChRs) were studied in sympathetic and parasympathetic major pelvic ganglion (MPG) neurons. MPG neurons isolated from male rat were enzymatically dissociated, and ionic currents were recorded by using the whole cell variant patch clamp technique. Total RNA from MPG neuron was prepared, and RT-PCR analysis was performed with specific primers for subunits of nAChRs. ACh dose-dependently elicited fast inward currents in both sympathetic and parasympathetic MPG neurons $(EC_{50};\;41.4\;{\mu}M\;and\;64.0\;{\mu}M,\;respectively)$. ACh-induced currents showed a strong inward rectification with a reversal potential near 0 mV in current-voltage relationship. Pharmacologically, mecamylamine as a selective antagonist for ${\alpha}3{\beta}4$ nAChR potently inhibited the ACh-induced currents in sympathetic and parasympathetic neurons $(IC_{50};\;0.53\;{\mu}M\;and\;0.22\;{\mu}M,\;respectively)$. Conversely, ${\alpha}-bungarotoxin$, ${\alpha}-methyllycaconitine$, and $dihydro-{\beta}-erythroidine$, which are known as potent and sensitive blockers for ${\alpha}7$ or ${\alpha}4{\beta}2$ nAChRs, below micromolar concentrations showed negligible effect. RT-PCR analysis revealed that ${\alpha}3$ and ${\beta}4$ subunits were predominantly expressed in MPG neurons. We suggest that MPG neurons have nAChRs containing ${\alpha}3$ and ${\beta}4$ subunits, and that their activation induces fast inward currents, possibly mediating the excitatory synaptic transmission in pelvic autonomic ganglia.

Short-course palliative radiotherapy for uterine cervical cancer

  • Purpose: The purpose of this retrospective study was to evaluate the efficacy and feasibility of short-course hypofractionated radiotherapy (RT) for the palliation of uterine cervical cancer. Materials and Methods: Seventeen patients with cancer of the uterine cervix, who underwent palliative hypofractionated 3-dimensional conformal radiotherapy between January 2002 and June 2012, were retrospectively analyzed. RT was delivered to symptomatic lesions (both the primary mass and/or metastatic regional lymph nodes). The total dose was 20 to 25 Gy (median, 25 Gy) in 5 Gy daily fractions. Results: The median follow-up duration was 12.2 months (range, 4 to 24 months). The median survival time was 7.8 months (range, 4 to 24 months). Vaginal bleeding was the most common presenting symptom followed by pelvic pain (9 patients). The overall response rates were 93.8% and 66.7% for vaginal bleeding control and pelvic pain, respectively. Nine patients did not have any acute side effects and 7 patients showed minor gastrointestinal toxicity. Only 1 patient had grade 3 diarrhea 1 week after completion of treatment, which was successfully treated conservatively. Late complications occurred in 4 patients; however, none of these were of grade 3 or higher severity. Conclusion: Short-course hypofractionated RT was effective and well tolerated as palliative treatment for uterine cervical cancer.

Bladder filling variations during concurrent chemotherapy and pelvic radiotherapy in rectal cancer patients: early experience of bladder volume assessment using ultrasound scanner

  • Purpose: To describe the early experience of analyzing variations and time trends in bladder volume of the rectal cancer patients who received bladder ultrasound scan. Materials and Methods: We identified 20 consecutive rectal cancer patients who received whole pelvic radiotherapy (RT) and bladder ultrasound scan between February and April 2012. Before simulation and during the entire course of treatment, patients were scanned with portable automated ultrasonic bladder scanner, 5 times consecutively, and the median value was reported. Then a radiation oncologist contoured the bladder inner wall shown on simulation computed tomography (CT) and calculated its volume. Results: Before simulation, the median bladder volume measured using simulation CT and bladder ultrasound scan was 427 mL (range, 74 to 1,172 mL) and 417 mL (range, 147 to 1,245 mL), respectively. There was strong linear correlation (R = 0.93, p < 0.001) between the two results. During the course of treatment, there were wide variations in the bladder volume and every time, measurements were below the baseline with statistical significance (12/16). At 6 weeks after RT, the median volume was reduced by 59.3% to 175 mL. Compared to the baseline, bladder volume was reduced by 38% or 161 mL on average every week for 6 weeks. Conclusion: To our knowledge, this study is the first to prove that there are bladder volume variations and a reduction in bladder volume in rectal cancer patients. Moreover, our results will serve as the basis for implementation of bladder training to patients receiving RT with full bladder.

항암치료와 통합암치료 병용으로 호전된 전이성 난소 및 자궁내막암 환자 1례 (A Case of Improvement of Metastatic Ovarian and Endometrial Cancer Treated by Integrative Medicine Therapy Combined with Chemotherapy)

  • This report is aimed to investigate the effect of Integrative Medicine Therapy (IMT) in treating metastasized ovary and endometrial cancer. A 51-year-old woman who was diagnosed double primary ovarian and endometrial cancer in 2009. The patient was treated with Laparoscopic Assisted Vaginal Hysterectomy (LAVH), Bilateral Salpingo Oophorectomy (BSO) Pelvic Lymph Node Dissection (PLND), adjuvant chemotherapy till Sep. in 2013. But metastases to Rt. External Iliac artery, Aortocaval area Lymph Nodes, Liver(caudate lobe), Rt. Buttock subcutaneous area, Lt. Gastric Area Lymph Nodes were found. Finally, the patient decided to be treated by IMT including Abnoba Viscum, Vitamin C, herbal medication and pharmacopuncture combined with chemotherapy. The efficacy was evaluated with Positron Emission Tomography and Computed Tomography (PET-CT) and Abdomen Computed Tomography (CT). The metastatic tumor in liver was disappeared and Rt. external iliac artery, aortocaval area Lymph Nodes, Rt. buttock subcutaneous area were also decreased after 6 months treatment. These results suggest that IMT may have a potential role for metastatic cancer.

Evaluation of Treatment Outcomes of Early-Stage Endometrial Cancer Radiotherapy: A Single Center Experience

  • Background: Postoperative adjuvant radiotherapy (RT) in the management of early stage endometrial cancer (EC) is still controversial. Here we report our institutional experience with patients who received postoperative RT for stage I-II EC over a period of 35 years and assess potential predictors of local recurrence (LR), distant metastasis (DM), and overall survival (OS). Materials and Methods: A total of 188 patients undergoing postoperative RT for stage IA-II EC between 1977 and 2012 were evaluated. Some 96 received median 46 Gy whole pelvic radiotherapy (WPRT) (range: 40-60 Gy), 37 were given WPRT with vaginal cuff therapy (VCT), and 55 received only VCT either with brachytherapy (BT) or stereotactic body radiotherapy (SBRT). Chemotherapy was given to 5 patients with uterine papillary serous carcinoma (UPSC). Logistic regression analysis was used to assess the effect of clinicopathological factors on LR, DM, and OS. Results: Median follow-up time was 11 years (range: 1-35 years). At the time of analysis, 34 patients were not alive. Of the 15 patients with LR, 7 (46.7%) recurred in the vaginal stump, 5 (33.3%) in the pelvic region, and 3 (20%) in the paraaortic nodal region, while 12 had distant metastasis. UPSC histology (p=0.027), sole VCT (p=0.041), high histologic grade (p=0.034), and age ${\geq}71$ (p=0.04) were poor prognostic factors on univariate analysis. Conclusions: In our patients receiving radiotherapy for early-stage EC, grade III disease and age ${\geq}71$ were associated with shorter OS whereas UPSC histology was an independent predictor for both LR and DM.

고양이에서 발생한 고양이전염성복막염에 의한 신경병증 증례 (Feline Infectious Peritonitis associated Neuropathy in a Cat)

  • A 8-month-old, spayed female, Domestic shorthair cat lived in a shelter was presented with pelvic limbs ataxia and dysuria. Serum biochemical profile abnormalities were hyperproteinemia and decreased albumin/globulin (A:G) ratio (0.70). Results of cerebrospinal fluid (CSF) analysis were mixed cells pleocytosis with predominance neutrophils and an increase in protein concentration. In addition, feline coronavirus was detected by realtime RT-PCR in CSF. Magnetic resonance imaging (MRI) findings revealed lesions of the lumbar spinal cord. Based on clinical signs, MR finding, CSF analysis and realtime RT-PCR result in CSF, this case was diagnosed as feline infectious peritonitis (FIP) associated meningomyelitis. Although prednisolone and mycophenolate mofetil were administrated, clinical signs were not resolved and progressed to tetraplegia and coma status. This case presentation describes that feline infectious peritonitis virus could affect the lumbar spinal cord only and cause meningomyelitis with pelvic limbs ataxia without other neurological signs.

자궁내막암의 수술 후 보조적 방사선치료 (Postoperative Adiuvant Radiation Therapy in Endometrial Carcinoma)

  • 목적 : 수술 및 수술 후 보조적 방사선치료를 시행한 자궁내막암 환자의 예후인자, 재발 장소 및 생존율 등을 분석해 보고자 하였다. 대상 및 방법: 1991년 9월부터 1997년 12월까지 서을중앙병원에서 수술 및 방사선치료를 받은 27예의 자궁내막암환자를 대상으로 하였다. 수술은 단순자궁적출술이 6예, 단순자궁적출술 및 림프절 절제술이 8예, 근치적 자궁적출 술이 13예에서 시행되었다. 모든 환자에서 외부 방사선치료가 시행되었으며 이 중 16예에서 ovoid를 이용한 질내조사가 추가되었다. 조사선량은 외부 방사선치료는 총 50.4 Gy를 조사하는 것을 원칙으로 하였고, 질내조사는 1회 분할선량 4$\~$5 Gy씩 4내지 5회 시행하였다. 추적기간은 6개월에서 95개월로서 중앙추적기간은 30개월이었다. 결과:FIGO 병기는 I-18예(67$\%$), II-1(4$\%$), III-8예(29$\%$)이었고, 조직분화도는 1$\~$14예(52$\%$), 2$\~$6예(9$\%$), 3$\~$7예(26$\%$)이었다. 수술 후 조직 소견상 조직분화도가 나쁠수록, 자궁근층의 침범 정도가 심할수록, 자궁부속기의 침범이있는 경우, 림프-혈관 침범이 있는 경우 골반내 림프절이 양성을 보이는 경우가 많았으나 통계적인 의미는 없었다. 추적이 종료된 시점에서 사망한 환자가 한 예도 없어 전체 생존율은 100$\%$이었으나 3예에서 병이 재발되어 3년 및 5년 무병생존율이 각각 89.6$\%$,, 76.8$\%$이었다. 재발 장소는 골반내, 대동맥방림프절, 폐를 포함한 다발성 등이 각각 I예 이었고 질 재발은 관찰되지 않았다. FIGO 병기(p=0.01), 림프-혈관 침범(p=0.03), 골반내 림프절 침범(p=0.0001)등이 무병생존을에 영향을 미치는 것으로 나타났고 조직분화도가 나쁠수록 무병생존율이 저조한 경향을 보였다(p=0.1). 치료부작용으로는 1예에서만 Grade 1의 직장출혈이 나타났으나 중등도 이상의 합병증을 보인 경우는 없었다. 결론 : 수술후 조직학적 소견에 따른 외부방사선치료 및 추가 질내조사는 국소재발을 억제하는데 효과적인 것으로 판단되며 결과적으로 생존율을 향상시키는데 기여하는 것으로 생각된다. 다만, 병기 및 조직분화도에 따라 질내조사를 추가하지 않는 경우 질 재발이 호발하는 환자군에 대한 연구가 더 진행되어야 할 것으로 판단된다.

Recombinant Tissue Plasminogen Activator Therapy for Aortic Thromboembolism in Four Dogs

  • Four dogs were brought to the Veterinary Medicine Teaching Hospital of Seoul National University (VMTH SNU) with a history of hind limb ataxia, three with pain, one without pain. Three of the four showed weak to absent femoral pulses and cold extremities. Thromboembolism was identified by ultrasonography in the external and/or internal iliac arteries. A thrombolytic agent, recombinant tissue plasminogen activator (rt-PA), was administered (0.5-1 mg/kg, every 60-120 min, 3-5 doses). Two dogs (Cases 2 and 3), which were instantly provided rt-PA treatment, survived 6 and 17 months, respectively, although hematemesis and hematochezia were observed during treatment. In the other two dogs (Cases 1 and 4), rt-PA was administered 4 and 28 days after the appearance of pelvic limb symptoms, which may have limited the benefits of the treatment. When rt-PA treatment is instituted instantly and the side effects are monitored thoroughly during treatment, a good prognosis might be expected in canine aortic thromboembolism. For this reason, we suggest that rt-PA treatment should be initiated immediately if thromboembolism is identified.

Prostatectomy Provides Better Symptom-Free Survival Than Radiotherapy Among Patients With High-Risk or Locally Advanced Prostate Cancer After Neoadjuvant Hormonal Therapy

  • Purpose: The purpose of this study is to compare the radiation therapy (RT) and radical prostatectomy (RP) of high-risk or locally advanced prostate cancer (PC) patients after neoadjuvant hormonal therapy (NHT). Materials and Methods: This retrospective study evaluated patients underwent RT (42 patients) or RP (152 patients) after NHT at a single center during 2003-2014. Times to biochemical recurrence (BCR), pelvic local recurrence (PLR), metastasis, clinical painful symptom progression (CPSP), castration-resistant PC (CRPC), and overall survival were compared between the RT and RP groups, after adjustment for TN stage, using the Kaplan-Meier method and log-rank test. Results: Significant inter-group differences were observed for age, Gleason score, initial PSA, and clinical and pathological T stages (all p<0.05). During a median follow-up of 71.7 months, the overall incidences of BCR, PLR, metastasis, CPSP, CRPC, and death were 49.5%, 16.5%, 8.3%, 7.7%, 7.7%, and 17.5%, respectively. The median times to BCR were 100 months for RT and 36.2 months for RP (p=0.004), although the median times were not reached for the other outcomes (all p>0.05). The independent predictor of CPSP was RP (hazard ratio, 0.291; p=0.013). Conclusions: Despite significantly different baseline parameters, RP provided better CPSP-free survival than RT among patients with localized high-risk or locally advanced PC.
