Palliative Care for Adult Patients Undergoing Hemodialysis in Asia: Challenges and Opportunities

  • This article underscores the importance of integrating comprehensive palliative care for noncancer patients who are undergoing hemodialysis, with an emphasis on the aging populations in Asian nations such as Taiwan, Japan, the Republic of Korea, and China. As the global demographic landscape shifts towards an aging society and healthcare continues to advance, a marked increase has been observed in patients undergoing hemodialysis who require palliative care. This necessitates an immediate paradigm shift to incorporate this care, addressing the intricate physical, psychosocial, and spiritual challenges faced by these individuals and their families. Numerous challenges impede the provision of effective palliative care, including difficulties in prognosis, delayed referrals, cultural misconceptions, lack of clinician confidence, and insufficient collaboration among healthcare professionals. The article proposes potential solutions, such as targeted training for clinicians, the use of telemedicine to facilitate shared decision-making, and the introduction of time-limited trials for dialysis to overcome these obstacles. The integration of palliative care into routine renal treatment and the promotion of transparent communication among healthcare professionals represent key strategies to enhance the quality of life and end-of-life care for people on hemodialysis. By embracing innovative strategies and fostering collaboration, healthcare providers can deliver more patient-centered, holistic care that meets the complex needs of seriously ill patients within an aging population undergoing hemodialysis.

가정간호대상자와 입원간호대상자의 간호만족도 및 간호사이미지 비교 (A Study of Nursing Care Satisfaction and the Image of Nurses As Compared Between Home Health Care and Hospitalized Clients)

  • The purpose of the study was to compare both the nursing care satisfaction and the image of nurses as experienced by home health care clients and hospitalized clients. For the descriptive survey study. data were collected from 69 home heath care clients and 342 in-patients in a university hospital. The tools used for the study were modified by Quality Patient Care Scale(Wandelt & Ager. 1974) and Image of Nurses (송인자, 1993). The data were analyzed using Pearson Correlation. Scheffe test. factor analysis. t-test. and ANOVA. The major findings were as follows: Regarding nursing care satisfaction, the mean score of total nursing care satisfaction in home health care clients was 3.28 out of 4.0. Among five domains, the domain with the highest score was the psychosocial domain, followed by the general. the professional. the physical. and the communication domain. The level of nursing care satisfaction was not significantly different according to demographic variables except for age: the age group of 41-60 showed the highest score (p<0.05). The mean score of total nursing care satisfaction in hospitalized clients was 2.95 out of 4.0. Among five domains, the domain with the highest score was the psychosocial domain, followed by the physical and the communication. the professional. and the general domain. The level of nursing care satisfaction was not significantly different according to demographic variables except age: the higher the age the higher the score (p<0.05). The levels of nursing care satisfaction in all five domains were significantly higher in home health care clients than in hospitalized clients(p=0.0005). Regarding image of nurses, the mean score of total image of nurses in home health care clients was 3.32 out of 4.0. Among four domains, the domain with the highest score was the sincerity domain, followed by the kindness. the spirit. and the knowledge and skill domain. The level of image of nurses was not significantly different according to demographic variables. The mean score of total image of nurses in hospitalized clients was 3.05 out of 4.0. Among four domains. the domain with the highest score was the sincerity domain, followed by the kindness, the knowledge and skill, and the spirit domain. The level of image of nurses was not significantly different according to demographic variables. The levels of image of nurses in all four domains were significantly higher in home health care clients than in hospitalized clients (p=0.001). Both the levels of nursing care satisfaction and image of nurses, part of an evaluation for quality of nursing care were significantly higher in home health care clients than in hospitalized clients. In light of the findings, we could consider that home health care nurses provided client-centered comprehensive nursing care. However, nurses need to have methods that more promote the social recognition of the image of nurses and nursing care services as well as professional knowledge and skills.

만성 폐쇄성 폐질환의 호스피스 완화의료 (Hospice and Palliative Care in Chronic Obstructive Pulmonary Disease)

  • 만성 폐쇄성 폐질환은 비가역적, 점진적 기도 폐쇄가 특징인 주요 사망 원인중의 하나이다. 말기 폐쇄성 폐질환 환자는 다양한 신체적, 정서적, 사회적, 기능적 문제로 인해 호스피스 완화의료가 필요하다. 따라서, 완화의료의 목적은 증상 조절, 운동 능력 및 삶의 질 향상이며, 아울러 급성 악화 빈도를 줄이고 질병의 진행을 늦추는 효과를 기대할 수 있다. 다양한 진행 과정 때문에 말기 시점을 예측하기 어렵다. 호흡곤란 정도, 폐기능, 혈액학적 임상 지표를 이용하여 대상자를 선정하는 기준을 마련하고 적용시점을 논의해야 한다. 질환의 특성상 진단 후 초기에는 완치 목적의 치료가 중심이 되어 완화의료가 이를 보완하는 방식으로 적용되다가, 완치 목적의 치료를 위해 입 퇴원을 반복하거나 치료의 반응이 낮고 나쁜 예후가 예상될 때 완화의료 비중이 확대되는 것이 바람직하겠다. 폐쇄성 폐질환 환자에게 적극적으로 호흡재활을 적용하고, 환자의 의사결정을 존중하여 증상 악화 이전에 미리 대책을 마련하고 앞으로의 치료 목표를 논의하기 위한 완화의료 팀을 구성하고, 국내 현실에 맞는 진료 연계 체계를 마련하는 것이 필요하다. 제도가 잘 정착하기 위해서는 일차 의료 및 암환자 대상의 완화의료 전문 팀과의 협업이 중요하며, 우선 고려되어야 할 것이다.

유산 가능성 임부의 정서, 불편감 및 간호요구에 관한 연구 (A Study on Emotions, Feelings of Discomfort, and Nursing Needs of the Women in the Possibility of Abortion : Centered on the Threatened Abortion and Habitual Abortion during Hospitalization)

  • Maternity nurses' roles are to find out women's various responses related to the health problems during pregnancy, to intervene adequate nursing and supports, and to help them adapt satisfactorily to new situation, such as hospitalization. The purposes of this study are : firstly, to inquire about women's emotional status, feeling of discomfort, and the nursing need during hospitalization because of discomfort, and the nursing needs during hospitalization because of the possibility of abortion by threatened and habitual abortion : secondly, to provide fundamental data for developing adequate nursing intervention and improving hospital management. The subjects of this study were 62 women who were hospitalized, visited out-patient clinic for follow-up, and stayed at home after discharge since August, 1993 from one hospital located in Seoul, from March 23 to April 13, 1994. The questionnaire was consisted of 21 items of emotion(Reliability Cronbach's alpa, .77), 19 items of feeling of discomfort(.79), and 21 items of nursing needs (.89), and nurses' performance according to nursing needs,(.90). These were measured by using Likert Scale and analyzed by using SPSS / $PC^+$ with the descriptive statistics, $X^2$-test, and ANOVA. Research findings are as follows : 1) The subjects' average age, hospitalized days, and gestational age are 32.2, 15.7, 12.9 by respectively. 2) 88.7% of the subjects are getting antepatal care, 66.1% are experiencing 1.75 times of spontaneous abortion, and 82.3% are nuclear family. 3) The Emotions were mainly comprized negative and subjective state of minds such as uneasiness, anxiety, and discomfort. The domains of emotions were related to 'fetus', 'herself', 'hospitalization', and 'husband's 'families', in rank. The highest scored item was "I, above anyone else, am mostly stressed from abortion." 4) The feelings of discomforts were firstly related to 'personal discomfort arising from absolute bed rest', 'meals provided', Usage of hospital, perse', 'health team', in rank. 5) The nursing needs frequently pointed out were 'Nurses kindly explain of me the purpose, method, and results of the lab. test', 'Nurses sincerely pay attention to my word in communication'. The least nursing needs were 'Nurses observe my feces and whether I have constipation or not'. 'When I can't move, nurses help me by holding my arms or pulling my bed'. 6) The highest performed items were 'Nurses observe whether I bleed or not', and 'Nurses give me drug with exact dose in time'. The least performed items was about hospital environment, such as 'Temperature, ventilation & humidity in the room should be controlled'. 7) Besides religion that is related to nursing need with statistically significant difference, none of general characteristics were related to emotion, feeling of discomfort, and nursing needs.

부산지역 일부병원 치료식 섭취 환자의 영양상담에 대한 인식 및 만족도 (Perception and Satisfaction on Nutrition Counseling Service for Patients Consuming a Therapeutic Diet at Hospitals in Busan)

  • 본 연구는 병원 영양과에서 실시하고 있는 치료식 섭취환자의 영양상담에 대한 인식과 영양상담 만족도 요인으로서의 지식전달 요인, 인지적 대화요인, 감성적 대화요인, 촉진능력 요인과의 관계를 파악함으로써 앞으로 병원 영양과에서 환자들의 영양상담에 대한 만족도를 높이는데 필요한 기초자료를 제공하고자 시도하였다. 조사대상은 부산지역 400 병상 이상의 병원에 입원중인 치료식 섭취환자를 퇴원하기 직전에 직접 1:1 면담법을 이용하여 설문조사를 실시하였다. 배부된 설문지 중 총 153부(98.7%)를 분석에 사용하였으며 본 조사는 2008년 1월 21일~2월 28일에 진행하였다. 조사대상 환자 중 영양상담을 받은 경험이 있는 환자가 전체의 57.5%로 나타났고, 영양상담을 받은 계기는 의료진이 권해서가 64.1%로 가장 높았고 영양상담 방법은 개별교육이 58.0%로 나타났다. 영양상담에 대한 인식에서 영양상담 이해에 대한 긍정적인 견해는 75.0%, 중요도는 83.0%, 영양관리에 도움이 되는 정도는 78.7%로 나타났다. 영양상담에 대한 전체 만족도 점수는 3.80점이며, 각 세부요인에서 지식전달 요인 3.80점, 인지적 대화 요인 3.71점, 감성적 대화 요인 4.05점, 촉진능력 요인은 3.61점으로 나타났다. 영양상담의 만족도 점수가 낮은 항목은 '퇴원 후 식사요법 정보제공', '맞춤형 영양정보 제공', '구체적인 식단 제시', '식단 작성 방법 설명'으로 나타났다. 영양상담에 대한 인식인 이해도, 중요도 및 영양관리 도움정도는 영양상담에 대한 만족도인 지식전달 요인, 인지적 대화 요인, 감성적 대화 요인, 촉진 능력 요인과 모두 유의적인(p<0.01) 양의 상관관계를 나타냈다. 이상의 결과를 살펴볼 때, 영양사들은 환자들의 영양에 대한 이해도 및 중요도를 높일 수 있는 다양한 영양상담 도구의 개발 및 활용 방안에 대한 노력이 필요하겠다. 환자와의 영양상담시 환자 스스로 영양관리를 독립적으로 실행에 옮길 수 있도록 환자들을 위한 구체적인 식단제시, 조리방법, 맞춤형 영양관리 방법 등의 촉진 요인들에 대해서 영양사들은 더 많은 관심을 가져야 하겠다. 즉, 실제 식사와 관련된 조리체험교실, 시식회 등의 개최를 통해 환자들에게 보다 직접적이고 실질적으로 다가갈 수 있는 환자 중심적인 영양상담이 필요하며 특히, 맞춤형 영양상담을 통해 환자의 치료 효과를 높이기 위해서는 현재로서는 부족한 영양사 인력의 증원이 절실히 필요하겠다.
