Staffs' Attitudes and Opinions toward Psychiatry in a Psychiatric Hospital (일 정신병원 직원의 정신과에 대한 인식과 태도)

  • Objectives : The aim of this study was to investigate the staffs' attitudes and opinions toward psychiatry in a hospital. The results of the investigation were taken as a basic data of the education programs of persons who worked in a hospital and community residents. Methods : The subjects of this study were persons who worked in a hospital. The tool used in this study was Attitudes and Opinions toward Psychiatry. Sixty-one persons were studied. Statistically one-way ANOVA or t-test were used. Results : Internal consistency was high, measuring 0.80 for the 26 items. Generally, hospital staffs have positive attitude toward psychiatry. Direct patient contact group scored more higher than indirect contact group, except 'overall merits of the field of psychiatry' and 'efficacy' factor. In factor 'overall merits of the field of psychiatry', and factor 'efficacy' female scored more higher than male averagely. In factor 'role definition and functioning of psychiatrists', 'possible abuses and social criticisms', and 'career and personal reward', direct patient contact group scored more higher than indirect contact group averagely. Conclusion This study results support the hypothesis that the direct patient contact group have more positive attitude toward psychiatry than the other group. It will be necessary to educate for a psychiatric hospital staffs and community residents on psychiatry.

The stability of tear film for Korean (한국인에 대한 누액 안정성 조사)

  • The purpose of our study was stability of tear film for Korean. In the 1970s, the major problem for the contact lens practitioner was the management of contact lens parameters. In the 1980s, oxygen permeability was the main problem in contact lens practice: now it is dry eyes. To appropriately fit the patient with contact lenses it is imperative to know in advance if the patient has a marginal dry eye. Based on the initial diagnosis appropriate care can be taken to minimize problems with the result that the patient can successfully wear contact lenses. A well known test for dry eyes is Schirmer test which has shown to be useful for diagnosing Sjogren syndrome. Biomicroscopy can often unmask numerous problems. The classical test for marginal dry eye is measuring the tear break up time(TBUT). A dry eye problem with contact lenses is often caused by the lid. If an incomplete blink is responsible for the dry eye it is possible to change the situation by blinking exercises. The result of these test, mean value of age was 22.8, mean value of blink rate was 19 times/min, mean value of Schirmer test was 25 mm/5 min, mean value of TBUT was 7.1 sec.

  • Objectives: This study reports using Hyungbangsabaksan to cure Soyangin Yangdokbalban (陽毒發斑) appealing Systemic Allergic Contact Dermatitis. Methods: After diagnosing the patient showing Papuloerythematous with no blisters in the body as a Soyangin Yangdokbalban (陽毒發斑), we prescribed the patient with Hyungbangsabak-san. We compared the condition of the patient before and after medication with photo shots. Results: After treatment, Papuloerythematous which was seen with the naked eye disappeared and various skin symptoms such as itching and warmth in the flare area also improved. Conclusions: After treatment, the symptoms of the patient who after eating lacquer chicken showed improvement.

The Effect of Head Posture Change on Initial Occlusal Contact in Temporomandibular Disorder Patient (측두하악 장애환자에서 두부자세 변화가 초기 교합접촉에 미치는 영향)

  • The purpose of this study was to evaluate an effect of change on head posture initial occlusal contacts with measuring the distances between initial occlusal contacts and maximum intercuspal position at different head posture in TMDs patient. For this study, 24 patients from age 13 to 36 were selected, they were examined health history taken, patients who have sign and symptoms of TMDs were examine before the study. For the normal group, 21 adults from age 23 to 25 were selected. They have normal or class I molar relationship, and have no other prosthetic restorations. Difference on distance between initial occlusal contact and maximum intercuspal position with mandibular kinesiograph$(MKG^R)$(K6 diagnostic system, Myo-tronic Inc, USA) in upright, supine, 45$^{\circ}$ extension, 30$^{\circ}$ flexion position of the head were measured. The Frankfort horizontal plane was used as a reference plane. The results were as follows : 1. There were significant differences between initial occlusal contacts of the normal and patient group on upright position and 30$^{\circ}$ flexion of the head(p<0.05, p<0.01) 2. The position of the initial occlusal contacts have a tendency to place anterior and inferior to maximal intercuspal position in upright position and 30( flexion of the head as well as posterior and inferior in supine position and 45$^{\circ}$ extension of the head in the normal and patient groups. 3. There were significant differences among the initial occlusal contacts between uptight and supine position; upright and 45$^{\circ}$ extension of the head(p<0.05); supine position and 30$^{\circ}$ flexion of the head, .and 30(flexion and 45$^{\circ}$ extension of the head in the patient group(p<0.01) The result have shown that after treatment on the supine position, it may be necessary to check occlusal contact on the upright position as well ass flexion of the head. It may need careful adjustment in occlusal condition on upright position of TMDs patient.

2 Cases of Occupational Contact Dermatitis (직업성 접촉 피부염 치험 2례)

  • Objectives : The purpose of this case report is to know the effect of Korean medical treatments on occupational contact dermatitis. Methods : We reported 2 cases who visited Allergy and Derma-Cosmetic Clinic of Kyung Hee University Hospital at Gangdong. They were diagnosed with occupational contact dermatitis and treated by Korean medical treatment. Results : One patient recovered after 13 days of admission and total 9 months of outpatient period. Another patient recovered after 9 days of admission and total 7 months of outpatients period through Korean Medical Treatment. Conclusions : We may expect that Korean medical treatment had therapeutic effects on occupational contact dermatitis, but the additional study will be conducted for revealing the exact therapeutic effects.

A Case Report of Contact Dermatitis treated by Mahwang-tang based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 마황탕(麻黃湯) 투여 후 호전된 접촉성피부염 1례)

  • Objective : The purpose of this paper is to report the improved contact dermatitis(CD) treated with the herbal medication based on Shanghanlun disease pattern identification diagnostic system. The level V steroid ointments were no use to the patient 3weeks before the treatment. Methods : In this case, the patient was diagnosed according to 'Disease Pattern Identification Diagnostic System based on Shanghanlun Provisions' and took Mahwang-tang for 36days since the final diagnosis was Taeyang-byung, number 46 provision. The photographs of patient's hands were taken to observe the process of the skin status and the discomfort from CD was measured with DLQI(Dermatologic Life Quality Index) and NRS(Numeric Rating Scale). Results : The DLQI score changed from 24 to 7 and NRS of itching changed form 8 to under 4. Conclusions : This case report shows that the herbal medication through the diagnostic system could be a remedy on CD, dealing with the skin status with no use of the level V steroid ointments. It is needed proving an alternative possibility of Korean Medicine, on other skin diseases, with later studies since there are many reports on the side-effects after some long-term steroid treatments.

Relation of Handwashing and Isolate of Bacteria from Mobile Phones of Healthcare Workers in a University Hospital

  • Mobile phones used by healthcare workers are not only an indicator of the contamination of healthcare associated bacteria, but can also be another source of infection. The number and time of handwashing, mobile phone operation time and disinfection were highly relation with the bacterial contamination on the surface of mobile phone. Healthcare associated bacteria isolated from the mobile phone surface were 28 MRCoNS (48.3%), 14 S. aureus (24.2%), 3 MRSA (5.2%), 5 A. baumannii (8.6%), 3 MRAB (5.2%), 3 Entrococcus spp. (5.1%), 2 Pantoea spp. (3.4%), 2 A. lowffii (3.4%), 1 E. cloacae (1.7%), 1 P. stutzeri (1.7%), and P. mirabillis (1.7%). For isolation according to department, 2 MRAB from the emergency room and 1 MRSA from intensive unit, the radiology team and the rehabilitation medical team, respectively were isolated. As a result of the relation of isolates from the department of patient contact (ER, RT, GW, CP, ICU, RMT), the bacterial isolation rate was 75% and the department of patient non-contact (MRT) was 10%.

Development of the Upper Wear Fixation Device for Chest AP X-ray Imaging on the Emergency Stretcher Bed (응급실 침대 위 흉부전후방향 엑스선 검사를 위한 상의고정장치 개발)

  • This study aimed to provide basic data for 3D printing in the medical health field by developing upper wear fixation device (UWFD), an auxiliary device for shortening chest AP examination time on emergency room beds and non-contact with patients. The standard of hooks was modeled according to the bed frame using the Autodesk Fusion 360. It was printed with Form2 (Formlabs, Somerville, MA, USA), as SLA (stereo lithography apparatus) method, and was washed and hardened using Form Wash and Form Cure. The completed UWFD conducted an online survey on 4 items of stability, convenience, availability, preference and general characteristics. The total stability average was 3.93±0.80, the total convenience average was 3.93±0.68, the total availability average was 4.01±0.89, and the total preference average was 3.80±1.08. This study was significant in suggesting improvements in the general X-ray examination process in the emergency room by designing and making aids to easily fixing the patient's top to the frame of the emergency bed while meeting promptness and non-contact with the patient.

The Current State of Lens Care in Contact Lens Wearer (콘택트렌즈 착용에 따른 사용자 실태조사)

  • The Hydroxyethyl methacrylate(HEMA) was developped in 1960's and later the use of contact lenses increased. At present more and more people use RGP contact lenses. Due to the wide range of optical corrections available and due to their high oxygen transmission Japanese and Korean people like them. Nevertheless one of nine CL users(beginners) stopped wearing the CL within the first year. We conducted a survey to evaluate the level of patient compliance in lens care and maintenance and to assess practitioner knowledge of contact lens information. Generally 6% of contact lens wearers drop out per year. The aim of this research was to find out what opticians can do to reduce the drop-out rate by getting information on handling, on general data of the patient and on optical prescription. Lens wearer were asked to complete a 10 question survey that focused on lens care-wear modality, lens replacement, storage, symptoms and case disinfection. There were total 180 participants in the study. Most of contact lens wearers have an inadequate understanding of contact lens care system. Therefore, it is important that contact lens specialists place more emphasis on practitioner education about general contact lens fields and reinforce patient education during the dispensing visit.

Survey of ICU Nurses' Knowledge of the Specific Moments of Hand Hygiene (일 대학병원 중환자실 간호사의 손 위생 수행 시점에 대한 지식 조사)

  • Purpose: This study was conducted to identify ICU nurses' knowledge of the five moments of hand hygiene and the ambiguity of these moments when demonstrating hand hygiene. Methods: The subjects were 200 intensive care unit nurses at a university hospital. Data was collected using self-report questionnaires, translated according to the instructions of training films developed by WHO, and analyzed using descriptive statistics and ranking tests. Results: The highest number of correct answers was regarding the moment before contact with a patient and the lowest was regarding the moment after contact with a patient. The rate of providing wrong answers regarding required moments of hand hygiene was high. Conclusion: The study identified ICU nurses' knowledge of specific moments of hand hygiene; they had difficulty differentiating between the moments that happened simultaneously, i.e. after touching a patient, and that patient's surroundings, and there was ambiguity concerning patient areas and medical treatment areas. It was concluded that it is necessary to educate nurses regarding both required and unrequired moments of hand hygiene and to ensure that they can distinguish between these moments.
