A study on Samchobyeonjeung(三焦辨證) of "OnByeongJoByeon(溫病條辨)" ("온병조변(溫病條辨)"의 삼초변증(三焦辨證)에 대한 고찰)

  • Byeon Jeung[辨證], which is to differentiate symptoms is a process of assessing the patient's condition. And like any other differentiation system, a ceratin pathological perspective is embedded. In Onbyeonghak(溫病學), new Byeon Jeung systems were designed to complement the insufficient, existing perspectives on acute epidemic febrile diseases. Oguktong(吳鞠通)'s Samchobyeonjeung(三焦辨證) is one of these newly designed systems. Though the meaning of Samchobyeonjeung varies among Onbyeong(溫病) researchers, they stand on common grounds of differentiating damp-heat disease[濕熱病]. However, Oguktong(吳鞠通) clearly demonstrated this system on epidemic febrile diseases. Researchers have acknowledged the importance of Oguktong(吳鞠通)'s approach, but thorough analysis of the subject has been insufficient. In this study, I have primarily studied the meaning of Samchobyeonjeung by analyzing "Onbyeongjobyeon(溫病條辨)", followed by assessment of negative and positive aspects.

Clinico-Hematological Findings for Classical Hodgkin's Lymphoma: an Institutional Experience

  • Background: Classical Hodgkin's lymphoma (cHL) is a B-cell lymphoid neoplasm characterized by a distinctive biological behavior with potentially curable disease characteristics. It is an uncommon hematological malignancy which primarily affects younger individuals. The rationale of this study was to determine its clinico-hematological profile along with stage stratification in Pakistani patients. Materials and Methods: In this descriptive study, adult patients with Hodgkin's lymphoma were enrolled from January 2010 to December 2014. Results: Sixty two histopathologically confirmed cases of cHL were identified. There were 42 males and 20 females, with a male to female ratio of 2: 1. The mean age was $29.7{\pm}13.8$ years with the median age of 30 years. B symptoms were present in 72.5% of patients. Histopathologically, the mixed cellularity type constituted 62.9% of cases, followed by nodular sclerosis in 25.8%, lymphocyte predominant in 9.6% and lymphocyte depleted in 1.6%. Stages I and II were present in 43.5% of patients at disease presentation, with 56.4% in stages III and IV. Conclusions: Our analysis shows that clinico-pathological features of Hodgkin's lymphoma in Pakistan are comparable to published data. Mixed cellularity is the commonest histological variant and advanced stage at presentation are common findings in our patients.

  • Pain and sensory disorder resulting from injury to peripheral nerves of the face and jaws are a major source of patient dissatisfaction and suffering. The majority of patient who sustain injuries to the peripheral sensory nerves of the face and jaws experience a slow but orderly return of sensation that is functional and tolerable in quality, if not "normal". For many patients, however, the long-term effects are a source of aggravation, and for a few, a significant cause of suffering. Common complaints relate to reduced sensory information causing embarrassing food accumulations or drooling, biting a burning the lip or tongue, and difficulty in performing routine activities such as shaving and apply makeup. For some patients posttraumatic symptoms become pathological and frankly painful. The predominent pain components are (1) numbing anesthesia dolorosa pain, (2) triggered neuralgiaform pain, (3) burning, aching causalgiaform pain, and (4) phantom pain. This is a report of cases about posttraumatic pain syndrome associated with dental treatment in a psychologically disabled patient.

Fistula Formation between Right Upper Bronchus and Bronchus Intermedius Caused by Endobronchial Tuberculosis: A Case Report

  • Endobronchial tuberculosis is defined as a tuberculous infection of the tracheobronchial tree and has a prevalence of up to 50% in active pulmonary tuberculosis cases. The most common complication of endobronchial tuberculosis is bronchial stenosis; benign fistula formation by endobronchial tuberculosis is rare, especially inter-bronchial fistula formation. We reported a rare case of a 73-year-old woman with a fistula between the right upper bronchus and bronchus intermedius. A diagnosis of inter-bronchial fistula caused by endobronchial tuberculosis was based on the results of chest computed tomography scans, bronchoscopy, and microbiological and pathological tests. The patient was treated with anti-tuberculous medication, and her symptoms gradually improved.

Ganglioneuroblastoma Associated with Malignant Hypertension and Cardiac Failure (심한 고혈압과 심부전을 동반한 신경절신경모세포종 1예)

  • Neuroblastoma is the most common extracranial solid tumor in children, and accompanies various clinical symptoms including hypertension. Hypertension is associated with catecholamines secreted from the tumor, and is usually not severe. We report one case of malignant hypertension with cardiac failure in a patient with adrenal neuroblastoma, successfully treated with adrenalectomy. A 3 year-old boy complained of protrusion of the chest wall. Physical examination revealed severe hypertension with cardiac failure. The levels of metabolites of catecholamine were increased in blood (norepinephrine >2000 pg/mL) and urine (norepinephrine 1350.5 ug/day). Abdominal CT showed a 7 cm-sized solid mass arising from the right adrenal gland. After stabilizing the hemodynamics with oral phenoxybenzamine, right adrenalectomy was performed. Pathological diagnosis was a ganglioneuroblastoma. The hypertension and cardiac failure were resolved after tumor removal.

Prevalence and Clinical Manifestations of Malaria in Aligarh, India

  • Malaria is one of the most widespread infectious diseases of tropical countries with an estimated 207 million cases globally. In India, there are endemic pockets of this disease, including Aligarh. Hundreds of Plasmodium falciparum and P. vivax cases with severe pathological conditions are recorded every year in this district. The aim of this study is to find out changes in liver enzymes and kidney markers. Specific diagnosis for P. falciparum and P. vivax was made by microscopic examination of Giemsa stained slides. Clinical symptoms were observed in both of these infections. Liver enzymes, such as AST, ALT, and ALP, and kidney function markers, such as creatinine and urea, were estimated by standard biochemical techniques. In Aligarh district, P. vivax, P. falciparum, and mixed infections were 64%, 34%, and 2%, respectively. In case of P. falciparum infection, the incidences of anemia, splenomegaly, renal failure, jaundice, and neurological sequelae were higher compared to those in P. vivax infection. Recrudescence and relapse rates were 18% and 20% in P. falciparum and P. vivax infections, respectively. Liver dysfunctions and renal failures were more common in P. falciparum patients, particularly in elderly patients. Artesunate derivatives must, therefore, be introduced for the treatment of P. falciparum as they resist to chloroquine as well as sulfadoxine-pyrimethamine combinations.

A Case of Idiopathic Portal Hypertension in a 3-year-old Girl (3세 여아에서 진단된 특발성 문맥 고혈압 1예)

  • A previously healthy 3-year-old girl was admitted to the Department of Pediatrics in Severance Hospital with sudden symptoms of melena. The vital signs were stable, and splenomegaly was found in a physical examination. The patient had moderate thrombocytopenia. There was no evidence of autoimmune disease. A upper gastrointestinal endoscopy and esophagogram showed a varix on the lower esophagus. Coarse liver parenchymal echoes and increased periportal echogenicity were seen on a Doppler sonogram. The velocity of the portal vein mildly increased. Magnetic-resonance-cholangiopancreatogram (MRCP) demonstrated normal portal structures. A sono-guided liver biopsy was performed, but the pathological findings were unremarkable. Based on these findings, we diagnosed the patient with idiopathic portal hypertension. The patient was discharged and was treated with oral beta blocker. We report a case of idiopathic portal hypertension with a brief review of the literature.

Occurrence of Blue Mold on Tomato Caused by Penicillium oxalicum in Korea

  • A blue mold on tomato fruit caused by Penicillium oxalicum occurred sporadically in a greenhouse at Gyeongsangnam-do Agricultural Research and Extension Services. Infection usually occurred through wounds or cracks on the fruits. Symptoms began with water soaking lesions, then became watery and softened eventually. Colony of the causal fungus was white at the early growing stage, turned green on Czapek yeast extract agar and malt extract agar. Conidia were ellipsoidal in shape and $2-6{\times}2-4{\mu}m$ in size. Stipes were septate, smooth, thin walled, and $90-280{\times}3-4{\mu}m$ in size. Penicilli were mostly biverticillate. Ramuli were 1-3 groups, smooth, and $10-16{\times}2-3{\mu}m$ in size. Rami were 1-2 groups and $6-30{\times}2-4{\mu}m$ in size. Metulae were 2-3(5) verticils, smooth, and $12-20{\times}3-4{\mu}m$ in size. Phialides were 5-7 verticilate, ampulliform to cylindroidal, smooth, and $8-12{\times}2-3{\mu}m$ in size. Optimum temperature for growth was about $25^{\circ}C$. Pathogenicity of the fungus was proved on tomato fruit according to Koch's postulation. On the basis of mycological and pathological characteristics, the fungus was identified as P. oxalicum Currie & Thom. This is the first report of the blue mold on tomato fruit caused by P. oxalicum in Korea.

Characterization of Sclerotinia sclerotiorum, an Emerging Fungal Pathogen Causing Blight in Hyacinth Bean (Lablab purpureus)

  • Stems and pods of hyacinth bean cultivated in a farmer's field in Gazipur District, Bangladesh, were found rotted in nearly 5% hyacinth bean plants. A fungus having fluffy mycelium and large sclerotia was isolated from affected tissues. Combined results of morphological, molecular and pathological analyses identified the fungus as Sclerotinia sclerotiorum (Lib) de Bary. Inoculating the fungus on healthy hyacinth bean plants and pods reproduced the symptoms previously observed in the field. The three isolates obtained from naturally infected plants were cross inoculated in hyacinth bean, okra and African-American marigold and they were pathogenic to these hosts. The optimum temperature and pH for its growth were $20^{\circ}C$ and pH 5.0, respectively. Sclerotial development was favored at pH 5.0. Sucrose and mannitol were the best carbon sources to support hyphal growth, while glucose was the most favourable for sclerotial development. The hyacinth bean genotypes, HB-82 (Rupban Sheem) and HB-102 were found highly resistant, while HB-94 (Ashina) was moderate resistant to the fungus. Finally, S. sclerotiorum was sensitive to Bavistin, Dithane M-45 and Rovral fungicides and Ca in the form of $CaCl_2$. This observation could possibly aid in eliminating field loss in hyacinth bean caused by an emerging pathogenic fungus S. sclerotiorum.

  • Phantasy is the psychic reality in which drive, sadism and anxiety are represented and constructed. It is one of important metapsychological concepts of the psychoanalysis which explains the formations of symptoms, internal psychic structure, and the mutual relationship between internal subjective world of man and external world. Through the understanding of phantasy we can approach the man's unconsciousness and fathom the structure of the subject itself. Especially S. Freud and M. Klein's psychoanalysis give us the deep insight into the phantasy, on the basis of which we can investigate the cause and structure of the pathological phenomena of the man.
