Inter- and Intra-rater Reliability of Pattern Identification Using Nasal Endoscopy for Allergic Rhinitis (비내시경을 활용한 알레르기 비염에 대한 한의학적 변증 지표의 관찰자간, 관찰자내 신뢰도 연구)

  • Objectives : We performed a pilot study to investigate inter- and intra-rater reliability of pattern identification using nasal endoscopy for allergic rhinitis(AR). Methods : Eight experts of ophthalmology, otolaryngology and dermatology of Korean medicine evaluated 20 nasal endoscopy photograph cases of AR patients with pattern identification index using nasal endoscopy for AR including the nasal membrane color(pale / hyperemia), nasal membrane humidity(dryness / dampness), rhinorrhea(watery / yellow), and membrane edema (atrophic / edematous) on nasal endoscopy. Results : Intra-rater agreement(%) and Kappa coefficient was generally from 'moderate' to 'good'(% agreement: 73.13-90% / Kappa coefficient: 0.547-0.748). Inter-rater agreement(%) and Kappa coefficient was also from 'moderate' to 'good' (% agreement: 65-85% / Kappa: 0.475-0.778) except 'humidity(dryness / dampness)' item (% agreement: 55.98% / Kappa: 0.340). In findings of subgroup analysis according to affiliation of raters, Inter-rater agreement(%) and Kappa coefficient of raters in same affiliation was higher than inter-rater agreement(%) and Kappa coefficient of raters in different affiliation except 'dryness / dampness' item. Conclusions : It is necessary to improve objectivity and reproducibility of pattern identification using nasal endoscopy for allergic rhinitis(AR) through the development of detail-oriented criteria and enhanced training of clinicians with development of standard operating procedures(SOPs).

The Development of Instrument of Korean Medical Pattern Identification and Functional Evaluation for Five Organ (한의 오장 변증·평가 설문지 개발을 위한 신뢰도 및 타당도 평가)

  • The aim of this study was to identify the reliability and the validity of Five Organs Questionnaire (FOQ). In this study, 335 data was collected from early adulthood of A-city, D-city and C-city in South Korea from Dec. 2016 to Jan. 2017, and the collected data was analyzed with SPSS 23.0 software. The result showed that the Cronbach's ${\alpha}$ of five organs was 0.775-0.853. The reliabilities of test and retest Intra Correlation Coefficient (ICC) for the five organs were .891-.929 in pattern identification and .874-.930 in functional evaluation, respectively. Each organ was divided by two factors, and the factor loadings of Liver, Heart, Spleen, Lung and Kidney were 52.973%, 54.534%, 57.060%, 53.803%, and 46.337%, respectively. The functional evaluation of five organs was associated with self-rated health status as r was -.443 to -.583(p<.001), and quality of life as r was -.5.17 to -.716(p<.001), respectively. The test-retest diagnostic agreement was 67.2% and Kappa was 0.562. This study revealed that FOQ is a reliable and valid questionnaire.

Suggestion of Standard Process in Developing Questionnaire of Pattern Identification (한의 변증 설문지 개발 표준프로세스 제안)

  • The aim of this study was to suggest the standard process in developing Questionnaire of Pattern Identification (QPI). The process in developing QPI was researched from validated and developed questionnaire and the standard process in developing QPI was suggested through review of the experts in research, statistics and clinics. Check list was also provided. The number of QPI reviewed in this research was 17(4 in disease in Korea Medicine, 5 in Pathological symptoms, 6 in Sasang constitutional Diagnosis, and 2 in etc), The standard process in developing QPI consisted of 11 phage and 33 check lists. 1) Composition of Research Member(3check lists), 2)Set up of the Aim(5), 3) Review for advanced research(3), 4) Finding an Important Index(3), 5) Review of item selection(4), 6) Developing the questions using items(5), 7) Developing Draft of Questionnaire(2), 8) 1st Survey of Reliability and Validity(2), 9) Revision and Correction of Item(1), 10) 2st Survey of Reliability and Validity(2), 11) Completion and Application(3). This study suggests the standard process in developing QPI for the first time in Korea. This following step may help A new QPI development.

Preliminary Research for Developing a Pattern Identification Tool for Idiopathic Pulmonary Fibrosis (특발성 폐섬유화증 변증 도구 개발을 위한 기초연구)

  • Objective: The purpose of this study was to develop a standard tool for identifying idiopathic pulmonary fibrosis patterns. Methods: Textbooks, published literature, and references with comments on patterns were reviewed. Using the Delphi method, we determined pattern identification based on the advice of a committee consisting of 11 Korean respiratory internal medicine professors. Results: Four pattern identifications were selected by the Delphi method: qi difficiency (氣虛), yin difficiency (陰虛), phlegm dampness (痰飮), blood stasis (瘀血). The tool was developed in a question-and-answer format containing 38 questions. Conclusions: An IPF pattern identification tool that can analyze IPF patterns for standardized diagnostics was developed with the consent of experts. Further research is needed on its reliability.

  • In this paper, we aimed to develop associative pattern recognizer based on neural network for aircraft identification. For obtaining invariant feature space description of an object regardless of its scale change and rotation, Log-polar sampling technique recently developed partly due to its similarity to the human visual system was introduced with Fourier transform post-processing. In addition to the recognition results, image recall was associatively performed and also used for the visualization of the recognition reliability. The multilayer perceptron model was learned by backpropagation algorithm.

Preliminary Study to Develop an Instrument for Pattern Identification and Evaluation for Chemotherapy-induced Peripheral Neuropathy (CIPN) (항암화학요법 유발 말초신경병증 변증평가도구 개발을 위한 기초연구)

  • Objectives: The aim of this study was to develop a standard instrument of pattern identification and evaluation for chemotherapy-induced peripheral neuropathy (CIPN).Methods: The advisory committee for this study was organized by 12 professors of traditional Korean medicine from the Korean Association of Traditional Oncology. The items and structure of the instrument were designed based on a review of previous publications. We revised the instrument in consultation with the advisory committee and received additional advice via email.Results: We divided the symptoms and signs of CIPN into four pattern identifications: wind arthralgia (風痺), cold arthralgia (寒痺), dampness arthralgia (濕痺), and arthralgia of the deficiency type (虛痺). We obtained the mean weights to reflect the standard deviations from each symptom of the four pattern identifications, which were scored on a 5-point scale by the advisor committee. After we obtained the answers to discrimination between variable symptoms (變症) and ordinary symptoms (素症) from the 12 experts, we gained the final weight from the combination of the ratio of pattern identification to the number of total answers of the advisory committee and the mean weight.Conclusions: The Instrument on Pattern Identification and Evaluation for CIPN was developed through a discussion between 12 experts. There was a limitation that the validity and reliability of this instrument have not been proven. However, the significance of this study was that it is the first Instrument on Pattern Identification and Evaluation aimed at assessing CIPN in traditional Korean medicine.

  • This study involves an experiment for the cognitive experiment design and the human reliability in software engineering. Its overall objectives are to analyze common-cause human domain error and reliability in human-software interaction. A laboratory study was performed to analyze software engineers' task behavior in software production and to identify software design factors contributing to the effects in common cause failure redundancy. Common-cause model and its function were developed, then the main experiment using programming experts was conducted in order to define a new cognitive paradigm, in the aspects of identification, pattern recognition, and behavior domain for human reliability and quality control in software development. The results and analytical procedures developed in this research can be applied to reliability improvement and cost reduction in software development for many applications. Results are also expected to provide guidelines for software engineering quality control and for more effective design of human-software interface system.

Preliminary Study to Develop the Instrument on Pattern Identification for Lung Cancer (폐암 변증도구 개발을 위한 기초연구)

  • This study is to develop a standard tool for pattern identifications in Korean Medicine for Lung Cancer. The advisor committee for this study was organized by 12 Korean Medicine professors from the Korean Association of Traditional Oncology. The item and structure of instrument were based on review of published literature. We took the consultation twice from the advisor committee and additional advices by e-mail correspondences. We divided the symptoms and signs of lung cancer into 6 pattern identifications. - lung spleen both deficiency (肺脾兩虛), lung kidney both deficiency (肺腎兩虛), dampness phlegm obstructing the lung (濕痰蘊肺), qi stagnation blood stasis (氣滯血瘀), yin deficiency toxin heat (陰虛毒熱), and heat toxin accumulation (熱毒蘊結). We obtained the mean weights which reflected the standard deviations from each symptoms of the 6 pattern identifications which were scored on a 5-point scale by 12 experts. We designed the Korean medicine pattern identification tool for lung cancer. It was composed of 57 questions in the question-and-answer format. Though there are some limits that this study is not proved about validity and reliability, the instrument is meaningful and expected to be applied to the subsequent research.

The Evaluation of Instrument for Cold-Heat & Deficiency-Excess Pattern Identification of Dementia (치매의 한열허실 변증 지표문항에 대한 예비분석)

  • Objectives: The study aimed to test the validity and reliability of the questionnaire for cold-heat & deficiency-excess pattern identification of dementia and establish a new version of the questionnaire. Methods: Mean, standard deviation, skewness, internal consistency, correlation and t-test of the 26 items derived from previous study was analyzed in 20 dementia patients. The items with Cronbach-${\alpha}$ coefficient below 0.7 were modified. Thus, we established a new version of the questionnaire consisting of 20 items. Results: Cronbach-${\alpha}$ of each cold, heat, deficiency and excess questionnaire was 0.662, -0.229, 0.722 and 0.778, respectively. The correlation coefficient between cold, heat, deficiency and excess was less than 0.4 and correlation coefficient between dementia and cold, deficiency was 0.518. On t-test, the t-value of cold, deficiency and dementia was -2.196. Conclusions: The results indicated that cold-heat, deficiency-excess questionnaires showed satisfactory discriminant validity. In addition, there was correlation between dementia and cold, deficiency. Finally, we established a new version of the questionnaire for cold-heat, deficiency-excess pattern identification that consisted of 20 items.

  • Objectives : The prevalence of allergic rhinitis (AR) is 10-40% worldwide and there are many demands for Traditionl Medicine (TM) treatment for AR. Pattern identification (PI) is essential process in diagnosis and treatment of TM, however, objective assessment measurement for PI of AR in TM is lacked. Methods : PI questionnaire for AR was developed in 2008 by specialists in the department of Otolaryngology of TM, based on systemic and nasal symptoms and signs. However, that questionnaire had many problems in application to clinical practice and has since been revised based on several studies and delphi method. Consequently, the PI questionnaire for AR version 3.0 has been developed. Results & Conclusions : This questionnaire classifies patients with AR as possessing lung-heat, lung-cold, or spleen qi deficiency based on nasal symptoms and general conditions of AR patients. This is first questionnaire for pattern identification of AR. We plan to conduct a validation and reliability study, and revise the questionnaire based on the results of this study.
