횡경막에 발생한 원발성낭종의 수술 치험 1예 (Primary Cyst of The Diaphragm: Report of A Case)

  • Primary cyst of the diaphragm is a rather uncommon disease. This is a report of a case of diaphragmatic cyst, located in the tendinous portion of the right diaphragm. The patient had no specific symptoms in the respiratory systems, but suffered from gastrointestinal symptoms [indigestion, epigastralgia, and loss of appetite etc.] for 2 months. Accidentally, on a simple chest x-ray examination, a round homogenous mass density was discovered. Tomography showed a well circumscribed parenchymatous mass. So a coin lesion in the medial segment of the right lower lobe was suspected. A thoracotomy was performed. There were no pathological findings in the lung and pleura. A ping-pong ball sized round mass, which was soft, elastic and fluctuated,was noted in the tendinous portion of the diaphragm. It was enucleated completely without diaphragmatic rupture, and diagnosed as a primary diaphragmatic cyst [fibrous-walled] by the histopathologic examination. Postoperative course was uneventful.

The treatment of actinomycosis mimicking a retained root tip: a confusing case

  • Actinomycosis is an infection caused by the actinomyces genus and is associated with trauma or previous infection. A 58-year-old male patient was referred from a private dental clinic for root extraction of the lower right molar. The x-ray showed fractured root-like material distal to the distal root of the lower right second molar. A biopsy during extraction of the root-like material was performed, which revealed a sequestrum with actinomycosis by a pathological examination. In this case, the radiopacity of the suspicious lesion was higher than that of the surrounding alveolar bone, which confused it with the root tip. The diagnosis of actinomycosis required long-term antimicrobial therapy, which is very different from simple extraction or removal of sequestrum.

  • Atherosclerosis, slow progressing inflammatory lesion in arteries, is one of the major causes of cardiovascular diseases. As mortality due to cardiovascular disease keeps increasing in Korea, researches on pathological mechanism of atherosclerosis may be beneficial in fighting against cardiovascular diseases. It is known that migration and MMP-9 secretion of Vascular Smooth Muscle Cell(VSMC) play a significant part in pathogenesis of atherosclerosis, although detailed mechanism of entire process is not clarified. We investigated whether the seeds of Trichosanthes kirilowii maxim (TS), inhibit migration and MMP-9 production of HASMC(human aortic SMC), which were induced by TNF-${\alpha}$ treatment. Migration assay showed that TS inhibited the migration of HASMC induced by TNF-${\alpha}$, in dose dependent manner. Also by Zymography MMP-9 production of HASMC was found to be reduced by TS, both in time and in dose dependent manner. Western blotting results suggest TS suppress activity of MAPkinases.

Studies on the Concurrent Administrations of Sosiho-Tang Extract and Methionine-Effects on the Liver Lesion Induced by Carbon Tetrachloride in Rats-

  • The preventive effect of Sosiho-Tang extract administered concurrently with methionine on hepatotoxicity induced by carbon tetrachoride (20% $CCl_4$ in olive oil, 5 ml/kg) in rats was studied. S-GOT values were decreased significantly by the concurrent administration of Sosiho-Tang extract 100 mg/kg with methionine 100 mg/kg and also Sosiho-Tang 200 mg/kg with methionine 100 mg/kg. S-GPT values were decreased remarkably by the concurrent administration of Sosiho-Tang extract 200 mg/kg with methionine 100 mg/kg. In histological observations, the pattern of the contrast group indicated the marked necrosis of the parenchymatous cells around the centrilobular area and the fatty changes of the hepatic cells around the midzonal area, but the concurrent administration of Sosiho-Tang extract 200 mg/kg and methionine 100 mg/kg revealed better improvements than their respective single administration in all pathological aspects.

Primary Eosinophilic Granuloma of Adult Cervical Spine Presenting as a Radiculomyelopathy

  • We report a case of 29-year-old man diagnosed as a primary eosinophilic granuloma (EG) lesion of the seventh cervical vertebra. He had paresthesia on both arms, and grasping weakness for 10 days. Cervical magnetic resonance image (MRI) showed an enhancing mass with ventral epidural bulging and cord compression on the seventh cervical vertebra. Additionally, we performed spine series MRI, bone scan and positive emission tomography for confirmation of other bone lesions. These studies showed no other pathological lesions. He underwent anterior cervical corpectomy of the seventh cervical vertebra and plate fixation with iliac bone graft. After surgical management, neurological symptoms were much improved. Histopathologic evaluation confirmed the diagnosis of EG. There was no evidence of tumor recurrence at 12 months postoperative cervical MRI follow-up. We reported symptomatic primary EG of cervical spine successfully treated with surgical resection.

Primary Intrapulmonary Thymoma Presenting as a Solitary Pulmonary Nodule

  • Primary intrapulmonary thymoma (PIT) is a very rare lesion of uncertain pathogenesis. PIT should be considered when the histopathological appearance of a lung tumor shows features that are uncommon but similar to those of a thymoma. In this case report, we discuss the case of a 5 9-year-old female with a solitary pulmonary nodule that was confirmed to be PIT on the basis of pathological tests. Treatment with complete resection showed good results.

Development of De Novo Cavernous Hemangioma after Radiosurgery for Cavernous Hemangioma

  • We report a rare case of cavernous hemangioma (CH) which developed in adjacent location to a preexisting CH after gamma knife radiosurgery (GKRS). A 36-year-old woman underwent GKRS for a CH in the left lentiform nucleus. Three-and-half years after radiosurgery, MRI revealed a new CH in the left caudate nucleus. Surgical excision of the new lesion was performed. The pathological examination confirmed the diagnosis of CH. In radiosurgery for CH, it should be noted that a new CH may develop, which is likely to result from the interaction between radiation and predisposing factors of the patient.

Spindle Cell Lipoma: A Rare, Misunderstood Entity

  • Spindle cell lipoma, a rare variant of lipoma, is a benign tumor found in the posterior neck and shoulder. A 24-year-old man with a close family history of malignant lymphoma had presented with a large, firm, nodular mass found in the right supraclavicular area. Excision of the deeply located mass revealed a pale yellow, rubbery nodule which grossly resembled an enlarged lymph node, with a variant of lymphoma as a primary suspect. However, pathological studies revealed the lesion to be a spindle cell lipoma. Although atypical in location, spindle cell lipoma should always be kept in differential diagnosis of a newly-noted soft tissue mass, as this entity may be easily cured by simple excision.

Spontaneous Pulmonary Hematoma with No Underlying Causes: A Case Report

  • A 57-year-old male patient was admitted to our center because of a cystic mass on the lower portion of the right major fissure that was found incidentally by chest X-ray. He did not have a history of trauma or anticoagulant use. The lesion was removed by video-assisted thoracoscopic surgery. Pathological examination revealed an organizing pulmonary hematoma without any complications, and a follow-up chest X-ray after 1 year showed no recurrence.

  • The salivary duct cyst is a rare disease and usually found in the parotid gland. A 55-years-old man presented swelling in the left buccal area and pathological diagnosis was a salivary duct cyst. Though its recurrence has been reported rare, the presented case showed recurrent swelling after enucleation. The recurrent lesion was successfully treated by incision and drain insertion.
