알파파 음악과 미술감상이 뇌졸중 환자의 손 기능에 미치는 영향 (The Effects of $\alpha$-Wave Music and Art Appreciation on Hand Function in Patient with Stroke)

  • Purpose:The purpose of this study was to investigate the effect of $\alpha$-wave music and art appreciation on hand function in stroke with hemiplegia. Methods:A total of 32 stroke with hemiplegia participated in this study experimental group(16 subjects) received $\alpha$-wave music and art appreciation with general neurologic therapy. Control group(16 subjects) received general neurologic therapy. All subjects were assessed for hand function(manual dexterity, power grip, pinch grip, two point discrimination(parm, finger), tactile sense(parm, finger) using a purdue pegboard, dynamometer, pinch gauge, two-point anethesiometer and semmes-weinstein monofilament wire. The data were analyzed using paired and independent t-test. Results:The results were as follows : 1. In the experimental group, manual dexterity were significantly increased between pre and post intervention(p<.05). 2. In the experimental group, tactile sesne in finger were sifnificantly increased between pre and post intervention(p<.05). Conclusion:The results of this study shows that $\alpha$-wave music and art appreciation affect the hand function of hemiplegic side with regard to manual dexterity and tactile sense.

  • Purpose: The purpose of this study was to investigate the effect of $\alpha$-wave music and art appreciation on hand function. Methods: A total of 22 university students participated in this study (10 males and 12 females). Twelve subjects received $\alpha$-wave music and art appreciation. The other subjects received neither. All subjects were assessed for hand function (manual dexterity, power grip, pinch, lateral pinch, tactile sense) using a Purdue pegboard, dynamometer, pinch gauge, and Semmes-Weinstein monofilament wire. The data were analyzed using paired and independent t-tests. Results: The results were as follows: 1. In the experimental group, manual dexterity and tactile sense were significantly increased between pre- and post-intervention (p<0.05). Within the control group, manual dexterity and power grip were significantly increased between pre- and post-test (p<0.05). 2. With regard to dexterity and tactile sense, the experimental group experienced a significant post-intervention increase compared to the control group (p<0.05). There was no significant difference in power grip, pinch, or lateral pinch changes between the two groups (p>0.05). Conclusion: The results of this study show that $\alpha$-wave music and art appreciation affect hand function with regard to manual dexterity and tactile sense.

Effects of Korean Computer-Based Cognitive Rehabilitation Program on the Memory in Healthy Elderly

  • The number of healthy older adults is rapidly increasing recently owing to the increase of the elderly population. Therefore, programs for improving the cognitive functions of these healthy seniors are actively being expanded. This study aimed to prevent the decline of cognitive function due to aging by applying a program enhancing cognitive functions to healthy older adults. The objective of this study was to evaluate the effects of Korean computer-based cognitive rehabilitation program (CoTras), which is commonly used in cognitive therapy for the aging, on the memory of the elderly. The subjects had scored at least 24 points in MMSE-K. CoTras was applied once a week (30 minutes) for one month. Electronic pegboard programs were used as an evaluation tool: order memory (difficulty=low) and location memory (difficulty=medium). The order and location memories were compared before and after the intervention. The Wilcoxon signed rank-sum test was used for the study at the significance level of ${\alpha}=.05$. The results showed that CoTras significantly improved order memory and location memory. Therefore, CoTras can be applied to the healthy elderly for improving that memory improvement training has a positive impact on healthy older adults result in the development of memory enhancement programs can be expanded in the future.

The Effect of Arm Movements in the during Standing Position on Lower Limb Global Synkinesis and Balance in Stroke Patients

  • Background: Stroke patients require arm movement exercising for various stimulations in standing position for various stimulations rather than in a sitting position because they require integrated skillful movements, such as stretching, holding, and controlling. Objective: This study was conducted to provide foundational clinical data about lower limb global synkinesis in stroke patients using arm movements in a standing position. Design: Randomized controlled trial. Methods: The subjects were divided into a control group (n = 10) and an experimental group (n = 10), and a pre-test was conducted to evaluate leg global synkinesis (GS) and balance. Intervention method is stretching an arm to hold a ball, repeating supination and pronation of the hand only while maintaining the arm extended as much as possible, repeating shoulder abduction and adduction while holding the pegboard. This was followed by a three-week intervention during which re-measurement was conducted in the same way as was done for the pre-test. Results: The control group showed a significant difference in GS and balance during plantar flexion (p<.05), and the experimental group showed a significant difference in GS and balance during all movements (p<.05, p<.01, respectively). There was a significant difference in GS and balance between the two groups during dorsiflexion (p<.05, p<.01, respectively). Conclusion: The findings demonstrate that human arm movements in a standing position can reduce GS in the affected limb, and balance can be improved by stimulating the surrounding tissues of the affected limb and changing them positively.

A Comprasion of the Activation of Mirror Neurons Induced by Action Observation between Simple and Complex Hand Movement

  • Purpose: We compared the activation pattern of the mirror neurons (MN) between two types of hand movement according to action observation using functional MRI. Methods: Twelve right-handed healthy subjects (5 male and 7 female, mean age $21.92{\pm}2.02years$) participated in the experiment. During fMRI scanning, subjects underwent two different stimuli on the screen: 1) video clips showing repeated grasping and releasing of the ball via simple hand movement (SHM), and (2) video clips showing an actor performing a Purdue Pegboard test via complex hand movement (CHM). paired t-test in statistical parametric mapping (SPM) was used to compare the activation differences between the two types of hand movement. Results: CHM as compared with the SHM produced a higher blood oxygen level dependent (BOLD) signal response in the right superior frontal gyrus, left inferior and superior parietal lobules, and lingual gyrus. However, no greater BOLD signal response was found by SHM compared with CHM (FWE corrected, p<0.05). Conclusion: Our findings provided that the activation patterns for observation of SHM and CHM are different. CHM also elicited boarder or stronger activations in the brain, including inferior parietal lobule called the MN region.

인지상태와 연령에 따른 손 기능의 비교 (Comparison of Hand Functions According to Cognitive Status and Age)

  • Purpose: The purpose of this study is to analyze the hand functions of elderly persons according to their cognitive status and age. Methods: A total of 65 persons voluntarily participated in the study. The subjects were divided into three groups: impairment cognitive group, normal cognitive group, adult group (persons in their twenties). Assessment of cognitive status was performed using a mini-mental state examination for Koreans (MMSE-K). Hand function was assessed using the Purdue pegboard test. The collected data were analyzed using a one-way ANOVA and Pearsonn Acorrelation. Results: There were significant differences in hand functions in the three groups. Post-hoc test results showed significant differences between each group. There were statistically significant differences in the correlation among hand functions, cognitive status, and age. The findings of this study suggest that hand functions have a positive correlation with cognitive status. However, a negative correlation was found between hand function and age. Conclusion: According to the study's results, hand functions are correlated with age and cognitive functions in elderly persons. This study suggests that hand rehabilitation with cognitive intervention increases hand functions in elderly persons.

가상현실 기반 작업치료프로그램이 학령기 지적장애 아동의 신체기능 및 학습능력에 미치는 영향 (The Effects of Virtual Reality-Based Occupational Therapy Program on the Physical Function and Learning Capacity of School-Age Intellectual Disability Children)

  • Purpose : The purpose of this study was to investigate the effects of a virtual reality-based occupational therapy program on the physical function and learning ability of intellectually disabled school-aged children. Methods : In this study, 20 intellectually disabled children of school age were randomly and evenly divided into experimental and control groups with children in the experimental group receiving a virtual reality-based occupational therapy intervention. The study adopted a pretest-posttest design. The intervention was conducted for a total of 12 sessions for six weeks, twice a week, and 50 minutes per session. As measurement tools, BOT-2 and grooved pegboard tests were used to compare physical function before and after the intervention program, and K-ABC was used to check changes in learning ability. Results : The occupational therapy program produced a significant improvement in both physical function and learning ability of the experimental group. A significant difference was observed between the experimental and control groups. Conclusion : This study confirmed the value and usefulness of virtual reality-based occupational therapy as a tool for enhancing the physical function and learning ability of intellectually disabled school-aged children. Based on the results, a variety of future studies are encouraged that would further test the effects of the occupational therapy program used here.

노인운전자 운전능력 선별검사로서 이중과제수행의 활용 가능성 연구 (A study on the possibility of using dual task performance as a screening test for driving ability of elderly drivers)

  • 본 연구는 노인운전자의 운전능력을 선별하는 검사로서 이중과제가 활용될 수 있는지 확인하고자 노인운전자의 이중과제 수행과 운전 행동간의 상관관계를 알아보았다. 총 32명의 정상 노인운전자를 대상으로 일반적 정보 및 운전 관련 정보를 조사하고 이중과제 수행정도를 알아보기 위하여 Y-DuCog(Yonsei-Dual task Cognitive screening test)를 평가하였다. 운전행동 수준은 K-DBQ(Korean-Driving Behavior Questionnaire)설문을 사용하였다. 연구결과 페그꽂기 과제와 동물이름 말하기를 활용한 이중과제 수행이 K-DBQ의 위반영역의 점수와 유의미한 상관관계를 보였다. 본 연구로 운전능력 선별검사로서 이중과제의 가능성을 확인할 수 있었다.

The Effects of Making Horcitultural Products and Selling Activities on Vocational Rehabilitation of Persons with Mental Disabilities

  • This study carried out a horticultural program for people with mental disabilities to determine suitability of the horticultural activity program and to actually apply to vocational rehabilitation. The program was carried out in 16 sessions from April 12 to June 7, 2018 with nine people with a strong will to participate in vocational rehabilitation at a health center for persons with mental disabilities. The tools used were the Horticultural Activity Performance Assessment (psychological behavior) and Purdue Pegboard Test that examines functions of hands and arms. The subjects sold the products they made during class time and obtained the profits. After the program, the scores increased with a significant difference (p = .000) in the psychological behavior area of the Horticultural Activity Performance Assessment. Hand and arm movement also showed continuous improvement along the program with a significant difference (p = .000), as well as finger dexterity also with a significant difference (p = .018). The net profit was 314,000 KRW (subtracting cost of goods 314,000 KRW from total sales 628,000 KRW). The profit was shared equally among the nine subjects, each receiving 34,900 KRW. Therefore this study proved horticultural activity class is suitable for vocational rehabilitation of people with mental disabilities proved by the positive effects. We also expect that the subjects will be able to make more profits if they learn more skills, since they are interested the process of making such profits.

겨울철 전자 기기 사용을 위한 전도성 보온장갑의 착용성 평가 - 손의 기민성과 사용성, 체온조절 반응을 중심으로 - (Performance of Conductive Gloves When Using Electronic Devices in a Cold Environment - Manual Dexterity, Usability and Thermoregulatory Responses -)

  • The present study evaluated the manual dexterity and usability of conductive gloves when operating touchscreen devices in the cold. Twelve male subjects (23.3±1.5 years in age) participated in three experimental conditions: no gloves, fabric conductive and lambskin conductive gloves. Manual dexterity was tested using both Purdue Pegboard (PP) and ASTM dexterity tests at an air temperature of 5℃ and air humidity of 30%RH. Glove usability was tested through the following touchscreen tests: tap, double tap, long tab, drag, flick, and multi-touch. The results showed that manual dexterity according to the PP (2.5 mm of a pin diameter) and ASTM tests (8 mm of a stick diameter) was worse for the two glove conditions than for the no glove condition (p<.005). PP dexterity was better for the fabric glove condition than for the lambskin glove condition (p<.05); however, there was no difference in ASTM dexterity between the two glove conditions. Hand and finger skin temperatures were higher for the glove conditions than the bare hand condition (p<.05), with no differences between the two glove conditions. The touchscreen usability was the best for the no glove condition, followed by fabric gloves (p<.05). Wearing either fabric or lambskin gloves diminishes hand dexterity while maintaining hand and finger temperatures at higher levels. For improved hand dexterity in dealing with small numbers, letters on a touchscreen in cold environments, we recommend wearing fabric conductive gloves rather than lambskin conductive gloves.
