Patients' Perception of Quality of Health Service and Utilization considering convergence factors: A Survey of Elderly Patients of Public Health Centers, Seoul, Republic of Korea (융합적 요소를 고려한 의료서비스 질 인식도와 의료이용의 관련성: 서울시 보건소를 이용하는 노인을 대상으로)

  • Health service quality is increasingly emphasized, and primary care service is no exception in this. This study identified patients' perception of quality considering convergence factors associated with increased utilization among patients over the age of 65 using community health centers. A total of 307 respondents were included in the analysis. The results showed that positive perceptions on facility and environment, patient-centered care, technical knowledge and skills of physicians, and emotional support with nurses and non-medical workers were statistically and significantly associated with higher utilization. In order to increase utilization of primary care service at community health centers, they need to manage and improve these factors.

Improving Hospital Referral System based on Perception of Delivery of Healthcare by Enrollees (국민건강보험 이용자의 의료전달체계 인식에 따른 진료의뢰 개선 방안)

  • This study was carried out to suggest hospital referral system improvements based on health insurance subscriber perceptions of the delivery of healthcare. Health insurance subscriber outpatients (n = 207) referred from a stage 1 medical institution to the S university hospital located in Seoul responded to the study survey. The study's item reliability is reliable as the Cronbach's alpha coefficient was greater than 0.7. This study results showed that 5.9% of patients were referred from a higher stage hospital to a same stage hospital. The main factor attracting patients to S university hospital were physician reputation and confidence. The highest factor ($4.40{\pm}.92$) was xxxx. In addition, survey respondents reported that the concentration of patients in extra-large hospitals in Seoul ($4.24{\pm}.97$) was an important issue, and the issue with the highest priority for improvement ($4.05{\pm}1.02$). A positive correlation was detected between the recognition and improvement of delivery of healthcare (p < 0.01). Based on the results, we suggest that improvements in the delivery of healthcare should focus on patients rather than suppliers of national health insurance or other insurers. Quality of life of Middle -Aged Persons Who have cancer (중년기 암환자의 삶의 질에 관한 연구)

  • This descriptive study was under taken to explore relationships among the quality of life, health locus of control and perceived state of health persons with cancer to contribute theoretical understanding about these phenomenon of interest to the quality of nursing care. The subjects of this were 200 persons with cancer (100- in patients and 100- out patients), both male and female, between 30 and 59 years of age. Data were obtained using a convenience sample technique from two university hospitals in seoul from August, 1989, to June, 1990. The instruments used for this study were the Quality of life scale developed by Ro, You - Ja and the Health Locus of Control scale developed by Wallston & Wallston. Data were analyzed using a SAS program for ANOVA, t-test, Schefffe test, Pearson Correlation Coefficients and Stepwise multiple regression. The results were as follows : 1. The scores on the quality of life scale ranged from 95 to 191 with as mean of 147.85(range 47 to 235). The Mean scores(range 1-5) on the different dimensions were family relationships 3.50, relationships with neighbours 3.48, self - esteem 3.17, physical state and function 2.99, economic life 2.93 and emotional life 2.91. 2. Significantly higher scores on the quality of life and demographic characteristics were as follows : the quality of life for women(t=2.80, p= .006), for those without complications(t=2.54, p= .013), and for those who perceived their illness as mild(F=4.85, p= .009). Higher scores on quality of life were correlated with the following : 1) emotional state and the age group 50-59(F=3.43, p= .34). 2) economic life and higher income(F=6.72, p= .002), those without complications(t=2.68, p= .00), and those who perceived their illness as mild(F=3.11, p= .05). 3) self-esteem and marriage(F=3.64, p=.028), those without complications(t=2.18, p=.03), and those who perceived their illness as mild(F=7.72, p=.000). 4) physical state and funciton and the age group 30-39(F=4.65, p=.010), those without complications (t=2.00, p=.05), and those who perceived their illness as mild(F=3.38, p=.04). 5) family relationship and those who live with their spouse(t=2.82, p=.005). 3. There was a significant positive correlation between the subjects perceptions of their current state of health and the quality of life score(r=.4364, p=.0001). 4. There was no relationship between Locus of control and quality of life in this sample. 5. Stepwise multiple regression analysis showed that: 1) the perception of current health status was the main predictor and accounted for 20.11% of the total variance. 2) sex and educational level accounted for an additional 21.71% of the total variance. 6. The quality of life and the perception of their current health status of these patients with cancer were generally lower than those of healthy adults as noted in previous studies. In conclusion, the quality of life for these cancer patients was generally low especially in regard to their emotional state. The current perceived state of health, sex, complications and perceived degree of illness were important variables relatiog to quality of life.

Orthodontic pain control following arch wire placement; a comparison between pre-emptive tenoxicam and chewing gum: a randomized clinical trial

  • Background: Pain during fixed orthodontic treatment can have a detrimental effect on patient treatment compliance. To overcome this, there is a definite need to establish the best pain-relieving methods suitable for orthodontic patients in terms of efficacy and use. The objective of this study was to compare the effect of chewing gum and pre-emptive tenoxicam on pain after initial archwire placement and to evaluate the pain perceptions of orthodontic patients in the two groups while performing various functions at specific time intervals. Methods: Forty-two patients were selected and randomly divided into two groups: group A (chewing gum) and group B (pre-emptive tenoxicam). Pain perception was documented by patients immediately; at 4 h; at bedtime on the day of archwire placement; the next morning; at 24 h; and at bedtime on the 2nd, 3rd, and 7th day after the initial archwire placement. Pain scores were noted during fitting of the posterior teeth, biting, and chewing using a visual analog scale. The data obtained were subjected to statistical analysis. Results: Group A showed a significant increase in pain until the next morning while fitting the posterior teeth, biting, and chewing [36.2, 52.0, 33.4, respectively]], followed by a gradual decrease by the 7th day. Group B showed a significant increase in pain at bedtime on biting, with a peak value of 47.5. Pain on chewing, fitting posterior teeth, peaked the morning of the next day (100.0, 45.0). The Freidman test showed a statistically significant difference with a p-value of < 0.01. Higher pain scores were observed while chewing and biting compared with that while fitting the posterior teeth in both groups. The overall comparison of pain control between the two groups was not statistically significant [P > 0.05] between the two groups. Conclusions: Chewing gum was not inferior to pre-emptive tenoxicam. Thus, chewing gum is a non-pharmacological alternative to analgesics for orthodontic pain control that eliminates the chance of adverse reactions and can be used in the absence of adult observation.

Expert Opinion Questionnaire About Chest CT Scan Using A Negative Pressure Isolation Strecher in COVID-19 Patients: Image Quality and Infection Risk (COVID-19 환자에서 음압격리들것을 이용한 흉부 CT 검사에 대한 전문가 의견 설문: 영상품질과 감염위험)

  • Purpose To survey perceptions of certified physicians on the protocol of chest CT in patients with coronavirus (COVID-19) using a negative pressure isolation stretcher (NPIS). Materials and Methods This study collected questionnaire responses from a total of 27 certified physicians who had previously performed chest CT with NPIS in COVID-19 isolation hospitals. Results The nine surveyed hospitals performed an average of 116 chest CT examinations with NPIS each year. Of these, an average of 24 cases (21%) were contrast chest CT. Of the 9 pulmonologists we surveyed, 5 (56%) agreed that patients who showed abnormalities in serum D-dimer required contrast chest CT. All 9 surveyed radiologists agreed that the image quality of the chest CT with NPIS was sufficient for CT image interpretation regarding pneumonia or pulmonary embolism. Furthermore, in our 9 surveyed infectionologists, 5 (56%) agreed that a risk of secondary infection in the CT room after temporary opening of NPIS could be prevented through a process of disinfection. Conclusion Experienced physicians considered that the effects of NIPS on chest CT image quality was minimal in patients with COVID-19, and the risk of CT room contamination was easily controlled.

A Study on Practitioner's Perceptions on Early Screening of Autism Spectrum Disorder (자폐스펙트럼장애의 조기선별에 대한 관련 분야 종사자의 인식 조사)

  • Objectives: The purpose of this study is to investigate the professional knowledge and perceptions of the early screening of Autism Spectrum Disorder (ASD) in practitioners who have contact with patients with ASD. Methods: A survey was carried out among 674 practitioners in total, where practitioners are defined as those who work at primary medical centers, public institutions, educational institutions and treatment institutions. The survey was carried out both online and offline, and it mainly focused on 1) knowledge about ASD symptoms, 2) knowledge about the early screening of ASD, 3) measures taken after ASD detection, 4) thoughts on the development of early screening tools for ASD, and 5) the current status of ASD treatment. The data collected were analyzed through descriptive statistics, analysis of frequency and cross tabulation analysis using SPSS WIN 22.0. Results: The results of this study suggest that the practitioners were not aware of the exact symptoms of ASD and their professional knowledge and the environment for early screening were insufficient. Furthermore, very few and inappropriate measures were taken after the detection of ASD. In addition, there was a high demand for early ASD screening tools to be used on site and, regarding treatment, the significance of the implementation of evidence based treatments as well as the continuity of relevant research came to the fore. Conclusion: It seems that there is a lack of knowledge and perception of the early screening of ASD and that education and training among practitioners is urgently required. This issue is discussed in more detail in the paper.

Defining the Core Competencies of the Nurses in A Tertiary Hospital and Comparing Different Units based on Their Respective Characteristics (일 종합전문요양기관 간호사의 핵심역량 도출 및 근무지 특성별 중요도 인식 비교)

  • Purpose: The purpose of this study was to define the core components required of nurses to provide quality nursing care to patients and analyze the significance of these components in the hospital's various units to maintain a high level of competence among nurses. Method: The study evaluated 3 categories included 35 subcategories with 148 core components that were derived from literature review and interviews of nursing professionals. The nursing professional category included 18 subcategories with 98 components, the organizational culture category included 4 subcategories with 16 components, and the temperament-attitude category included 13 subcategories with 34 components. The study included 335 nurses with more than one year of hospital experience and measured disparities among different hospital units. The data was analyzed with SPSS-Win 10.0, differing perceptions of the importance of general traits among the participants were measured using standard deviation, and differing perceptions of the importance of professional traits among the participants were assessed using ANOVA and subsequently with the Bonferroni Test. The reliability of the aforementioned research tools were evaluated using the Cronbach's ${\alpha}$. Result: The results of the study were as follows: 1. Among the three categories, temperament-attitude category was perceived to be the most important, followed by nursing professional category and organizational culture category. Among the ten most important subcategories within the three categories, safety and infection prevention as well as responsibility were perceived to be the most important, followed by promotion of physiologic adaptation, document management and presentation, self-control, ethics, observance law, coping with emergency, humanity, and medication. 2. The relative importance of category associated with the core competencies within the hospital unit were as follows : 1) The units that rated nursing professional category as being the most significant were: internal medicine, surgical unit, mother-child unit, emergency room, intensive care unit, and operating room, in that order. 2) The units that rated the organizational culture competencies as being the most significant were: mother-child unit, internal medicine, surgical unit, emergency room, operating room, and intensive care unit, in that order. 3) The units that rated temperament-attitude category as being the most significant were: internal medicine, surgical unit, emergency room, mother-child unit, operating room, and intensive care unit, in that order.

Effect of infection control practice on exposure prevention of dental hygienist (치과위생사의 감염관리 실천이 노출 예방에 미치는 영향)

  • Objectives : In the field of dental hygiene, infections pose a serious problem. This issue has left many patients and dental staff exposed to microbes with potentially far-reaching effects. The purpose of this study was to find solutions which could improve Dental health policies and promote improved methods for the control of infections. This survey was conducted between March 3, 2008 and March 30, 2008, in the metropolitan area. The research was carried out in dental clinics and dental hygienists were surveyed through a questionnaire. Methods : Collected data was examined using the SPSS 14.0 program, using frequency, mean and standard deviation analysis, T-test, one-way ANOVA, Scheffe's test and Duncan's test. Further analysis was given using Logistic Regression. Results : The Performance of Infection Control by dental hygienists in Dental Clinics was shown at $2.96{\pm}0.42$. Disinfection, sterilization and the wearing of masks is high at $3.65{\pm}0.64$, $3.64{\pm}0.65$. The perception of the importance of infection control is higher among dental hygienists than dentists. Of the dental hygienists surveyed, 83.9% were exposed to at least one accident while at work. Age and years of experience were important in relation to infection control practices. The rate of infections and the number of accidents experienced related to the dental hygienists performance of infection control. The performance of infection control is influenced by the dental hygienists own perceptions, and the perceptions of their dentists. The dental hygienist's working conditions and their beliefs related to infection control were shown to influence their performance of infection control practices. Conclusion : At the conclusion of this investigation, it was determined that systematic refresher training of infection control should be delivered through further education and various media. Dental health care workers should be encouraged to practice these action items presented in the training. For successful implementation of infection control in every dental health-care setting, it is highly demanded that effective safe-guard tools, strategic support and standardized action items against infection problems are developed.

  • The purpose of this study was to obtain basic data for dental implants prosthetic maintenance care after analysis perceptions of dental implants. A question was used to questionnaire by 210 dental patients at five dental clinics in the Jinju-city. In conclusion, 1. The knowledge level about implant is 'Never'(35.7%). 2. A feeling of unrest during implant surgery is 'pain'(37.6%). 3. The expectation level about dental implants function is 70% of natural teeth(36.2%). 4. The main function of implant teeth is 'mastication'(70.5%). 5. The implant hygiene supplies for the dental implants maintenance is 'proximal brush'(60%). 6. The important maintenance attitude for the dental implants is 'regularly dental examination'(42.9%). 7. The interval regularly for implant examination is 'six monthly'(44.8%).

A Study on the College Student's Recognition and Consumption of Antioxidant in Seoul Area (대학생의 항산화에 대한 인식 및 항산화 식품 섭취 실태 -서울 지역을 중심으로-)

  • This research contains awareness of antioxidant and intakes of antioxidant foods for the present evaluate college students in Seoul, 375 patients were investigated. The subjects, the woman college student more than male's responded, showed a uniform distribution in the allowance, grade and the most type of residence is living apart from their family. All male and female college students recognize a lot about health, but male college students had higher than female students interested in the health, on the other hand, female college students had higher than male college students for the health efforts for the promotion of a healthy. Awareness about the oxide and active oxygen is moderate level, but knowledge about active oxygen is low level, they responded that active oxygen was caused when received stress or do strenuous exercise. General Health Functional Foods recognized that the usual intake, but intake of antioxidant was when the activity was caused by active oxygen. They recognized that the antioxidant effect is anti-aging and vitamin, wine and tea, were perceived as antioxidant foods, are popularly known. Usually, people was initially recognized through the internet in university or high school, they desire to obtain information was high but the effort to gain understanding and knowledge about antioxidant are relatively low. The result of comparing the difference of natural antioxidant foods and antioxidant healthy functional foods, recognizes of effects and absorption rate are similar, but recognizes that natural food intake is better recognition in the economics and health functional food is better recognition in the easy intake and nature foods was more preferred than functional foods because of nature friendly. Trying to intake of antioxidant foods is low, but people is expected anti-aging and fatigue recovery through the intake of antioxidant food. People think that intake is irrelevant to the season, but summer is higher than other seasons. Showed that efficacy perceptions about health supplements are higher, but efficacy perceptions about antioxidant health supplements when ingested are at a moderate level, which is lower, due to low antioxidant for understanding. Antioxidant functional health food intake will be affected the gifts or the people around them and purchase is also more influenced by surround people than themselves. So showed that most college students prefer natural antioxidant foods than antioxidant health supplements, in case of ingested antioxidant health supplements also showed that it was consumed by surround people than personal will.
