Intentional partial odontectomy-a long-term follow-up study

  • Background: The surgical extraction of the third molar is the most frequently encountered procedure in oral and maxillofacial surgery and is related with a variety of complications. This study examined the efficacy of intentional partial odontectomy (IPO) in the third molars which have no periapical lesions and are located near important anatomical structures such as inferior alveolar nerve. Methods: Seven patients (four males, three females, $39.1{\pm}11.6years$), who received IPO to reduce the risk of inferior alveolar nerve injury (IANI), were followed long-term. The treated teeth were horizontally impacted third molars in the mandibular left (n = 5) or mandibular right (n = 4) areas and were all ankylosed with the surrounding alveolar bone. During the IPO, the bone around the crown was removed to expose the crown, and then the tooth was resected at cement-enamel junction (CEJ). Any secondary trauma to the healthy root was minimized and remained intact after primary suture. Results: The mean follow-up time was $63.2{\pm}29.8months$, and all sites showed good bone healing after the crown removal. Also, sensory abnormality was not found in any patients after IPO. In one patient, the bone fragments erupted 4 months after IPO. In other patient, an implant placed on second molar site adjacent to the third molar that received IPO was explanted about 2 years after the patient's persistent discomfort. Conclusions: In case where high risk of IANI exists, IPO may be chosen alternatively to surgical extraction to reduce the risk of nerve damage.

Injury of submandibular gland and lingual nerve as complication third molar tooth extraction in mandible : a case report (하악 제3대구치 발치 시 합병증으로 발생할 수 있는 악하선과 설신경 손상: 증례보고)

  • The extraction of an impacted third molar tooth is associated with many complications during the procedure and postoperative care. These complications include bleeding, swelling, pain, infection, as well as root fracture, proximal tooth injury, alveolar bone fracture, lingual nerve and inferior alveolar nerve injury etc. With the exception of a fractured root dislocation in the submandibular space, no direct submandibular gland injury related to extraction surgery has been reported until now. A 40 year old man visited the department of oromaxillofacial surgery at Yeouido St. Mary's Hospital for an extraction of the right mandible third molar. A partial third molar impaction was diagnosed by a clinical and radiographic examination. A surgical tooth extraction was practiced including buccal cortical bone osteotomy. During socket curettage, an encapsulated cyst-like lesion and a verified $3{\times}3\;cm$ neoplasm in the apically lingual direction were found during process of dissection. A biopsy confirmed that the neoplasm involved the submandibular gland and nerve trunk. This unusual anatomical organ injury during the surgical tooth extraction procedure is reported as a new complication during impacted third molar extraction.

  • When teeth are extracted, patient face social, psychological and aesthetic problems which can be minimized by fabricating a interim immediate denture. Interim immediate denture manufactured using digital technology can be completed with reduced number of patients' visits and simple laboratory process. Implant-supported removable partial denture (ISRPD) has been suggested as alternative treatment option when fixed implant prosthesis is not feasible. In this case, interim immediate dentures were fabricated using digital technology for patient after teeth extraction and treatment using ISRPD by installing implants and surveyed crowns is found to be successful with better support, stability and maintenance of removable partial dentures.

  • We propose a multi-strategic concept-spotting approach for robust spoken language understanding of conversational Korean in a hostile recognition environment such as in-car navigation and telebanking services. Our concept-spotting method adopts a partial semantic understanding strategy within a given specific domain since the method tries to directly extract predefined meaning representation slot values from spoken language inputs. In spite of partial understanding, we can efficiently acquire the necessary information to compose interesting applications because the meaning representation slots are properly designed for specific domain-oriented understanding tasks. We also propose a multi-strategic method based on this concept-spotting approach such as a voting method. We present experiments conducted to verify the feasibility of these methods using a variety of spoken Korean data.

  • The shape of an object plays a very important role in pattern analysis and classification. Roughly, the researches on this topic can be classified into three fields, i.e. (i) edge detection, (ii) dominant points extraction, and (iii) shape recognition and classification. Many works have been done in these three fields. However, it is very seldom to see the research that discusses the connection relationship of objects. This problem is very important in robot assembly systems. Therefore, here we focus on this problem and discuss how to recover the connection relationship of planar objects. Our method is based on the partial curve identification algorithm. The experiment results show the efficiency and validity of this method.

  • This paper proposes a novel electrical characterization approach for a high-performance package system using an improved Partial Element Equivalent Circuit (PEEC). As the effect of interconnects becomes a pivotal factor for the performance of high-speed electronic systems, there is a great demand for an accurate equivalent model for interconnects. In particular, an equivalent model of interconnects is established in this paper for the Fine-Pitch Ball Grid Array (FBGA) package using the improved PEEC method. Based on the equivalent model, electrical characteristics are analyzed; furthermore, these are verified through the measurement results of a Vector Network Analyzer (VNA).

  • Presurgical prosthetic treatment planning is critical for the success of the implant prosthesis. Inadequate treatment plan, due to insufficient discussion between prosthodontist, and surgeon, may result in poor prognosis. A 26-year-old male patient was referred for prosthodontic treatment after implant was placed in the area of teeth #17,16, 22, 25 and 27, without adequate discussion nor the treatment planning between oral surgeon and prosthodontist. It was found that the patient had two hopeless teeth, and a severely resorbed alveolar ridge. Additional tooth extraction was needed and the type of definitive prosthesis was shifted from fixed type to removable one. Proper pre-surgical treatment planning is essential for the good prognosis. Implant-supported removable prosthesis on milled bars may be a useful treatment option in patients with incorrect angled placement on severely resorbed alveolar ridge.

  • The direct acid leaching of domestic Marmatite concentrate at elevated temperatures and pressures was investigated. Almost 100 percent of zinc was extracted from the concentrate liberating free sulfur in 4 hours at $100^{\circ}C$ when the oxygen partial pressure was 5 atm. in sulfuric acid solution. By applying the Arrhenius equation to leach reaction in the range of $60^{circ}$ to $100^{\circ}C$ at the same oxygen partial pressure, 15.7 kcal per mole of activation energy was calculated. At the initial stage of leaching, the rate of reaction increased linearly by increasing temperature and pressure. The concentration of sulfuric acid gave minor effect to leaching velocity in the range of 5 to 20 percent. The particle size should be under 270 mesh for 100 percent extraction of zinc.

Flashover Prediction of Polymeric Insulators Using PD Signal Time-Frequency Analysis and BPA Neural Network Technique

  • Flashover of power transmission line insulators is a major threat to the reliable operation of power system. This paper deals with the flashover prediction of polymeric insulators used in power transmission line applications using the novel condition monitoring technique developed by PD signal time-frequency map and neural network technique. Laboratory experiments on polymeric insulators were carried out as per IEC 60507 under AC voltage, at different humidity and contamination levels using NaCl as a contaminant. Partial discharge signals were acquired using advanced ultra wide band detection system. Salient features from the Time-Frequency map and PRPD pattern at different pollution levels were extracted. The flashover prediction of polymeric insulators was automated using artificial neural network (ANN) with back propagation algorithm (BPA). From the results, it can be speculated that PD signal feature extraction along with back propagation classification is a well suited technique to predict flashover of polymeric insulators.

Extraction and Characteristics of Anti-obesity Lipase Inhibitor from Phellinus linteus

  • To develop a potent anti-obesity lipase inhibitor from mushroom, the lipase inhibitory activities of various mushroom extracts were determined. Methanol extracts from Phellinus linteus fruiting body exhibited the highest lipase inhibitory activity (72.8%). The inhibitor was maximally extracted by treatment of a P. linteus fruiting body with 80% methanol at $40^{\circ}C$ for 24 hr. After partial purification by systematic solvent extraction, the inhibitor was stable in the range of $40\sim80^{\circ}C$ and pH 2.0~9.0. In addition to lipase inhibitory activity, the inhibitor showed 59.4% of superoxide dismutase-like activity and 56.3% of acetylcholinesterase inhibitory activity.
