A STUDY ON THE SIZE OF THE PERMANENT TEETH (영구치의 치아크기에 관한 연구)

  • After 800 students of Chonbuk National University was examined, 86 people (male : 43, female : 43, mean age : 22.2 years old) was selected as a group of normal occlusion. From their gypsum cast, this conclusion was obtained. 1. Intra-observer measurement errors in buccolingual diameter, maxillary lateral incisors have somewhat bigger errors. In mesiodistal diameter, maxillary first molars and maxillary second molar have bigger numerical value. Mean errors of measurement are 0.051mm at buccolingual diameter of crown and 0.083mm at mesiodistal diameter. 2. Fluctuating asymmetry is 0.030 average in buccolingual diameter, and 0.037 average in mesiodistal diameter. Statistically there are no big differences. 3. Male has longer buccolingual diameter than female in every permanent teeth. Teeth which have statistical difference in buccolingual diameter are maxillary lateral incisor, maxillary canine, maxillary second molar, mandibular central incisor, mandibular canine, mandibular second premolar, and mandibular first molar. In mesiodistal diameter maxillary central incisor, maxillary canine, and mandibular first molar have statistically difference. 4. Tooth which has the biggest difference depending on gender is maxillary lateral incisor in buccolingual diameter and mandibular canine in mesiodistal diameter. 5. Both sexes have similar crown index. Male has bigger value of crown module measurement and crown area measurement in every tooth. Crown area considered as size of tooth from occlusal surface was bigger in male than in female statistically except some teeth, maxillary first premolar, mandibular lateral incisor, first premolar and second premolar.

  • Incidence of crown-root fracture due to traumatic injury, have been reported 3% in the permanent dentiton, 2% in the deciduous dentition. There are two treatment methods for crown-root fractured teeth with pulp exposure, when the fracture line was located under the alveolar crest. One way is the extrusion by orthodontic force the other way is intra-alveolar transplantation which occlusally repositioning of apical fragment in the alveolar socket. Since intra-alveolar transplantation has introduced in 1970s, it was practiced as alternative to orthodontic extrusion. As the result, this method may thoughted that had a good prognosis. As a result of trauma, completely crown-root fracture was occured in the maxillary right central incisor in this case. We couldn't reposition the deepest fracture line above the alveolar crest by the conventional surgical extrusion, because apical fragment was too short. Thus, after extraction of apical fragment, we repositioned it to the socket following demineralized freezed dried bone graft, which possible to support the apical fragment. At the 15-month recall examination, the root still showed normal mobility and there was not observed any in flammatory or replacement root resorption in the periapical radiograph.

Changes of Occlusion following the Placement of Preformed Metal Crown to the Permanent First Molar in Children and Adolescents (소아청소년에서 제1대구치의 기성금속관 수복 후 교합 변화)

  • The purpose of this study is to evaluate changes of occlusion following the placement of preformed metal crown in the permanent first molar affected by severe caries or hypomineralization. The study was conducted on 24 patients who met the criteria among the patients with occlusion in the permanent first molar reaching the occlusal plane. Restorative treatment was performed on one first molar using preformed metal crown. Before treatment, immediately after treatment, after 4 weeks, and after 8 weeks, the T-scan III was used for measuring distribution of bite force at the maximum intercuspal position, and overbite was measured with digital vernier calipers and change of dental midline was surveyed. After 8 weeks, questionnaires survey was conducted. In most cases, immediately after treatment, the distribution of bite force between the restored side and non-restored side of arch was reversed compared to before treatment, and the overbite was decreased. However, after 4 weeks, bite force of the left and right side was balanced and the distribution of bite force on the restored and non-restored permanent first molars had similar values. Also, the changed overbite and midline were returned to pre-treatment status and there was no significant temporomandibular joint and masticatory discomfort after treatment. In this study, it was confirmed that spontaneous occlusal equilibrium was achieved one month after the placement of preformed metal crown.

EVALUATION OF CONSERVATIVE TREATMENT BETWEEN AMALGAM FILLING AND STAINLESS STEEL CROWN IN PRIMARY TEETH. (乳齒(유치)에 있어서 Stainless steel crown Amalgam과 充塡(충전)과의 비교연구(比較硏究))

  • The study was performed to evaluate effects of Conservative treatment between amalgam filling and stainless steel crown. The selected subjects for this study was 350 children who have amalgam filling or stainless steel crown. The obtained results are as followings. 1. In amalgam filling teeth, 120 teeth (60.0%) showed marginal leakage, 57 teeth (28.5%) showed fracture of filling material and 28 teeth (14.0%) teeth showed periapical lesion. 2. In crowned teeth, 6 teeth (3.0%) showed hole on the surface of crown, 29 teeth (14.5%) showed avulsion of crown and 10 teeth (5.0%) showed periapical lesion. 3. After 12 months from examining date, 69 teeth (34.5%) showed sound condition in amalgam filling teeth and 155 teeth (75.2%) showed sound condition in crowned teeth.

  • Data derived from odontometric studies are useful in many areas. Especially crown diameter of primary teeth is important in not only anthropology but also clinical dentistry. The purposes of this study were to compare diameters of primary molars of Korean children with those of preformed stainless steel crown, and examine racial characteristics of primary molars of Korean children. The mesiodistal and buccolingual diameters of primary molars were measured on dental casts taken from 235 children(male 105, female 131), with digital calipers. And the data were compared with those of preformed stainless steel crowns. The results were as follows; 1. No significant difference was observed between the right and left members of antimeric teeth and all primary molars of male were larger than those of female. 2. The diameters of primary molars of Korean are smaller than those of Austrailian aborigines and there was significant difference between diameters of primary molars of Korean and those of other populations. 3. There were size differences between diameters of maxillary, mandibular first molar and preformed stainless steel crown than other primary molars, relatively.

THE IATROGENIC DAMAGES OF THE FIRST MOLARS FOLLOWING THE STAINLESS STEEL CROWN RESTORATION THE SECOND PRIMARY MOLARS (제 2유구치 기성금관 수복에 따른 제 1대구치의 의원성 손상)

  • Preformed stainless steel crown is an useful restorative material for the treatment of badly broken down primary teeth. However iatrogenic damage to adjacent teeth might occur during the process of tooth reduction. Such damages might lead to plaque accumulation and increase the risks of caries initiation. Especially the damage can make a problem in the first permanent molar. Purpose of this study was to investigate an iatrogenic damage to the first permanent molar during preparation of second primary molar for preformed stainless steel crown. Twelve children restored with preformed stainless steel crown to second primary molar were selected. Contact areas were separated with separation elastics, and tooth surfaces were cleaned. After taking negative impression using vinylpolysiloxane impression material, the specimens were examined by scanning electron microscope for the detection of iatrogenic damage. The prevalence of iatrogenic damage was 66.7% and variable appearances and So we can suggest that when preparing teeth for preformed stainless steel crown, we should be careful about adjacent teeth not to make an iatrogenic damage.

  • The purpose of this study was to elucidate the size and morphologic characteristics of maxillary primary first molars in Korean children using three-dimensional laser scanner and compare three-dimensional image with preformed stainless steel crown. Scanned three-dimensional images of dental cast taken from 132 children(male 62, female 70) by three-dimensional laser scanner(Breuckmann opto-Top HE100, INUS, Korea) were used. Mesiodistal diameter, buccolingual diameter, occlusogingival height and crown shape of each image were calculated by Rapidform 2004 program(INUS, Korea). The values were statistically compared by independent samples t-test with 95% of significant level. The results were as follows : 1. No significant difference in crown size was found between left and right maxillay primary first molar(p>0.05). 2, Significant difference in mesiodistal diameter, buccolingual diameter, buccal occlusogingival height was found between male and female (p<0.05), and crown size of male was bigger than that of female. 3. Average image of maxillay primary first molar was shaped three-dimensionally and measured. In comparison with 3M stainless steel crown, this image was similar with No.4 or No.5 SS crown in male, No.4 in female. In comparison with ILSUNG SS crown, this image was similar with No.5 in male, No.4 in female. 4 Mesiolingual line angle area, distolingual line angle area and buccogingival ridge were more obvious in average image than 3M stainless steel crown. ILSUNG SS crown was more square and had longer mesiodistal diameter than average 3D image.

ORAL REHABILITATION OF UNSTABLE OCCLUSION DUE TO SEVERELY DESTRUCTED FIRST PERMANENT MOLARS (제1대구치의 광범위 치관파괴로 인한 불안정한 교합의 재건)

  • A 9-year old boy visited Department of Pediatric dentistry, Chonbuk National University Hospital due to unstable occlusion. He had permanent 1st molars destructed severely by dental caries and lost vertical stop. He could not occlude in the same position on closing of the jaw since he could not find his centric occlusion. The treatment plan was made, which was oral rehabilitation with crown restorations on permanent 1st molars based on clinical and radiographic examinations and centric relation of the jaw. Treatment plan consisted of endodontic treatment, crown lengthening and restoration of the occlusal vertical dimension (OVD) with gold crown by 3mm-increase of OVD. After endodontic and periodontal treatments, temporary crowns made of acrylic resin were set for assessment of tolerance to an increased OVD. After a month for tolerance, final prosthetics were made and set temporarily. In a month, gold crowns were set finally. Immediately after setting, the patient showed anterior open bite, but at the appointment after 4 months, he showed normal overjet, overbite and stable occlusion.

One-visit Apexification Using MTA and Reattachment of a Crown-root Fractured Tooth with Severe Coronal Damage: A Case Report (심한 치관 손상이 발생한 치관-치근 파절 치아의 일회 내원 치근관형성술 및 파절편 재부착 : 증례 보고)

  • In dental trauma, reattachment of the original tooth fragment improves the reproduction of original tooth shape, texture, color, and radiolucency; thus, it provides good aesthetics. A 9-year-old boy was referred due to complicated crown-root fracture of the maxillary right central incisor. Although it had poor prognosis due to severe coronal damage and subcrestal fracture, reattachment of the tooth fragment was chosen due to the patient's age. One-visit apexification with mineral trioxide aggregate (MTA) was performed, followed by osteotomy and reattachment of the tooth fragment with post placement. Regular observation revealed no clinical signs or symptoms and no radiologic complications.

  • Dentinogenesis imperfecta is an inheritable disease of dentinal defect, generally is inherited as a single autosomal dominant trait. It has a prevalence of 1 in 8000 with the trait, and no significant difference between male and female, with involvement of the primary and permanent teeth. Shields proposed three types of Dentinogenesis imperfecta. Affected teeth have various discoloration, separation of enamel rapid destruction of underlying dentin, and severe attrition. Radiographically, the teeth have cervical constriction, bulbous crown, thin root, obliteration of the root canals and pulp chambers, and periapical lesions in a sound tooth. The objective of treatment is rehabilitation of the esthetics, the masticatory function, and the vertical dimension of occlusion. In these cases, two pediatric patients reported to the Kyungpook University Pediatric clinic, with a chief complaint of discolored teeth and severe attrition. As a result of clinical and radiographic exam, it was diagnosed as Dentinogenesis imperfecta. The posterior teeth were restored with Stainless Steel Crown, and the anterior teeth were restored with composite resin veneering.
