PET-CT (Positron Emission Tomography-Computed Tomography)의 융합영상(Fusion image)에서 긴 영상 획득 시간에 의한 환자의 움직임은 융합영상에서 부적합한 결과로 나타나 영상의 질과 진단에 큰 영향을 준다. 의료장비 회사에서 제공하는 팔 지지대는 환자 자세에서의 편의성과 안전성을 고려하지 않아 PET/CT를 검사하는 동안 팔과 머리의 움직임 때문에 뇌기저부의 영상 결손이 생기고 머리의 좌우, 상하 움직임으로 인하여 재촬영이 빈번하다. 이에 우리는 두경부와 팔의 움직임을 최소화할 수 있는 환자보정기구를 개발하여 PET/CT 촬영 시 환자에게 편의성과 안전성을 제공하고 환자의 움직임으로 인한 재촬영을 줄이고자 한다. 2012년 6월부터 7월까지 PET Torso 검사를 위해 핵의학과에 내원하는 환자들 중 움직임에 불편함이 전혀 없는 환자 20명과 어깨 통증으로 팔을 올리기가 불편한 환자 20명을 대상으로 Patient Holding System (PHS)와 팔의 경사도($25^{\circ}$, $27^{\circ}$, $29^{\circ}$, $31^{\circ}$, $33^{\circ}$, $35^{\circ}$)를 변화시켜 가장 편안한 자세와 경사도를 선정 하였다. 또한 지지대 재질의 투과성에 대하여 유관업체의 검증을 하였고, 환자의 머리와 팔을 밴드(벨크로 형태)로 고정하여 이에 대한 편의성을 질의하였다. 그리고 움직임에 의한 재촬영 빈도를 알아보기 위해 2012년 1월부터 12월까지 PET Torso를 시행한 환자들 중 환자보정기구 사용 전/후 재촬영 여부를 분석하였다. 움직임에 불편이 전혀 없는 환자에서는 18명이 PHS와 팔의 각도가 $29^{\circ}$에서 가장 편안함을 느낀다고 답하였으며, 2명은 각각 $27^{\circ}$, $31^{\circ}$이라고 답하였다. 어깨 통증을 느끼는 환자에서는 15명이 $31^{\circ}$에서 가장 편안함을 느낀다고 답하였고, $33^{\circ}$에서 2명, $35^{\circ}$에서 3명이었다. 이를 위해 손잡이 부분이 상, 하 움직임이 가능하도록 제작하였다. 환자보정기구 재질의 투과성은 검증된 상태이며, 환자보정기구 자체의 움직임을 방지하기 위하여 PHS와 보정기구를 밴드(벨크로 형태)로 고정시켰다. 두경부의 움직임을 최소화할 수 있는 홈을 만들고 머리를 고정할 수 있게 고정밴드를 부착 하였고, 손목 및 전완과 상완의 움직임을 제한할 수 있는 고정밴드를 부착하여 팔의 흔들림으로 인한 움직임을 최소화 하였다. PET Torso 촬영 시 움직임에 의한 재촬영 빈도는 환자고정기구 사용 전 10.56% (191건/1,808명), 사용 후 2.77% (48 건/1,732명)로 7.79%의 발생 빈도를 줄일 수 있었다. 최근 의료 환경의 변화와 혁신으로 고가의 영상검사가 늘어남에 따라 고객이 요구하는 차별화된 서비스 제공은 필수적이다. 따라서 PET/CT를 검사하는 동안 환자의 편안함과 안전성을 확보하기 위해 인체공학적인 환자보정기구를 제공하여야 한다. 따라서 본 연구를 통해 PET Torso 검사 시 환자의 체형과 상황에 따라 인체공학적 경사도를 상, 하 조절할 수 있는 환자보정기구를 제작하였으며, 팔의 움직임에 의한 뇌기저부의 영상결손을 줄이고 머리의 움직임으로 인한 재촬영을 줄일 수 있었다.
In robotic surgery, a surgeon checks only a surgical site of patient in the progress of surgery by vision and sound information. In order to solve this limited information, the haptic function is necessary. And haptic surgical robot is also necessary to design a haptic master device. The master device for laparoscope operation with cable-conduit was developed in previous research to give haptic function. It suggested a possibility of developing a master device by using the cable-conduit. However, it is very inconvenient to use. Therefore, this paper suggests a new mechanism design structure to solve the problems of the previous work by new forming a new master device. And it has proved that it's usability is better than previous one. Furthermore it has also experimented and analyzed that a backlash of new master device is compensated by smooth backlash inverse algorithm.
Intrafractional motion of patients, such as respiratory motion during radiation treatment, is an important issue in image-guided radiotherapy. The accuracy of the radiation treatment decreases as the motion range increases. We developed a control system for a robotic patient immobilization system that enables to reduce the range of tumor motion by compensating the tumor motion. Fusion technology, combining robotics and mechatronics, was developed and applied in this study. First, a small-sized prototype was established for use with an industrial miniature robot. The patient immobilization system consisted of an optical tracking system, a robotic couch, a robot controller, and a control program for managing the system components. A multi speed and position control mechanism with three degrees of freedom was designed. The parameters for operating the control system, such as the coordinate transformation parameters and calibration parameters, were measured and evaluated for a prototype device. After developing the control system using the prototype device, a feasibility test on a full-scale patient immobilization system was performed, using a large industrial robot and couch. The performances of both the prototype device and the realistic device were evaluated using a respiratory motion phantom, for several patterns of respiratory motion. For all patterns of motion, the root mean squared error of the corresponding detected motion trajectories were reduced by more than 40%. The proposed system improves the accuracy of the radiation dose delivered to the target and reduces the unwanted irradiation of normal tissue.
A new transcutaneous energy transmission (TET) system was developed for transmitting electrical power to an implanted device, such as an artificial heart in a patient's body. This new design can maintain a stable output voltage independent of the load resistance. The system includes a compensation capacitor to reduce energy loss and increase power transfer efficiency. Experimental results show that the output voltage of the receiving coil changes very little as the load resistance varies from 14.8 ${\Omega}$ to 15 $k{\Omega}$, which corresponds to a change in output power from 0.1 to 97 W.
The purpose of this study was to develop a hand rehabilitation training system for hemiplegic patients. We also tried to find out five hand postures (WF: Wrist Flexion, WE: Wrist Extension, BG: Ball Grip, HG: Hook Grip, RE: Rest) in real-time using multi-channel EMG-based deep learning. We performed a pre-processing method that converts to Spider Chart image data for the classification of hand movement from five test subjects (total 1,500 data sets) using Convolution Neural Networks (CNN) deep learning with an 8-channel armband. As a result of this study, the recognition accuracy was 92% for WF, 94% for WE, 76% for BG, 82% for HG, and 88% for RE. Also, ten physical therapists participated for the usability evaluation. The questionnaire consisted of 7 items of acceptance, interest, and satisfaction, and the mean and standard deviation were calculated by dividing each into a 5-point scale. As a result, high scores were obtained in immersion and interest in game (4.6±0.43), convenience of the device (4.9±0.30), and satisfaction after treatment (4.1±0.48). On the other hand, Conformity of intention for treatment (3.90±0.49) was relatively low. This is thought to be because the game play may be difficult depending on the degree of spasticity of the hemiplegic patient, and compensation may occur in patient with weakened target muscles. Therefore, it is necessary to develop a rehabilitation program suitable for the degree of disability of the patient.
The using of compensator is required to adjust the irregular dose distribution due to irregular thickness of the body in Total Body Irradiation. Aluminuim, copper or lead is generally used as compensator. In our study, we would like to introduce a result of the attenuation and compensation effect of radiation use compensator made by duralumin and its clinical use. The thickness of compensator was calculated by the attenustion of radiation, which was measured by polystyrene phantom and ionization chamber(farmer). The compensation effect of radiation was measured by diode detector. All of conditions were set as in real treatment, and the distanc from source to detector was 446 cm. We also made fixation of device to easily attach the compensator to LINAC. Beam spoiler was menufactured and placed on the patient to irradiate sufficient dose to the skin. diode detector were placed on head, neck, chest, umbilicus. pelvis and knee with each their entranced exit points, and datas of dose distribution were evaluated and compared in each points for eleven patients(Feb. 96-Feb. 97). The attenuation rate of irradiation by duralumin compensator was measured as $1.4\%$ in 2mm thickness. The mean attenuation rate was $1.3\%$ per 2mm as increasing the thickness gradually to 50 mm. By using duralunim compensator, dose distribution in each points of body was measured with ${\pm}2.8\%$ by diode detectior. We could easily calculate the thickness of compensator by measuring the attenuation rate of radiation, remarkably reduce the irragularity of dose distribution duo to the thickness of body and magnify the effect of radiation therapy.
우리나라는 최근 세월호 사건 등 안전 불감증으로 인한 많은 사고로 인해 수많은 인명피해를 입었다. 따라서 안전교육이 어느 때 보다 중요한 시기이며 여기에는 '어떤 콘텐츠로 어떻게 교육할 것인가'가 중요한 주제이고 특히 안전교육의 특성상 이론 교육이 아닌 체험 교육이 효과적이다. 그러나 이런 안전교육 프로그램을 접하기란 쉬운 일이 아니며 안전교육의 일환인 응급 처치에 대한 교육은 공공기관을 통해 의무적으로 배우지 않으면 접할 기회가 많지 않아 아직까지도 우리나라의 안전 교육에 대한 프로그램 활성도는 미미한 수준이다. 본 논문은 이런 문제를 인식하고 의료 응급처치 교육을 위해 재미와 몰입을 가미한 효과적인 기능성게임을 제안한다. 이를 위해 응급처치 정보 애플리케이션 20가지 사례를 통해 의료 게임 5개를 분석하고 기능성게임의 지속 사용성을 높이는 5가지 요소를 도출하였다. 5개의 의료게임분석을 통해 1개의 게임을 선택하여 게임 방식을 차용하고, 5가지 요소를 level-up 구조, 반복학습, 보상결과, 경쟁 구조, 정보전달의 형태로 적용하였다. 제안된 의료 교육 기능성 게임은 1) 환자의 역할을 하는 캐릭터가 있어야 하며, 2) 상황을 보여주는 내러티브 흐름에서 3) 사용자가 상황을 판단하고 응급 처치를 하도록 유도해야한다. 또한 4) 보상과 레벨 그리고 단순하게 반복하는 기능이 디자인되어야 하며 5) 커뮤니티로 타인과 정보가 공유될 수 있어야 한다. 향후 본 연구의 결과로 구현된 콘텐츠는 우리나라 의료 응급처치 교육의 대중화에 기여할 것이라 사료된다.
오늘날에는 기술의 발전으로 인간의 뇌에 직접 자극을 가하여 질병을 치료하거나 뇌파를 통해 직접 기계를 조정하는 것이 가능해졌다. 이러한 뇌신경과학기술은 비침습적으로 이루어질 수도 있으나 적어도 현재까지는 뇌에 직접 전극이나 마이크로칩을 이식하는 침습적 방법이 필요한 자극을 더 정확하게 가하거나 뇌파를 더 정밀하게 측정할 수 있다. 뇌심부자극술(DBS)의 경우 파킨슨병, 본태성진전증에 대해 안정적인 치료방법으로 인정받고 있으며, 그 외 알츠하이머나 우울증 등에도 활용할 수 있는지 연구가 진행중이다. 뇌-컴퓨터 인터페이스(BCI)의 경우 임상단계이지만 신경이 손상되어 신체를 움직이지 못하는 사람들의 신체기능을 대체하거나 재활치료를 지원하는 역할을 할 것으로 기대되고 있다. 문제는 이러한 침습적 뇌신경과학기술을 필요로 하는 사람들의 상당수는 질병 또는 신경 손상으로 인해 판단능력이 손상되어 있거나 의사표시를 제대로 할 수 없는 사람들인데 반해, 이 기술들을 이용한 시술은 고도의 침습적인 시술이어서 반드시 환자 본인의 동의를 필요로 한다는 것이다. 특히 뇌신경과학기술이 아직 임상시험단계에 머물러 있는 영역에서는 위험은 그만큼 커지고 이익은 불확실하기 때문에 이를 고려하여 수술을 받을지 여부를 판단할 수 있도록 더 충분한 설명이 이루어져야 한다. 환자에게 성년후견이 개시되어 있다면 성년후견인이 - 경우에 따라서는 법원의 허가를 받아 - 이 시술에 대한 동의를 대신할 수 있을 것이다. 성년후견이 개시되어 있지 않은 경우 환자의 판단능력이 손상되어 있거나 의사표시를 할 수 없는 경우가 문제이다. 우리 의료 실무에서는 환자가 동의할 수 없는 경우 환자의 보호자에게 동의를 받는 경향이 있지만, 환자의 보호자라는 개념은 우리법상 근거를 찾기 어려운 개념이어서 문제가 있다. 장기적으로는 환자의 배우자나 근친이 보충적으로 환자의 의료행위 동의대행권을 갖도록 법률상 규정하는 편이 타당할 것이다. 뇌신경과학시술을 받은 환자에게 부작용 등 손해가 발생한 경우도 검토를 요한다. 만일 환자에게 수술에 수반되는 위험에 대해 제대로 설명이 되지 않았다면 위자료 청구가 가능하다. 의료과실과 부작용 사이에 인과관계가 있다면 그 부작용에 대해서도 손해배상청구를 할 수 있다. 또한 BCI나 DBS 모두 뇌에 전극이나 마이크로칩등을 이식하고 이를 외부의 컴퓨터를 통해 제어하기 때문에 인체의식형 의료기기가 사용된다. 인체이식형 의료기기에는 제조물책임법이 적용되므로 그 결함으로 인해 부작용이 발생했다고 인정된다면 제조자에 대해서 손해배상을 청구할 수 있다. 최근 우리 의료기기법에는 인체이식형 의료기기 책임보험제도가 시행되어 피해자가 구제를 받을 가능성이 더 강하게 보장된다.

Background and Objectives We aimed to develop a Korean version of the velopharyngeal insufficiency (VPI) speech corpus system. Subjects and Method After developing a 3-channel simultaneous speech recording device capable of recording nasal/oral and normal compound speech separately, voice data were collected from VPI patients aged more than 10 years with/without the history of operation or prior speech therapy. This was compared to a control group for which VPI was simulated by using a french-3 nelaton tube inserted via both nostril through nasopharynx and pulling the soft palate anteriorly in varying degrees. The study consisted of three transcriptors: a speech therapist transcribed the voice file into text, a second transcriptor graded speech intelligibility and severity and the third tagged the types and onset times of misarticulation. The database were composed of three main tables regarding (1) speaker's demographics, (2) condition of the recording system and (3) transcripts. All of these were interfaced with the Praat voice analysis program, which enables the user to extract exact transcribed phrases for analysis. Results In the simulated VPI group, the higher the severity of VPI, the higher the nasalance score was obtained. In addition, we could verify the vocal energy that characterizes hypernasality and compensation in nasal/oral and compound sounds spoken by VPI patients as opposed to that characgerizes the normal control group. Conclusion With the Korean version of VPI speech corpus system, patients' common difficulties and speech tendencies in articulation can be objectively evaluated. Comparing these data with those of the normal voice, mispronunciation and dysarticulation of patients with VPI can be corrected.
일반 X-선 촬영에서 경험이 없는 비숙련자의 경우, 검사 실패의 경험을 통해 숙련되는 과정을 겪는다. 이것은 불필요한 의료 피폭 가중의 문제를 유발한다. 본 연구에서는 배치 전 비숙련 방사선사가 임상 숙련도를 높일 수 있는 방안으로, 시각적, 공간적 보조 장치를 이용한 두개골 측방향 촬영 실습을 통해, 촬영 보조장치 사용에 따른 정확도와 유용성을 평가하였다. 육안적 관찰과 학습에 의존해서 촬영한 경우 영상 왜곡을 나타내는 회전 이격도 7.85 ± 1.45 mm, 기울임 이격도 4.84 ± 0.5 mm였다. 시각적 보조 장치를 이용해서 실습한 경우 회전 이격도 4.4 ± 0.76 mm, 기울임 이격도 3.01 ± 0.87mm였다. 공간적 보정 기구를 이용해서 실습한 경우 회전과 기울임 이격도는 5.2 ± 0.69 mm, 3.33 ± 0.61 mm로 나타났다. 배치 후 작업 환경과 동일한 조건에서 경험적 촬영 실습에 의한 영상 왜곡은 5.4% 감소하였으나, 기울임에 의한 영상 왜곡은 1.2% 증가하였다. 시각적 보조 장치를 이용해서 실습한 경우, 경험적 촬영 실습보다 회전이격도 40.1% 기울임 이격도 30.7% 감소하였다. 공간적 보정 기구를 사용할 경우, 기존의 경험적 실습 방식보다 회전 이격도 41.7%, 기울임 이격도 23.7% 감소하였다. 따라서 비숙련 방사선사의 배치 전 시각적, 공간적 촬영 보조 기구를 이용한 실습은 배치 후 검사 정확도 향상과 재촬영률 감소에 도움을 줄 수 있을 것으로 사료된다.

제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
