All-Inside Technique of Anterior Cruciate Ligament Reconstruction using Central Quadriceps Tendon and Patella Bone Block (관절강 내에서 모든 수술 과정을 시행하는 관절경적 전방십자인대 재건술)

  • The all inside anterior cruciate ligament reconstruction technique places an anterior ligament substitutes within two bony sockets rather than hone tunnel. This approach is accomplished through arthroscopic three portal which avoids the surgical exposure and morbidity associated with creating traditional bone tunnel. This technique has several distinct advantages when compared with the traditional ACL reconstruction through the bone tunnels. It offers the surgeon a less morbid method for ACL reconstruction that positions an ACL substitute at the anatomic attachment sites of the original ACL with two bone sockets, obviating the need for traditional bone tunnels. Graft fixation at or near the anatomic attachment points of the original ACL minimizes creep with early range of motion and reduces the abrasive 'wind-shield wipe' motion of the graft which occur with bone plugs positioned inside bone tunnels. The sagittal posterior angle to the tibial socket increases fixation strength to pullout with anterior translation force for the tibia on the femur. This technique is not graft specific and can accomodate any graft in which graft length can be customized to the intraarticular native ACL length.

Anatomic Analysis of Rectus Femoris Functional Flap in Korean (대퇴직근 기능성 유리 피판술을 위한 신경 및 혈관에 대한 해부학적 고찰)

  • The purpose of this article to evaluate the availability of the rectus femoris flap in Korean subjects. Material and Methods is that Cadaveric dissections were done on 51 femoral triangles of 26 cadevers. We measured the length of the direct head of rectus femoris from anterior superior iliac spine to patella upper pole, ASIS to lateral border of femoral nerve, and entry point of femoral nerve and vessel branches to rectus. Usually, there were three terminal branches to rectus femoris from the femoral nerve. The entry point of the first branch was at the proximal $17.5{\sim}31.4%$ portion of the rectus femoris. The second and the third branch entered at the proximal $22.5{\sim}40.7%$ and $26.3{\sim}42.3%$, respectively. The vessel entry was at $20.2{\sim}37.3%$. The length from ASIS to femoral nerve was $3.5{\sim}8.5\;cm$. Among the 51 rectus femoris muscles, 44 had one nutrient artery, and 7 had 2 nutrient arteries. The nutrient artery originated from the descending branch of the lateral femoral circumflex artery in 18(40.9%) cases, directly from the lateral femoral circumflex artery in 8(18.0%) cases, and from proximal(6 cases, 13.6%) and distal(12 cases, 27.3%) portion of the deep femoral artery. The average length of the nutrient artery was 29.8 mm and the width was 2.14 mm. The point where it meets the main feeding artery of the rectus femoris was $9.0{\sim}15.0\;cm$ from the ASIS. In all cases, the main artery's entrance was proximal to the first nerve branch. Conclusion is that rectus femoris has available data for functional flap.

Arthroscopic Cannulated Screw Fixation Technique for Avulsion Fracture of the Intercondylar Eminence of the Tibia (삽관 나사못(Cannulated screw)을 사용한 경골 과간 융기부 견열 골절의 관절경적 치료 기법)

  • Avulsion fractures of the intercondylar eminence of the tibia are not uncommon. In the displaced avulsion fracture, anatomical reduction and firm fixation of fracture fragments are needed but the most of the conventional operative techniques including arthroscopic technique are relatively complex and need. The results were not always satisfactory due to the risk of postoperative complications such as wound infection, premature epiphyseal closure and loss of fixation after early motion etc. So we describe a simple and safe modified method of arthroscopic reduction and fixation for avulsion fractures of the intercondylar eminence of the tibia. In our thirteen cases, we achieved anatomical reduction and secure fixation using cannulated screw through the three arthroscopic portals (anterolateral, medial mid-patellar and central). Postoperatively, immediate limited range of motion of the knee and partial weight bearing were possible. Additional use of the washer afforded safe fixation of comminuted avulsion fracture. The advantage of this technique includes its technical simplicity, easy removal of hardware, ability to treat comminuted type IV fracture with washer, no additional skin incision, no damage to growing plate in growth children and less morbidity.

Transpatellar Cannulated Screw Fixation of Displaced Tibial Intercondylar Eminence Fractures (전위성 경골 과간 융기부 골절의 슬개골 하단을 통한 삽관 나사못 고정술)

  • Purpose : To evaluate the clinical results of displaced tibial intercondylar eminence fractures which were treated with transpatellar cannulated screw fixation. Materials and Methods : Ten patients with displaced tibial intercondylar eminence fractures were treated between December 1998 and May 2001 and then followed up for more than one year. They were treated arthroscopic reduction and fixation of fracture site by cannulated screw through the hole of nonarticular surface of inferior patella. They were prospectively evaluated with regard to their clinical and radiologic results. Results : Radiologic unions occurred at an average of 9.2 weeks. Average anterior displacements were 1.8 mm in stress x-rays and 1.1 mm in KT-2000 arthrometer. Average loss of extension was $4.1^{\circ}$. Functional results were excellent in 7 cases and good in 3 cases. Conclusion : Arthroscopic transpatellar cannulated screw fixation is one of the useful methods for the treatment of displaced tibial intercondylar eminence fractures.

Patellar Tendinitis in the adult Volleyball Players (성인 배구 선수에서 발생하는 슬개건염)

  • Purpose: To estimate the prevalence and incidence of patellar tendinitis, to discover the patellar tendinitis in top level male volley ball players. Materials and Methods: Thirteen members of the male adult volleyball players were included in this study. From history and physical examination, 3 of the 13 players had patellar tendon pain associated with activity and were diagnosed as patellar tendinitis at the time of the study. Investigators were blinded about the injury status of the players. For all players, data such as age, volleyball experience, amount of training and competition hours were recorded. Results: Of the thirteen cases, two spikers complained pain on the lower pole of the patella, one setter on the upper pole. Conclusion: This findings suggest that patellar tendinitis in the volleyball players are influenced by the amount of the practice and their position. In most cases, the supervised control of the practice will reduce the overuse injuries in the volleyball players.

Percutaneous Lateral Release and Medial Reefing for Recurrent Patellar Dislocation (재발성 슬개골 탈구의 경피적 외측부 유리술 및 내측부 중첩술)

  • Purpose: This study seeks to address the results of percutaneous lateral release and medial reefing for patients with recurrent patellar dislocation. Materials and Methods: This paper focuses on 27 cases from 25 patients who had a surgical operation at our hospital in a span of eight years and six months from the time of December 1996 to May 2005. The subjects consist of 11 males and 14 females, and their average age was 22.2 years old. All the patients had their trauma history, and the average frequency of dislocation before surgery was 11.5 times. Results: The congruence angle before operation was 23.3 degree on the average, while the lateral patellofemoral angle was -5.7 degree on average. However, the congruence angle after surgery came to -2.4 degree, while the lateral patellofemoral angle recovered to the normal range of within 11.5 degrees. Four cases showed the recurrent dislocation, and two out of those four cases had -35, -12 degrees of their lateral patellofemoral angle respectively and 59, 14 degrees of their congruence angle respectively. Conclusion: Using percutaneous lateral release with medial reefing can be considered as an effective treatment for recurrent patellar dislocation

Usefulness of Ultrasound for the Loose Body in Knee (슬관절내 유리체에 대한 초음파 검사의 유용성)

  • Purpose: We wished to evaluate the clinical usefulness of ultrasound, when loose body is palpated and is shown radiographically in knee. Material and Methods: Twenty-five knees of twenty-two patients with loose body in knee radiographically from March, 2007 to July, 2009 were involved in this study, there were 8 males and 14 females, there were 11 right knees, 8 left knees and 3 both knees, the average age was 56.3 years (range, 32~76 years). The location of loose bodies were 13 cases at suprapatella pouch, 1 case at infrapatella anterior compartment and 11 cases at posterior compartment. In loose bodies of suprapatella pouch, we examined the mobility of loose bodies longitudinally with ultrasound placed at superior patella pole while flexing and extending the knee. In loose bodies of infrapatella anterior and posterior compartment, we examined the mobility of loose bodies longitudinally and transversely with ultrasound while flexing and extending the knee. Results: All cases of suprapatella pouch and infrapatella anterior compartment were diagnosed to movable loose bodies and were removed surgically, all cases of posterior compartment were diagnosed to immovable loose bodies and were not removed. Conclusion: When we determined the surgical removal according to mobility of loose body in knee, ultrasound may be helpful clinically.

A Clinicopathologic Review of Eight Cases of Chondroblastoma (연골모세포종 8례의 임상 및 병리학적 검색)

  • Eight cases of chonproblastoma were studied by analyzing the clinical and pathologic findings. The age of eight cases ranged from 17 to 38 years old(median age, 22.7 years old). The tumors developed in the femur(3 cases), patella(2 cases), tibia(1 case), fibula(l case), and ulna(1 case). The mean diameter of tumors was 4.0cm (range, 1.5 to 8.0cm). Grossly, tumors showed grayish brown solid area with foci of secondary aneurysmal bone cyst. Histologically, the tumor cells were round or polygonal in shape with nuclear groove. And there were chondroid differentiation(7 cases), mitosis(3 cases), calcific deposits(3 cases), secondary aneurysmal bone cyst(4 cases), hemosiderin deposits(4 cases), necrosis(3 cases), vascular invasion(1 caes), and foamy histiocytes and cholesterol cleft(l cases). All cases showed no metastasis to lymph node and distant organ. Seven cases (87.5%) were immunoreactive for S-100 protein. None was immunoreactive for cytokeratin.

Surgical Correction of Medial Patellar Luxation including Release of Vastus Medialis without Trochleoplasty in Small Breed Dogs: A Retrospective Review of 22 Cases

  • In 22 dogs with medial patellar luxation (MPL) of grade 3 or lower, resection of the vastus medialis oblique muscle, patellar anti-rotational suture, fascia lata overlap, and tibial tuberosity transposition (TTT) were undertaken to stabilize the patella without trochleoplasty. Data including signalment, clinical symptoms, details of the affected hindlimb, preoperative and postoperative patellar luxation grades, postoperative recovery time, and postoperative complications were obtained from medical records. The grade of lameness was evaluated preoperatively and postoperatively. Mean (${\pm}SEM$) grade of medial patellar luxation was $2.64{\pm}0.11$ preoperatively and $0.2{\pm}0.27$ postoperatively. Mean (${\pm}SEM$) grade of lameness was $1.73{\pm}0.27$ preoperatively and $0.18{\pm}0.15$ postoperatively. Patellar reluxation occurred in 1 of 22 (4.5%) cases requiring additional surgery. At final follow-up, 2 of the 22 (9.0%) dogs, including one with reluxation, had occasional lameness. Client-based questionnaire results demonstrated significant improvements in all parameters. Surgical treatment of MPL that included resection of the vastus medialis oblique without femoral trochlear groove deepening improved surgical outcomes in dogs with up to grade 3 MPL.

  • Anterior knee pain syndrome would best be defined as a painful condition that arises in or around the patellofemoral joint and is insidious in onset and bilateral, with an enigmatic entity with multiple causes. Although its etiology is uncertain, the cause is often considered to be abnormal lower limb biomechanics, pathology of extensor mechanism, disorder of patellofemoral joint, malalignment or lateral tracking of the patella, soft tissue tightness, muscle weakness. The measurement of patellar alignment has come to be accepted as an integral part of the examination of anterior knee pain syndrome. Various measurement techniques exist, both clinical and radiological, and these have been frequently used in the diagnosis and treatment of the condition.?Treatment depends on the underlying cause of anterior knee pain and should be directed to the cause rather than to the results. Most often, this involves non-surgical measures, such as anti-inflammatory medications, quadriceps exercises, and hamstring stretching. Shin splint, or medial tibial stress syndrome refers a syndrome of pain running along the inner distal 2/3 of tibia shaft. Shin splint is a common problem for athletes whose sport involves a repeated, jarring impact to the leg. A major factor determining the efficacy of the treatment is that correct diagnosis be made of the problem. The varied etiology has led to the development of several theories as to the cause, treatment, rehabilitation and prevention of shin splint. The management is rest, ice massages, pain relief by medication, and muscle strengthening exercise. Proper rehabilitation and preventative measures can ensure that there is no further recurrence.
