공공의료기관 간호사의 조직몰입, 환자안전문화인식이 환자안전간호활동에 미치는 영향 (The Effect of Organizational Commitment and Perceived Patient Safety Culture on Patient Safety Nursing Activities in Public Hospital Nurses)

  • Purpose: This study was conducted to identify and analyze organizational commitment, patient safety culture, and patient safety nursing activities on patient safety in public hospitals. Methods: The data were collected from 190 nurses in 6 public hospitals within the Gyeonggi region and were analyzed using SPSS 23.0 for descriptive statistics and multiple regression. Results: Patient safety nursing activity was positively correlated with the number of participants in patient safety training programs, organizational commitment, and patient safety culture awareness. Patient safety culture awareness was positively correlated with the number of participants in patient safety training programs and organizational commitment. Organizational commitment was positively correlated with age and total work experience. Factors affecting patient safety activities, frequency of patient safety training, and patient safety culture awareness were significant influencing factors with an explanatory power of 30.1%. Conclusion: In order to increase patient safety nursing activities in public hospitals, systematic patient safety training policies are required. In addition, diverse interventions are required to increase organizational commitment.

병원 간호사의 환자안전문화 인식, 사건보고 및 안전간호활동: 일 대학병원을 중심으로 (A Study on Patient Safety Culture, Incident Reporting and Safety Care Activities of Clinical Nurses in a University-Affiliated Hospital)

  • Purpose: This study aimed to investigate perception of patient safety culture, incident reporting, and safety care activities among clinical nurses and to identify factors associated with the safety care activities. Methods: Structured questionnaires were used to collect data from 155 nurses who were involved in direct patient-care. Results: Descriptive statistical anaylses revealed that the mean score of patient safety culture was $3.26{\pm}0.32$ and $4.19{\pm}0.41$ was for the safety care activities. In incident reporting, reporting intention ($3.56{\pm}0.68$), belief in improvement ($3.42{\pm}0.60$), worry about appraisal ($3.37{\pm}0.65$) and reporting knowledge ($3.36{\pm}0.72$) respectively. Correlational analyses showed that perceived patient safety culture (r=.36), reporting intention (r=.34), belief in improvement (r=.32), and the knowledge (r=.38) in incident reporting were positively correlated with safety care activities, while the worry about appraisal in incident reporting attitude was negatively correlated. The factors associated with safety care activities were incident reporting knowledge (${\beta}=.31$, p<.001), supervisor/managers' attitudes toward patient safety culture (${\beta}=.29$, p<.001), belief in improvement of incident reporting attitude (${\beta}=.16$, p=.041). Conclusion: These results suggest that to improve safety care activities among hospital nurses, it is necessary to educate nurses on incident reporting. Also, a system-level approach is needed to support leadership in patient safety and to provide positive feedback on incident reporting.

  • 본 연구는 의료기관인증제도 도입에 따른 환자안전과 의료서비스의 질이 병원경영활동에 미치는 복합영향을 확인하고 조직문화의 매개효과를 파악하고자 하였다. 이를 위해, 2014년 6월말까지 의료기관인증을 획득한 대구 경북지역에 위치하는 300병상 미만과 300병상 이상, 6개 의료기관 종사자를 대상으로 2014년 9월 22일부터 4주간 설문조사 하였다. 해당 병원에서 1년 이상 근무한 377명을 연구대상으로 하였다. 연구결과, 300병상 미만 중소병원과 300병상 이상 병원 및 종합병원 모두, 환자안전과 의료의 질, 조직문화와 병원경영활동은 변수 간 정(+)의 상관관계가 있는 것으로 나타났다. 환자안전과 의료의 질, 병원경영활동 사이에 조직문화가 부분 매개효과가 있는 것으로 나타났으며 인과효과가 높았다. 따라서 의료기관인증제도 도입에 따른 환자안전과 의료의 질 향상 활동은 병원경영활동을 증진시키는데 효과가 있다 여겨진다.

병원 간호사의 안전 간호활동에 영향을 미치는 요인 (Factors Influencing Safety Care Activities of Hospital Nurses)

  • Purpose: The purpose of this study was to investigate the relationships among fatigue, patient safety culture and safety care activities of hospital nurses, and to identify and explain factors influencing safety care activities. Methods: The research participants were 187 nurses from a urban general hospital located in Korea. Self-evaluation questionnaires were used to collect the data. Data collection was done from January 10 to 31, 2019. Data were analyzed using descriptive statistics, independent t-test, One-way ANOVA, Pearson correlation coefficients and multiple regression with the SPSS 24.0 program. Results: There were significant negative relationships between fatigue and safety care activities (r=-.22, p=.003), and significant positive relationships between patient safety culture and safety care activities (r=.22, p=.003). Factors influencing safety care activities in hospital nurses were identified as type of unit (ICU) (${\beta}=.28$), patient safety culture (${\beta}=.24$) and fatigue (${\beta}=-.19$). The explanation power of this regression model was 16% and it was statistically significant (F=8.29, p<.001). Conclusion: These results suggest the need to develop further management strategies for enhancement of safety care activities in hospital. To improve the levels of patient safety, education programs on patient safety should be developed and provided to nurses in hospitals.

노인요양시설 요양보호사의 환자안전문화, 환자안전관리 중요성 인식 및 환자안전관리활동: 혼합연구방법 적용 (Patient Safety Culture, Perception of Importance on Patient Safety Management, and Patient Safety Management Activities of Care Workers in Nursing Homes: Mixed-method Approach)

  • Purpose: This study investigated the patient safety culture (PSC), the perception of importance on patient safety management (PIPSM) and the patient safety management activities (PSMA) of care workers in nursing homes. This was a descriptive study that attempted to provide basic data for the patient safety education program of care workers. Methods: Data were collected using questionnaires and interviews from July 1 to 31 in 2020. One hundred and seventy-four care workers participated in quantitative research. The collected data were analyzed by the SPSS/WIN 25.0 program using descriptive statistics, t-test, ANOVA, Bonferroni, and Pearson's correlation. The qualitative data were collected through semi-structured, audio-recorded interviews with six representatives and six care workers from six nursing homes. Content analysis was performed to analyze the data. Results: Positive correlations were observed between PSC and PIPSM, and between PIPSM and PIPSM. Care workers' experience in patient safety management was in the following six categories: "Safety accident risk factors", "Type of safety accidents", "How to prevent safety accidents", "Effective safety management education", "Emphasis on occupational ethics of care workers", and "Needs for standard protocol" Conclusion: These findings indicate that considering the care workers' age and facility size, nurses should enhance patient safety education for care workers and establish a management activity system.

  • Purpose: The objective of this research was to explore levels of patient safety and safe nursing activities depending on the level of nurse staffing, in order to provide effective management of nurse personnel. Methods: The research was conducted with 455 nurses from eight hospitals in B city. Data were collected according to the level of nurse personnel from second (nurse vs. patient ratio of 2.0-2.5) to fifth (ratio of 3.5-4.0) rank. The survey tools were, 'Questionnaire on Patient Safety, a Hospital Survey on Patient Safety Culture developed by AHRQ (2007), and 'Questionnaire on Patient Safety Nursing Act, in which the questions were selected from nursing-related items (Medication 6 & Safety Nursing Assurance Act 4) in the Safety Evaluation developed by Evaluation Institute of Medical Institution. Data were analyzed with SPSS PC 12.0 program using descriptive statistics, $x^2$ test, ANCOVA and $Scheff{\grave{e}}$. Results: The nurses' overall cognition level on patient safety and safe nursing activities showed that nurses who are in the second and third rank had higher scores than those in lower ranks. Conclusion: The results of this study indicate that hospitals need a higher ratio level for nurse personnel in order to assure patient safety and safe nursing activities.

  • 본 연구는 의료기관 인증제의 의무 대상인 정신병원과 요양병원을 대상으로 환자안전 및 질 변화와 경영활동의 변화 간의 관계에 대해 의료기관 인증을 받은 5개 정신병원과 5개 요양병원에 근무하는 종사자들을 대상으로 조사 연구한 결과, 정신병원과 요양병원 종사자들의 인증업무 참여유무는 환자안전 및 질 관리 변화 측정내용 중 '안전보장활동 수행정도' '환자를 위한 양질의 의료서비스 제공정도' '환자의 권리와 책임에 대한 존중정도' '감염발생 위험예방 위한 감염관리활동 수행정도'의 향상 변화에 유의하게 긍정적인 상관관계를 보였다. 그리고 환자안전 및 질 관리 내용 중 안전보장활동 지속적인 질 향상 활동 환자의 권리와 책임에 대한 존중의 지속적인 발전적 변화를 추구하기 위해서는 교육훈련을 통해서 조직구성원들의 역량을 강화시키는 경영활동이 필요하며, 진료전달 체계와 평가의 관리 환자를 위한 양질의 의료서비스 제공의 지속적인 발전적 변화를 위해서는 고객지향성 업무프로세스를 사회 환경의 변화에 맞추어 추구하는 경영활동이 필요함을 알 수 있었다.

간호대학생의 환자안전관리활동에 영향을 미치는 요인 (Factors Influencing the Patient Safety Management Activity of Nursing Students)

  • Purpose: This purpose of this study was to identify the level of knowledge and attitude of patient safety and patient safety management activity (PSMA) and identify influencing factors of PSMA in nursing students. Methods: The participants were 210 fourth-year nursing students in C and G city. Data were collected with structured questionnaires from October 10 to November 10, 2017. Descriptive statistics, t-test, one way ANOVA, Pearson's correlation and multiple regression with SPSS 21.0 were used. Results: As a result, the level of knowledge of patient safety was 9.05, attitude of patient safety was 4.07, and PSMA was 4.22. The factors influencing PSMA were knowledge and attitude of patient safety. The regression model explained 77% of PSMA. Conclusion: Based on the results of this study, it is suggested that a systematic education program considering factors influencing the patient safety management activities of nursing students be developed.

치과위생사의 감염관리활동과 환자안전문화에 대한 인식 (Perception of infection control activities and patient safety culture among dental hygienists)

  • Objectives: The study was to promote patient safety by analyzing the effect of dental hygienist's perception of patient safety culture on infection control activities. Methods: The study is based on a survey of 377 dental hygienists in total working in dental settings. The questionnaire consisted of 119 questions, including 34 questions on perception of patient safety culture, and 85 questions on infection control activities. Hierarchical regression analysis was used to examine the relationship between the perception of patient safety culture and infection control activities. The data was analyzed using the SPSS version 20.0, and p<0.05 was adopted to decide on significance. Results: The longer dental hygienists have worked n the dental settings, the more active they become in infection control activities. Among the different types of dental care settings, general (university) hospitals had the largest number of infection control activities, followed by dental clinics, and network dental clinics, in descending order. The dental settings possessing a higher number of dental hygienists were found to conduct more infection control activities than other dental settings. In addition, it was found that when a dental setting adopts a patient safety policy across all the units in the hospital, more systems and procedures for patient safety tend to be established, and that stricter management response to error leads to improvement of infection control activities. Conclusions:In order to enhance infection control activities, infection control activity programs should develop and implement periodic reinforcement of infection control education. regular monitoring of infection control activities.

공공병원 간호사의 안전간호활동에 미치는 영향 요인 (Factors Influencing Safe Nursing Activities of Public Hospital Nurses)

  • Purpose: In this descriptive study, we aimed to identify factors influencing safe nursing activities of nurses working at public hospitals. Methods: We collected data in March 2021 through an online survey. The subjects were 199 nurses who had worked for at least six months at public hospitals. The self-reported questionnaire consisted of items pertaining to safe nursing activities, perception of patient safety culture, and the general characteristics of the subjects. To examine the differences in the safe nursing activities and perception of patient safety culture by general characteristics, we employed a t-test and ANOVA. We conducted a multiple linear regression analysis to explore the factors affecting safe nursing activities. Results: The mean score for safe nursing activity was 4.59±0.39. Perception of patient safety culture and age group were found to be the main factors affecting the safe nursing activities of nurses working at public hospitals (F=17.24, p<.001, Adj. R2=.40). When the score for perception of patient safety culture increased by 1 point, the score for safe nursing activities increased by 0.58 points. In addition, the score for safe nursing activity in the 40s age group was 0.16 points higher than for those in the 20s age group. Conclusion: Effective management and leadership in public hospitals is needed to improve the culture of patient safety at various levels so as to ensure safe nursing activities.
