간호대학생에게 적용한 노인환자 유사체험에 관한 연구 (A Study on Nursing Students' Elderly Patient Simulation Experience)

  • 본 연구는 충청북도 소재 일개 대학교 간호학과에 재학 중인 2학년 학생 88명(실험군: 47명, 대조군: 41명)을 대상으로 노인환자 유사체험의 효과를 검정하기 위해 수행되었으며 결과변수로는 노인 및 노인환자에 대한 태도와 공감 정도가 측정되었다. 연구 결과 노인환자 유사체험은 간호대학생의 노인 및 노인환자에 대한 태도에는 유의한 효과를 미치지 못하였으나 공감 수준을 향상시키는데 효과가 있는 것으로 나타났다. 또한 대상자들은 노인환자 유사체험에 대한 제한점으로 1회성의 단편적 경험, 노화에 대한 불안감 야기, 체험복장의 현실감 부족, 체험 과정 중의 사고 위험성 등을 제시하였고 이러한 제한점이 유사체험의 긍정적 효과를 약화시킬 수 있는 것으로 제기되었다. 따라서 이러한 제한점들을 고려하여 중재 프로그램을 개선하여 제공한다면 중재가 간호 대학생들의 노인환자에 대한 긍정적 태도와 공감적 이해증진으로 이어지는데 크게 기여할 것으로 판단되었다.

간호 조직구조와 간호결과의 관계 (Relationship Between Nursing Organizational Structure and Nursing Outcome)

  • Purpose: This study was investigated to identify the relationship among hospital nursing organizational structure(decentralization, communication, bed/nurse ratio), process(nurses' job satisfaction, nurses' clinical experiences, nurses' unit experiences), and nursing outcome(patient satisfaction, physiologic adaptation, length of stay, number of complication). Method: The subjects consisted of 86 hysterectomy patients and 23 nurses in gynecology unit. Data were collected from May 16, 2002 to August 15. 2003 by the structured questionnaires and chart review. Data analysis was done with ANOVA, Pearson's correlation coefficient, path analysis. Result: Relationship between organizational structure and process ; Bed/nurse ratio was negatively related to nurses' job satisfaction(r=- .37, p<.05), and nurses' clinical experience(r=- .69, p<.00). Decentralization(r=.42, p<.05) and comunication(r=.61, p<.00) were positively related to nurses' clinical experiences. Relationship between process and nursing outcome ; There was a significant relationship between nurses' unit experiences and patient satisfaction(r=.63, p<.00), nurses' job satisfaction and physiologic adaptation(r= .44, p<.05), nurses' unit experiences and physiologic adaptation(r=.64, p<.00), Relationship between organizational structure and nursing outcome ; Decentralization and communication were positively related to patient satisfaction(r=.86, p<.00 ; r=.88, p<.00) and physiologic adaptation(r=,51, p<.01, r=.64, p<.00). Conclusion: Nurses' unit experience, communication, decentralization were significant variables for patient satisfaction. Nurses' unit experience, nurses' job satisfaction, communication were significant variables for patient physiologic adaptation.

응급구조(학)과 학생들의 환자 개인정보보호에 대한 인식 및 태도 (Recognition and attitude of paramedic students regarding patient privacy protection)

  • Purpose: This study sought to analyze perceptions and attitudes regarding patient personal information protection according to the general characteristics of paramedic students and their clinical practice experience. Methods: A total of 215 paramedic students from G university in I metropolitan city and D university in G do were surveyed. Frequency analysis, percentage analysis, and one-way distribution analysis were conducted using the software SPSS version 23.0. Results: First, recognition regarding the operation of laws and systems related to medical law and personal information protection was high among students who had no clinical practice experience, and there were no differences with regard to students' general characteristics. Second, the level of recognition regarding patient personal information protection and exposure did not differ depending on students' general characteristics and clinical practice experience. Conclusion: Based on the results of this study, students' recognition and attitude should be improved by carrying out continuous education on patient personal information protection. Furthermore, more specialized and systematic training related to patient personal information protection should be conducted to nurture appropriately trained paramedic students.

중소병원 간호사의 환자안전태도, 안전통제감이 안전간호활동에 미치는 영향 (Factors affecting the Patient Safety Attitude and Safety Control on Safety Care Activities among Nurses in Small and Medium-sized Hospitals)

  • 본 연구는 중소병원 간호사의 안전간호활동에 영향을 미치는 요인을 파악하기 위한 서술적 조사연구이다. 연구대상은 G광역시에 소재하는 7개 중소병원 간호사 227명을 대상으로 하였다. 자료수집은 2016년 5월 16일부터 5월 23일까지 이루어졌으며, 수집된 자료는 SPSS 18.0 program을 이용하여, 기술통계, t-test, one way ANOVA, Pearson's correlation, multiple regression으로 분석하였다. 분석 결과 환자안전태도 4.11점, 안전통제감 3.74점, 안전간호활동 4.33점이었다. 일반적 특성에 따른, 대상자의 환자안전태도는 학력과 직무만족도에서 통계적으로 유의한 차이를 보였으며, 대상자의 안전통제감은 연령, 학력, 근무경력, 직위, 안전교육경험, 안전사고경험, 직무만족도에서 유의한 차이를 보였고, 대상자의 안전간호활동은 학력, 근무부서, 환자안전교육, 직무만족도에 따라 통계적으로 유의한 차이를 보였다. 환자안전태도, 안전통제감, 안전간호활동 간에 유의한 양의 상관관계가 있었다. 대상자의 안전간호활동에 환자안전태도, 안전통제감, 환자안전교육이 긍정적인 영향을 미치는 것으로 나타났다. 본 연구를 기초로 중소병원 간호사들의 안전간호활동에 대한 강화를 위해서 환자안전태도와 안전통제감을 향상시킬 수 있는 인식전환이 필요하다. 또한, 지속적인 환자안전교육을 위한 다양한 안전 간호사례 제공과 체계적인 지원이 필요하다.

전문병원 입원환자의 환자경험 및 추천의향 (Patient experience and recommendation intention at specialty hospitals)

  • Purposes: Patient experience is a tool to evaluate the process and results of medical services provided by medical institutions from the patient's point of view. Patient satisfaction surveys are a meaningful and essential source of information for improving quality in healthcare organizations. This study aims to provide basic data for improving the quality of medical service that patients can feel by analyzing the recommendation intention and satisfaction of inpatients in specialty hospitals. Methodology: The subjects of this study were 879 inpatients in 28 specialty hospitals in 14 designated fields. We conducted a telephone survey with a structured questionnaire on the satisfaction and recommendation intention for specialty hospitals. Findings: In inpatients, hospital satisfaction was higher in nursing care services and hospital satisfaction was low in physicians care services. The overall patient satisfaction score was 91.4(SD=11.9) out of 100, and the intention of recommendation was 92.0(SD=14.1) out of 100. The factors affecting patient experience were designated fields, sex, age, residential area, monthly household income, and perceived health status. Practical Implications: This study confirmed the high level of patient satisfaction and recommendation intention among inpatients of specialty hospitals. Patient satisfaction can be of great value to healthcare providers in recognizing and improving the quality of care, as well as predicting patients' willingness to revisit medical institutions. This study can be used to improve the quality of hospital care services in specialty hospitals rather than general and tertiary general hospitals.

Affordance design for the Management of Patients' Experience in Small Scale Hospitals

  • In the recent years, with a purpose to provide visual and psychological stability to patients and hospital users, Affordance design theory based on perceptual psychological thought has been applied as hospital design. In the past, hospital service design was focused on but Affordance design theory focuses on the patients' experience and methodological research on . The characteristics of the application of Affordance design in the case of patients' experience and service point is as follows:- Firstly, Affordance design is applied when the objective of the patient's experience and the functionality of the object are combined. In other words, affordance design is a condition that arises when encounter between service contact point and patients' experience takes place. Secondly, In the field of Hospital service design, Affordance concepts in and its application can be divided on the functions of experience, cognition and sense following the application of Affordance concepts in Hospital service design. Thirdly, Affordance design method sets meaning of the action received through information provided by morphological features. The condition and surrounding has to be designed in a way that it can induce active behavior of the patient. This study has significance as it has been studied from the perspectives of Affordance theory of design psychology in addition to the physical environment of the hospital, the point of interaction between the hospital and patient, along with the elements of experience design. This study proposes the necessity of analytical study in the field of service design implementation considering various stakeholders in the service contact points of small and medium hospitals in Korea. Furthermore, study of the application of Affordance design can be complemented by reflecting the size of hospitals, the characteristics of specialties, and the specific service environment of each hospital can also be conducted.

CarboMedics 기계판막의 임상경험 (Mid term experience with CarboMedics Medical Valve)

  • The CarboMedics valve is a bileaflet prosthesis with excellent hemodynamic characteristics, but the long term surgical experience with this valve, its durability and its biocompatibility are unknown. During a 5 year period from october 1988 to July 1993, 748 prostheses [402 mitral, 261 aortic, 58 tricuspid, 27 pulmonic] were inserted in 552 patients [mean age 40.2 years]. The operative mortality was 6.6% [37/560, 13.2% in age group below 15 years and 5.7% above 15 years]. and the main causes of death were complex congenital malformation and left ventricular failure. Follow up was totaled 1182 patient- years and mean follow up was 28.3 months/patient. No structural failure has been observed. Hemorrhage was the most frequent valve related complication[1.78% / Patient-year]. Embolism occurred at a rate of 0.93% / Patient-year. There were 5 cases of valve thrombosis [0.42% / Patient-year, two fatal]. There occurred 11 late deaths[6 valve related] and 42 valve related complications. Actuarial survival at 5 years is 97.18 0.94% and actuarial complication free survival at 5 years is 89.07 1.54%. In summary, the CarboMedics valve stands for a durable valve substitute, with low valve related complications.

신규간호사의 환자안전동기, 환자안전관리태도가 환자안전관리행위에 미치는 영향 (The Effects of New Nurses' Patient Safety Motivation, Patient Safety Management Attitude on Patient Safety Management Behavior Korean)

  • 본 연구는 신규간호사를 대상으로 환자안전동기, 환자안전관리태도, 환자안전관리행위 정도와 상관관계를 확인하고 환자안전관리행위에 영향을 주는 요인을 알아보고자 실시하였다. 신규간호사 127명을 대상으로 2020년 9월 11일부터 9월 30일까지 설문조사 하였으며, 수집된 자료는 SPSS 22.0 프로그램을 이용하여 t-test, ANOVA, Scheffe's test, Person's correlation coefficients, 다중회귀분석을 하였다. 환자안전동기는 환자안전관리태도, 환자안전관리행위와 양의 상관관계를 보였으며, 환자안전관리태도는 환자안전관리행위와 양의 상관관계를 보였다. 환자안전관리행위에 가장 유의한 영향을 준 요인은 환자안전동기, 환자안전관리태도, 환자안전관리 교육경험 이었다. 따라서 신규간호사의 환자안전관리행위 능력을 증진시키기 위해 환자안전동기, 환자안전관리태도를 포함한 환자안전교육 프로그램의 개발과 적용을 제안한다.

노인간호에 대한 간호원의 태도조사연구 (A Study of the Attitudes of Nurses toward the Geriatric Nursing Care)

  • Presently, there is increasing demand for geriatric nursing care because of increasing preparation of elderly population due to extended age. Of particular importance in determining the effectiveness of nursing care given to the elderly is the attitude of the nurse toward the elderly patient. Knowledge of the various changes that usually occur with aging will enable the nurse to help an aged Person maximize his potential in illness and in health. The objectives of this study were : 1. To delineate the attitudes of nurses toward the geriatric nursing care. 2. To learn the influencing factors affecting the attitudes of nurses toward the geriatric nursing care. The study population defined and randomly selected is 225 nurses at one general hospital, one private hospital, two national hospitals in Seoul during the period of October l0th -20th, 1975. The questionnaire method was used. Respondents were 140 nurses. X$^2$- test and t- test were employed in analyzing the data. The questionnaire form included 54 statements which concerned the attitudes of nurses to-ward geriatric nursing care. It was divided into five areas : 1) General characteristics of study population. 2) The nurse's concepts of elderly. 3) Care of the geriatric patient. 4) Interpersonal relationship with geriatric patient. 5) Teaching of the geriatric patient. Each of the 54 statements of the questionnaire was considered to be either Positive or Negative. A Positive response was assigned the value of+1 , and a Negative response or no response was assigned the value of O. The results of the study were as follows : 1. The Investigation of attitudes of nurses toward geriatric nursing care. a. Data indicated , respondents have negative attitudes in their. concepts of elderly (74.3%) b. Data indicated respondents have positive attitudes in care of geriatric patient (64.3%). c. Data indicated respondents have positive attitudes in interpersonal relationship with geriatric patient (85% ). 4. Data indicated respondents have positive attitudes in teaching of geriatric patient (89.3%). 2. The results of study regarding the five hypothesis were as follows : a There was significant difference in the mean scores between the attitudes of nurses toward the geriatric nursing care and level of education of the nurse. b. There was no significant difference between the attitudes of nurses toward the geriatric nursing care and clinical experience, but there was significant difference between care of the geriatric patient and clinical experience. c. There was no significant difference between the attitudes of nurses toward the geriatric nursing care and shift most frequently worked, but there was significant difference between care of the geriatric patient and shift most frequently worked. d. There was no significant difference in the mean scores between the attitudes of nurses toward the geriatric nursing care and marital status, but there was significant difference in the mean scores between interpersonal relationship, teaching of geriatric patient and marital status. e, There was no significant difference in the mean scores between the attitudes of nurses toward the geriatric nursing care and experience with elderly, but there was significant difference in the mean scores between care of geriatric patient and experience with elderly.

중소병원 간호사의 투약 근접오류경험 영향요인 (Influencing Factors of Near Miss Experience on Medication in Small and Medium-Sized Hospital Nurses)

  • 본 연구는 중소병원 간호사의 투약 근접오류경험의 영향요인을 확인하여 근접오류 예방과 안전한 투약간호를 위한 교육프로그램 개발의 기초자료를 제공함으로써 중소병원의 환자안전문화 구축에 기여하고자 시도된 서술적 조사연구이다. 수집된 자료의 분석은 SPSS/WIN 20.0을 이용하여 𝑥2-test, Independent t-test, one-way ANOVA, Logistic regression analysis를 실시하였다. 연구결과, 투약 근접오류경험에 영향을 미치는 요인은 일반적 특성 중 근무부서와 환자안전문화였다. 모수 추정치 값의 승산비(odds ratio)는 특수부서 근무자보다 일반병동 근무자가 근접오류를 경험하지 않을 교차비가 2.23(95% 신뢰구간: 1.07~4.67, p=.032)이었으며, 환자안전문화 점수가 1점 증가할 때 근접오류를 경험하지 않을 교차비가 2.24(95% 신뢰구간: 1.02~4.95, p=.045)인 것으로 나타났다. 즉, 특수부서 근무자가 일반병동 근무자보다 근접오류를 경험할 확률이 높고, 환자안전문화 인식정도가 높을수록 근접오류를 경험할 확률이 낮은 것을 알 수 있었다. 따라서 간호사들의 환자안전문화 인식 개선을 위한 병원차원의 오류감시시스템의 개발과 간호조직 차원에서 경력 및 부서별 특성에 따른 맞춤형 투약교육 프로그램을 개발하여 이론 교육과 함께 시뮬레이션 훈련이 필요할 것으로 사료된다.
