지문인식 기반의 전자의무기록 시스템 인증 모델 (An Authentication Model based Fingerprint Recognition for Electronic Medical Records System)

  • 의료정보는 환자에게 중요한 개인정보로써 반드시 보호돼야 한다. 특히 전자의무기록에 접근할때, 의료인의 강화된 신원확인에 대한 인증방식이 필요하다. 기존의 공인인증서 기반 인증모델은 개인키 관리, 권한위임 등 문제점으로 전자의무기록의 특성을 반영하지 못했다. 본 논문에서는 전자의무기록 시스템에 의료인이 접근하는 경우 지문인식 기반 인증 모델을 적용하여 강화된 인증방식을 제안한다. 전자의무기록의 지문인증 모델은 의료업무의 특성을 반영하여 개인키 관리, 권한위임 문제를 원천적으로 해결하였다.

민간의료보험의 현황 및 활성화에 관한 연구 (A Study on Private Health Insurance in Korea)

  • This study explores the feasibility of activating private health insurance in Korea. The rationale for expanding private supplementary health insurance can be found in many cases of health care reforms in the European countries. Private health insurance can not only relieve the financial distress of the government health insurance programs but also offer the medical institutions incentives to improve the quality of medical care. In Korea there is no supplementary health insurance that reimburses for various kinds of diseases based on a well designed fee schedule. Recently, the cancer insurance is the best seller in the health related insurance market. As observed in the U. S. case, the cancer insurance which pays the predetermined amount (indemnity coverage) regardless of the medical charges incurred to the patient is limited in its coverage for the insured. To provide better protection against catastrophic diseases, the government should give insurance companies incentives to develop health insurance products that cover multiple diseases rather than a single disease. Consumers can hardly understand and compare complex insurance products. To resolve the information asymmetries, the government should publish a consumer report that compare various health insurance products in a user friendly way. In the long run, insurance companies will plan to sell health insurance products that charge risk related premium only when insurers accumulate the underwriting know-hows, the government shares data on various health statistics including claims and demographics, and risk pool for high risk patients is well established and subsidized by the government.

민영의료보험 가입이 일부 경·요추부 염좌 환자의 입원에 미치는 영향 (The Influence of Private Health Insurance on Admission among Some Patients with Cervical or Lumbar Sprain)

  • 이 연구는 2008년 6월 1일부터 2008년 8월 31일까지 광주 전남지역에 소재하고 있는 20개 의료기관(병원 3, 의원 17)에서 신경학적 증상이 없는 단순 경 요추부 염좌 진단을 받고 입원 및 외래치료를 받은 환자 449명(입원=384명, 외래=85명)을 대상으로 민영의료보험 가입이 입원에 미치는 영향을 규명하기 위해 시행되었다. 구조화된 설문지를 이용하여 조사한 응답 결과에 대해 단일변량분석을 시행하고 입원여부와 유의한 관련성이 있는 변수를 독립변수로 하여 다단계 로지스틱회귀분석을 실시한 결과와 그 함의는 다음과 같았다. 관련된 요인을 통제한 후에도 민영의료보험 가입여부의 입원에 대한 비차비는 3.31(95%CI 1.41-9.58)로서 민영의료보험 가입은 입원여부에 독립적으로 유의한 영향을 미치는 요인임이 확인되었다. 그 밖에 조사대상 환자군의 입원에 영향을 미치는 요인들을 변수군별로 보면 환자의 사회인구학적요인 변수군의 결정계수($R^2$)가 0.226로 가장 컸고 다음으로 의료기관 및 의료인 특성 변수군의 $R^2$=0.122, 질병 특성 변수군의 $R^2$=0.108 등이었으며 민영의료보험의 가입여부의 $R^2$=0.013으로 결정계수 값이 크지는 않았다. 이 연구는 경증질환 진단을 받은 환자를 대상으로 현재 중소규모 의료기관에서 진료중인 환자를 대상으로 민영의료보험 가입여부와 입원여부의 관련성을 구명한 국내 최초의 실증연구로서 민영의료보험의 가입이 초래하는 도덕적 해이의 존재와 정도를 일정부분 규명하였다는 점에서 그 의의를 찾아 볼 수 있다. 전 국민 의료보험제도 하에서 이는 민영의료보험 자체 뿐 아니라 건강보험에 대한 외부효과도 존재할 수 있음을 시사하는 결과로서 이를 내부화하기 위한 적절한 조치의 마련이 필요할 것으로 판단된다.

의료기관의 소셜 미디어 마케팅 역량이 인지된 위험 및 의료관광 환자 만족도에 미치는 영향에 관한 연구 (The Effect of Social Media Marketing Capability on International Patient Satisfaction through Perceived Risk in the Medical Tourism Context)

  • Purpose: The purpose of the research is to explore the role of social media in attracting international patients for cosmetic services in South Korea during the COVID-19 pandemic. The study aims to conceptualize social media as an effective marketing tool for minimizing perceived risks associated with cosmetic services and increasing patient satisfaction. Methods: The study validated proposed hypotheses using the PROCESS macro for SPSS with overseas patients who received cosmetic treatment in private Korean plastic surgery clinics in Busan. Results: This study found that information delivery capability reduced perceived risk and contributed to patient satisfaction, while communication capability did not show any significant relationship with perceived risk and satisfaction. In addition, information delivery capability had a significant direct effect on patient satisfaction, but communication capability did not. Conclusion: It is expected that the outcomes of this study will broaden our understanding of the use of social media in reducing perceived risk and increasing satisfaction.

우리나라 전자의무기록의 개선방안 (Improvement Plan of the Korean Electronic Medical Record)

  • The rapid development and distribution of information communication industry facilitates the changes of hospital administration, introducing EMR(Electronic Medical Record) instead of paper-based medical record in the medical field. The developed countries such as U.S. have established EMR system after in the middle of 1970s because the primary advantages of EMR is to store and handle vast amounts of records efficiently and increase the quality of health care. Most of health organizations in Korea also apply medical record system to their administration. As the result, they have accomplished a scientific administration system through the use of medical record to handle a variety of patient's information including patient's confidentiality and privacy such as family history, social status, income level, and so on. However, access to and the misuse of EMR causes illegal infringement of patient's information and finally it becomes a very serious medical issue. Potential leakage and misuse of records may seriously infringe patient's privacy rights. In this respect, the related agencies in the public and private sector have been making efforts to prevent patient's records leakages. Especially, the revision bill of Medical Law in 2002 establishes the ways on the security and standards of electronic records. However, it does not provide the proper guidelines which is applied to the rapid changes of the medical environment. One of the most priorities in the hospital administration is the production and maintenance of an accurate medical records fulfilled by medical recorders. Therefore, it is very important for health care providers to hire ethical-based medical recorders. But, unfortunately most of hospitals overlook the importance of their roles. All parts including government, physician and patient must have more concerns on the problems related to EMR. Therefore, this study aims to propose the proper ways to resolve the problems coming from EMR.

환자 개인정보보호에 대한 간호대학생의 인식과 실천 (Recognition and Performance of Patient Private Information Protection (PPIP) in Nursing Students)

  • 본 연구는 환자 개인정보보호에 대한 간호대학생의 인식과 실천정도를 규명하기 위한 조사연구이다. 조사대상은 간호대학생 383명으로 측정도구는 Lee & Park (2005)이 개발한 설문지를 학생용으로 수정하여 사용하였다. 자료는 descriptive statistics, t-test, ANOVA, Pearson correlation coefficients로 분석하였다. 분석 결과 대상자의 96.1%이상이 학교나 병원에서 환자 개인정보보호에 대한 교육을 받았으나 10분이내의 교육이 48.0%이었다. 환자 개인정보보호에 대한 인식정도는 평균 4.13점, 실천정도는 평균 3.84점이었다. 영역별로는 의사소통 업무 영역에서 인식과 실천 정도가 가장 높았다. 학력과 나이에 따라 영역별로 인식정도에 차이가 있었고, 병원에서의 교육경험 유무, 병원교육시 정규프로그램 유무, 병원교육 시 일 회당 교육시간에 따라 영역별로 실천정도에 차이가 있었다. 인식과 실천의 총점과 영역별 점수간에 유의한 상관관계가 있었다. 본 연구의 결과는 임상실습을 하는 간호대학생을 대상으로 환자의 개인정보를 보호하기 위한 교육 프로그램을 개발하는데 도움이 될 것이다.

만성 질병환자를 위한 CDSS를 적용한 PHR 시스템 (CDSS enabled PHR system for chronic disease patients)

  • With the advance of Information Technology (IT) and dynamic requirements, diverse application services have been provided for end users. With huge volume of these services and information, users are required to acquire customized services that provide personalized information and decision at particular extent of time. The case is more appealing in healthcare, where patients wish to have access to their medical record where they have control and provided with recommendation on the medical information. PHR (Personal Health Record) is most prevailing initiative that gives secure access on patient record at anytime and anywhere. PHR should also incorporate decision support to help patients in self-management of their diseases. Available PHR system incorporates basic recommendations based on patient routine data. We have proposed decision support service called "Smart CDSS" that provides recommendations on PHR data for diabetic patients. Smart CDSS follows HL7 vMR (Virtual Medical Record) to help in integration with diverse application including PHR. PHR shares patient data with Smart CDSS through standard interfaces that pass through Adaptability Engine (AE). AE transforms the PHR CCR/CCD (Continuity of Care Record/Document) into standard HL7 vMR format. Smart CDSS produces recommendation on PHR datasets based on diabetic knowledge base represented in shareable HL7 Arden Syntax format. The Smart CDSS service is deployed on public cloud over MS Azure environment and PHR is maintaining on private cloud. The system has been evaluated for recommendation for 100 diabetic patients from Saint's Mary Hospital. The recommendations were compared with physicians' guidelines which complement the self-management of the patient.

  • The pre-existing medical treatment was done in person between doctors and patients. EMR (Electronic Medical Record) System computerizing medical history of patients has been proceed and has raised concerns in terms of violation of human right for private information. Which integrates "Identification information" containing patients' personal details as well as "Medical records" such as the medical history of patients and computerizes all the records processed in hospital. Therefore, all medical information should be protected from misuse and abuse since it is very important for every patient. Particularly the right to privacy of medical record for each patient should be surely secured. Medical record means what doctors put down during the medical examination of patients. In this paper, we applies fingerprint identification to EMR system login to raise the quality of personal identification when user access to EMR System. The system implemented in this paper consists of embedded module to carry out fingerprint identification, web server and web site. Existing carries out it in client. And the confidence of hospital service is improved because login is forbidden without fingerprint identification success.

  • The Internet of Medical Things (IoMTs) are to be considered an investment and an improvement to respond effectively and efficiently to patient needs, as it reduces healthcare costs, provides the timely attendance of medical responses, and increases the quality of medical treatment. However, IoMT devices face exposure from several security threats that defer in function and thus can pose a significant risk to how private and safe a patient's data is. This document works as a comprehensive review of modern approaches to achieving security within the Internet of Things. Most of the papers cited here are used been carefully selected based on how recently it has been published. The paper highlights some common attacks on IoMTs. Also, highlighting the process by which secure authentication mechanisms can be achieved on IoMTs, we present several means to detect different attacks in IoMTs

유헬스케어에서 환자의 프라이버시 보호 방안 연구 (A Study of Patient's Privacy Protection in U-Healthcare)

  • 유헬스케어 서비스의 급속한 발전과 보급에 힘입어 유헬스케어 서비스 기술은 많은 변화가 이루어지고 있다. 그러나 유헬스케어 서비스는 보안상의 문제로 인하여 사용자의 민감한 의료정보가 제3자에게 유출되고 사용자의 프라이버시가 사용자의 동의없이 침해되는 문제가 발생되고 있다. 본 논문에서는 유헬스케어 환경에서 사용되고 있는 환자의 프라이버시 정보에 안전하게 접근하기 위해서 병원관계자의 권한 및 접근 레벨에 따라 환자의 생체정보를 분산 접근하는 모델을 제안한다. 제안 모델은 환자의 생체정보의 접근을 제어하는 동시에 타임스탬프를 통해 DoS 공격 예방과 최신성을 유지한다. 또한, 제안 모델은 병원관계자를 중앙에서 서버가 통합 관리하는 동시에 병원마다 병원관계자의 권한 및 레벨에 따라 접근을 제어하기 때문에 환자의 프라이버시 침해 및 의료정보 유출을 예방한다.
