A Case Report of Symptom Improvement after lobectomy in Male Breast Cancer with Lung Metastasis Patient Treated with Korean Medicine based Integrative Cancer Treatment (남성 유방암 폐전이 환자의 폐절제술 후 한의기반 통합암치료로 증상 호전에 대한 증례보고)

  • Objective: The purpose of this study is to report improvement of symptoms after lobectomy of male breast cancer lung metastasis treated with Korean Medicine based Integrative Cancer Treatment (ICT). Methods: A male left breast cancer patient diagnosed with metastasis on lung at July 2019. After Video assisted thoracic surgery (VATS) left lower lobe (LLL) lobectomy and En bloc wedge resection the patient visited the Daejeon korean medicine hospital of Daejeon university East West Cancer Center (EWCC) to treat operation-site (op-site) pain, dysphagia, anorexia with Korean Medicine Treatment. The patient was treated with Korean Medicine based ICT for an approximately 20 days. The clinical outcomes were measured by National Cancer Institute Common Terminology Criteria for Adverse Event (NCI-CTCAE), Numeral rating scale (NRS) and Eastern Cooperative Oncology Group (ECOG). The safety of treatment was verified by blood tests. Results: After treatment, op-site pain was improved from NRS 9 to 6, dysphagia and anorexia were relieved from NRS 9 to 2. And ECOG score of the patient was improved from grade 2 to 1. Conclusion: This case study suggests that Korean Medicine based ICT may help to improve post operative sequelae in metastatic lung cancer patient.

A Case Report of Pancreatic Cancer with Liver Metastasis Patient Treated with Integrative Cancer Treatment (췌장암 간전이 환자의 통합 암 치료에 대한 증례보고)

  • Objective: The purpose of this study is to report effects and potential of Integrative Cancer Treatment (ICT) on metastatic pancreatic cancer patient. Methods: A 79-year-old pancreatic cancer patient diagnosed with metastasis on liver visited the Daejeon Korean medicine hospital of Daejeon university East West Cancer Center (EWCC) on May 2021. The patient has been received chemotherapy (gemcitabine plus abraxane) and concurrently treated with ICT since May 2021. The clinical outcomes were measured by computed tomography, laboratory findings including tumor markers (CEA, CA19-9) and numeric rating scales (NRS). Laboratory analysis and National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE), version 5.0 were used to evaluate the safety of ICT. Results: After treatment, constipation was relieved from NRS 5-6 to 2, both leg numbness was improved from NRS 9 to 2. Tumor size was generally decreased accompanying by reducing the levels of tumor markers. There were no severe adverse events induced by ICT based on NCI CTCAE version 5.0. Conclusion: This case study suggests that ICT in combination with chemotherapy may help in the treatment of patients with metastatic pancreatic cancer.

  • The small and medium sized dams have the fill dam type of a lot of occasions, which are often weak in cases of major floods. For this reason, although a countermeasure is in great need, due to the importance of the facilities and financial situations, no direct safety measures have been taken. In this study, in order to minimize construction expenditure for practical safety measures in cases of major floods, the overflow section of spillway has been analyzed focusing on how the overflow capacity will increase in the case of partially rebuilding a part of the overflow section of spillway favorable for hydraulic conditions. The Labyrinth weir and movable weir was chosen for reconstruction models of the overflow section. Moreover, for analyzing the after-effects of the reconstruction, a small scale dam was temporarily chosen for various experiments such as the hydraulic model testing and the three dimension numerical evaluation through the use of Flow-3D.

A Prospective and Open-Label Trial of Quetiapine and Haloperidol in the Treatment of Delirium (섬망의 치료에 대한 Quetiapine과 Haloperidol의 전향적 개방형 연구)

  • Objectives: This prospective and open-label study was conducted to evaluate the efficacy and safety of quetiapine and haloperidol in patients with delirium. Methods : Fourty patients(19 patients in a quetiapine group : 21 patients in a haloperidol group) with delirium by DSM-IV were treated with flexible doses of open-label qvetiapine and haloperidol. To evaluate the primary efficacy of the medication, scores from the Korean version of Delirium Rating Scale(K-DRS) were assessed every seven days and to evaluate the secondary efficacy and safety, scores from the Clinical Global Impression-Severity, Korean Version of Mini-Mental State Examination, and the Drug-Induced Extrapyramidal Symptoms Scale were assessed at the baseline and the seventh day. Data were gathered from November 2004 to June 2005. Results : K-DRS scores for each group decreased significantly over the study period; however, no significant differences between groups were found. The group-by-time effect was not significant. In addition, there was no significant difference in the frequency of response to drugs between the two groups. No patients reported clinically significant side effects. Conclusion : These data show no significant difference in the efficacy and safety between quetiapine and haloperidol in the treatment of delirium. Since haloperidol has a great possibility of causing a extrapyramidal side effect resulted by previous studies, it is expected that quetiapine, a renowned medication with low side effects, may be a useful alternative agent to haloperidol, the classical antipsy-chotics, in the treatment of delirium.

  • Objectives : The purpose of this study is to evaluate the feasibility of massive clinical research and to make a basic analysis on the effectiveness and safety of moxibustion treatment on knee osteoarthritis compared to usual care. Methods and Results : This study is a protocol for a pilot randomized controlled trial. Forty participants are assigned to the moxibustion group (n=20) and usual care group (n=20). Participants assigned to the moxibustion group receive moxibustion treatment on the affected knee(s) at six standard acupuncture points (ST36, ST35, ST34, SP9, Ex-LE04 and SP10) three times per week for four weeks (total of 12 sessions). Participants in the usual care group don't receive moxibustion treatment during the study period and follow-up are made on the 5th, 9th and 13th weeks after random allocation. Both groups are allowed to use any kind of treatment, including surgery, conventional medication, physical treatment, acupuncture, herbal medicine, over-the-counter drugs and other active treatments. Education material that explains knee osteoarthritis and current management options and self-exercise is provided for each group. The pain scale of the Korean Western Ontario and McMaster Universities Questionnaire (K-WOMAC) is the primary outcome measurement used in this study. Other subscales of the K-WOMAC, the Short-Form 36 Health Survey (SF-36), Beck Depression Inventory (BDI), Physical Function test, Patient Global Assessment, and Pain Numeric Rating Scale (NRS) are used as outcome variables to evaluate the effectiveness of acupuncture. Safety is assessed at every visit. Conclusions : The result of this trial will provide a basis for the effectiveness and safety of acupuncture treatment for knee osteoarthritis.

Evaluating the Usability of Medical Body Wrap in Whole Body Bone Scan (전신 뼈 검사에서 의료용 신체 고정구의 유용성 평가)

  • Purpose: When performing nuclear medicine examinations, body wraps or plastic supports are used to support and immobilize the patient's upper extremities to prevent patient safety accidents. However, the existing plastic supports compromised patient and staff safety, including finger entrapment and falls. Moreover, the body wrap provided by manufacturers compromised image quality such as upper extremities cutoff during whole body bone scan. Therefore, a new design of body wrap was developed to improve the issue, and this study aims to evaluate the usability of this medical body wrap. Materials and Methods: To evaluate the usability of the newly designed medical body wrap, a quality assessment of whole body bone scan images and a user satisfaction survey were conducted. Adult patients (male:female=129:152, age: 60.3±12.4 years, BMI: 24.0±4.2) aged 16 years or older who underwent a whole body bone scan during two periods: June to July 2022 (before improvement, n=139) and June to July 2023 (after improvement, n=142) were randomly selected for image quality evaluation. Five radiotechnologists visually evaluated the posterior view of the whole body bone image, including the left and right elbow (2 points), arm (2 points), whether the hand is extended (2 points), whether the hand is included (2 points), and the number of visible fingers (10 points), with a total of 18 points, which were converted to 100 points and analyzed for difference before and after improvement using an independent sample t-test. The user satisfaction questionnaire was evaluated using a 5-point Likert scale among 16 radiotechnologists from three general hospitals who experienced the new body wrap. Results: The image quality assessment was 82.0±13.8 before the improvement and 89.3±10.1 after the improvement, an average of 7.3 points higher, with a statistically significant difference (t=5.02, p<0.01). The user satisfaction survey showed an overall satisfaction rating of 4.1±0.8 for ease of use, 3.8±0.7 for scan preparation time, 3.9±0.7 for patient safety, 3.8±1.2 for scan accuracy, and 4.2±0.7 for recommendation (87.5% questionnaire response rate). Conclusion: The developed body wrap showed higher image quality and user satisfaction compared to the old method. Considering these results, it is deemed that the new body wrap may be more useful than existing methods.

AN OBSERVATIONAL MULTI-CENTER STUDY FOR EVALUATION OF EFFICACY, SAFETY AND PARENTAL SATISFACTION OF METHYLPHENIDATE-OROS IN CHILDREN WITH ADHD (주의력결핍과잉운동장애 아동에게 Methylphenidate-OROS 투여시 효용성과 안전성 및 부모 만족도를 평가하기 위한 다기관관찰연구)

  • Objectives : This study was performed to evaluate the efficacy and safety of MPH-OROS and parental satisfaction for treatment of children with ADHD Method : The 569 participants were clinically diagnosed for ADHD using DSM-IV criteria. We switched current medication to MPH-OROS or introduced MPH-OROS for treatment of ADHD. We assessed the clinical global impression severity of illness (CGI-S), the clinical global impression severity of improvement(CGI-I). And the parents of participants measured the Korean version of Conners rating scale at baseline, the 1 st week and the 3rd week after MPH-OROS trial. At the 3rd week, the parents measured the parent satisfaction questionnaire. Results : $13\%$ of participants dropped out because of several causes including side effects. The change of CGI-S was significantly decreased. Using CGI-I, the improvement was $72.3\%$ at the 1st week and $87.4\%$ at the 3rd week. The total score of the Korean version of Conners parent rating scale was significantly decreased. The participants complaining one or more of side effects were 119$(20.7\%)$, and the most common side effect was anorexia. The $94\%$ of parents replied that they were overall satisfied with MPH-OROS trial. Also the advantages of MPH-OROS of parental report were the long duration of the drug, the improvement of schoolwork and attitude, the improvement of home behavior and homework and the improvement of overactivity. Conclusion : MPH-OROS is effective and well-tolerated in actual clinical use for ADHD.

  • This paper is intended to show some new approaches and concepts for the development of system model and prototype software for computer-aided Monitoring and Assessment(M&A) of grand bridges. The system model that incorporates optimal strategies for M&A of grand bridges is suggested. Reliability models are developed and a reliability-based capacity rating method is established for the evaluation of the bridge safety and reliability based on the real responses measured from a series of field load tests. Based on the proposed models, an integrated prototype software is then developed for computer-aided M&A of grand bridges under the environment of the graphic user interface, which is successfully applied to the M&A of an existing cable-stayed bridge. It may be stated that the system model and prototype software developed in this paper can be utilized and implemented in the development of the computer-aided M&A system for cable-stayed and suspension bridges.

Electroacupuncture for Treatment of Primary Dysmenorrhea: Systematic Review of Randomized Controlled Trials (원발성 월경통의 전침 치료 연구에 관한 체계적 문헌 고찰)

  • Objectives: This review aims to report the efficacy and safety of electroacupuncture in the treatment of primary dysmenorrhea. Methods: We searched for randomized controlled studies (RCTs) using electroacupuncture on primary dysmenorrhea up to Dec 2016. The following databases were searched : OASIS, PubMED, EMBASE, CENTRAL. The results of the studies were analyzed and the risk of bias was assessed. Results: Five studies were included in this review. In most studies, the effect of electroacupuncture at Sameumgyo (SP6) was better than that of electroacupuncture at Hyeonjong (GB39), nonacupoint and no intervention about Visual analog scale (VAS), Verbal rating scale (VRS) and Retrospective symptom scale (RSS). But there were no significant effects on Resistance Index (RI) and S/D ratio (the ratio between peak systolic to end-diastolic flow velocity in uterine arteries). Serous adverse events were not reported. Conclusions: This review shows that electroacupuncture is effective on primary dysmenorrhea with no serious adverse events.

Analysis of Recent Trends in Clinical Research Publications on Acupuncture and Moxibustion Studies (침구 임상연구 논문동향 분석)

  • Objective : This study was aimed to review the trend of clinical research articles of acupuncture and moxibustion from the year of 1995 to 2006. Methods : The retrieving and analyzing period were from January 1995 to July 2006. Articles were collected by retrieving Pubmed database. Search terms were as follows; 'Acupuncture', 'Acupuncture & Meridian', 'Moxibustion'. After reviewing articles by our pre-defined criteria, Correlation was assessed qualitatively between contents of the final selected articles and those of Korea Institute of Oriental Medicine. Results : The number of acupuncture research articles have increased with higher rate since the year of 2000. In the field of medical devices, the articles were sparsely existed. The percentage of clinical acupuncture research articles was 75%. However, in the category of Korean investigators' researches, it was 46%. Publications by Korean investigators were concentrated to several specific journals of which their impact factor were below 1 point. Conclusion : There needs more qualifying studies in acupuncture studies which are represented by impact factor in Journal Citation Reports. It is necessary to research and develop the medical devices quantatively rating the outcome of acupuncture trials. The secondary research, such as, systematic reviews, meta-analysis, would be a key approach to evaluate the effectiveness and safety of acupuncture treatments. Moxibustion research would be noticeable in regard to its historical effectiveness and its variable indications.
