Factors Related to Diabetic Patients' Quality of Life: The 8th Korean National Health and Nutrition Examination (1st Year, 2019) (당뇨 환자의 삶의 질 관련 요인: 제 8기 1차년도(2019년) 국민건강영양조사)

  • Objectives: This study aims to examine diabetic patients' quality of life by using the data of the 8th Korea National Health and Nutrition Examination Survey (1st year, 2019), identify the factors related to this, and utilize the results as basic data for intervention that can improve diabetic patients' quality of life. Methods: For the research subjects, this study extracted 624 patients who were diagnosed with Diabetes by a doctor from the total sample of 8,110 participants of the 8th Korea National Health and Nutrition Examination Survey. The SPSS(version25.0) program was used for the analysis of the collected data. Then, this study used a backward elimination multiple regression analysis method that applied complex sample, to examine the factors related with the finally estimated quality of life. Results: The results of this study revealed that diabetic patients' quality of life was related with gender, age, occupation, restriction of activity, subjected health status. The final model explained 35.7% of the variance (Wald F=28.210, p<.001). Conclusions: In order to improve the quality of life of diabetic patients, it would be desirable to provide differentiated management by developing a customized intervention strategy that takes into account gender, age, and occupation. When managing diabetic patients, the state, local governments, and hospitals should include content that prevents and copes with restrictions on activities that may occur due to disease. In addition, it is required to prepare a strategy to induce positive perception of the subject's own health status.

Analysis of Nursing Interventions and Nursing Outcomes of the Patients with Liver Transplantation (간호중재와 간호결과 분석- 간이식 환자를 중심으로 -)

  • Purpose: This study was designed to investigate the survey of analyzing nursing interventions and nursing outcomes of the patients with liver transplantation. Method: The subjects of this study consisted of 83 patients and fifteen nurses who took care of them. The methods of this study were retrospective and descriptive survey. Result: The mean age of patients was 42.7 years and the subjects were 62.7% males. 56.7% of these patients were diagnosed with liver cirrhosis of B type. The mean duration of hospitalization was 48.6 days. Ninety-five nursing interventions were performed at least daily. The most frequent used interventions were "environmental management: comfort", "medication administration: parenteral", "cough enhancement", and " oxygen therapy". Even though SGOT level to identify nursing outcomes was decreased, SGOT was not within normal limits. Therefore, even after discharge of patients, There is a need to take care of them carefully. Conclusion: These findings revealed the significance and need of nurse practitioners who performed professional nursing intervention for the patients with liver transplantation. Especially, it is necessary needs to develop the nursing intervention programs for comfort.

Prevalence of Musculoskeletal Pain in Patients With End-Stage Renal Failure Receiving Hemodialysis (말기 신부전 환자의 근골격계 통증 발생 실태 -혈액투석 환자를 대상으로-)

  • Purpose: The objectives of this study were to investigate the current status of the pain in and the dysfunction related to the musculoskeletal system experienced by patients with end-stage renal failure receiving hemodialysis. Methods: A questionnaire survey was given to 107 patients undergoing hemodialysis in M Hospital. The survey was conducted by using a brief pain inventory(BPI) to investigate the body parts in pain, degree of pain, difficulty in daily life, correlation with hemodialysis and the degree of satisfaction with their own health. Results: The investigation of the patients undergoing hemodialysis showed that 89(83.2%) of the 107 patients experienced pain in their musculoskeletal system. Their pain's incidence was highest in the knees(46.1%) and 68.5% of the patients reported that the most severe pain they had experienced within the past 24 hours was at a level between "moderate" and "severe." The limitation of daily life by pain is high most as 47.7% in the accomplishment of works. Conclusion: Most hemodialysis patients experience musculoskeletal related pain that affects their daily life. Hemodialysis patient management programs should include an assessment and management of such pain.

A survey on netritive value of general hospital diet and appetite of hospitalized patients in Pusan city (부산시내 종합병원 일반식의 영양가 및 환자식욕상태에 관한 조사연구)

  • To study the nutritional value of general hospital diet, the author surveyed in 5 hospitals which are located in Pusan. Thd information on appetite and dietary habits was taken from the questionnaire results of 166 patients and 185 non patients. The survey of nutritional value was conducted on 104 males and 61 females hospitalized patients, taking three consecutive days in each hospital. RESULTS : 1. The average nutritive value of general diet per patient per patient per day served in hospitals was 2446.601$\pm$57.31 kcal, 100.23$\pm$8.18g protein, 42.35$\pm$6.46g fat, 413.35$\pm$31.46g carbohydrate. 2. The average percentage of diet consumed by patients per day was 69.7$\pm$4.5%. 3. The average nutrient intake by patients per day was 2025.94$\pm$194.46kcal, 77.04$\pm$6.38g protein, 31.71$\pm$6.80g fat, 356.20$\pm$26.40g carbohydrate. 4. The meals were taken more regularly by patient group than by non patient group. 5. The psychological appetite was lower in patient group. 6. The patients thought hospital diet as reliable and were willing to take diets well in order to get well as soon as possible.

Review of Reliability and Validity of Medical Service Experience Survey: Focused on the Differences by Type of Medical Institutions (의료서비스경험조사의 신뢰도 및 타당도 검토: 의료기관 특성별 차이를 중심으로)

  • Background: The efforts to build more "people centered," "patient centered" health system has been emerging all over the world. Aligning with it, the Korean government is conducing the survey called "Medical Service Experience Survey (MSES)." There are critics, however, that MSES is not scrutinizing the medical experiences of patients in various healthcare settings. For this reason, this study aims to perform an empirical analysis of the differences in answers of patients responding to various healthcare settings. Methods: There are two steps in this study. First, explanatory analysis is conducted to compare the tendency of statistical concentration on questionnaires by divided healthcare settings. Second, confirmative analysis is carried out to evaluate the construct validity, reliability, and discriminant validity of the questionnaire in each healthcare setting. The raw data of MSES, which was conducted in 2020 by the Ministry of Health and Welfare in Korea and the Korean Institute for Health and Social Affairs is used. Results: As a result of exploratory factor analysis for all outpatients, the items were classified into four factors statistically: "doctor experience," "nurse experience," "outpatient service experience," and "patient satisfaction." It was confirmed that the reliability of all factors extracted was secured. However, for patients who visited hospitals, questionnaires related to personal privacy, such as "experiences on medical staffs considering physical exposure" or "experiences related to personal information exposure," were answered in conjunction with items of "nurse experience." Besides, patients responded that administrative elements of medical services, such as "experiences of comfort in medical institutions" and "experiences of satisfactory administrative services," were related to the items of "nurse experience." The answers of patients who visited traditional medical hospitals and clinics about "doctor experience" and "nurse experience" were not discerned statistically, and the answers to "doctor experience," "nurse experience," and "medical institution experience" were entangled with the responses of patients who visited dental hospitals and clinics. On the other hand, as a result of the confirmatory factor analysis, it was found that the inquiries of MSES generally had intensive validity. Conclusion: The collection of objective and scientific data is the prominent component to enlighten the patient-centered healthcare system alongside with change of the worldwide paradigm of measuring the healthcare system performance as follows the transition of perspective of health care from provider-centered to patient-centered. This study empirically shows that the patient experience can vary as the healthcare settings. Furthermore, to make an advance in measuring the experience of patients with medical services, this article proposes the deliberate consideration of the different kinds of healthcare settings and articulate design of the survey.

Research for the Development of an Integrated Medical System for Idiopathic Pulmonary Fibrosis: Based on an Analysis of Questionnaire Survey in Doctor and Patient Groups (특발성 폐섬유증의 통합의료 협진 모델 개발을 위한 의료진 및 환자 대상 설문조사 연구)

  • Objective: In this study, a questionnaire survey was conducted for doctors and patients to obtain basic data necessary for the development of an integrated medical system for idiopathic pulmonary fibrosis (IPF). Methods: Questionnaires were developed separately for doctors and patients through an expert group meeting. The survey subjects were recruited online and offline, and finally, responses from 231 doctors and 59 patients were used for statistical analysis. Results: The most important parts in the treatment of IPF for both doctors and patients were the "improvement of respiratory symptoms," "improvement of quality of life," and "prevention of disease progression." Antifibrotic agents were prescribed at a high rate, and 100% of the specialists in Western medicine (WM) and 45.8% of patients reported experiencing side effects. As for the additional payment costs that patients considered as affordable for an integrated medical system, "under 50,000 won (about 38$)" was reported the most in both doctor and patient groups. With regard to the reasons for their reluctance to recommend an integrated medical system for IPF, specialists in WM responded highly to "uncertain evidence for treatment effectiveness." Regarding complementary and alternative medicine therapies that can be beneficial in patients with IPF, "lifestyle management," "diet management," "herb," "relaxation therapy," and "psychotherapy" were ranked high in both doctor and patient groups. Conclusions: In this study, a questionnaire survey on IPF was conducted to review actual treatment status, analyze shortcomings, and identify considerations for the development of an integrated medical system for IPF in the future.

The Nursing Needs of Post-Surgical Colon Cancer Patients at Discharge (대장암 수술 환자의 퇴원 시 간호요구도 조사)

  • Purpose: This study was a descriptive survey of nursing needs for post surgical colon cancer patients at discharge. Method: A survey was done utilizing questionnaires about the nursing needs a target sample of 61 patients who had colon cancer surgery during April May 2006 in a general hospital in Seoul. Results: Levels for treatment & prognosis were the highest in all domain, high in order of psychological support & stability, complications & discomfort, diet, daily life style, recovery & health promotion, and support system. Patient factors affecting nursing needs were age, job, duration of colon cancer and handling of stoma. Conclusion: Using discharge education for colon cancer patients based on the results of this study, nurses should focus on the domains of treatment & prognosis, psychological support & stability and complication & discomfort, and should tailor teaching content to be specified for age, job, duration of colon cancer, and handling of stoma.

A Study on the Wearing Conditions of Mastectomy Bras and Breast Prosthesis (유방암 환자용 브래지어에 대한 착용실태조사)

  • There are approximately 1,000 patients who undergo mastectomies each year in Korea. However, there is no company who manufactures mastectomy brassieres and breast prosthesis in Korea. The purpose of this study is to evaluate the wearing conditions of the imported mastectomy brassieres with breast prosthesis and to develop the better-fitted and comfortable mastectomy brassieres in reasonable price. 254 patients participated in this survey. In the results of our survey, 117 out of the 254 patients were wearing mastectomy brassieres and breast prostheses, which have been imported mostly from USA. The degree of satisfaction at wearing mastectomy brassieres was low. The patients generally feel hot when wearing mastectomy brassieres, specially in summer and also pointed out the heavy weight of breast prosthesis and the discomfort in wearing mastectomy brassiere for a long period of time.

Pain Evaluation in Patients with Knee Osteoarthritis (퇴행성슬관절염 환자의 통증에 대한 연구)

  • We performed this study to assess pain in patients with knee osteoarthritis. Participants were 120 Patients(men 26 and women 94) with knee osteoarthritis. Pain evaluations were conducted using survey composed of 12 items by interview with oriental medicine doctor. Collected data were analyzed by IBM SPSS Statistics 19.0. Women were 3.8 times more than men(21.7% vs 78.3%). Approximately 80% of participants were overweight and obesity. Descending stair is most painful action for knee osteoarthritis patients. The average degree of knee pain was more than moderate. 5. People diagnosed as arthritis before had the much higher knee pain than never diagnosed before. This study showed female gender, overweight and obesity were associated with the risk of knee arthritis and participants complained knee pain more than moderate. It is hoped that our findings provide information that help the treatment of knee osteoarthritis.

Self-care Behavior and Metabolic Control in Diabetic Patients - Based on the 3rd National Health and Nutrition Survey - (당뇨병환자의 자기간호행위와 대사조절 - 2005년 국민건강영양조사에 의한 -)

  • Purpose: We analyzed the self-care behavior and metabolic control rates in diabetic patients based on the National Health and Nutrition Examination Survey in Korea (2005). Methods: The study group included 311 patients who were over 126 mg/dL on the FBS test. Data were analyzed using SPSS PC WIN 12.0. Results: The average score of self-care behavior was $12.08{\pm}1.05$ points, and significantly different according to DM treatment status, disease duration (years), admission experiences (within 1 year) and education about DM. Glucose, total cholesterol, triglycerides were decreased in self care subjects, but not significantly. Conclusion: An educational program for diabetic patients would help maintain metabolic control rates to improve self-care behavior.
