함치성 낭종과 관련된 미맹출 영구치의 감압술을 이용한 정상 맹출유도 : 증례보고 (NORMAL ERUPTION GUIDANCE OF UNERUPTED PERMANENT TEETH ASSOCIATED WITH DENTIGEROUS CYST BY DECOMPRESSION : 5 CASES REPORT)

  • The purpose of this report is to describe a treatment method of dentigerous cyst associated with unerupted permanent teeth in mixed dentition patients. In our cases, extraction of infected primary teeth was followed by decompression of the cyst. At the same time, parts of the cystic walls were sent for histopathological examination. Decompression was performed by inserting a rubber tube into the cystic cavity through the extraction socket. The cystic cavity was kept open by means of vigorous use of a syringe by patient. Postoperative panoramic radiograph was taken bimonthly. After $5{\sim}12$ months, the impacted permanent teeth were erupted on the desired position. All cases presented favorable result. By extracting the infected primary teeth, and opening the cyst for continuous drainage, it was possible to achieve spontaneous eruption of the involved permanent teeth into the proper position. In all our cases, there was no sign or symptom of recurrence of the cyst up to postoperative 18 months.

Orthodontic treatment of a transposed maxillary canine and first premolar in a young patient with Class III malocclusion

  • A 12-year-old girl was referred to our clinic for evaluation of an unaesthetic dental appearance. All permanent teeth were erupted, while the deciduous maxillary right canine was retained. Cone-beam computed tomography revealed a complete transposition of the maxillary left canine and first premolar involving both the crowns and the roots. Initial cephalometric analysis showed a skeletal Class III pattern, with a slight maxillary retrusion and a compensated proclination of the upper incisors. The patient's teeth were considered to be in the correct position; therefore, we decided to attempt treatment by correcting the transposition and using only orthodontic compensation of the skeletal Class III malocclusion. After 25 months of active orthodontic treatment, the patient had a Class I molar and canine relationship on both sides, with ideal overbite and overjet values. Her profile was improved, her lips were competent, and cephalometric evaluation showed acceptable maxillary and mandibular incisor inclinations. The final panoramic radiograph showed that good root parallelism was achieved. Two-year follow-up intraoral photography showed stable results.

하악에 발생된 법랑모세포 암종 (Ameloblastic carcinoma of the mandible)

  • The ameloblastic carcinoma is an extremely rare, aggressive odontogenic neoplasm of the jaws. It is described as an ameloblastoma in which there is histologic evidence of malignancy in the primary or recurrent tumors, regardless of whether it has metastasized. We report an aggressive case of ameloblastic carcinoma of the mandible. A 68-year-old man with the complaint of the left facial swelling and intermittent pain was referred to our hospital. Serial images of panoramic radiograph, computed tomograph, and magnetic resonance imaging showed an ill-defined destructive radiolucent lesion of the left mandible. The lesion had typically aggressive behavior with extensive local destruction of bone and extended to the adjacent soft tissues. Bone scan revealed increased uptakes in the left mandibular body and ramus regions. Histological features were generally resembled with those of an ameloblastoma but with cytologic features of epithelial malignancy.

상악동에 발생한 aspergilloma에 대한 증례보고 (Aspergilloma of the maxillary sinus: report of a case)

  • Aspergilloma of the paranasal sinus is a non-invasive form of aspergillosis, most often in the maxillary sinus. This case presents an 86-year-old female with aspergilloma of the left maxillary sinus. The patient's chief complaint was intermittent pain on the left maxillary first premolar area. A radiopacification of the left maxillary sinus was observed on the panoramic radiograph. Cone-beam computed tomography revealed complete radiopacification of the left maxillary sinus and scattered multiple radiopaque mass inside the lesion. Biopsy was performed under local anesthesia. On microscopic examination, numerous fungal hyphae, which branch at acute angle, were observed. The diagnosis was made as an aspergilloma based on the histopatholgic examination.

  • Aneurysmal bone cyst (ABC) is relatively rare, non-neoplastic expansile lesion of bone. The case of a IS-year-old male with a ABC of the left mandibular condyle is presented. Panoramic radiograph showed a unilocular radiolucency with thinned coritces and a subcondylar fracuture which was due to the trauma. Computed tomography (CT) revealed expansile lesion which had similar attenuation soft tissue. The patient was treated surgically including iliac crestal bone graft.

  • Osteopathia striata with cranial sclerosis (OS-CS) is a bone dysplasia characterized by a linear striated pattern of sclerosis, especially in the long bones, and cranial sclerosis. It has variable clinical findings but distinctive radiological findings. Multiple oral and dental findings have been associated with this disease and can be seen during dental and/or medical imaging of the head and neck. Dentists and clinicians must be familiar with these signs to differentiate them from pathosis or erroneous radiographs. In the following case, we present a patient with OS-CS that presented at The University of Florida College of Dentistry with multiple craniofacial manifestations of this syndrome that were seen on a panoramic radiograph, which is one of the most commonly requested radiographs by dentists.

  • Hemangioma is a benign tumor characterized by the proliferation of blood vessels. Although it often appears in soft tissues, its occurrence in bone tissue, particularly the mandible, is extremely rare. A 32-year-old female sought attention at the dental clinic complaining of a painless swelling in the posterior region of the left side of the mandible. A panoramic radiograph and computed axial tomography scan were taken, showing honeycomb and sunburst images, respectively, in the affected area. The patient underwent a biopsy, which led to the diagnosis of intraosseous hemangioma. Having assessed the characteristics of the lesion, it was decided to perform complete excision including safety margins, followed by an iliac crest bone graft to reconstruct the mandible. Awareness of the possible clinical and radiographic presentations of intraosseous hemangioma is considered important, as non-diagnosis could have severe consequences given its possible relation to dental structures.

Giant lipoma of the tongue: A case report and review of the literature

  • This report presents the case of a 49-year-old man who presented with giant masses that had recently grown on the bilateral sides of the tongue. A clinical examination revealed rubbery yellowish lesions protruding from the tongue. A panoramic radiograph showed an enlarged soft tissue shadow of the tongue. Computed tomography showed well-defined circumscribed mass exhibiting a homogeneous low density on the bilateral sides of the tongue. On magnetic resonance images, the masses showed a high signal intensity on T1-weighted images and iso-signal intensity with partially hyperintense margin on fat-suppressed T2-weighted images. Surgical excision was performed, and a histopathologic examination confirmed the diagnosis of lipoma. The patient recovered well with no sign of recurrence. A giant lipoma is defined as a lipoma larger than 5 cm in diameter. A literature review of giant lipomas of the tongue is also presented herein.

두부 위치 변화에 따른 파노라마 방사선사진상의 치축각도의 왜곡 (Distortion of tooth axes on panoramic radiographs taken at various head positions)

  • 본 연구는 파노라마 방사선사진에서 두부 위치 변화에 따라 인접 치아장축 사이에 이루는 각도가 어떻게 변화되는지 알아보기 위해 시행하였다. 정상교합자의 모델을 기초로 하여 3차원 solid 모델을 구성하고 이를 신속 조형모델(rapid prototyping, RP)로 제작하였다. 다음 9가지 두부위치들을 5주 동안 일주일 간격으로 재위치시키고 촬영하였다: ideal head position, $5^{\circ}$, $10^{\circ}$ up, $5^{\circ}$, $10^{\circ}$ down, $5^{\circ}$, $10^{\circ}$ right up, and $5^{\circ}$, $10^{\circ}$ right rotation. 파노라마 방사선사진 촬영은 디지털 방사선 촬영기(Planmeca ProMax)를 이용하였고 모니터상에서 직접 인접 치아장축을 표시하고 그 사이 각도를 계측하였다. 그 결과 이상적인 두부 위치와 다양한 두부 위치 변화에 따라 촬영한 파노라마 방사선사진 사이에 인접 치아장축 사이의 각도가 대부분 유의한 차이를 보였다. 수평축(X축)을 중심으로 고개를 든 경우, 각 치아들의 장축이 교합평면을 향하여 수렴하는 경향을 보였고 고개를 숙인 경우에는 치근단측을 향하여 수렴하는 경향을 보였으며, 이는 특히 전치부에서 가장 큰 차이를 보였다. 전후방축(Y축)을 중심으로 좌우측으로 고개를 기울인 경우, 교합평면이 올라간 쪽의 치아장축은 치근단측을 향하여 수렴하는 경향을 보였고 교합평면이 내려간 쪽은 교합평면을 향하여 수렴하는 경향을 보였다. 수직축(Z축)을 중심으로 고개를 좌우로 회전시킬 때, 고개를 돌린 쪽의 측절치와 견치는 치근단측을 향하여 수렴하고 반대측의 측절치와 견치는 교합평면을 향하여 수렴하였다. 이상의 결과들로 볼 때 파노라마 방사선사진 촬영 시 두부의 위치 변화에 따라 치근 평행도가 달라지기 때문에 환자를 올바르게 위치시키는 것이 중요하며, 교정 환자에서 파노라마 방사선사진을 이용하여 치근 평행도를 평가할 때 교합평면의 경사(전후방, 좌우측방)나 악궁의 비대칭 등이 반드시 고려되어야 할 것이다.

쇄골두개 이형성증 환아의 치아발육이상 (THE ANOMALIES OF PERMANENT DENTITION IN CLEIDOCRANIAL DYSPLASIA)

  • 쇄골두개 이형성증(Cleidocranial Dysplasia)은 autosomal dominant skeletal dysplasia로 쇄골의 부재 두개골 봉합지연 및 치아이상의 특징을 갖는 질환이다. 치아이상 중에 유치열 발달은 정상인데 반해 영구치 맹출 실패가 가장 특징적인 소견이며 다수의 과잉치의 존재, 치아형태이상과 치근형태이상 등이 존재한다. 영구치의 지연 혹은 맹출 부전의 원인으로 (1) 다수의 과잉치 존재, (2) cellular cementum이 없는 기형적 치근형태, (3) 악골의 높은 골밀도, (4) 유치와 골의 비정상적 흡수를 들 수 있으며 저하된 골대사가주원인이며 두번째로 다수의 과잉치의 존재를 들 수 있다. 이의 치료방법은 더 많은 치아 맹출의 환경조성을 위한 교정치료, 외과적 처치 및 보철적 수복이 필요하다. 본 증례는 쇄골두개 이형성증으로 진단받은 7세 10개월 여 환아로 영구치 미맹출을 주소로 본원에 내원하였다. 임상구강 검사에서 유치의 만기잔존, 악궁의 협착, 전치부 반대교합과 다수의 치아우식증이 관찰되었으며 악골의 방사선사진에서 유치의 만기잔존 및 상하악에 다수의 과잉치가 관찰되었고 두부방사선사진에서 미폐쇄된 봉합과 봉간골(wormian bone)이 관찰되었으며 흥부방사선사진에서 쇄골의 부재가 보였다. 이 환아의 성장에 따른 치열 발달을 이해하고 시기에 따른 적절한 치과적 처치에 대해서 본 증례에서 보고하는 바이다.
