The Influence of Learning Agility, Grit, and Positive Psychological Capital on the Performance of Clinical Nurses (임상간호사의 학습민첩성, 그릿, 긍정심리자본이 간호업무성과에 미치는 영향)

  • Purpose: The goals of this study were to identify the relationships among learning agility, grit, positive psychological capital, and nursing performance and to determine the impact of these factors on clinical nursing performance. Methods: Between November and December 2021, a descriptive survey design was conducted with a convenience sample of 260 clinical nurse participants from two regional hospitals. Data were collected using the Learning Agility Scale, Grit Scale, Psychological Capital Questionnaire, and the Nursing Performance Scale. Data from 248 nurses were analyzed using descriptive statistics, independent t-test, one-way analysis of variance, Pearson's correlation, and multiple linear regression analysis using SPSS 27.0. Results: Results revealed significant positive correlations between nursing performance and learning agility, grit, and positive psychological capital. Stepwise multiple regression analysis identified positive psychological capital as the strongest predictor of nursing performance, followed by work experience, learning agility, grit, and educational level. These variables accounted for 51% of the variance in nursing performance. Conclusion: This study underscored the importance of positive psychological capital, learning agility, and grit in improving nursing performance. Healthcare organizations should prioritize the development of these attributes among nurses. Implementing strategies to enhance these factors in nursing practice and education could improve nursing performance, potentially resulting in better patient outcomes and increased job satisfaction. Future research should explore specific interventions to cultivate these qualities and assess their long-term impact on nursing practice.

Evaluation of the Usefulness of Virtual Reality Equipment for Relieving Patients' Anxiety during Whole-Body Bone Scan (전신 뼈 검사 환자의 불안감 해소를 위한 가상현실 장비의 유용성 평가)

  • Purpose When performing a whole-body bone scan, many patients are experiencing psychological difficulties due to the close distance to the detector. Recently, in the medical field, there is a report that using virtual reality (VR) equipment can give pain relief to pediatric patients with weak concentration or patients receiving severe treatment through a distraction method. Therefore, in this paper, VR equipment was used to provide psychological stability to patients during nuclear medicine tests, and it is intended to evaluate whether it can be used in clinical practice. Materials and Methods As VR equipment, ALLIP Z6 VR (ALLIP, Korea) was used and the experiment was conducted after connecting to a mobile phone. The subjects were 30 patients who underwent whole-body bone examination from September 1, 2021 to September 30, 2021. After intravenous injection of 99mTc-HDP, 3 to 6 hours later, VR equipment was put on and whole body images were obtained. After the test, a survey was conducted, and a Likert scale of 5 points was used for psychological anxiety and satisfaction with VR equipment. Hypothesis verification and reliability of the survey were analyzed using SPSS Statistics 25 (IBM, Corp., Armonk, NY, USA). Results Anxiety about the existing whole-body bone test was 3.03±1.53, whereas that of anxiety after wearing VR equipment was 2.0±1.21, indicating that anxiety decreased to 34%. When regression analysis of the effect of the patient's concentration on VR equipment on anxiety about the test, the B value was 0.750 (P<0.01) and the t value was 6.181 (P<0.01). decreased and showed an influence of 75%. In addition, overall satisfaction with VR equipment was 3.76±1.28, and the intention to reuse was 66%. The Cronbach α value of the reliability coefficient of the questionnaire was 0.901. Conclusion When using VR equipment, patients' attention was dispersed, anxiety was reduced, and psychological stability was found. In the future, as VR equipment technology develops, it is thought that if the equipment can be miniaturized and the resolution of VR content images is increased, it can be used in various clinical settings if it provides more realistic stability to the patient.

Infection control realities and relevant factors in dental hygienists (치과위생사의 감염관리실태와 관련요인)

  • Objectives : The purpose of this study was to improve the practice level on dental hygienists'infection prevention by examining the actual condition of dental hygienists'infection control and analyzing factors of having influence upon this. Methods : A questionnaire survey was carried out from July 18, 2011 to August 26 targeting dental hygienists who work at medical institutions where are located in Daegu, Busan and Ulsan Metropolitan Cities and Gyeongsangnam buk-do. After then, the following conclusions were obtained. Results : 1. As a result of surveying practice by item in the practice level of infection prevention, the items with high practice level in the management of infectious diseases were surveyed to be 12 months(89.6%) for health-checkup cycle and to be having experience of vaccination(78.0%) for hepatitis type B. The items with high practice level in the management and practice of washing hands were indicated to be in order of regularly paper towel(87.7%) and hands cleaning after regular medical examination(80.5%). In the item of the practice on wearing and managing individual protection equipment, regularly wearing(93.1%) rubber globes given washing implements was indicated to be high. 2. As a result of analyzing working career, working institution, working region, and practice level of infectious-disease management, the appearance of fulfilling infection control guidelines at medical institution and the experience of education for infection control were indicated to have difference depending on working institution. Regularly health checkup was indicated to have difference depending on respondents' working career and working institution. 3. The whole average in the practice level of infection prevention according to working career, working institution, and working region was indicated to be 2.55 out of 3-point perfection. 4. Wearing latex gloves was indicated to have statistically significant difference depending on working institution(p<0.001) and working region(p<0.001). The exchange of latex gloves every patient and the use of paper apron had statistically significant difference depending on working region(p<0.001). 5. As a result of comparing the frequency of using protection equipment for preventing infection according to the management of infectious diseases, the statistically significant difference was shown depending on the appearance of infection control guidelines at medical institution(p<0.001), the appearance of having experience of health checkup(p<0.01), and the appearance of having experience of vaccination for hepatitis type B(p<0.05). Conclusions : The above-mentioned findings showed that the denture satisfaction of the denture-wearing senior citizens was linked to their subjective oral health awareness. Therefore it will be possible to improve denture-wearing elderly people's quality of life when oral health plans geared toward boosting their denture satisfaction are carried out.

Limitation of Psychiatric Intervention for Suicidal Drug Intoxication Patients in Emergency Room (응급실에 내원한 약물음독의 자살 시도자에서 정신건강의학과 진료 참여의 제한점)

  • Purpose: This study was designed to determine the factors hindering psychiatric intervention for suicide attempters in the emergency room (ER). Methods: Participants were 299 patients aged 18 years or older admitted to the ER for suicidal drug overdose between July 2012 and June 2014. Patients were divided into two groups according to whether they had received psychiatric treatment in the ER. Medical histories and follow-up treatments were determined by examining patients' medical records and through phone surveys, and were then compared using ${\chi}^2$-test and Fisher's exact test. In addition, the rate of satisfaction of the treatment group and the reasons for their dissatisfaction were also determined. Results: The treatment and non-treatment groups comprised 135 (45%) and 164 patients (55%), respectively. Factors influencing participation in psychiatric intervention were previous history of suicide attempts (p=0.004), history of psychiatric disorder (p<0.001), time of day (p=0.039), and day of the week (p=0.040) of arrival in the ER. Whether or not the patient received follow-up psychiatric treatment was not significantly relevant (p=0.300). Of the 82 patients who participated in the treatment satisfaction survey, 50.2% reported being unsatisfied, mainly because of unfriendly medical personnel (36.6%), discomfort regarding other people's perceptions (24.4%), and cursory care (14.6%). Conclusion: To raise the participation rate of psychiatric consult, cooperation with psychiatry at night and on weekends is required, and incorporation of patients without previous history of suicidal attempt or other psychiatric disorder is important. Resolution of complaints toward psychiatric consult in suicide attempt survivors is also required.

A Prospective Observational Study on Symptoms of the Postpartum Women with Korean Medicine Treatment during Six Weeks after Childbirth (한의치료를 받은 산모의 산후 6주간 증상에 대한 전향적 관찰 연구)

  • Objectives: This study aims to investigate the changes of women's postpartum symptoms, including their weights, edema index and quality of life (QOL) over the first six weeks after childbirth. Methods: The study participants were 31 postpartum women treated in the Dept. of Obstetrics & Gynecology, at Woo-Suk University Korean Medicine Hospital, from May 20th to August 5th, 2016. In the first 2 weeks of the postpartum period, the main data collection method was to interview each patient and write down all of their complaints, and additionally referring to each participant's PHR (Personal Health Records). At the 3rd, 4th, 5th and 6th week, the researcher interviewed participants by phone once a week. The participants' weight, edema index (ECW/TBW) and the quality of life (by EQ-VAS) were measured at 1st and 2nd week after childbirth. And, EQ-VAS was assessed once more at the 6th week after childbirth. As a last step, the participants responded to a survey on satisfaction regarding their postpartum care with Korean Medicine. Results: 1. In the 1st one week after childbirth, edema was the most frequent complaint. It was about joint pain of the upper limbs in the 2nd and 3rd weeks, sweating in the 4th week, and joint pain of the upper limbs in the 5th and the 6th weeks. 2. In the 1st two weeks of the postpartum period, the weight of the participants decreased from $66.33{\pm}9.30kg$ to $62.60{\pm}8.92kg$ (p<0.001) and the edema index ECW/TBW decreased from $0.399{\pm}0.010$ to $0.385{\pm}0.0 4$ (p<0.001). The EQ-VAS significantly improved from $61.77{\pm}17.72$ to $73.51{\pm}14.67$ (p<0.001). In the last 4-weeks of the postpartum period, the EQ-VAS decreased from $74.30{\pm}14.25$ to $73.63{\pm}13.35$, but this difference was not statistically significant (p=0.749). 3. Regarding the satisfaction with postpartum care with Korean Medicine, 60% of the participants responded that it was 'Excellent' and 40% said it was 'Good'. Conclusion: Over the entire postpartum period, the most frequent complaints were about musculoskeletal symptoms. Postpartum care with Korean Medicine treatments in the early postpartum period improves various postpartum symptoms and the quality of life for postpartum women.

The Levels of Psychosocial Stress, Job Stress and Related Factors of Medical Doctors Practicing at Local Clinics (일부 개원의사들의 사회심리적 스트레스 및 직무 스트레스 관련 요인)

  • Objectives : This study was conducted to investigate the levels of psychosocial stress, job stress and their related factors among medical doctors practicing at local clinics. Methods : A survey using a self administered questionnaire was administered to 1,456 doctors practicing at private clinics via post for 2 months (2006. 1 - 2006. 3). Psychosocial stress, job stress, demographic factors, job related factors and health related behaviors were investigated. Among the eligible study population, the respondents were 428 doctors (29.4%). Results : The average scores of psychosocial stress and job stress were 2.19 and 3.13, respectively. The levels of psychosocial stress and job stress were statistically lower in older respondents, those who worked shorter or who were more satisfied with their job, and those with higher socioeconomic status. The level of psychosocial stress was related with smoking status, drinking status and exercise. The level of job stress was related with smoking status and exercise. In multiple linear regression analysis using psychosocial stress as a dependent variable, age, working hours per day, job satisfaction and perception on socioeconomic status were significant independent variables. In analysis using job stress as a dependent variable, age, working hours per day and job satisfaction were significant independent variables. Conclusions : Stress affects the doctor-patient relationship, productivity and overall health level of people. Therefore, it is important to manage and relieve the stress of doctors. It is suggested that more advanced studies on stress level and related factors and ways to improve the stress and health related behaviors of medical doctors should be conducted.

Satisfaction Evaluation of Diabetic Foot Disease Measurement using AI-based Application (AI기반 에플리케이션을 활용한 당뇨병성 족부질환 측정의 만족도 평가)

  • The purpose of this study is to develop a customized foot disease analysis and management system for diabetic patients to prevent foot ulcers in diabetic foot disease patients. This system utilizes image analysis technology to measure not only foot pressure, but also ankle deformation, body balance, and foot wounds. Through various data, it is possible to accurately analyze the state of foot deformation, and based on this, the exact state of deformation of the foot of a patient with diabetic foot disease was identified and a customized insole was produced. This study was conducted to examine the satisfaction level of using an application that checks the status of diabetic foot disease wounds and to identify the degenerative status of diabetic foot disease patients and foot disease patients by wearing customized insoles and to survey the satisfaction of wearing insoles. As a result of the study, the knee angle measured for plantar pressure was -0.8 ± 1.3 degrees and ranged from a minimum of -2.4 degrees to a maximum of 1.1 degrees, and there was no significant difference in valgus knee between both lower extremities (p = 0.534). There was a significant difference in tibial angle between both lower extremities (p < 0.001). Ankle angle on the left side was 2.6 ± 2.0 degrees, ranging from a minimum of 0 degrees to a maximum of 6.3 degrees, and on the right, it was 4.5 ± 2.1 degrees, with a distribution of minimum 1.5 degrees to a maximum of 9.1 degrees. There was a significant difference in ankle angle between both lower extremities (p = 0.011). They responded that they felt an average of 4.3 points of satisfaction with the plantar pressure measurement application. Respondents responded that they felt an average of 3.9 points of satisfaction with the use of customized insoles.

A study on the change of the views on E.M.T. profession of the E.M.T. college students (일부 전문대 응급구조학생의 응급구조전문직관의 변화에 대한 연구)

  • The purpose of this study was to identify the change of perceptions related to E.M.T. profession of E.M.T. student and to provide the basic data for development of curriculum. The subjects of the survey were 216 students from 3 junior college by questionaire from November 25 to November 29, 1996 in Kwang-Ju, Cheong-Ju, and Kong-Ju. The results were analyzed using the SAS and can be summerized as follows : 1. The view on profession was a statistically significant difference inspecial knowledge & skill training (t=-1.686, P<.001), high income (t=-0.753, P<.05) 2. The view on competence as a E.M.T. was a statistically significant differncein sufficient professional knowledge on the E.M.T. (t=-1.144, P<.001), own's health (t=-0.808, P<.001), sincerity and responsibility (t=-1.429, P<.01), satisfaction and effort (t=-1.335, P<.01), boundary of affairs (t=-1.356, P<.01) 3. The view on E.M.T. profession was a statistically significant difference inprotect of pt. life & to relieve the pain (t=-2.388, P<.001) 4. The view on necessary knowledge of E.M.T. was a statistically significant difference in patient assessment (t=-2.168, P<.001), ACLS (t=1.678, P<.001), hemorrhage & shock (t=-3.252, P<.001), trauma (t=-4.284, P<.001), cardiopulmonary disturbance (t=-2.897, P<.001), acute abdomen (t=-4.284, P<.001), neonatal disease (t=-4.032, P<.001), OBGY disease(t=-4.151, P<.001), emergency delivery (t=-2.825, P<.001), infectious disease (t=-2.930, P<.001), environmental emergency (t=-3.123, P<.05), recording(t=2.22, P<.05) 5. The view on ideology of E.M.T. was a statistically significant difference in belief & faith (t=-2.344, P<.001), knowing of own (t=-2.142, P<.01), humanity (t=-2.581, P<.05), knowing of patient (t=-2.079, P<.05).

Nursing Work Related to Low Back Pain in Intensive Care Unit (중환자실 요통관련 간호업무특성)

  • This study is a descriptive comparative study to identify personal characteristics and nursing work characteristics according to the presence or absence of back pain of nurses in intensive care units in general hospitals and to explore major factors that affect back pain induction. A survey was conducted on nurses in the intensive care unit of a general hospital located in D City, and a total of 100 copies were used for analysis. For data analysis, descriptive statistics, chi-square test, Fisher accurate test, and logistic regression analysis were performed using IBM SPSS 24.0. The study found that 69% of intensive care unit nurses had back pain, 65.2% of which experienced back pain for more than three months in the past year, and 84.1% had back pain after working in the intensive care unit. In the back pain intensity score measured by NRS, 49% answered that they experienced 4 or more points. As factors related to back pain, gender (p=.03) were significant variables in personal characteristics, in the characteristics of nursing work, the number of patient assistance (p=.03) and the number of patient exchanges (p=.01), heavy lifting (p=.04), satisfaction with the condition of the shoes (p=<.001) was found to be a significant variable affecting back pain induction. It is considered that hospital organization and national policies are needed to prevent and manage low back pain in intensive care unit nurses.

Structured Assessment to Evaluate a Family Medicine Clerkship Program (가정의학 임상실습과정에 대한 구조화된 평가)

  • This study was conducted to analyze the strengths and weaknesses of a 3-week family medicine clerkship program based on the results of an online survey taken by the students (N=127) and a structured interview with a focus group (n=10), aimed to improve the quality of the clerkship program. The online survey contained questions pertaining to goals, schedule, contents, arrangement, atmosphere, environment, evaluation, and satisfaction regarding the clerkship. The focus group interview addressed the schedule and achievements of the program. Scores were reported on a 5-point Likert scale. Most students were highly satisfied with the overall quality of the clerkship. The structured interview results showed that 97.6% of the clerkship program was executed according to the schedule. The focus group reported a perfect score of 5 points on several measures including: accomplishment of the educational goals of the family medicine clerkship, providing many chances to obtain medical histories and perform physical examinations on real patients, experience with various symptoms and diseases, positive attitudes of faculty members when teaching, notification of the guidelines for evaluation beforehand, well-constructed and effective clerkship schedule, and reflection of student feedback. However, the focus group gave low scores on: support for health accidents of students, access to patient information, enough opportunities to practice clinical skills, appropriate rest facilities for students, and fairness of clerkship evaluation process. In conclusion, the structured evaluation performed after the 3-week clerkship program motivated students and helped them ensure an efficient clerkship. This structured evaluation also suggested basic data to make the professor who is subject of the assessment. This study shows that structured assessment is an effective method which can be used to improve the quality of clerkships.
