Prevalence of Allergic Rhinitis between Urban and Rural Residents in a Local Community (일개 시 지역의 도심과 농촌 주민의 알레르기비염 유병률)

  • Objectives: The purpose of this study was to examine the prevalence of allergic rhinitis between urban areas in the adjacent areas to a steel industrial complex and rural areas and the impact of pollutants in the industrialized city on allergic rhinitis. Methods: From July 28 to August 9 of 2008, 1,043 residents of urban and rural areas in a local community had enrolled in health screening and questionnaire survey. One thousand thirty-three patients also underwent a skin prick test. Prevalence rates of allergic rhinitis were calculated according to residential areas, and the used statistical analysis were Fisher's exact test and chi-square test. Results: In a survey, the fraction of adults, who complained of sneezing, nasal obstruction and rhinorrhea, during a recent 1 year period, showed the significant difference between urban (30.5%) and rural areas (22.4%). The fractions of positive skin prick tests were not different between two areas in each age group. The prevalence of allergic rhinitis was 8.4% in urban areas and 6.9% in rural areas. Considering the age groups, the adults group only showed the significantly higher prevalence of allergic rhinitis in urban areas (8.2% vs. 3.7%). Conclusions: Unlike the children and adolescents groups, the prevalence of allergic rhinitis in adults group was higher in the industrialized urban areas.

The Effect of Fumanet Exercise Program for Life care on Cognition Function, Depression in Dementia (라이프케어 증진을 위한 후마네트 운동프로그램이 치매노인의 인지기능, 우울기능에 미치는 영향)

  • Purpose: As dementia progresses, cognitive function decreasing leads to memory loss, speech degradation, time and space degradation and judgment degradation, which causes difficulties in carrying out tasks related to daily life. It was said that community-based non-drug intervention therapy for early dementia patients was important to participate in entertainment treatment, including activities such as awareness and exercise therapy, exercise rehabilitation, aerobic exercise, and art. Methods: This study conducted 15 experimental and 15 control groups(experimental group : Fumanet exercise, control group : general occupational therapy) for eight weeks at the Daycare Center in Gyeonggi-do to find out the impact of the Fumanet exercise program on cognitive and depression functions of the elderly. The pre-post evaluation used KGDS, MMSE. Results: There were significant differences between the two groups in the function of menopause, memory recall, attention concentration and calculation, and depression, and no significant results were obtatined in memory registration, language function, understanding and fracture. The Fumanet movement was judged to be effective in improving cognitive function and reducing depression for the elderly with dementia. Conclisions: The Fumanet movement was judged to be effective in improving cognitive function and reducing depression for the elderly with dementia.

Fragile X Syndrome and it's Association with Autism (약체X염색체 증후군과 자폐증과의 연관)

  • The fragile X syndrome, which is considered to be synonymous with the Martin-Bell syndrome, is a relatively common form of X-linked mental retardation. The syndrome seems to occure in many different ethnic groups and its prevalence among mentally retarded males has been estimated to be in the order of 2 to 6%. The karyotypic hallmark of the syndrome is made up with a pronounced constriction near each tip of the long arm of the X chromosome(fragile site), shown in vitro only under conditions in which thymidylate production is blocked(lowered folate levels). Special culture media are needed to demonstrate this constriction site. Major clinical features associated with the syndrome include macroorchidism, large or prominent ears, significant emotional and behavioral dysfunctions such as hyperactivity, self-injury, lack of eye contact and social interaction, schizophrenia, autism, etc., and speech and language dysfunctions ranging from nonverbal to verbal speech with moderate to severe expressive language delays. Some have minor clinical features in common such as an increase in birth weight high forehead, prognathism, increased head circumference in infancy and childhood which did not persist into adult life. The recent research findings have shown that the fragile X syndrome is associated with infantile autism. Many patients with the fragile X syndrome fulfill the diagnostic criteria for infantile autism. Therefore it is recommendable that any patient with developmental delays and autism or autistic manifestations should have a chromosomal analysis, including fragile X examination. In the present review, historical aspects, incidence, and clinical features are presented. Recent anecdotal reports of the association with autism and the clinical improvement following high dose folic acid treatment will be discussed.

An Open-Label Study of the Improvements in Clinical Symptoms and Neurocognitive Functions in Korean Children and Adolescents with Attention-Deficit Hyperactivity Disorder after Treatment with Metadate CD (국내 주의력결핍 과잉행동장애 아동 및 청소년에서 메타데이트CD의 임상증상 및 신경인지기능 개선 효과에 대한 개방 연구)

  • Objectives : This study aimed to investigate the efficacy and safety of Metadate CD (MCD) when given to Korean children and adolescents with attention-deficit hyperactivity disorder (ADHD). We also explored the effects of the drug on diverse neuro-cognitive functions. Methods : Ninety-one subjects with ADHD (mean age 8.6${\pm}$2.2 years) were recruited at 6 outpatient clinics in Seoul, Korea. We used the ADHD Rating Scale (ARS), Clinical Global Impression (CGI), and comprehensive attention test (CAT) to measure the drug's effects. Results : After 0.92${\pm}$0.32mg/kg/day of MCD were administered for 57.4${\pm}$7.6 days, there was a 48.5% reduction in the mean total ARS scores (p<.001). Fifty-seven subjects (64.8%) showed either much improved or very much improved outcomes on the CGI-Improvement scale. The CGI-Severity scale also decreased from an average of 4.7 to an average of 2.9 (p<.001). Errors and response time standard deviations of the CAT, sustained attention test-to-response tasks, the flanker test, and divided attention test scores decreased after treatment (p<.05). The forward memory span of the spatial working memory test scores increased (p<.05). Thirty-five patients (39.8%) experienced side effects, of which the most common were headache (14.8%), nausea (12.5%), and anorexia (9.1%). Conclusion : This open-label study suggests that MCD is effective and safe in improving the symptoms and neurocognitive functions of Korean children and adolescents with ADHD.

No Association Between Single Nucleotide Polymorphisms in Distal-Less Homeobox-6 (DLX6) and Autism Spectrum Disorders (ASD) from the Korean Male Population (한국인 남성에서 자폐스펙트럼장애와 DLX6 유전자 단일염기다형성간 연관성 연구)

  • Objectives : Autism spectrum disorder (ASD) is a neurodevelopmental disorder that is characterized by abnormalities of social functioning, communication and behavior. The association of the 7q21-34 region with ASD has been reported. The DLX6 gene, which is located at the 7q22 region, is one of the positional and functional candidate genes for ASD. We found that there is no association between DLX6 polymorphisms and ASD in the Korean male population. Methods : We selected three single nucleotide polymorphisms (SNPs) that might be implicated in the change of the DLX6 gene expression. The genomic DNA was collected from the venous blood of 147 male controls and 179 male patients with ASD. The genotypes of the selected SNPs were determined using the Illumina GoldenGate assay, and the statistical analyses were performed using HapAnalyzer software and SAS Enterprise. Results : We found no association of the three SNPs in the DLX6 gene with ASD in the Korean male population. Conclusion : Our study suggests that the three SNPs in the DLX6 gene are not associated with ASD, and we need to analyze the previously reported regions for their associations with ASD.

DENTAL TREATMENT UNDER GENERAL ANESTHESIA IN A PATIENT WITH LOWE SYNDROME : A CASE REPORT (로웨 증후군 환아의 치아우식 치료 증례 보고)

  • Lowe syndrome is X-linked gene disorder, characterized by cataracts, renal dysfunction and brain abnormalities. Oral healthcare of young patients with Lowe syndrome could be easily neglected due to the uncooperative behavior or other systemic condition of the child. We are presenting a case of successful treatment under general anesthesia with uncooperative child with Lowe syndrome. A 3-year old boy with Lowe syndrome visited Seoul National University Dental Hospital for multiple caries. He had been suffering from congenital cataract, medullary nephrocalcinosis and hypotonia. Multiple caries were observed from clinical and radiographic examination. Concerning behavior management problem and possibility of aspiration due to hypotonia, dental treatment under general anesthesia was planned. Left upper primary first molar was extracted to prevent further infection. Other teeth were treated with Stainless steel crown or composite resin restoration based upon the extent and the severity of dental caries. Under general anesthesia, dental procedure was carried out successfully and safely. Considering uncooperative behavior and other medical conditions of the patient, general anesthesia could be effective. Home oral care and periodic visit to dental clinic should be emphasized to the caregiver of patient with Lowe syndrome, considering the susceptibility of dental caries and other oral manifestation.

  • Pierre Robin syndrome (PRS) is characterized by the triad of congenital mandibular hypoplasia, glossoptosis and cleft palate. Infant PRS patients are frequently suffering from upper airway obstruction, gastroesophageal reflux and growth retardation caused by above mentioned problems. We report a dental caries treatment of 3-year old girl with Pierre Robin syndrome with multiple caries. The cause of multiple caries was mainly presumed as patient's eating habit caused by her general condition. She had some feeding problems and had history of gastric tube. She was still using milk bottle and took more than an hour to finish a meal. The treatment was performed under general anesthesia considering patient's condition; mild autism, poor cooperation and respiratory problem due to micrognathia. Severely affected upper incisors were treated with pulp treatment and restored with zirconia crown for esthetic purpose. Lower incisors were treated with pulp treatment and restored with composite resin. Upper right first primary molar was restored with stainless steel crown and other primary molars were treated with composite resin. There were no postoperative complications. According to her parents, the patient's compliance to oral hygiene management was greatly improved after the treatment since she was very pleased with the esthetic result and highly motivated by her looks. The treatment without sedation or general anesthesia would be possible once the airway is improved as the mandible grows.

Esthetic Root Coverage for Gingival Recession (심미적인 결과를 얻기 위한 치근 피개술)

  • In dental esthetics, soft tissue plays an important part, probably very large portion of it. A clear understanding of the periodontal tissues and its management around teeth and implants help us to develop concepts for a modern dental treatment that addresses the needs of demanding patients in regard of esthetics and durability. When we talk about esthetic, we can say that one of the most important element is a harmonization with gingiva (soft tissue) called 'Pink Esthetic' As for the pink esthetics, gingival line(contour) takes most of the influence on esthetic result; it consists of labial gingival level, interproximal papilla height, and a line that connects them. In the gingival recession, labial gingival level and gingival contour move to the apical portion, and the root area is exposed. It leads to the unesthetic result. Root coverage technique is classically used to treat gingival recession (marginal tissue recession) of natural teeth. It is an essential technique on periodontal plastic surgery part. It is also a very useful technique to recover soft tissue problems in implant dentistry. So, root coverage technique must be mastered for a good implant esthetic result. The general overview of root coverage procedures will be discussed with step by step explanation to get more esthetic result.

A Study on the Degrees of Death Orientation and Terminal Care Performance of Nurses (간호사들의 죽음에 대한 성향과 임종간호수행 정도)

  • This study has been done for the purpose of investigating the degrees of death orientation and terminal care performance. The factors related to these two variables, and the relationship between death orientation and terminal care performance. The subjects of study were 128 nurses who implemented nursing care for terminally ill patients at C University Hospital in Kwang Ju city. The data were collected from March 13 to 19, 1996, by means of Death Orientation by Thorson Powell(1988) and Terminal Care Performance Scale by researcher. The data were analysed by t-test, ANOVA, Duncan test and Pearson's correlation coefficient. The Results of this study were summarized as follows : 1. The mean score of death orientation was 61.4. The degree of death orientation showed no significant difference depending on the general characteristics of nurses. 2. The mean score of terminal care performance was 45.5. In comparison of the degree of terminal care performance among three domains, the mean score of each item tended to show higher degrees in order of 'Psychological domain(2.4)', 'Physical domain(2.2)', 'Spiritual domain(1.9)'. 3. The degree of terminal care performance showed significant differences in age(F=11.48 p=.0001), marital status(t=10.49 p=.0015), religion(t=5.01 p=.0270), period of clinical experience(F=10.30 p=.0001) and ward unit(F=3.73 p=.0036). The degree of terminal care performance in physical domain showed significant differences in age(F=7.26 p=.0010), marital status(t=9.72 p=.0023), period of clinical experience(F=7.03 p=.0013), ward unit(F=6.23 p=.0001). The degree of terminal care performance in psychological domain showed significant differences in age(F=8.73 p=.0003), marital status(t=4.22 p=.0419), religion(t=5.59 p=.0196), period of clinical experience(F=6.36 p=.0023), ward unit(F=3.33 p=.0075). The degree of terminal care performance in spiritual domain showed significant differences in age(F=8.30 p=.0004), marital status(t=10.45 p=.0016), religion(F=5.41 p=.0216), period of clinical experience(F=8.80 p=.0003). 4. The relationship between the degrees of death orientation and terminal care performance showed no correlation(r=-.026 p=.7746).

A Classification of Death Orientation of Cancer Patient's Family Members : A Q-Methodological Approach (암환자 가족의 죽음 태도 유형에 관한 연구)

  • This study was designed to identify, describe and classify orientations of cancer patient's family members to death and to identify factors related to their attitudes on death. Death to the male is understood as a comprehensive system and believed to be highly subjective experience. Therefore attitude on death is affected by personalities. As an attempt to measure the subjective meaning toward death, the unstructured Q-methodology was used. Korean Death Orientation Questonaire prepared by Kim was used. Item-reliability and Sorting-reliability were tested. Forty five cancer patients' family members hospitalized in one university medical center in Seoul were sampled. Sorting the 65 Q-itmes according to the level of personal agreement ; A forced normal distribution into the 11 levels, were carried out by the 45 P-samples. The demographic data and information related to death orientation of the P-sample was collected through face to face in depth interviews. Data was gathered from August 30 till September 22, 1995. The Z-scores of the Q-items were computed and principal component factor analysis was carried out by PC-QUANL Program. Three unique types of the death orientation were identified and labeled. Type I consists of twenty P-samples. Life and death was accepted as people's destiny, They firmly believed the existence of life after life. They kept aloof from death and their concern was facing the and of the life with dignity, They were in favor of organ donation. Type II consists of Nine P-Samples. They considered that death was the end of everything and did not believed the life after life. They were very concerned about the present life. Type III consists of Sixteen P-samples. They regarded the death as a natural phenomena. And they considered that the man is just a traveller and is bound to head for the next life which is believed to be free of agony, pain or darkness. They neither feared death nor its process. Their conserns were on the activities to prepare themselves for the eternal-life after death. Thus, it was concluded that there were three distinctiven type of attitudes on death among cancer patient family members, and their death attitudes were affected by demographic and socio-cultural factors such as sex, education, and religion.
