A Case of Congenital Paraesophageal Hiatal Hernia in Infancy

  • Esophageal hiatal hernia is the hernia of a part of or the whole of stomach to posterior mediastinum through esophageal hiatus. Esophageal hiatal hernia can be classified as sliding hiatal hernia (type I), paraesophageal (type II), combined sliding and paraesophageal (type III), and complex paraesophageal (type IV). Type III and IV are clinically classified as paraesophageal hernia. The authors by chance found cystic mass filled with air in the lower lobe of the right lung during the treatment of mycoplasma pneumonia of 10 month-old patient. It was found to be paraesophageal hernia on the chest computed tomography and treated with the operation. As complex paraesophageal hernia is not usual among infants, the authors report it here with literature review.

입원과 수술시행의 적정성 평가 (An Appropriateness Evaluation of Cesarean Section, Cholecystectomy, and Admission in Pediatric Pneumonia)

  • The aim of this study was to evaluate the appropriateness of some kinds of surgery and admission, such as cesarean section (C/S), cholecystectomy, and pediatric pneumonia. For appropriateness evaluation, we ourselves developed some criteria, which were included in the category of explicit and linear criteria, with the assistance of specialists of relevant clinical field. The evaluation of appropriateness was performed by two family physicians. The major findings were as follows: 1. For cesarean section, 77.6% of deliveries were determined to be 'appropriate', but the level of appropriateness was not significantly different among hospitals and between hospital groups by size. The most freqeunt indication of C/S was repeated operation, followed by cephalopelvic disproportion (CPD). The labor trials for vaginal delivery among repeated C/S and CPD cases were performed in 24.5% of pertinent deliveries. 2. About 73.8% of cholecystectomy cases was appropriate to one of the surgical indications, without significant differences among hospitals. Of surgical indications, 'sufficiently frequent and intense symptom recurrence' was the most frequent, and 'confirmed acute cholecystitis' was the second. 3. Of children admitted due to pneumonia, only 57.4% of cases satisfied admission criteria, and the level of appropriateness of admission was different among hospitals. The common reasons fur admission were 'failure to initial treatment', 'suspected bacterial pneumonia', 'young infant', etc. We could find that there were differences of quality among hospitals in some procedures, especially in the pediatric pneumonia and labor trial before C/S, which suggested that the implementation of quality assurance activiteis would be necessary in this country. In this study, we used some simple and primitive research tools and the numbers of subjects and tracer procedures were limited. So advanced studios with plentiful subjects and more representative diseases or procedures should be tried.

성장 장애를 보인 영아에서 발현된 주폐포자충 폐렴 1례 (A Case of Pneumocystis carinii Pneumonia in an Infant with Failure to Thrive)

  • 주폐포자충 폐렴은 면역 기능이 억제된 환자군에서 발병률이 높은 감염성 질환으로 특히, 후천성 면역 결핍증이나 백혈병 등의 혈액 종양 환자군, 심한 영양 결핍 환자군, 장기 이식 환자군, 고용량의 부신 피질 호르몬 사용 환자군 등이 고위험군으로 알려져 있으며, 일차성 면역 결핍증에 이환된 환아에서 주폐포자충 폐렴의 증례가 보고되어 있다. 저자들은 상기 위험군에는 속하지 않으나, 10여일 정도의 수유부진 후 빈호흡을 보인 영아에서 흉부 방사선 소견상, 폐양측에 간질성 침윤이 관찰되었고 기관지내 삽관으로 흡인한 객담의 Gomori-methenamine silver 염색 검사상 주폐포자충이 검출되어 주폐포자충 폐렴으로 확진 후 TMP-SMX 14일 투여로 호전을 보인 예를 경험하여 보고하는 바이다.

국내 마이코플라스마와 클라미디아 감염 - 폐렴을 중심으로 - (Mycoplasma and chlamydia infection in Korea)

  • Pneumonia, which usually requires hospitalization for children, is caused by various pathogens. According to recent surveys, the prevalence of atypical pneumonia caused by Mycoplasma or Chlamydia has increased, especially in preschool children. Also, the evidence has been accumulated that Mycoplasma or Chlamydia infection is associated with asthma including both inception and exacerbation. Therefore, it is important to consider how the clinical aspects of Mycoplasma pneumonia have changed. In particular, Chlamydia pneumonia, which has not been prevalent in Korea, needs greater attention.

  • The major pathogens that cause atypical pneumonia are Mycoplasma pneumoniae, Chlamydophila pneumoniae, and Legionella pneumophila. Community-acquired pneumonia (CAP) caused by M. pneumoniae or C. pneumoniae is common in children and presents as a relatively mild and self-limiting disease. CAP due to L. pneumophila is very rare in children and progresses rapidly, with fatal outcomes if not treated early. M. pneumoniae, C. pneumoniae, and L. pneumophila have no cell walls; therefore, they do not respond to β-lactam antibiotics. Accordingly, macrolides, tetracyclines, and fluoroquinolones are the treatments of choice for atypical pneumonia. Macrolides are the first-line antibiotics used in children because of their low minimum inhibitory concentrations and high safety. The incidence of pneumonia caused by macrolide-resistant M. pneumoniae that harbors point mutations has been increasing since 2000, particularly in Korea, Japan, and China. The marked increase in macrolide-resistant M. pneumoniae pneumonia (MRMP) is partly attributed to the excessive use of macrolides. MRMP does not always lead to clinical nonresponsiveness to macrolides. Furthermore, severe complicated MRMP responds to corticosteroids without requiring a change in antibiotic. This implies that the hyper-inflammatory status of the host can induce clinically refractory pneumonia regardless of mutation. Empirical macrolide therapy in children with mild to moderate CAP, particularly during periods without M. pneumoniae epidemics, may not provide additional benefits over β-lactam monotherapy and can increase the risk of MRMP.

소아암 환자에서 2009 대유행 인플루엔자 A(H1N1) 감염의 임상적 고찰 및 계절 인플루엔자와의 비교 분석 (2009 Pandemic Influenza A(H1N1) Infections in the Pediatric Cancer Patients and Comparative Analysis with Seasonal Influenza)

  • 목적: 본 연구는 단일 기관에서 소아암 환자의 2009 인플루엔자 A(H1N1) [A(H1N1)pdm09] 감염 양상을 조사하고 이전의 계절 인플루엔자 감염과 비교 분석하고자 하였다. 방법: 삼성서울병원에서 2009년 8월부터 2010년 2월까지 A(H1N1)pdm09 감염, 2000년 1월부터 2009년 5월까지 계절 인플루엔자 A 감염이 확진된 소아암 환자의 의무기록을 후향적으로 분석하였다. 결과: 82명의 소아암 환자에서 A(H1N1)pdm09 감염이 확진되었다. 10명(12.2%)의 환자에서 하기도 감염증 또는 호흡기 외 감염이 발생하였다. 3명(3.7%)의 환자가 사망하였고 그 중 2명은 A(H1N1)pdm09 기여 사망이었다. 합병된 감염증과 관련하여 유의성을 가지는 위험인자는 감염의 시점(2009년 44-45주)과 병원 내 감염이었다. 이전의 계절 인플루엔자 A 감염과 비교하였을 때 임상적 특성에는 유의한 차이가 없었으나 A(H1N1) pdm09 감염에서 적극적인 항바이러스제 치료가 시행되었다. 결론: 소아암 환자에서 A(H1N1)pdm 감염은 경증부터 중증까지 다양한 임상 경과를 보였으며 이전의 계절 인플루엔자와 비교했을 때 임상 양상의 유의한 차이는 없었다.

Risk factors for childhood pneumonia: a case-control study in a high prevalence area in Indonesia

  • Background: Acute respiratory infections (ARIs), especially pneumonia, remain a major cause of infant mortality worldwide. In Indonesia, pneumonia is the second most common cause of infant and toddler deaths. Exclusive breastfeeding and basic immunization can protect infants and children from contracting pneumonia. Purpose: Our goal was to assess the risk factors for childhood pneumonia in regions with a high prevalence of pneumonia in Indonesia. Methods: This case-control study was conducted between March and April 2019. A total of 176 infants and toddlers aged 10-59 months were enrolled and selected from among patients who visited the community health center. Cases of pneumonia were diagnosed clinically based on the World Health Organization guidelines, and the control was nonpneumonia. Results: The risk factors for the diagnosis of pneumonia included no or nonexclusive breastfeeding (odds ratio [OR], 7.95; 95% confidence interval [CI], 3.52-17.94), incomplete basic immunizations (OR, 4.47; 95% CI, 2.22-8.99), indoor air pollution (OR, 7.12; 95% CI, 3.03-16.70), low birth weight (OR, 3.27; 95% CI, 1.19-8.92), and a high degree of wasting (OR, 2.77; 95% CI, 1.06-7.17). Other variables such as nutritional status (height-for-age z score), age, sex, and educational status of the mother were not risk factors for pneumonia. Conclusion: No or nonexclusive breastfeeding, incomplete basic immunizations, indoor air pollution, a history of low birth weight, and severe malnutrition were risk factors for childhood pneumonia. Breastfeeding was the dominant factor, while sex modified the relationship between exclusive breastfeeding and the incidence of pneumonia.

Increased risk of refractory Mycoplasma pneumoniae pneumonia in children with atopic sensitization and asthma

  • Purpose: A nationwide outbreak of Mycoplasma pneumoniae pneumonia (MP) refractory to macrolide antibiotics occurred in Korea during 2011. Steroid therapy has been reported to be both efficacious and well tolerated in pediatric patients with refractory MP. We compared clinical features and laboratory characteristics between children with refractory MP requiring steroid treatment and those with macrolide-responsive MP and evaluated the risk factors associated with refractory MP. Methods: We investigated 203 children who were admitted to our institution with MP from June to November 2011. Refractory MP was defined by persistent fever over $38.3^{\circ}C$ with progressive pulmonary consolidation or pleural effusion despite administration of appropriate macrolide antibiotics for 5 days or longer after admission. Steroid therapy was initiated on the fifth day after admission for refractory cases. Results: There were 26 patients with refractory MP requiring steroid therapy. The mean duration of steroid therapy was 5.4 days and most of the patients were afebrile within 24 hours after initiation of steroid therapy. The prevalence of refractory MP was higher in patients with pleural effusion, lobar pneumonia affecting more than 2 lobes, higher levels of serum lactate dehydrogenase, increased oxygen requirements, and longer duration of hospitalization. Atopic sensitization and history of asthma were also associated with refractory MP after adjusting for age and gender. Conclusion: Children with refractory MP had more severe pneumonia. Atopic sensitization and history of asthma may be risk factors for refractory MP requiring steroid therapy in Korean children.

폐렴에 속발한 가와사끼병 2예 (Two cases of Kawasaki disease following pneumonia)

  • 가와사끼병은 전신성 혈관염을 일으키며 드물게 폐렴 등 호흡기 질환의 임상 양상으로 발현될 수 있다. 가와사끼병은 아직 원인이 밝혀지지 않아 진단을 위한 검사 소견이 없고 전형적인 임상 양상에 의해 진단되어지므로 비전형적 임상 양상으로 나타날 경우 그 진단이 늦어질 수 있다. 항생제 치료에도 불구하고 폐렴이 악화되며 염증 반응이 증가되는 경우 가와사끼병을 의심하는 것이 중요하다. 저자들은 폐렴으로 발현된 가와사끼병 2예를 경험하였기에 보고하는 바이다.
