산부인과 간호사의 환자 프라이버시 보호행동에 관한 연구 (A Study on Protecting Patients' Privacy of Obstetric and Gynecologic Nurses)

  • Purpose: This study aims to determine obstetric and gynecologic (OBGY) nurses' perception and performance propecting patients' privacy, and to contribute to develop educational program and improve the quality of nursing care. Methods: 206 OBGY nurses in 6 hospitals using an electronic medical record or an order communicating system were chosen by convenience sampling and agreed to participate in the study. The questionnaire, explored 4 domains of privacy: direct nursing, linked business, patient information management, communication with relatives. Results: Perception and performance of protecting patient privacy averaged 4.29 (of 5) and 3.55 (of 5), respectively. Most nurses (94.2%) recognized the importance of protecting patient privacy, 80.1% received patient privacy education. There was a distinct difference between the perception and performance of protecting patient privacy of nurses. Performance of protecting patient privacy had a positive correlation with perception. Conclusion: Proper performance of protecting privacy protection requires improving perception of each nurse on the patient privacy, and various efforts should be made to minimize the affect from external factors such as hospital environment. It is needed to educate nurses for patient privacy. It is also needed for medical organizations to improve their policies and facilities to ease the performance for privacy protection.

  • Purpose: The purpose of this study was to investigate the patient's perception of the nurse's behavior in protecting patient privacy and to make suggestions for medical facilities to increase protection of patient privacy. Method: The research was a survey study The data on protection of privacy in primary nursing, for physical privacy, of patient information and in private conversations were collected in October 2005 from 187 patients in a university hospital. Frequencies, means, t-test, ANOVA, and $x^2$-test were used to analyze the data. The SPSS 13.0 for Windows program was used. Results: The mean score for patients' perception of nurse protection of their privacy was 3.33. There were significant differences in perception of protecting patients' privacy according to gender for private conversation, according to level of education for all but physical nursing and for number of admissions for total score and for primary nursing. Conclusion: The results of this study suggest the following: a) Institutional polices and nursing guideline should be clearly stated as to the nurses' duty to protect patient privacy. b) Medical facilities should be arranged in a way that allows for the protection of patients' medical information, and should focus on patient privacy. c) It is necessary of nurses to receive periodic in-service education on protecting patient privacy.

  • Purpose: This study aims to examine level of perception and performance of privacy protection behavior of anesthesia and operating room (OR) nurses for patients who underwent general anesthesia surgery. Methods: Data collection was conducted from August 2020 to January 2021 for a total of 101 participants, consisting of 49 patients and 52 nurses. Independent t-test and Pearson's correlation were conducted using SPSS 21. Results: Anesthesia and OR nurses showed the highest score in patient privacy, followed by patient information management, body privacy, and the lowest score in communication. There was a significant difference between the patient information and the communication. Conclusion: Anesthesia and OR nurses had the highest level of perception and performance of patient privacy protection behavior for body privacy, and the lowest for communication. In addition, there was a significant difference in patient information management and communication. In order to protect the privacy of patients undergoing general anesthesia surgery, efforts are needed to learn standardized nursing knowledge, attitudes, and practice.

간호대학생의 환자 프라이버시 보호행동 영향요인 (Factors Influencing Patient Privacy Protection Behavior among Nursing Students)

  • Purpose: The purpose of this study was to identify factors influencing patient privacy protection behavior among nursing students and examine the relationships between these factors. Methods: Participants in this study were 144 nursing students who have experienced clinical practice. The data were analyzed using descriptive statistics, one-way ANOVA, Scheffe test, Pearson's correlation coefficient, and multiple regression with IBM SPSS Win 23.0 program. Results: Professional self-concept and ethical values were factors influencing patient privacy protection behavior among nursing students. These variables explained 21.9% of the variance for patient privacy protection behavior. A higher level of patient privacy protection behavior was associated with higher levels of professional self-concept and ethical values. Conclusion: The findings demonstrate that strategies for enhancing patient privacy protection behaviors of nursing students should include methods for forming images of positive nurses and firming ethical values.

  • Purpose: With the rapid increase in information technology in the medical environment, protection of patient's privacy is a crucial issue to hospital nurses. The purpose of this study was to determine neonatal nurses' perception and performance of behavior to protect patient privacy, and professional self-concept, and to investigate the relationships among these variables. Methods: The participants in this descriptive correlation study were 182 nurses in nursery and neonatal intensive care units using EMR or OCS in November, 2011. Results: Perception and performance averaged 4.61 and 4.04 (out of 5) respectively, and the average score for professional self-concept was 2.73 (out of 4). There was a significant difference between perception and performance. The performance of behaviour to protect patient privacy had a positive correlation with perception and professional self-concept. Multiple regression analysis showed that the key determinants of performance were recognition of necessity of patient privacy education, professional self-concept and perception, and these explained 36% of the total variance of performance. Conclusion: Study results indicate a need to establish policy to protect privacy of neonates and their families, and to develop educational programs to enhance neonatal nurses' perception and performance.

  • 본 연구는 간호사의 전문직 자아개념과 윤리의식이 환자의 개인정보보호 실천도에 미치는 영향을 파악하고자 시도되었다. 연구대상은 K도에 소재한 종합병원에 재직 중인 간호사 196명으로, 2018년 6월 1일부터 7월 5일까지 전문직 자아개념과 윤리의식 및 환자 개인정보보호 실천도를 설문조사하였다. 연구결과 대상자의 전문직 자아개념은 4점 만점에 2.62점, 윤리의식은 4점 만점 중 2.93점, 환자 개인정보보호 실천도는 5점 만점 중 3.69점이었다. 환자 개인정보보호 실천도에 영향을 미치는 요인은 윤리의식, 전문직 자아개념으로 확인되었으며, 이들 변수들은 환자 개인정보보호 실천도를 30% 설명하였다. 따라서 병원 간호사의 환자 개인정보보호 실천도를 높이기 위해서는 간호사의 윤리의식을 높이고 전문직 자아개념을 향상시킬 수 있는 신규간호사를 위한 교육과 경력간호사를 위한 주기적인 재교육 프로그램이 마련되어야 할 것으로 사료된다.

계층적 다중 속성을 이용한 헬스케어 환자의 프라이버시 보호 기법 (Privacy Protection Scheme of Healthcare Patients using Hierarchical Multiple Property)

  • 최근 헬스케어는 다양한 의료 서비스를 제공받으려는 사용자가 급격하게 증가하고 있으며, 환자의 정보가 제3자에게 쉽게 노출되어 악용될 수 있어 환자에 따라 병원 관계자(의사, 간호사, 약사 등)의 역할이 명확하게 분류될 필요가 있다. 본 논문에서는 헬스케어 환경에서 환자의 정보가 제3자로부터 안전하게 사용하기 위해서 환자의 속성정보를 분류하고, 병원 관계자는 역할에 따라 권한을 분류하여 계층적 다중 속성을 이용한 환자의 프라이버시 보호기법을 제안한다. 제안 기법은 환자의 프라이버시 속성정보(데이터 소비자, 시간, 센서, 목적, 의무, 위임 그리고 상황 등)를 수학적 모델로 표현하고, 제 3자로부터 환자 정보가 불법적으로 악용되는 것을 예방하기 위해서 환자와 병원관계자 사이의 속성정보를 동기화하여 환자의 프라이버시 정보의 유출을 예방한다.

A Privacy-aware Graph-based Access Control System for the Healthcare Domain

  • The growing concern for the protection of personal information has made it critical to implement effective technologies for privacy and data management. By observing the limitations of existing approaches, we found that there is an urgent need for a flexible, privacy-aware system that is able to meet the privacy preservation needs at both the role levels and the personal levels. We proposed a conceptual system that considered these two requirements: a graph-based, access control model to safeguard patient privacy. We present a case study of the healthcare field in this paper. While our model was tested in the field of healthcare, it is generic and can be adapted to use in other fields. The proof-of-concept demos were also provided with the aim of valuating the efficacy of our system. In the end, based on the hospital scenarios, we present the experimental results to demonstrate the performance of our system, and we also compared those results to existing privacy-aware systems. As a result, we ensured a high quality of medical care service by preserving patient privacy.

정보화시대의 환자진료정보 보호에 관한 법.제도적 고찰 (A Study on Medical Information Privacy Protection Law and Regulation in the Information Age)

  • This study discusses the direction of legislation to strengthen the legal protection of medical records privacy in information age. The legislation trends on privacy protection of medical records in European Union and United States are analysed and the current law and regulation of Korea on medical records are compared. The issues discussed include the ownership of medical records, the patient's right of access to medical records, medical information publication for other than treatment or insurance processing use, confidentiality responsibility of provider organizations, medical information management in provider organizations, penalty for the unlawful use of patient information. This study concludes that the patients' right on medical record and provider organization's responsibility in processing patient information should be strengthened in order to protect patients' privacy and to conform to the international standard on medical record protection in the information age.

병원간호사의 정보활용능력과 개인정보보호에 대한 인식이 윤리적 가치관에 미치는 영향 (Influence of Information Literacy and Perception of Patient Data Privacy on Ethical Values among Hospital Clinical Nurses)

  • Purpose: The aim of this study was to elucidate clinical nurses' ethics germane to information literacy and perception of patient data privacy and thus help nurses to develop more positive and consolidated ethical values. Methods: For this study a descriptive survey design was used. Participants were 142 nurses who worked in a hospital and completed self-report questionnaires. Data were collected from August 1 to 5, 2016 and were analyzed using independent t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficients, and stepwise multiple regression with SPSS 22.0. Results: Ethical value had a positive correlation with information needs (r=.25, p=.002) in information literacy as well as in direct patient care (r=.27, p=.001), shift work (r=.20, p=.016), patient information management (r=.39, p<.001), and communication (r=.24, p=.004) in perception of patient data privacy. Patient information management, educational background, and age were significant variables predicting the level of ethical values and accounted for 21% of the variance. Conclusion: Ethical values education with particular emphasize on managing patient information should be encouraged for nurses who are younger and have a lower education level. Findings indicate a need for education programs to guide clinical nurses to utilize appropriate information when solving ethical challenges in every day nursing practice.
