치과병원 종사자의 환자안전문화인식과 환자안전관리활동의 관련성 (The association between patient safety culture perceptions and patient safety management activities in dental hospital workers)

  • Objectives: This research was conducted to recognize dental health care employees' awareness of patient safety culture and to examine the association between awareness of patient safety culture and patient safety activities. Methods: The subject of this research includes the dental health care employees from 11 different dental hospitals in the nation, dental offices in the 6 different general hospitals in Seoul, and 4 different private dental offices in 2014. A self-reported questionnaire was completed by 485 dental health care employees from March 17 to April 3, 2014. Multiple regression models were used in the analysis. Results: First, the dental health employees' awareness of patient safety culture was 3.25 on average. The extent of patient safety activities was 2.75. The behavior of the supervisor/manager and the adequate explanation showed positive correlation with r=0.213 (p<0.001). The process of communication and the preventative inspection of the medical equipment showed negative correlation with r=-0.258 (p<0.001), and especially the awareness of patient safety culture and the infection control exercise showed the most significant correlation (r=0.293, p<0.001). Second, hospital environment factor of patient safety perceptions positively were related to most of patient safety management activities after controlling several covariates. Conclusions: The result of this research showed that most of the patient safety culture perceptions positively were correlated with patient safety activities. We also found that proper hospital environment, simple communication process, and positive perceptions for patient safety level were more likely to perform patient safety management activities after controlling several covariates. Improving the patient safety perceptions of dental hospital workers is an important consideration.

  • Purpose: This study was conducted to identify the effects of organizational health and patient safety culture on nursing activities for patient safety as perceived by hospital nurses. Methods: A self-report survey was administered to staff nurses of one advanced general hospital and two general hospitals in South Korea. Of the questionnaires, 188 were analyzed. Results: Organizational health had a significantly positive correlation with patient safety culture (r=.52, p<.001) and patient safety nursing activities (r=.31, p<.001). Conclusion: The findings in this study indicate that among the factors influencing patient safety nursing activities, organizational factors were more important than individual factors, and organizational health had a big effect on patient safety nursing activities.

  • Purpose: This study was conducted to identify and analyze organizational commitment, patient safety culture, and patient safety nursing activities on patient safety in public hospitals. Methods: The data were collected from 190 nurses in 6 public hospitals within the Gyeonggi region and were analyzed using SPSS 23.0 for descriptive statistics and multiple regression. Results: Patient safety nursing activity was positively correlated with the number of participants in patient safety training programs, organizational commitment, and patient safety culture awareness. Patient safety culture awareness was positively correlated with the number of participants in patient safety training programs and organizational commitment. Organizational commitment was positively correlated with age and total work experience. Factors affecting patient safety activities, frequency of patient safety training, and patient safety culture awareness were significant influencing factors with an explanatory power of 30.1%. Conclusion: In order to increase patient safety nursing activities in public hospitals, systematic patient safety training policies are required. In addition, diverse interventions are required to increase organizational commitment.

간호학과 실습생이 관찰한 간호사의 환자안전 간호활동 수행 정도 (Nurses' patient safety activities observed by nursing students)

  • Purpose: Objective of this study was to investigate how often nurses to perform patient safety activities. Methods: 521 observations were collected in 9 hospitals by 107 nursing students. Nurses' patient safety care activities were measured 0 (not at all) to 10 (all the time) scores. Descriptive statistics, independent t-test, one way ANOVA, correlation, multiple regression used to analyse data. Results: Items like 'Initial nursing assessment', 'drug management bring on', 'preparation for radiology test', 'falls assessment', 'nursing record' got high scores. But, scores of 'patient identification', 'verbal order management', 'hand hygiene' were lower than others. Each scores were different significantly according to institutions and departments. Within a same institution, the variance of scores, especially in 'patient identification', 'hand hygiene' were great. Scores of activities were different according to characteristics of institutions like type, location, number of beds, teaching hospital, number of accreditation, JCI accreditation. Predictors influencing nurses' patient safety activities were type, location and accreditation. These predictors account for 19.4% of variance. Conclusion: Performance of nurses' patient safety activities were different significantly according to characteristics of institutions. The important items like 'patient identification', 'hand hygiene' had achieved lowest performance. Further researches are needed to improve the basic safety activities.

환자안전사고에 의한 손상환자의 병원내 사망 관련 요인 : 2013-2017 퇴원손상심층조사자료 활용 (Factors Related to In-Hospital Death of Injured Patients by Patient Safety Accident : Using 2013-2017 Korean National Hospital Discharge In-depth Injury Survey)

  • This study aimed to analysis factors related to in-hospital death of injured patients by patient safety accident. A total of 1,529 inpatients were selected from Korea Centers for Disease Control and Prevention database(2013-2017). Frequency, Fisher's exact test, t-test, ANOVA, logistic regression analyses by using STATA 12.0 were performed. Analysis results show that the mortality rate was lower for female than male but the mortality rate was higher for the older age, the higher the CCI, head (or neck), multiple, systemic damage sites, internal and others, metropolitan cities based on Seoul and 300-499 based on the bed size of 100-299. Based on these findings, the possibility of using the in-depth investigation of discharge damage from the Korea Centers for Disease Control and Prevention as a data source for the patient safety survey conducted to understand the actual status of patient safety accident types, frequency, and trends should be reviewed. Also, it is necessary to prevent injury and minimize death by identifying factors that affect death after injury by patient safety accident.

상급종합병원 환자와 간호사의 안전 인식 비교와 간호사의 환자안전관리 직무수행 영향요인 (Comparison of Safety Perception between Patients and Nurses and Factors Affecting Nurses Safety Management Activities in Tertiary Hospitals)

  • 본 연구는 상급종합병원 환자와 간호사의 안전 인식을 비교하고, 간호사의 환자안전관리 직무수행에 미치는 영향요인을 확인하기 위해 수행된 서술적 조사연구이다. 경상남도 J시 G대학교병원에 입원한 환자 147명과 간호사 147명을 대상으로 하여 2018년 7월 24일부터 9월 23일까지 구조화된 설문지를 사용하여 자료수집 후 SPSS/WIN 23.0으로 통계 분석하였다. 분석 결과, 환자의 안전 인식이 간호사의 인식 정도보다 유의하게 높았으며, 간호사의 환자안전관리 직무수행에 미치는 유의한 영향요인은 교육수준, 근무형태, 의사소통, 환자안전 인식, 환자안전관리 중요성 인식이었다. 다중회귀분석 결과, 환자안전 지식의 하위영역인 '의사소통'과 환자안전관리 중요성 인식의 하위영역인 '환자안전관리에 대한 관심'이 간호사의 환자안전관리 직무수행을 54.8% 설명하는 것으로 나타났다. 본 연구 결과를 바탕으로 상급종합병원 간호사의 환자안전관리 직무수행 향상을 위해 간호사는 환자와 조직 구성원간 원활한 의사소통과 관심 정도를 높이는 전략을 개발할 필요가 강조된다.

의료기관 인증 후 환자안전 및 질 관리 변화와 경영활동 변화 간의 관계: 정신병원과 요양병원 대상 (Relationship between Changes of Patient Safety & Medical Service Quality and Changes of Management Activity after Medical Institution Accreditation: Mental Hospitals and Geriatric Hospitals)

  • 본 연구는 의료기관 인증제의 의무 대상인 정신병원과 요양병원을 대상으로 환자안전 및 질 변화와 경영활동의 변화 간의 관계에 대해 의료기관 인증을 받은 5개 정신병원과 5개 요양병원에 근무하는 종사자들을 대상으로 조사 연구한 결과, 정신병원과 요양병원 종사자들의 인증업무 참여유무는 환자안전 및 질 관리 변화 측정내용 중 '안전보장활동 수행정도' '환자를 위한 양질의 의료서비스 제공정도' '환자의 권리와 책임에 대한 존중정도' '감염발생 위험예방 위한 감염관리활동 수행정도'의 향상 변화에 유의하게 긍정적인 상관관계를 보였다. 그리고 환자안전 및 질 관리 내용 중 안전보장활동 지속적인 질 향상 활동 환자의 권리와 책임에 대한 존중의 지속적인 발전적 변화를 추구하기 위해서는 교육훈련을 통해서 조직구성원들의 역량을 강화시키는 경영활동이 필요하며, 진료전달 체계와 평가의 관리 환자를 위한 양질의 의료서비스 제공의 지속적인 발전적 변화를 위해서는 고객지향성 업무프로세스를 사회 환경의 변화에 맞추어 추구하는 경영활동이 필요함을 알 수 있었다.

Current Status of Patient Safety Regulations, Guidelines and Support Mechanisms in Korean Hospitals

  • Purpose: This study was conducted to investigate patient safety regulations and guidelines in order to understand their current status, and to examine support measures to improve patient safety in Korean hospitals. Methods: The participants were the safety officers from hospitals with 200 or more beds and 112 hospitals responded to the online survey. The questions covered patient safety regulations, the performance level of patient safety activities, patient safety incident reporting systems, the dedicated professional, training, support mechanisms, and expectations of reporting systems. Results: Among preventative measures, fall prevention and hand hygiene were reported to be most widely practiced (92% and 91%, respectively). Time-out for invasive procedures showed a relatively low practice rate at 70%. Among patient care activities, transfusion, surgery and sedation, medication, and infection management were performed by 84, 74, 93 and 93% of the hospitals, respectively. Patient safety activities included patient safety committee, patient safety cooperation between decision-making bodies, patient safety workshops, seminars, lectures, and training for employees. Conclusion: Patient safety regulations and guidelines have not yet been sufficiently prepared, and a public institution such as a certification authority is of crucial importance to enforce these guidelines.

병원간호사의 긍정심리자본과 역할갈등이 환자안전역량에 미치는 영향 (Effects of Positive Psychological Capital and Role Conflict of Hospital nurses on Patient safety competencies)

  • Purpose: This study was performed to determine whether positive psychological capital and role conflict among hospital nurses influence patient safety competencies. Methods: Data were collected from nurses working at one hospital with more than 800 beds in J Province. Data were analyzed using descriptive statistics, t-test, ANOVA, Scheffé's test, Pearson's correlation analysis and hierarchical regression analysis using SPSS 26.0 program. Findings: In the hierarchical regression analysis, positive psychological capital, role conflict, bachelor's degree or higher as the highest level of education achieved, and three or more experiences reporting accidents impacting patient safety were found to significantly correlate with subjects patient safety competency. Among these, positive psychological capital emerged as the strongest factor, and the explanatory power of Model 4 was determined to be 38.1%. Conclusion: This study confirmed that hospital nurses' positive psychological capital and role conflict impact patient safety competency, underscoring the importance of organizational measures to increase patient safety awareness. Therefore, it is necessary to develop an educational program to strengthen hospital nurses' patient safety capabilities and conduct follow-up research to test its efficacy.

마취·회복실 간호사의 환자안전문화와 환자안전역량이 안전간호활동에 미치는 영향 (Effect of Patient Safety Culture and Patient Safety Competence on Safety Nursing Activity among Nurses working in Anesthetic and Recovery Rooms)

  • Purpose: This study was to investigate the relationships among patient safety culture, safety competence and safety nursing activity among nurses in anesthetic and recovery rooms, and to identify the factors contributing to safety nursing activity. Methods: A descriptive correlational study was conducted. Participants were 156 nurses from 13 hospitals. Data were collected from February 11 to March 15th, 2019, and analyzed using descriptive statistics, t-test, Mann-Whitney U test, one-way ANOVA, Pearson's correlation and multiple regression analysis with SPSS statistics 24.0 Program. Results: Safety nursing activity was significantly different in relation to nurses' level of education, position at work, clinical career, clinical career at anesthetic and recovery rooms, and work experience in patient safety. Safety nursing activity demonstrated a significant positive correlation with patient safety culture and patient safety competence. Factors contributing to safety nursing activity were patient safety knowledge, skill and attitude, clinical career, clinical career at anesthetic and recovery rooms, and the patient safety improvement system which explained 57.0% of total variance of safety nursing activity. Conclusion: To improve safety nursing activities at anesthetic and recovery rooms, it is necessary to develop patient safety programs with enhanced knowledge, skill and attitude to take patient safety as a top priority.
