  • Through USN (Ubiquitous Sensor Network) is collected the patient's vital information in real-time, also information collected will be stored in the DB (Date Base), frequent use hospital saved patient's vital information for DB. Stored in the patient's vital medical information stored in the patients with frequent hospital patient to hospital if the patient's vital information is stored in DB. But, stored location is within hospital server or stored in a PC environment, because If utilize other Hospital existing hospitals will need to request. However, Existing hospital have problem for security, authentication, management, cost, manpower, such as, because other hospitals and the exchange of information does not come easily. So, If has the advantage of the patient and the patient's vital information is stored on mobile devices that you can use as DB. It is important to find information quickly and accurately, in this study, Is A study on mobile circulation loop DB systems for patient-centered serbices.

Convergence outpatient medical service patient experience research using data mining (데이터마이닝 기법을 이용한 융복합 외래 의료서비스 환자경험조사 연구)

  • The purpose of this study is to find out specific measures that can help the management strategy of patient-centered medical institutions by conducting research on patient experience surveys of convergence outpatient medical services using data mining techniques according to changes in patient-centered medical culture. Using the raw data of the 2018 Medical Service Experience Survey, 8,843 people over the age of 15 who had patient experience in outpatient medical services were analyzed. Decision tree analysis was performed. The determinants of satisfaction with outpatient medical services patient experience were the doctor's area and patient's rights protection area, and the determinants of intention to recommend outpatient medical services were the doctor's area and facilities comfort. Women evaluated the experience positively in overall satisfaction as compared to men, and those over the age of 60 positively evaluated the overall satisfaction and intention to recommend. It is significant that the outpatient experience decision-making model is presented, and that the doctor's area, patient's rights protection area, and facility comfort are important factors. Long-term research on the 'Medical Service Experience Survey' is needed, and research on the inpatient medical service experience is needed.

Quality of life, patient preferences, and implant survival and success of tapered implant-retained mandibular overdentures as a function of the attachment system

  • Purpose: A novel attachment system for implant-retained overdentures (IRODs) with novel material combinations for improved mechanical resilience and prosthodontic success (Novaloc) has been recently introduced as an alternative to an existing system (Locator). This study investigated whether differences between the Novaloc and Locator attachment systems translate into differences in implant survival, implant success, and patient-centered outcomes when applied in a real-world in-practice comparative setting in patients restored with mandibular IRODs supported by 2 interforaminal implants (2-IRODs). Methods: This prospective, intra-subject crossover comparison compared 20 patients who received 2 intra-foraminal bone level tapered implants restored with full acrylic overdentures using either the Locator or Novaloc attachment system. After 6 months of function, the attachment in the corresponding dentures was switched, and the definitive attachment system type was delivered based on the patient's preference after 12 months. For the definitive attachment system, implant survival was evaluated after 24 months. The primary outcomes of this study were oral health-related quality of life and patient preferences related to prosthetic and implant survival. Secondary outcomes included implant survival rate and success, prosthetic survival, perceived general health, and patient satisfaction. Results: Patient-centered outcomes and patient preferences between attachment systems were comparable, with relatively high overall patient satisfaction levels for both attachment systems. No difference in the prosthetic survival rate between study groups was detected. The implant survival rate over the follow-up period after 24 months in both groups was 100%. Conclusions: The results of this in-practice comparison indicate that both attachment systems represent comparable candidates for the prosthodontic retention of 2-IRODs. Both systems showed high rates of patient satisfaction and implant survival. The influence of material combinations of the retentive system on treatment outcomes between the tested systems remains inconclusive and requires further investigations.

  • U-healthcare is real-time monitoring of personal biometric information using by portable devices, home network and information and communication technology based healthcare systems, and fused together automatically to overcome the constraints of time and space are connected with hospitals and doctors. As u-healthcare gives health service in anytime and anywhere, it becomes to be a new type of medical services in patients management and disease prevention. In this paper, recent changes in prevention-oriented care is analyzed in becoming early response for Healthcare Information System by requirements analysis for technology development trend. According to the healthcare system, PACS, OCS, EMR and emergency medical system, U-healthcare is presenting the design of a patient-centered integrated client system. As the relationship between the meaning of the terms is used in the ontology, information models in the system is providing a common vocabulary with various levels of formality. In this paper, we propose an ontology-based system for patient-centered services, including the concept of clustering to clustering the data to define the relationship between these ontologies for more systematic data.

Long Term Assessment of Outcome of Essential Competencies in CPPE at Tertiary and Secondary Hospitals Located in Seoul and Gyeonggi-do: College of Pharmacy Students' Evaluation from 2014 to 2018 (서울과 경기도 소재 상급종합병원과 종합병원의 필수실무실습 핵심역량교육 성과에 대한 장기적인 고찰)

  • Objective: As the demands of healthcare environment change, it is necessary to advance human health care by improving students' essential competencies including knowledge, skills, abilities, inter-professional collaboration and patient centered care. This study identified long term accomplishment and improvement of the essential competencies in clinical pharmacy practice education (CPPE) at Korean hospitals over time. Methods: This study was conducted for pharmacy students who completed CPPE evaluation related to tertiary hospitals and secondary hospitals located in Seoul and Gyeonggi-do regional area from 2014 to 2018. Results: Over the past 5 years, overall results of student evaluation on the essential competencies in CPPE at both tertiary and secondary hospitals showed a decreasing trend or did not change. Essential competency in CPPE at tertiary hospitals had been identified as superior on 'Learn clinical knowledge in the treatment of diseases' to secondary hospitals [average number of students (%): 210 (72.9%) vs 68 (68.0%)]. On the other hand, essential competencies in CPPE at secondary hospitals had been identified as better at 'inter-professionals collaborative teamwork and direct patient care' than tertiary hospitals [average number of students (%): 64 (64.0%) and 56 (56.0%) vs 121 (42.0%) and 90 (31.3%)]. A total of 176 (61.1%) students in tertiary hospitals and 66 (66.0%) in secondary hospitals evaluated that 'patient-centered care' education was good. Conclusion: In tertiary hospitals, all six essential competency outcomes have not been improved, whereas four essential competency outcomes showed an increasing trend in secondary hospitals. It will be necessary to develop outcome-based CPPE education program to better reflect the essential competencies.

  • Objectives: To evaluate the effects of concurrent therapy with cognitive behavior therapy and person-centered counseling in addition to general Korean herbal medicine treatment for a patient with anorexia nervosa. Methods: A patient diagnosed with anorexia nervosa was treated with Korean herbal medicine treatment, cognitive behavior therapy, and person-centered treatment. The results were evaluated through a feeding interview, self-checklist, beck depression inventory (BDI) and InBody test. Results: After the treatment period, the treatment effects such as improvement of dietary restriction and elimination behavior, weight gain, and reduced depression were confirmed. Conclusions: This study suggests that concurrent therapy together with cognitive behavior therapy, person-centered counseling, and general Korean herbal medicine treatment is effective on patients with anorexia nervosa.

Concept Analysis of Patient Safety (환자안전(patient safety) 개념분석)

  • This study was conducted to investigate the clear concept of patient safety and obtain theoretical evidences. Methods Research was conducted using Walker & Avant's conceptual analysis process. Results: Patient safety was defined as a activity that minimizes and removes possible errors and injuries to patients. It includes a basic desire to secure the patient's right to safety, and the legal regulations and duties of medical teams. The results of the establishment of a safety culture are patient-centered medical treatment and caring. Antecedents were found to be open and clear communications, continuous education and training for health care personnel, sufficient allocation of qualified personnel, cooperation among departments, improvements in the recognition of patient safety. Consequences were found to be the provision of high quality medical care and treatment, and increase in patient satisfaction. Conclusion: Patient safety as defined by the results of this study will contribute to the foundation of institutionalization of the pursuit of patient safety and creation of a hospital culture focusing on patient safety as a first priority.

Effect of Hospital Selection Factors on Patient Satisfaction and Reuse Intention (병원선택요인이 환자만족도 및 재이용에 미치는 영향)

  • This study was performed to identify effects of hospital selection factors on patient satisfaction and reuse intention. For this purpose, a survey was performed subject to outpatients and inpatients in 10 hospitals located in Busan district from March 7, 2013 to June 21, 2013 and total 447 copies were used as final study data. As results of this study, it was found that among the hospital selection factors of outpatients, internal conditions and medical service gave effects to patient satisfaction and internal conditions, service of medical staffs and medical service gave effects to reuse intention. It was found also that among the hospital selections factors of inpatients, internal conditions, service of medical staffs and medical service gave effects to patient satisfaction and external conditions, internal conditions, service of medical staffs and medical service gave effects to reuse intention. It was suggested that in the effect of patient satisfaction on reuse intention, both inpatients and outpatients gave the effect and thus higher patient satisfaction was related to higher reuse intention. Therefore, instead of hospital-centered medical treatments, hospitals should restructure its medical and service systems that are patient-centered. Also, it is necessary for the hospital managers to recognize that patient satisfaction is an important factor in increasing its profit.

The Influence of Working Environment on Person-centered Care in Home Care Visiting Dementia Caregivers (재가방문 치매 요양보호사의 근무환경이 인간중심돌봄에 미치는 영향)

  • Purpose: The purpose of this study was to examine the impact of the working environment on person-centered care for home visiting dementia caregivers. Methods: Participants in this study were 146 caregivers, with the least 6 months of experience working at visiting care centers in Busan, and used a total of 133 questionnaires suitable for analysis. Data were collected from June 9 to July 20, 2023, and were analyzed using descriptive statistics, t-test, ANOVA, pearson's correlation coefficients, and multiple linear regression using IBM/SPSS 27.0. Results: As a result of multiple linear regression analysis, organizational factors (β=-.37, p<.001), job-related factors (β=-.27, p=.001), and religion (β=.18, p<.013) were factors affecting person-centered care of home visiting dementia caregivers, and working environment explained 37% (F=10.63, p<.001) of person-centered care. Conclusion: Based on the results of this study, the exploration of strategies to improve the working environment will be necessary in order to increase person-centered care for home visiting dementia caregivers.
