Sudden sensorineural hearing loss (SSNHL) is a common disorder; however, sequential, bilateral presentation of the disease is rarer than unilateral presentation. Clinical otologists usually focus on treating the side with impaired hearing when patients first present with unilateral SSNHL, and therefore, may not warn patients of the possibility of subsequent hearing impairment in the contralateral ear. Furthermore, it is professionally discouraging when a patient presents with profound, sequential SSNHL after initial treatment. This may adversely impact the doctor-patient relationship, even if the patient is offered the best possible care from their first visit. Herein, we report the case of a patient with profound, idiopathic, bilateral SSNHL with a time interval of 37 days between involvement of both ears. Even though high-dose steroids were administered intraorally and intratympanically, the patient's hearing was not restored, and the patient eventually required bilateral cochlear implant surgery. Our report demonstrates that sequential, profound, bilateral SSNHL may manifest without any specific signs.

Primary ciliary dyskinesia (PCD) is characterized by the congenital impairment of mucociliary clearance. When accompanied by situs inversus, chronic sinusitis and bronchiectasis, PCD is known as Kartagener syndrome. The main consequence of impaired ciliary function is a reduced mucus clearance from the lungs, and susceptibility to chronic respiratory infections due to opportunistic pathogens, including nontuberculous mycobacteria (NTM). There has been no report of NTM lung disease combined with Kartagener syndrome in Korea. Here, we report an adult patient with Kartagener syndrome complicated with Mycobacterium abscessus lung disease. A 37-year-old female presented to our hospital with chronic cough and sputum. She was ultimately diagnosed with M. abscessus lung disease and Kartagener syndrome. M. abscessus was repeatedly isolated from sputum specimens collected from the patient, despite prolonged antibiotic treatment. The patient's condition improved and negative sputum culture conversion was achieved after sequential bilateral pulmonary resection.

Objective : The purpose of this case report is to offer an unprecedented target for Gyejigabuja-tang by analyzing a case in which the symptoms were improved by the herbal decoction therapy, and to accumulate a fundamental knowledge in herbal medicine therapy for urinary incontinence. Method : We studied a patient's chart with the patient's consent and displayed patient's timeline and adherence in a figure according to the CARE guideline. The progress of urination symptoms were assessed with the frequency of urination and how long the patient can stand urine. The accompanied lower limb pain was estimated with Numerical Rating Scale(NRS) Results : The patient's adherence was good during the medication period and symptoms related with urination and accompanied lower limb pain were improved with no specific side effects. NRS of the lower limb pain decreased from 7 to 2, the frequency of urination also improved from over 10 times to 6~8 times per day, and the patient can stand urine from less than 1 second to 2~3 minutes. Conclusions : Gyejigabuja-tang was effective for an urinary incontinence patient which corresponds to the provision No.20 in Shanghanlun.

A 2-day-old Thoroughbred foal weighing 58 kg was admitted to the Equine Hospital of Korea Racing Authority (KRA) Jeju Stud Farm with clinical signs including loss of suckling behavior and barking. Neonatal encephalopathy (NE) was diagnosed based on history and typical clinical signs of NE. The foal seemed to recover in 5 days of intensive care and treatment but then was complicated with the septic arthritis of left hock joint on the $6^{th}$ day of admission. A course of aggressive systemic antimicrobial therapy with joint lavage for 8 days was conducted and the foal was fully recovered and discharged. The follow up on the patient after 2 years revealed that the patient achieved a great success as a racehorse without any unexpected sequel. This report describes a course of NE complicated with septic arthritis in a foal and the clinical outcome of the intensive care and treatment in detail. To our knowledge, this is the first report which describes NE complicated with septic arthritis in a foal in Republic of Korea.

Traumatic tricuspid regurgitation is a rare complication of blunt chest trauma caused by chordal rupture, anterior papillary muscle rupture and anterior leaflet tear. Since clinical symptoms are vague, early diagnosis is difficult and some patient exhibit symptoms of right heart failure. Right heart failure has been the traditional indication for surgical treatment, such as tricuspid valve replacement. Recently, early detection using transthoracic echocardiography and surgical treatment, like valve repair, prior to overt right heart failure have been shown to better prognosis. We report a case of traumatic tricuspid regurgitation with chordal rupture in patient due to traffic accident.

Background: Hypoglycemia is uncommon in people without diabetes. There have been only a few reports of cardiac arrest in conjunction with hypoglycemia in non-diabetic patients. Case Report: A 66-year-old man visited the emergency room with dizziness. He was a chronic alcoholic. Laboratory test showed no evidence of diabetes mellitus. Brain magnetic resonance imaging revealed a left cerebellar infarction. Abdomen computed tomography demonstrated liver cirrhosis with minimal ascites. During his hospital stay, he consumed only a small amount of food because of nausea and headache. On hospital day 4, he had a cardiac arrest after two seizure episodes. His blood glucose was 10 mg/dL. The combination of liver cirrhosis, renal failure and poor oral intake was presumed to be the causes of the severe hypoglycemia. Conclusion: We report a rare case of cardiac arrest occurring in conjunction with severe hypoglycemia in a non-diabetic patient with cerebral infarction.

Purpose: The purpose of this study was to identify factors influencing fall-prevention behaviors of nurses working in long-term care hospitals. Methods: Participants included 147 nurses working in 10 long-term care hospitals in B city. Data were collected from September 20-October 12, 2016. SPSS/WIN 21.0 was used for analysis with t-test, ANOVA, Scheffé test, Pearson correlation coefficients, and multiple regression. Results: It was found that attitude toward fall (r=.29, p<.001) and patient safety culture (r=.25, p=.002) had a significant positive correlation with fall-prevention behaviors of nurses working in long-term care hospitals. The factors influencing fall-prevention behaviors in participants were clinical career and patient safety culture (β=.21, p=.012), contributing to 19% of the total variance in fall- prevention behaviors. Conclusion: The findings showed that systematic delivery of differentiated fall prevention education is preferred to nurse's clinical career as a private factor to improve fall-prevention behaviors of nurses in long term care hospital. Particularly, it is imperative to conduct periodical and practical fall-prevention education for nurses to prevent career discontinuity. An independent report system and open communication system as well as a scheme that can disseminate patient safety culture in individual departments to implement patient direct nursing are required to encourage patient safety culture in organizations.

Chromosome 6p duplication is very rare and clinically characterized by short stature, mental retardation, and congenital heart diseases. Patients with mental retardation may present with poor oral health conditions. Dental treatment may need to be performed under general anesthesia in such patients. Our case report deals with induction of general anesthesia to a patient with chromosome 6p duplication, for dental treatment. The selection of a nasotracheal tube of an appropriate size, because of the patient's short stature, was especially important for airway management. In the present case, the patient with chromosome 6p duplication was intubated with a nasotracheal tube, which was not age-matched but adapted to the height and physique of the patient.

Objective : This case report aims to show the effect of Bekhogainsam-tang on Overactive Bladder and Rheumatoid arthritis. The patient was diagnosed with Yangmyung-byung based on Shanghanlun provisions. Methods : According to Diagnostic system based on Shanghanlun provisions, the patient was diagnosed with Yangmyung-byng and has been unusual dietary habits. So she treated with Bekhogainsam-tang decoction for 75 days. We evaluated the results of treatment by Overactive Bladder Symptom Score(OABSS), Modified Overactive Bladder Validated 8-question Screener(OAB-V8), and Rheumatoid Arthritis Pain Score(RAPS). Results : After taking Bekhogainsam-tang decoction for 75 days, the patient's symptoms and OABSS was decreased from 20 to 11, OAB-V8 was from 42 to 20, and RAPS was from 130 to 63. Conclusions : This case report showed an effectiveness of using Bekhogainsam-tang according to Diagnositic system based on Shanghanlun provisions on Overactive Bladder and Rheumatoid Arthritis.
