비체중지지자세에서의 수동적 발목 발등굽힘과 동적균형검사 수행력의 상관관계 (The Relationship Between Passive Ankle Dorsiflexion With a Non-Weight Bearing Condition and the Performance of the Dynamic Balance Test)

  • The purpose of this study was to examine the relationships between the ankle dorsiflexion passive range of motion (DF PROM) under a non-weight bearing condition and the normalized reach distance in three directions of the Y-Balance Test (YBT). Sixty-one healthy adults (32 males and 29 females, age: $23.0{\pm}3.0$ years, height: $169.3{\pm}8.9cm$, weight: $61.9{\pm}5.4kg$) participated in this study. The ankle DF PROM was measured using a goniometer. To assess dynamic balance, all subjects performed three trials to determine the maximum lower extremity reach in the anterior, posteromedial, and posterolateral directions of the YBT. The relationship between the ankle DF PROM and both the normalized reach distance in each direction and the composite score of the YBT were analyzed using the Pearson correlation. Only the normalized reach distance in the anterior direction of the YBT was significantly related to the ankle DF PROM measured under a non-weight bearing condition (r=.50, p<.001). Neither the normalized reach distances in the posterior directions nor the composite score of the YBT were significantly correlated with the ankle DF PROM measured under a non-weight bearing condition. These findings suggest that ankle DF PROM does not affect the overall dynamic balance of the lower extremity, with only the anterior dynamic balance affected among the three directions.

고리 봉합법을 이용한 심부 수지 굴건 종지부에서의 건봉합 (Loop Suture Technique for Flexor Digitorum Profundus Tendon Repair in the Insertion Site)

  • Purpose: In the case of repair for far distal parts of FDP (Flexor digitorum profundus) division, the method of either pull-out suture or fixation of tendon to the distal phalanx is preferred. In this paper, the results of a modified loop suture technique used for the complete division of FDP from both zone 1a and distal parts of zone 1b in Moiemen classification are presented. Methods: From July 2006 to July 2009, the modified loop suture technique was used for the 10 cases of FDP in complete division from zone 1a and distal parts of zone 1b, especially where insertion sites were less than 1 cm apart from a tendon of a stump. In a suture technique, a loop is applied to each distal and proximal parts of tendon respectively. Core suture of 2-strand and epitendinous suture are done with PDS 4-0. Out of 10 patients, the study was done on 6 patients who were available for the followup. The average age of the patients was 49.1 years (in the range from 26 to 67). 5 males and 1 female patients were involved in this study. There were 3 cases with zone 1a and distal parts of zone 1b. The average distance to the distal tendon end was 0.6 cm. There were 5 cases underwent microsurgical repair where both artery and nerve divided. One case of only tendon displacement was presented. The dorsal protective splint was kept for 5 weeks on average. The results of the following tests were measured: active & passive range of motion, grip strength test, key pinch and pulp pinch test. Results: The follow-up period on average was 11 months, in the range from 2 to 20 months. There was no case of re-rupture, but tenolysis was performed in 1 cases. In all 6 cases, the average active range of motion of distal interphalangeal joint was 50.8 degree. The grip strength (ipsilateral/contralateral) was measured as 88.7% and the pulp pinch test was 79.2% as those of contralateral side. Flexion contracture was presented in 2 cases (15 degree on average) and there was no quadrigia effect found. Conclusion: Despite short length of tendon from the insertion site in FDS rupture in zone 1a and distal parts of zone 1b, sufficient functional recovery could be expected with the tendon to tendon repair using the modified loop suture technique.

  • Objective : The aim of this study was to report the effectiveness of Korean medicine on a Intracerebral hemorrhage patient with shoulder subluxation induced shoulder pain. Method : The patient was treated with acupuncture, electroacupunture, soyeom pharmacopuncture, electrical stimulation therapy, and rehabilitation exercise. The treatment effect was evaluated through the Numeral Rating Scale(NRS), Passive Range Of Motion(PROM) and Fingerbreadth Palpation Method(FPM). Results : After 31days of Korean Medicine including acupuncture, electroacupunture, soyeom pharmacopuncture, electrical stimulation therapy, and rehabilitation exercise treatment, the pain parameters and range of motions improved. Conclusion : This study suggested that Korean medicine might be effective for shoulder pain and range of motions in patients with intracerebral hemorrhage.

  • Objectives : The purpose of this study is evaluate of the effect of Dong-gi Acupuncture(DGA) on rehabilitation after stroke. Methods : 32 patients who had a stroke of paralysis were randomized into two groups. 17 patients were treated with DGA on sound side combined active or passive exercise on affected side. The other 15 patients were exclusive DGA group and the method retaining needles on acupoints of affected side for about 20 minutes were applied. We evaluated by Manual muscle test, Range Of Motion(ROM), Visual Analog Scale(VAS) and Modified Barthel index(MBI) before treatment, after 10 days and after 20 days. Results : In terms of power of lower limb, active ROM of shoulder abduction and MBI, the sample group showed statistically significant improve. In terms of power of lower limb ROM of hip joint abduction and affected side pain, significant difference between two groups was not shown. Conclusions : In this clinical study, Dong-gi Acupuncture therapy can be used for improving the power of upper limb, active ROM of shoulder abduction and MBI score after hemiplegic stroke.

  • The purpose of this study was to investigate the effects of a task-oriented approach on weight-bearing distribution and muscular activities of the paretic leg during sit-to-stand movement in 18 chronic stroke patients. Both groups were received neurodevelopmental treatment for 30 min/day and then the experimental group ($n_1$=9) followed additional a task-oriented approach (sit-to stand training with controlled environment) and the control group ($n_2$=9) followed a passive range of motion exercise for 15 min/day, five days/week, for four weeks. Weight-bearing distribution and muscular activities of the paretic leg during sit-to-stand movement were measured before and after four weeks of training. There was significantly improved weight-bearing distribution of the paretic leg during sit-to-stand movement in the experimental group compared with that of the control group after four weeks of training (p<.05). But electromyographic activities of the quadriceps and the tibialis anterior of the paretic leg were not significantly different (p>.05). Thus, it is necessary to apply a task-oriented approach to improve the weight-bearing distribution of the paretic leg during sit-to-stand movement in chronic stroke patients.

견갑하근 건 파열과 동반된 상완 이두근 건 탈구 (Subscapularis Tendon Rupture with Medial Dislocation of Biceps Tendon - Case Report -)

  • Biceps tendon dislocation combined with rupture of subscapularis tendon is not a common lesion and there has been few case reported in Korea. We experienced one fifty Six years old male patient who shows typical features on physical examination and roentgenographic finding. He was injured by direct trauma on his right shoulder in adducted and external rotated position. He showed positive findings on passive external rotation test and lift-off test. On MR!, the subscpaularis tendon was totally ruptured and the biceps tendon was dislocated to anteromedial aspect of the glenoid labrum, which was typical finding. On the arthroscopic examination, the subscapularis tendon was totally ruptured from its humeral attachment and the biceps tendon was not seen in its normal anatomical position and it was dislocated antermedially to the glenoid labrum. We repaired the subscapularis tendon to humerus by use of suture anchor and the biceps tendon was relocated to its normal anatomical position in the intertubercular groove. On the post operative 6 months follow up, the patient shows improvements in his subjective symptoms and active range of motion.

Influence of Forward Head Posture on Electromyography Activity of Hyoid Muscles During Mouth Opening

  • Although the relationship between temporomandibular disorder and forward head posture (FHP) is controversial, it is generally accepted that altered head posture can affect mandible position and masticatory muscles activity. Because suprahyoid (SH) and infrahyoid (IH) muscles are stretched by increased passive tension in FHP, this study investigated their activity during mouth opening in FHP compared to neutral head posture (NHP). Twenty healthy subjects (10 males and 10 females) participated in this study. Head postures were evaluated with a cervical range of motion instrument. Electromyography (EMG) activity of bilateral SH and IH muscles was measured while an open mouth was maintained at each head posture. Paired t-test was used to identify significant differences in normalized EMG activity between head postures. Statistical significance was set at .01. Results showed the normalized EMG activity of SH and IH muscles were significantly lower in FHP compared to NHP. This finding indicates that FHP affects the EMG activity of hyoid muscles when they are stretched.

  • Rehabilitation treatments after orthopedic surgery promote postoperative healing in humans. In veterinary medicine, there is increased interest in rehabilitation treatments because they are potentially beneficial to animals in the post-operation period. This study examined the effectiveness of rehabilitation treatment in dogs that underwent orthopedic surgeries, including femoral head & neck ostectomy (FHNO) and medial patellar luxation treatment (MPLT). The group that received the rehabilitation treatment after FHNO (1.88 weeks) showed a significantly shorter recovery length of 6.62 weeks compared to those that did not receive the treatment (8.50 weeks). The other group that received the rehabilitation treatment after MPLT (4.38 weeks) showed a significantly shorter recovery length of 5.01 weeks compared to those that did not receive the treatment (9.39 weeks). For the qualitative evaluation, the types and frequencies of rehabilitation treatments were monitored. The rehabilitation programs used frequently were heat therapy, laser therapy, passive range of motion, exercise therapy, and aquatic therapy. A standard rehabilitation program after hindlimb surgery was suggested based on the quantitative and qualitative investigation. The results add additional evidence that showed the beneficial effects of rehabilitation treatments in dogs.

  • Hemiplegic shoulder pain(HSP) is one of the most frequent and difficult problems affecting poststroke hemiplegic patients. Honeybee venom acupuncture therapy(BVAT) is known for its pain relieving effects in arthralgia. To evaluate the effectiveness of BVAT on HSP, 24 patients were sequential1y allocated into BVA T treatment group and control group and monitored for 4 weeks at time interval of initial(T0), 1 week(T1), 2 weeks(T2) and 4 weeks(T4). In treatment group, 1:10000 honeybee venom solution 0.2㎖ was injected into acupoint(s) following Deqi three times a week. Kyonu(LI15) was used in the first week. Thereafter Kyonu(LI15) and Nosu(SI10) were used. Visual analogue scale of pain severity showed significant decrease in treatment group compared to control group at T2 and T4 evaluation. Painless passive range of motion of shoulder external rotation showed significant increase in treatment group compared to control group at T4 evaluation. Fugl-Meyer Motor Assessment of upper limb motor function and Modified Ashworth scale of the spasticity of upper limb showed no difference between two groups. BVAT showed as an effective therapy in HSP and further extensive clinical studies are expected.

Development of a Tele-Rehabilitation System for Outcome Evaluation of Physical Therapy

  • This paper presents a portable tele-assessment system designed for remote evaluation of the hypertonic elbow joint of neurologically impaired patients. A patient's upper limb was securely strapped to a portable limb-stretching device which is connected through Internet to a portable haptic device by which a clinician remotely moved the patient's elbow joint and felt the resistance from the patient. Elbow flexion angle and joint torques were measured from both master and slave devices and bilaterally fed back to their counterparts. In order to overcome problems associated with the network latency, two different tele-operation schemes were proposed depending on relative speed of tasks compared to the amount of time delay. For slow movement tasks, the bilateral tele-operation was achieved in real-time by designing control architectures after causality analysis. For fast movement tasks, we used a semi-real-time tele-operation scheme which provided the clinicians with stable and transparent feeling. The tele-assessment system was verified experimentally on patients with stroke. The devices were made portable and low cost, which makes it potentially more accessible to patients in remote areas.
