시력교정수술환자의 병원선택요인과 만족도 (Hospital Selection Factors and Patient Satisfaction of Refractive Surgery Patients)

  • The study compared refractive surgery patients' hospital selection factors and satisfaction, through this to see the factors influencing patients' satisfaction, aiming at 235 patients visiting two Eye hospitals. The research results are followed. First, target group's hospital selection factors appeared facilities and environment factor, reliability factor, personal service factor, and accessibility factor in the order. Second, patients' satisfaction appeared reliability factor, facilities and environment factor, and personal factor in the order. Fourth, in the result comparing patients' expectation on hospital selection and satisfaction score, all factors of facilities and environment, personal service, and reliability had high satisfaction score than expectation. Third, in the result of factors influencing refractive surgery patients' satisfaction, in surgery determination period, surgery expenditure, and hospital selection factors among characteristics relating to refractive surgery, facilities and environment, personal service, and reliability appeared as satisfaction influencing factors. In conclusion, the study found that hospital selection factors and satisfaction influencing factors by refractive surgery patients' surgery type are considered to be able to be used to establish marketing strategy in the fields excluded from insurance application, the researches to improve patients' satisfaction in patient-oriented medical market should be continuously proceeded.

치과의원 내원환자가 평가한 치과 의료서비스 질과 관련 요인 (Factors related to dental service quality evaluated by dental clinic patients)

  • Objectives : This study purposed to identify factors related to dental service quality perceived by dental clinic patients, and for this purpose. Methods : The data conducted a questionnaire survey of 362 patients who had visited a dental clinic in Daejeon twice or more in August 2010 and aged over 20 during the period from the 1st to 31st of August 2010 and obtained results as follows. Results : The patients' general characteristics, perceived dental service quality was higher in older ones, married ones, high school or lower school graduates, civil servants, self-employed ones, and housewives, and the differences were statistically significant. according to the patients' dental clinic use characteristics, perceived dental service quality was higher when the stay in dental clinic was short, and the difference was statistically significant. Conclusions : Perceived dental service quality was related to the patients' general characteristics and dental clinic use characteristics. therefore, dentists and dental hygienists should make efforts to be more patient-oriented and to create pleasant dental care environment.

Aprepitant Regimen의 고용량 Cisplatin 유발 오심 및 구토 예방 효과 (The Effect of Aprepitant Regimen on the Prevention of High-Dose Cisplatin-Induced Nausea and Vomiting)

  • Aprepitant is a substance P/neurokinin-1 (NK1)-receptor antagonist that was approved in 2003 for prevention of CINV. In addition, updated anti-emetic guidelines that include the aprepitant regimen have been published by NCCN and ASCO. However there is scarce clinical data in Korea. The prospective study was performed to evaluate the prevention of high dose cisplatin induced nausea and vomiting in all patients who started high-dose cisplatin-based chemotherapy at our hospital. We checked the nausea severity and vomiting episodes by calling patients within 4 to 5 days after chemotherapy. The retrospective study was performed to compare the prevention of CINV in solid tumor patients who switched their anti-emesis regimen from the standard regimen to the aprepitant regimen. In aprepitant regimen, aprepitant was added to the same anti-emetic regimen used during previous cycles. We checked the nausea, vomiting grades and adverse events in electronic medical records (EMR). In prospective study, 195 patients were included in the analysis. 88.2% of patients achieved a complete response (no emesis and no rescue therapy). In retrospective study, 54 patients were reviewed. With aprepitant regimen, nausea and vomiting grades were improved in 22 patients (40.7%) and in 9 patients (16.7%), respectively. Compared with standard regimen, addition of aprepitant provided superior prevention against CINV in Korean patients receiving highly emetogenic cisplatin-based chemotherapy. Moreover, aprepitant significantly prevented CINV in patients who received the standard regimen to prevent CINV in previous chemotherapy cycles.

A Comparative Analysis on Korean Medical and Western Medical Service Usage Tendency of Rotator Cuff Surgery Patients - Using HIRA's Patients Sample Data

  • Objectives: To lay the foundation for future research into Korean Medicine treatment for Rotator Cuff repair surgery patients by analyzing Korean Medical and Western Medical service utilization and treatment duration. Methods: Data sampling was performed on 2015's HIRA patient data (confidence level of 97%) to analyze patients' Korean Medical and Western Medical service usage tendency. Sampled groups were divided into two groups: i) Patients who completed their treatment within five months after the rotator cuff surgery (termination group), ii) Patients who were treated for more than five months after the surgery (continuation group). Then the patients' Korean Medical and Western Medical service usage tendency was investigated and information of these patients are arranged. Results: Out of 1,453,486 patients who were gathered for sampling, 2,461 patients in total had gone through rotator cuff repair surgery. The termination group had 517 patients and the continuation group had 541 patients. The proportion of patients who visited a Korean Medicine clinic was lower in the termination group than the continuation group. Conclusion: The continuation group received more treatments (both in Western Medicine and Korean Medicine) and spent more on medical expenses compared to the termination group. Further research is highly recommended for more efficient Western Medicine and Korean Medicine treatments and reduced medical expenditure.

특정 암 환자를 PHR 파일롯 서비스 (The PHR Pilot Service for Specific Cancer Patients)

  • PHR(Personal Health Records : 개인평생건강기록) 은 진료정보를 제공하는 것을 기본서비스로 하며, 환자와 서비스 제공자에게 유용한 서비스를 제공해야 한다. 본 연구는 진료정보의 제공 외 사용자에게 적합한 서비스를 발굴하는데 목적이 있다. 서비스 대상은 암환자로 한정하였고 PHR 서비스 발굴을 위하여 기존 사례 분석, 의료진 인터뷰, 환자 설문을 진행하였다. 그 결과로 발굴된 서비스는 3가지이다. 첫째, 환자에게 치료계획(patient's clinical pathway)을 제공하고 두 번째 온라인 질의응답기능 제공, 세번째 환자 본인의 상태를 입력하는 기능 제공이다. 발굴된 명지 PHR 서비스는 웹과 앱(안드로이드)으로 약 3개월간 파일롯 테스트를 하였고 유용성을 확인하였다. 향후 상업화된 PHR 서비스가 되기 위해서는 치료계획의 등록을 위한 표준화 및 사용자 편의성을 고려한 모델이 되어야 할 것이다.

의료서비스 이용에서 보호자의 서비스품질인식이 환자의 서비스품질인식 및 구매행동의도에 미치는 영향 (Effects of guardians' service quality perception on the service perception and behavioral intention of patients in healthcare service)

  • In this study, we examined the effects of guardians' service quality perception(G-SQP) on patients' service quality perception(P-SQP) leading to patients' behavioral intention(P-BI) and the effects of G-SQP on guardians' behavioral intention(G-BI). To investigate the causalities of the variables, we collected national wide samples of 144 hospitals, 1456 patients and 1455 guardians of the patients and analyzed 1146 guardian-patient coupled cases refined with prerequisites such as missing value, outliers, normality and covariance conditions. Followed were contributions of the present study. First, G-SQP was a predictor of P-SQP. Second, in the first contribution statement, there was no statistically significant difference between inpatient and outpatient group. Third, proven was that G-SQP was a predictor of G-BI. Fourth, verified was that P-SQP was a predictor of P-BI. Fifth, G-SQP was a predictor of P-SQP being led to P-BI that meant P-SQP had a mediating role between G-SQP and P-BI. Since the guardians' perception affected patients' buying decision, the present study implied service managers of hospital settings should pay attention to guardians' perception of service quality as well as patients'. With such strategy, hospitals could improve financial performance in long-term.

요양병원 노인환자의 구강건강상태와 구강건강관리 요구도 (Oral health status and care needs of elderly patients in long-term care hospital)

  • Objectives: The purpose of this study is to identify oral health status and oral health care needs of elderly patients in long-term care hospital. Methods: Oral health examination was carried out by a dentist and questionnaire was completed by direct individual interview by a dental hygienist for 245 elderly patients in seven long-term care hospitals in Daegu from March 12 to April 16, 2011. Results: Those who need dental care were 188(76.7%) and was done by oral health examination by a dentist. 93 patients(38.0%) wanted dental care and 63 patients(25.3%) need dental care. The most important dental service in the elderly patients was denture and prosthetic service. As demand for denture and prosthetic service exceeds the supply, but only 35.9 percent of the elderly received the dental care service by the estimation of the caregivers. Conclusions: It will be necessary to establish the customized dental care service for the elderly patients in long term care hospitals. The oral health education for the caregivers is very important and the continuing education program must be implemented in the future.

환자와 간호사의 개인적 특성과 간호서비스 질이 병원 재이용 의도에 미치는 영향에 대한 비교연구 (A Comparative Study of how Subjects' Characteristics and Nursing Service Quality Influence on Hospital Revisiting Intent between Patients and Nurses)

  • Purpose: This study was aimed to examine how subjects' characteristics and nursing service quality influence hospital revisiting intent, to compare perceptions of patients with those of nurses, Method: The questionnaire was developed and distributed to 300 patients and nurses at six general hospitals in a provincial city, Korea, For data analysis, the SPSS/PC program was used. Result: The nursing service quality's scores of patients is 3.61, that of nurses is 3.77, and there is a significant difference. The hospital revisiting intent's score of patients is 4.84, that of nurses is 4.61, and there are no significant differences. In subjects' characteristics, patients perceive that the hospital is the only different factor, and place hospital revisiting intent at $3.6\%$. Nurses perceive that education level and position are different factors, and place hospital revisiting intent at $3.3\%$. In nursing service quality, patients perceive that 2 attributes explain hospital revisiting intent at $17.8\%$ and 2 factors explain it at $16.5\%$; whereas, nurses perceive that 2 attributes explain hospital revisiting intent at $15.3\%$ and 3 factors explain it at $12.2\%$. Conclusion: There are perception gaps between consumers and providers. So nursing and hospital managers must recognize these facts and provide various marketing strategies to overcome them.

중환자실 환자의 욕창 발생 시기에 따른 관련요인의 차이 (Differences in Associated Factors according to the Time of Occurrence of Pressure Ulcers in Intensive Care Unit Patients)

  • Purpose : This study aimed to present the incidence of pressure ulcers and identify different associated factors according to the time of occurrence of pressure ulcers in intensive care unit (ICU) patients. Methods : The participants were 313 patients who reported pressure ulcers among 2,908 patients in ICUs at a large tertiary hospital in Gyeonggi-do. Among them, 220 patients (70.3%) had a pressure ulcer before admission, and 93 patients (29.7%) reported newly developed pressure ulcers after admission to the ICU. Data were collected between August 2018 and April 2019. Along with the time of occurrence and characteristics of pressure ulcers, diverse associated factors were gathered through electronic medical records. Data were analyzed using descriptive statistics, independent t-tests, and 𝑥2-tests. Results : Different risk factors associated with pressure ulcers in ICU patients according to the time of occurrence were main diagnosis, score of acute physiology and chronic health evaluation, score of Richmond agitation sedation scale, level of consciousness, administered sedatives, use of a ventilator, insertion of a feeding tube, and the duration of fasting period. Conclusion : Based on the results of this study, healthcare providers, especially ICU nurses, should try to detect early signs and symptoms of pressure ulcers, taking into account the derived factors associated with pressure ulcers in ICU patients. Practical intervention programs and strategies considering the factors associated with pressure ulcers must be developed to prevent and alleviate such ulcers in ICU patients in the future.

이화여자대학교 학생들의 의료실태에 관한 조사 연구 (A Status of Student Sickness and Medical Care in University Health Service, Ewha Womans University)

  • A study was carried out in order to obtain the status of student sickness and medical care in University Health Service, Ewha Womans University. This study was based on the clinical records of University Health Service and hospitals 'for student insurance pay claims during the year of 1981. And the findings from the study were as follows; 1. A total number of student patients cared at University Health Service in 1981 was 9,822 and the incidence rate of primary cared was 773 per 1,000 students. 2. A total number of student patients cared at hospitals was 393 and the incidence rate of secondary cared was 31 per 1,000 students and 5 student out of 31 per 1000 was cared under the haspitalization. 3. The evacuation rate of student patients from University Health Service to hospital was 393 out of 9,822 student primary cared or 4.0 percent. 4. The order of 5 major diseases of primary cared in University Health Service was respiratory system diseases (36.6%), Digestive system diseases (17.4%), Skin and subcutaneous tissue diseases (16.0%), Symptoms and undetermined diagnosis (13.7%) and Nerve and sensory organ diseases (12.0%) respectively. 5. The disease order of student patients(333) cared in hospitals as out-patients was Skin and subcutaneous tissue diseases (40.3%), Nervous and Sensory organ disease (19.2%), Digestive system diseases (10.8%) respectively. 6. The disease order of student patients (60) cared in hospitals as in-patients was Digestive system diseases (35.0%), Respiratory system diseases (13.3%), Nerve and sensory organ diseases (10.0%), Infectious and parasitic diseases (10.0%), and Symptom and Undetermined diagonsis (10.0%) respectively. 7. The evacuation rate of student patients in University Health Service to hospital was varied according to disease groups; the lowest rate was the diseases evacuated to Internal Medicine Department 1.5% or 75 out of 5,072 patient primary cared and the highest rate was Neuropsychiatry department 63.7% or 7 out of 11 patients. 8. The monthly distribution of student patients in University Health Service was the highest in September (17.9%) and April (15.5%) each semester. 9. The monthly number of student patients treated in hospitals was the range 20 to 40 in out patients and 2 to 9 in in-patients. 10. The hospital ill days per case were $4.3{\pm}5.0$ days in out-patients and $9.7{\pm}9.5$ days in in-patients.
