Lipomas are a common soft-tissue tumor of mesenchymal origin, but intermuscular lipomas are very infrequently found especially in the cervical region. Although lipoma itself rarely causes complications, an accurate diagnosis and appropriate treatment are important. Surgical excision is indicated for deep lipomas that are 5 cm or larger, grow, or are accompanied by symptoms such as pain. It is also important to differentiate malignant tumors through pathological examination. Since the size of the lipoma was large, lipoma was divided into several pieces to successful removal, and the patient in this case showed successful recovery after surgery.
Purpose: Sinus tarsi syndrome (STS) is caused by various pathologies. However, the exact etiology of STS remains controversial. This study evaluated the imaging and arthroscopic findings of patients who underwent surgical treatment after conservative treatment for STS failed. Materials and Methods: Between December 2014 and August 2018, 20 patients (21 cases) who underwent surgical treatment for STS were included in the study. The clinical results were analyzed using the visual analog scale (VAS) and the American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot functional scale. The radiographic results were analyzed using Meary's angle, calcaneal pitch angle, and hindfoot alignment angle. The pathologic conditions of sinus tarsi were confirmed by magnetic resonance imaging (MRI) and subtalar arthroscopy. Synovitis, bone edema, and accessory anterolateral talar facet (AALTF) were evaluated on MRI. Synovial thickening, cartilage damage, interosseous talocalcaneal ligament (ITCL) and cervical ligament rupture, soft tissue impingement, AALTF, and accessory talar facet impingement (ATFI) were evaluated by subtalar arthroscopy. Results: The mean duration of symptoms was 28.7 months (4~120). All patients showed significant improvement in the VAS and AOFAS ankle-hindfoot scale. Significant improvements in hindfoot alignment angle and Meary's angle postoperatively were noted in patients who underwent medial displacement calcaneal osteotomy. MRI confirmed synovitis in all patients, AALTF in 19 cases (90.5%), and ATFI with bone edema in seven cases (33.3%). In subtalar arthroscopy, pathologic conditions were observed in the following order: synovitis in 21 cases (100%), AALTF in 20 cases (95.2%), ITCL partial rupture in nine cases (42.9%), and soft tissue impingement in seven cases (33.3%). All cases had two or more pathological conditions, and 15 (71.4%) had three or more. Conclusion: In cases of STS that do not respond to conservative treatment, a comprehensive examination of the lesions of the tarsal sinus and lesions around the subtalar joint is essential.
직장 매독은 매독 감염(syphilis infection)에서 드물게 발생하며 증상 및 내시경, 영상 소견이 비특이적이다. 직장 매독은 일반적으로 동심성 장벽비후, 점막 충혈, 직장간막 지방침윤, 림프절병증 등으로 나타난다. 직장암은 비대칭적 장벽비후, 림프절 괴사를 보여 두 질환 감별에 도움을 얻을 수 있다. 하지만 두 질환의 중복되는 소견이 많아 환자의 병력을 고려하지 않고 직장 매독과 직장암을 구분하기는 매우 어렵다. 직장 매독은 직장암 외에도 염증성 장 질환 등 다른 양성 질환으로 오인하여 필요 없는 검사를 하거나 치료가 지연되는 경우가 많다. 저자들은 악성 종양으로 의심되었던 직장 매독 증례를 경험하여 이 증례 보고에서 내시경 및 영상, 병리학적 소견에 대하여 자세히 보고하고자 한다.
Objectives: The purpose of this study is to investigate the domestic awareness of Korean medicine treatment for dysmenorrhea. Methods: We conducted word frequency analysis, 2-gram analysis, degree centrality analysis, and CONCOR analysis using big data searched for '월경통', '생리통', and '한방' as main key words. The searching period was set from 2019 to 2023. Results: The number of original text data searched through main key words was on the rise. Specific words related to Korean medicine treatment were in the top 100, words related to herbal medicine ranked particularly high. The top 100 words were divided into 4 clusters (A; Body parts or body substances related to dysmenorrhea, B; Symptoms that accompany dysmenorrhea, C; Physiological or pathological situations related to dysmenorrhea, and D; Treatment related words of dysmenorrhea) and some clusters showed close relationships between each others (B-C, B-D, and C-D). Conclusions: Our results suggest that interest in Korean medicine treatment for dysmenorrhea is increasing. Also, as public understanding is estimated to be high, specific information such as method, process and mechanism of Korean medicine treatment seems to be needed.
Objectives : Herbal formula consist of multiple herbs, which can potentially interact with conventional drugs. If these interactions are not properly understood, they may reduce treatment efficacy or cause unexpected side effects. Thus, this study collected and analyzed papers on herbal formula and conventional drug interactions from PubMed to analyze various research trends. Methods : To analyze research trends on herbal formula and drug interactions, we first created search queries using a dictionary of herbal formula terms and collected related papers from PubMed using the Entrez API. The PubTator API was applied to identify compound names in the abstracts, recognizing compounds registered in the DrugBank as conventional drugs. Sentences describing interactions between herbal formulas and drugs were extracted using pattern matching, and relevant papers were selected. Trends were then analyzed by year, country, major formulas, major drugs, and interaction networks. Results : Yearly analysis showed a gradual increase in paper counts with a significant rise after 2010. Country analysis revealed that China published the most papers (53), followed by Japan (19) and South Korea (8). formula analysis identified 'sosiho-tang' and 'siryung-tang' as the most frequently mentioned (7 times each). Drug analysis showed '5-fluorouracil', 'acetaminophen', 'entecavir', and 'streptozotocin' were the most frequently mentioned (4 times each). Network analysis revealed 'sosiho-tang and tolbutamide' and 'siryung-tang and prednisolone' as the most frequently, mentioned interactions (3 times each). Disease analysis indicated 'urogenital diseases' were the most discussed (32 mentions), Followed by 'pathological conditions, signs, and symptoms' and 'digestive system diseases' (25 mentions each). Conclusions : Analyzing research trends on herbal formula and conventional drug interactions provides basic data for subsequent research, aiming to reduce side effects and enhance treatment efficacy in clinical settings.
Alginase$Alginase^{ⓡ}$ (Arginine esterase) is one of the snake venoms which is mainly consisted of arginine esterase and acts as a thrombus -forming inhibitor/thrombus-lysin. These present studies were performed to investigate of the acute and subacute toxicity of the Alginase$Alginase^{ⓡ}$ in rats. In acute toxicity study, rats were single administered intravenously with dosages of 0.001, 0.01. 0.1, 1 and 10U/kg B.W. and examined the number of death, clinical sign, body weight and pathological change for 7days after administration of Alginase$Alginase^{ⓡ}$. At maximum dose level (10U/kg B.W.), Alginase$Alginase^{ⓡ}$ induced symptoms of shock with cyanosis and dyspnea. But these symptoms dissappeared after 30~50 minutes and we could not find any other toxic effect in rats. Therefore, $LD_{50}$ Value of Alginase was over 10U/kg B.W. in rats. In four-week intravenous toxicity study of Alginase$Alginase^{ⓡ}$, rats were administered intravenously seven days per week for 28 days, with dosages of 0, 0.0125, 0.125 and 1.25U/kg B.W./day, respectively. Alginase$Alginase^{ⓡ}$ did not caused any death and showed any clinical signs in rats. No significant Alginase$Alginase^{ⓡ}$ -related changes were found in feed uptake, water consumption, hematology, serum biochemistry, urinalysis, ocular examination, organ weight and histopathological examination. From the results, Alginase$Alginase^{ⓡ}$ seems not to have any toxic effect in rats when it were given daily intravenous injections below the dosage 1.25U/kg B.W./day for four weeks.
Objectives Eyes, one of visual inspection regions, present important clues to pathological patterns including deficiency and excess patterns to the clinicians. The purpose of this study was to develop Eyes Inspection Questionnaire (EIQ) and to examine which items among the EIQ were more predictive of clinicians' determination for the deficiency and excess patterns. Methods Nine questionnaire items for Visual Inspection of Eyes were extracted through the literature review. These items were presented to the 4 Korean medical doctors who are specialized in visual inspection to conduct the Delphi method. The Korean medical doctors were asked to rate the importance of each items for the corresponding Visual Inspection of Eyes, using a Likert 5-point scale(the 3 points of importance as a cut-off point). Then, out of 75 photographs submitted to the Society of HyungSang Medicine in 2009, 30 portrait pictures were selected as samples. The samples were copied to make 60 sample pictures, and then randomly assigned to 4 clinicians. The 4 clinicians evaluated the 60 samples for excess and deficiency of the eyes and were asked to check the 6 questionnaire items. The results were recorded as 5-points-scale, and their average and standard deviations were calculated. Intra- class reliability test and multi regression test were performed using SPSS 13. Results Intra-class correlation coefficient (ICC) was between 0.750 to 0.841 (P<0.05). Indices for visual inspection of the eyes were: endowment of the bone structure around the eyes; brightness of the eyes; upward deviation of the eyes; eye shapes; and definition of iris. 76.92% of deficiency symptom patterns and 86.42% of the excess symptom patterns matched the patterns predicted by the visual inspection of the eyes, according to the frequency analysis. According to the multiple regression analysis, were significantly related to the excessive symptoms, and to the deficiency symptoms. Conclusion This study is the first attempt of development for checklist of excess and deficiency of Visual Inspection of Eyes and quantitative measurement of excess and deficiency using the Visual Inspection of Eyes by the visual inspection experts. Still, additional studies are needed regarding the relationship visual inspection methods have with existing standards of diagnosis.
Wang Cheong-Im(1768-1831) in the Ching dynasty conducted the therapy of blood statis syndrome using the methods for promoting blood circulation and removing blood statis in accordance with the description in the book Uirimgaechak(醫林改錯). Thus, this study examined his methods for promoting blood circulation and removing blood statis, therapy of blood statis syndrome and mechanism of function. Since the blood statis symptoms varied on the parts where the blood statis were occurred, the therapy for promoting blood circulation and removing blood statis applied different treatments and drugs depending on the symptoms. The therapy also valued much of qi and blood. Qi is important to the human body so that the therapy replenished qi and simultaneously promoted the blood circulation as mainly focusing on the regulation of the qi flow. Moreover, the therapy adjusted the level of removing blood circulation and rate of replenishing qi. The degree of prescription and drug was differentiated in treating a variety of blood statis because of the difference in the seriousness and characteristics of blood circulation, amount and application of drug and combination of drugs. The therapy was careful about the regulation of qi for the replenishment of qi and the nurishment of blood for the promotion of blood circulation. If the blood circulation was blocked, the blood statis was formed. Accordingly, the phlegm and the blood statis were closely related. Then, the phlegm and blood statis were simultaneously treated. The therapy divided the properties of blood statis into the stagnant excess syndrome, the stagnant deficiency syndrome, the stagnant cold syndrome and the stagnant heat syndrome and treated the patients suitable for each symptom. The function of mechanism in the treatment using the methods for promoting blood circulation and removing blood statis was divided into the stagnation of blood and interruption in blood circulation. The therapy laid stress on promoting blood circulation and removing blood circulation. With such emphasis, the therapy facilitated the circulation in the bloood vessel, prevented the coagulation of blood, removed the blood statis and promoted the metabolism. Then, the physiological function and pathological change internal organs were improved. Furthermore, the therapy strengtened the heart and promoted the blood circulation by improving the systemic blood circulation. Moreover, the therapy facilitated the micro-circulation by adjusting the balance of body.
Objective : This study aimed to review the performance of traditional herbal prescriptions for treating dementia and present a strategy for research on dementia therapy utilizing herbal medicine. Methods : A definition was made to clarify the technology regarding the development of herbal prescriptions for treating dementia. The queries were compounded based on the initial keywords provided by experts in the field, then applied to the Web of Science database search engines from January 1986 to September 2011 to search related scientific articles. Before performing the analysis, papers were extracted from the initial search reviewed by experts and 80 articles were selected. Then, the selected papers were analyzed in terms of publish year, country, and type of herbal prescriptions. Furthermore, the research performance evaluation for treating dementia by herbal prescriptions was also created in terms of country and organization based on forward citation analysis. In addition to, for the evaluation regarding research quality, we classified and reviewed papers into two types: clinical studies and experimental studies. Results : According to the quantitative information analysis of 80 articles, the number of papers has increased by 21.9% per the yearly mean from 1995, and Japan had the largest portion within this research field. There were 34 kinds of traditional herbal prescriptions, among them Ukgansan had the highest number of studies followed by Jodeungsan, Dangkisoosan and so on. In addition, quality index as calculated by cites per paper is higher than average in Switzerland, Turkey and Japan. In the view of the evaluation on quality there were 12 clinical studies, 8 RCT reported that herbal prescriptions had efficacy at cognition, behavioral & psychological symptoms (BPSD) and activity of daily life (ADL) in various type of dementia. In experimental studies most of the studies were performed using animal models. The studies using Ukgansan were aimed at improving BPSD. The papers studied with Jodeungsan and Dangkisoosan targeted vascular dementia. Conclusions : In this study, research to develop traditional herbal prescriptions for treating dementia has the potential to improve symptoms since herbal medicines work as both multi-function and multi-target in dementia with multiple pathological or neurotoxic pathways. Therefore, the results of the research should be used in order to establish strategies to develop technology for treating dementia with traditional herbal prescriptions in the future.
Background: Gastric cancer is the second leading course of cancer death worldwide and H. pylori infection is an important risk factor for gastric cancer development. This study was design to evaluate the clinical, pathological features, survival rate and prevalence of H. pylori infection in gastric cancer in Thailand. Materials and Methods: Clinical information, histological features, endoscopic findings and H. pylori status were collected from gastric cancer patients from Thammasat university hospital during June 1996-December 2011. H. pylori infection was assessed by histological evaluation, rapid urease test and serological test. Clinical information, endoscopic findings and histopathology of all patients were recorded and compared between patients with active or non-active H. pylori infection. Results: A total of 100 gastric cancer patients (55 men and 45 women with mean age of $55{\pm}16.8years$) were enrolled in this study. Common presenting symptoms were dyspepsia (74%), weight loss (66%), anemia (63%) and anorexia (38%). Mean duration of symptoms prior to diagnosis was 98 days. Overall prevalence of H. pylori infection was 83% and active H. pylori infection was 40%. 1-year and 5-year survival rates were 43% and 0%. There was no significant difference between active H. pylori infection in different locations (proximal vs non-proximal: 47.1% vs 48.5%; P-value = 0.9, OR=0.9; 95%CI=0.3-3.1) and histology of gastric cancer (diffuse type vs intestinal type: 47.4% vs 50%; P-value = 0.8, OR=0.9, 95%CI=0.3-2.7). However, linitis plastica was significantly more common in non-active than active H. pylori infection (27.9% vs 0%; P-value<0.0001, OR=13.3, 95%CI=3.2-64.5). Moreover, gastric cancer stage 4 was higher in non-active than active H. pylori infection (93% vs 50%, P-value<0.001). Conclusions: Prevalence of H. pylori infection in Thai gastric cancer patients was high but active infection was low. Most gastric cancer patients presented in advance stage and had a grave prognosis. Screening for gastric cancer in high risk individuals might be an appropriate tool for early detection and improve the treatment outcome for this particular disease in Thailand.
