The Changes in Psychopathological Behavior of Schizophrenics in the Ward (정신분열증환자 간호에 대한 임상적 고찰)

  • A study designed to provide effective nursing care for schizophrenic patient was carried out to 22 patients who were admitted to the psychiatric in-patient service, St. Malays Hospital, Catholic Medical Center. The main purpose of the study was to provide effective means of discriminating the effects of nursing care for schizophrenic patients. The experimental group sampled consisted of 10 male and female patients who have been given patient-centered nursing care while the control group consisted of 12 male and female patients who have been given only routine care. The administration of the WBI manual in both groups obtained the changes in the psychopathological behavior of them. The result were found to be as follows. 1. The greater number of the patient in both groups were below 30 years of age (70%). 2. Uptill 15 days after admission there was no difference between the change of the psychopathological behavior the subject group and that of the control. 3. There as a difference between the change of the psychopathological behavior of the subject group and that of the control uptill 30 days after. admission(p<0.08).

Validity and Reliability of the Korean Version of Person-Centered Practice Inventory-Staff for Nurses (간호사 대상 한국어판 인간중심돌봄 측정도구의 타당도와 신뢰도)

  • Purpose: The purpose of this study was to evaluate the validity and reliability of the Korean version of Person-Centered Practice Inventory-Staff (PCPI-S) for nurses. Methods: The English PCPI-S was translated into Korean with forward and backward translation. Data were collected from 338 nurses at one general hospital in Korea. Construct validity was evaluated with confirmatory factor analysis, convergent validity, and discriminant validity. Known-group validity was also evaluated. Cronbach's α was used to assess the reliability. Results: The PCPI-S Korean version consisted of 51 items in three areas: prerequisites, the care environment, and person-centered process. The comparative fit index (CFI) and values of person-centered care process were improved after engagement and having sympathetic presence items were combined as one component. The construct validity of PCPI-S Korean version was verified using four-factor structures (.05 < RMSEA < .10, AGFI > .70, CFI > .70, and AIC). The convergent validity and discriminant validity of the entire PCPI-S question were verified using a two-factor structures (AVE > .50, construct reliability > .70). There was an acceptable known-group validity with a significant correlation between the PCPI-S level and the degree of person-centered care awareness and education. Internal consistency was reliable with Cronbach's α .95. Conclusion: The Korean version of PCPI-S is valid and reliable. It can be used as a standardized Korean version of person-centered care measurement tool. Comparison of Patient Needs, Patient Satisfaction, and Communication Type of Nurses between Integrated Nursing Care Service Ward and General Ward Patients (간호·간병통합서비스병동과 일반병동 환자의 간호요구도, 간호만족도 및 간호사의 의사소통 유형 비교)

  • Purpose: The purpose of this study is to provide patient-centered nursing by comparing the patient needs, patient satisfaction, and communication type of nurses for patients admitted to integrated nursing care service ward and general ward. Methods: Data were collected by self-report questionnaires between September 7 and October 9, 2021. Patients were patients recruited from a hospital in D city, 100 each from comprehensive nursing care service ward and general ward. The data were analyzed using descriptive statistics, 𝜒2-test, t-test, ANOVA, and Pearson's correlation. Results: There was no significant difference in patient needs between the patients of integrated nursing care service ward and the general ward (t=-1.12, p=.263). However, patient satisfaction was higher among the patients in integrated nursing care service ward than in the general ward (t=-3.55, p<.001). Pertaining to the communication type of nurses, the informational communication type (t=-4.02, p<.001) and the friendly communication type (t=-3.92, p<.001) were stronger in the integrated nursing care service ward than the general ward. It was, hence, confirmed that the educational requirements were positively correlated with patient satisfaction in the reliability domain and technical-professional domain, and that the nurse informational communication type and friendly communication type had a significantly positive correlation with patient satisfaction. Conclusion: The expansion of the integrated nursing care service ward may be considered in the future, with an informational and friendly communication method as an effective response that can be adapted in practice as a reflection the type of communication among nurses.

Concept Analysis of Patient Safety (환자안전(patient safety) 개념분석)

  • This study was conducted to investigate the clear concept of patient safety and obtain theoretical evidences. Methods Research was conducted using Walker & Avant's conceptual analysis process. Results: Patient safety was defined as a activity that minimizes and removes possible errors and injuries to patients. It includes a basic desire to secure the patient's right to safety, and the legal regulations and duties of medical teams. The results of the establishment of a safety culture are patient-centered medical treatment and caring. Antecedents were found to be open and clear communications, continuous education and training for health care personnel, sufficient allocation of qualified personnel, cooperation among departments, improvements in the recognition of patient safety. Consequences were found to be the provision of high quality medical care and treatment, and increase in patient satisfaction. Conclusion: Patient safety as defined by the results of this study will contribute to the foundation of institutionalization of the pursuit of patient safety and creation of a hospital culture focusing on patient safety as a first priority.

A Review Study on Interprofessional College Education in Health Care Sector (보건의료분야 전문가간 대학 교육에 대한 사례연구)

  • Background & Objectives: Rising concerns about patient safety and looming health provider shortages were generating new recognition for an old idea. Interprofessional education means that two or more professions learn with, from and about other to improve collaboration and the quality of healthcare. The University of British Columbia established the College of Health Disciplines in 2001 to examine the merits of interprofessional health education. The objective of this study was to review UBC's interprofessional health education and to introduce the theoretical framework of interprofessional education for collaborative patient-centered practice. Methods: This study was conducted with the materials relevant to the interprofessional education. We reviewed the journals and Web site for this subject and we obtained interview data from administrators in the University of British Columbia, College of Health Disciplines. Results: We introduced interdisciplinary education for collaborative patient-centered practice model. It is assumed valuable model in preparing the interprofessional education as well as theoretical framework for implementation. We preseuted the example for interprofessional education of the College of Health Disciplines, University of British Columbia. Conclusion: We need to introduce the interprofessional education at the health care departments of University or College in Korea.

The Relationship between Person-Centered Nursing and Family Satisfaction in ICUs (중환자실에서의 인간중심 간호와 가족 만족도 관계)

  • Purpose : This study aims to identify the relationship between person-centered critical care nursing (PCCN) and family satisfaction for intensive care unit (ICU) nurses. Methods : This research used a cross-sectional survey. The participants were 142 nurses and 121 family members from 8 ICUs in 4 hospitals in B city. The questionnaire survey was conducted between December 2018 and March 2019. The eight ICUs were divided into those that scored high and those that scored low for person-centered care relative to the average score for PCCN, and the difference in family satisfaction between the ICUs with high and low scores was compared using t-test. Results : The mean score of PCCN was $3.68{\pm}0.40$ out of 5. In the subcategories of PCCN, "comfort"was the highest at $3.95{\pm}0.49$, followed by "respect" at $3.73{\pm}0.57$, "compassion" at $3.59{\pm}0.57$, and "individuality" at $3.47{\pm}0.54$. The average score of family satisfaction with the ICUs was $3.45{\pm}0.67$ out of 5. In its subcategories, "emotion" was the highest at $3.69{\pm}0.72$, followed by "information" at $3.61{\pm}0.75$, "participation" at $3.30{\pm}0.79$, and "resources" at $3.20{\pm}0.80$. The family satisfaction ($3.75{\pm}0.54$) with the ICU of the highest-scoring PCCN was significantly higher than the family satisfaction ($3.25{\pm}0.53$) with the ICU with the lowest-scoring PCCN (t=4.97, p<.001). Conclusion: The results suggest that the PCCN scores of ICU nurses should be promoted to improve family satisfaction. It is necessary to pursue a variety of ways to secure the provision of person-centered nursing practice in ICUs.

  • A holistic approach to diabetes considers patient preferences, emotional health, living conditions, and other contextual factors, in addition to medication selection. Human and social factors influence treatment adherence and clinical outcomes. Social issues, cost of care, out-of-pocket expenses, pill burden (number and frequency), and injectable drugs such as insulin, can affect adherence. Clinicians can ask about these contextual factors when discussing treatment options with patients. Patients' emotional health can also affect diabetes self-care. Social stressors such as family issues may impair self-care behaviors. Diabetes can also lead to emotional stress. Diabetes distress correlates with worse glycemic control and lower overall well-being. Patient-centered communication can build the foundation of a trusting relationship with the clinician. Respect for patient preferences and fears can build trust. Relevant communication skills include asking open-ended questions, expressing empathy, active listening, and exploring the patient's perspective. Glycemic goals must be personalized based on frailty, the risk of hypoglycemia, and healthy life expectancy. Lifestyle counseling requires a nonjudgmental approach and tactfulness. The art of diabetes care rests on clinicians perceiving a patient's emotional state. Tailoring the level of advice and diabetes targets based on a patient's personal and contextual factors requires mindfulness by clinicians.

Design and Implementation of a Personal Health Record Platform Based on Patient-consent Blockchain Technology

  • In the 4th Industrial Revolution, the healthcare industry is undergoing a paradigm shift from post-care and management systems based on diagnosis and treatment to disease prevention and management based on personal precision medicine. To optimize medical services for individual patients, an open ecosystem for the healthcare industry that allows the exchange and utilization of personal health records (PHRs) is required. However, under the current system of hospital-centered data management, it is difficult to implement the linking and sharing of PHRs in practice. To address this problem, in this study, we present the design and implementation of a patient-centered PHR platform using blockchain technology. This platform achieved transparency and reliability in information management by eliminating the risk of leakage and tampering/altering personal information, which could occur when using a PHR. In addition, the patient-consent system was applied to a PHR; thus, the patient acted as the user with ownership. The proposed blockchain-based PHR platform enables the integration of personal medical information with scattered distribution across multiple hospitals, and allows patients to freely use their health records in their daily lives and emergencies. The proposed platform is expected to serve as a stepping stone for patient-centered healthcare data management and utilization.

  • With the recent advances in medical technology and health care, the prevention and treatment of diseases has developed. Accordingly aging has rapidly progressed. In this life span and aging society, demand for diagnostic centered medical care is increasing rapidly. In this paper, we propose a wearable patient check system based on FPGA that can be controlled by sensors. In the existing hospital, a doctor or nurse visited the patient every hour to check the condition. However, in this paper, patients, doctors and nurses can check the patient's condition at the desired time using patient check system. In addition, the tilt sensor is used for the patient who is uncomfortable to easily control. The proposed FPGA-based hardware architecture consists of an algorithm for enlarged image processing, a TFT-LCD Controller, a CIS Controller, and a Memory Controller to output the patient's status image. Implemented and validated using the DE2-115 test board with Cyclone IV EP4CE115F29C7 FPGA device and its operating frequency is 50MHz.

Gerontological Nursing in the Era of the Fourth Industrial Revolution (4차 산업혁명시대 노인 간호)

  • Purpose: There is a need to examine changes in the health care environment and the impact on gerontological nursing care in the era of the Fourth Industrial Revolution. In this article recent healthcare paradigm changes, gerontechnology, high tech and high touch, person-centered approaches are discussed. Methods: A narrative review was used. Results: Cyber physical system, artificial intelligence, advance and convergence of bioengineering, and information communication technology are changing the health care paradigm to "precision", "prediction" and "personalization". Entry into the global aging society and the surge in the elderly population worldwide has led to searches for a new means to prepare for projected demands of this growing population. Thus, efforts such as gerontechnology have been made to apply and utilize recent innovative science and technology in order to promote the health and life of elders. There is a great emphasis on the convergence of high tech and high touch, in which humanistic and artistic approach are critical in order to assure that technology is beneficial to human beings rather than harmful. Conclusion: Positive healthcare experiences among patients and their families are emphasized by utilizing new technology and employing high touch while providing personalized care with a person-centered approach.
