Phase II Study of Preoperative Intra-Arterial Epirubicin, Etoposide, and Oxaliplatin Combined with Oral S-1 Chemotherapy for the Treatment of Borrmann Type 4 Gastric Cancer

  • Purpose: A phase II study was conducted to evaluate the safety and efficacy of preoperative, intra-arterial perfusion of epirubicin, etoposide, and oxaliplatin combined with oral chemotherapy S-1 (SEEOX) for the treatment of type 4 gastric cancer. Materials and Methods: A single-center, single-arm phase II trial was conducted on 36 patients with histologically proven type 4 gastric cancer without distant peritoneal or organ metastasis. Patients received 3, 21-day courses of SEEOX preoperative chemotherapy. The primary endpoint was overall survival (OS) and the secondary outcomes assessed were chemotherapeutic response, radical resection rate, pathological regression, toxicities, postoperative morbidity, and mortality. Results: All patients were at an advanced stage of cancer (stage III or IV) and completed the entire course of treatment. Based on changes in tumor volume and peritoneal metastasis, the objective response rate was 55.6% (20/36; 95% confidence interval [CI], 38.5%-72.6%) and the disease control rate was 69.4% (25/36; 95% CI, 53.6%-85.3%). The radical resection rate was 75% (27/36; 95% CI, 60.1%-89.9%) and the proportion of R0 resections was 66.7% (21/36; 95% CI, 50.5%-82.8%). The pathological response rate was 33.3%, of which 13.9% showed complete pathological regression. The median survival was 27.1 months (95% CI, 22.24-31.97 months), and the 2-year OS was 48.5% (95% CI, 30.86%-66.1%). Conclusions: Preoperative SEEOX is a safe and effective treatment for type 4 gastric cancer. Based on these preliminary data, a phase III study will be conducted to confirm the superiority of this regimen over standard treatment.

Cytotoxicological and Pathological Studies of 6-Paradol, a Pungent Principle of Ginger (생강성분 6-Paradol의 세포 독성 및 병리학적 연구)

  • It is previously reported that 6-paradol can induce prolonged analgesia in experimental animals. In order to investigate the mutagenicity of 6-paradol, Ames Samonella/microsome plate assay was carried out with Salmonella typhimurium strains, TA 98, TA 100, TA 1535 and TA 1538, 6-Paradol was nonmutagenic in Salmonella typhimurium with and without rat liver microsomal activation. The rec assay with Bacillus substilis strains H 17 $rec^+$ and M 45 $rec^+$ was carried out ot test 6-paradol and other compounds (1-3 mg/disc) for DNA damaging activity, 6-Paradol was also nonmutagenic in DNA damaging activity. The relative size of the inhibition zone for 6-paradol was smaller than that of capsaicin. We have also determined the pathological effects of this compound on the various tissues of rats after administrating(i.p.) with increasing doses of 4, 8, 12, 16 mg/kg at 2 hour intervals and found no significant changes in terms of histology.

Somatostatin Receptors 3, 4 and 5 Play Important Roles in Gallbladder Cancer

  • Expression changes of somatostatin receptor subtypes (SSTRs) including SSTR1, SSTR2, SSTR3, SSTR4 and SSTR5 in the development of gallbladder cancer were assessed with attention to relationships with clinical pathological characteristics. SSTRs in 29 gallbladder cancer and 25 normal gallbladder tissue specimens were examined by immunohistochemical staining. Differences between SSTRs expressions and clinical pathological parameters were analyzed by chi-square test. The five subtypes of SSTR were all expressed in gallbladder cancer tissues and SSTR3 presented the highest expression. SSTR5 expression was increased significantly in gallbladder cancer (P<0.05) compared with that in normal gallbladder tissue. SSTR3 expression in highly and moderately differentiated gallbladder cancer was significantly higher than that in poorly differentiated lesions (P<0.05). SSTR4 expression was lower in gallbladder cancer with lymph node metastasis than that in gallbladder cancer without lymph node metastasis (P<0.05). Therfore, these results indicated that SSRT5, SSTR3 and SSTR4 may play important roles in the formation and development of gallbladder cancer.

Acute Oral Toxicity of the Bark of Diospyros kaki in Mice (마우스에 대한 감나무 껍질의 급성경구독성시험)

  • This study was carried out to investigate the acute toxicity of the bark of Diospyros kaki (Ebenaceae) in mice. The aqueous extract of the bark of Diospyros kaki (AEDK) was administered orally at a doses of 5 mg/kg, 50 mg/kg, 300 mg/kg and 2,000 mg/kg. In this study, number of deaths, clinical signs, body weights and pathological examinations were investigated after administration of AEDK. There were neither dead animals nor significant changes of body weights during the experimental period. In addition, no differences were found between control and AEDK treated groups in clinical signs, organ weights and gross pathological findings. AEDK did not show any toxic effect in mice.

  • Extracellular acidification occurs not only in pathological conditions such as inflammation and brain ischemia, but also in normal physiological conditions such as synaptic transmission. Acid-sensing ion channels (ASICs) can detect a broad range of physiological pH changes during pathological and synaptic cellular activities. ASICs are voltage-independent, proton-gated cation channels widely expressed throughout the central and peripheral nervous system. Activation of ASICs is involved in pain perception, synaptic plasticity, learning and memory, fear, ischemic neuronal injury, seizure termination, neuronal degeneration, and mechanosensation. Therefore, ASICs emerge as potential therapeutic targets for manipulating pain and neurological diseases. The activity of these channels can be regulated by many factors such as lactate, $Zn^{2+}$, and Phe-Met-Arg-Phe amide (FMRFamide)-like neuropeptides by interacting with the channel's large extracellular loop. ASICs are also modulated by G protein-coupled receptors such as CB1 cannabinoid receptors and 5-$HT_2$. This review focuses on the physiological roles of ASICs and the molecular mechanisms by which these channels are regulated.

An Outbreak of Porcine Pleuropneumonia in Korea (돼지 흉막폐렴(胸膜肺炎)의 발생(發生))

  • Authors observed an outbreak of porcine pleuropneumonia at a piggery in Gyeongbug, Korea, which were diagnosed on the ground of clinical and pathological findings with the following results. 1. One hundred and forty weanlings out of 200 ones died suddenly without noticiable signs during the period of 15 days. All of the dead pigs showed formy, blood-tinged discharge from the nose and mouth. 2. Main clinical findings observed from 60 alive pigs from group consisted of fever, cyanosis severe degree of dyspnea and pleuritic friction rub. 3. On hematological observation for typical 5 cases leukocytes showed marked degenerative left shift with leukopenia (10500-13070/cmm), neutropenia (7-13%) and increase in the number of band neutrophils (6-27%). 4. The most prominent and constant pathological changes observed at necropsy for dead or alive pigs consisted of fibrinous pleurisy with partial adhesion, overlying of the pulmonary surface with fibrinous menbrane and necrotic pneumonic lesions with marked edema and congestion of lung parenchyma. 5. From these results the outbreak was considered to be typical porcine pleuropneumonia, although no bacteriological isolation was attempted.

An Acute Outbreak of Calf Salmonellosis Caused by Salmonella dublin (Salmonella dublin에 의한 소의 살모넬라증의 발생)

  • An epizootic of calf diarrhea occurred in an extensive dairy farm located in Gangwondo province in December of 1985. The patients showing fever, chills, complete anorexia and watery diarrhea sometimes dysentery were concentrated among the calves ranged from 8 days to 3 months old and many of them died in acute course. The five carcases were examined by pathological and bacteriological means. The predominant gross lesions were edematous swelling of the mesenteric lymph nodes, profuse catarrhal or diphtheritic enteritis and extensive purulent bronchopnenomonia or lobar fibrinous pneumonia. Microscopically the lesions of the liver and spleen were characterized by coagulative necrotic foci and granulomatous nodules. The spleen and lymph node showed also proliferative changes of reticular cells and involution of intestinal lymphatic nodules were noticed. The strains of Salmonella(S) species isolated from the liver, spleen, bile juice, peritoneal fluid and thoracic fluid were identified serologically as S. dublin. These clinical and pathological findings of the disease were those of acute enteritis form of salmonellosis and characterized by high morbidity and mortality among the calf herd. The report also signifies the first description of an epizootic of bovine salmonellosis caused by S. dublin in Korea.

Pathological study on rabbit haemorrhagic disease in young rabbits

  • We investigated the pathological changes in young rabbits which were experimentally infected with rabbit hemorrhagic disease virus (RHDV). Experimental infection of RHDV was carried out in both thymectomized and non-thymectomized young immature rabbits and adult rabbits. None of young rabbits infected with RHDV died during the experiment. Histologically, single or focal hepatocellular degeneration and necrosis with mild lymphocyte infiltration were observed in the rabbits killed at 30 hours and 5 days PI. Lymphocyte infiltration was more severe at 5 days PI than at 30 hours PI. RHDV antigens were mainly detected in the degenerating hepatocytes adjacent to the infiltrated lymphocytes at 30 hours PI and 5 days PI. In electron microscopical observation, infiltrated lymphocytes in the lesions had large nuclei without cytoplasmic granules and interdigitated with adjacent hepatocytes. It is assumed that infiltrated lymphocytes in hepatic lesions in RHDV infected young rabbits are T-lymphocytes and originate from peripheral lymphoid organs or tissues rather than from thymus.

  • This study was carried out to investigate the pathological changes with paraquat(1.1'-dimethyl-4.4'-dipyrildiylium dichloride) administered by intraperitoneally, orally, skin applied in mice, rats and rabbits. Results were obtained as follows; In 2 days after paraquat administration clinical signs anorexia, depression, tachypnea, and tachycardia, respiratory failure, coma and death were observed in mice, rats and rabbits. Compared toxicity of paraquat with mouse were observed toward to paraquat that resistance was strong than rats and rabbits. Also, mortality of skin application were found the low than intraperitoneal and high than oral administration. In the case of gross observation were appear lips moisture in orally administered rats and rabbits by skin application. Lung of all laboratory animals were observed congestion and haemorrhage, swelling or atrophy. In the case of microscopic findings were severe congestion and haemorrhage, interstitial pneumonia of lung. Congestion and haemorrhage of liver, congestion and haemorrhage, renal tubule epithelium necrosis of kidney were observed in mice, rats and rabbits. Skin application group of mice, rats and rabbits showed infiltration of inflammatory cells and folliculitis of epidermis and dermis. Also, in oral administration group showed congestion and haemorrhage, tachment, necrosis of alimentary tract mucosa.

Pathological study of a infectious lymphoid organ necrosis virus infection in penaeid shrimp (Penaeus orientalis) (보리새우류(대하)의 전염성 Lymphoid organ 괴사 바이러스 감염에 대한 병리학적 연구)

  • Since 1993, sudden and massive death has occurred among penaeid shrimp, Penaeus orientalis cultured in Korea. We investigated the pathological characterizations on the spontaneously and experimentally infected penaeid shrimp. The major gross findings of the spontaneous cases were 2-6mm sized-white spots on the inside of the carapace and reddish discolorization. Histopathologically, massive necrosis of lymphoid organ, degeneration and necrosis of epithelia of epidermis and foregut were observed in both spontaneous and experimental cases. Amphophilic to basophilic intranuclear inclusion bodies were readly observed epithelia of epidermis, foregut and gills. Electron microscopy revealed enveloped, non-occuluded ellipsoid to rod shaped virus particles, within the nucleus, in the lymphoid organs and interstitial cells of hepatopancreas of both spontaneously and experimentally infected shrimps. The size of the virion was $375{\times}167nm$, and the nucleocapsid was $290{\times}75nm$. The causative agent causing massive death in penaeid shrimps in Korea resembles baculovirus associated with white spot syndrome (WSBV) occurred in Taiwan in virus morphology and gross and histological changes of the shrimps.
