Evaluation of the clinical efficacy of a TW3-based fully automated bone age assessment system using deep neural networks

  • Purpose: The aim of this study was to evaluate the clinical efficacy of a Tanner-Whitehouse 3 (TW3)-based fully automated bone age assessment system on hand-wrist radiographs of Korean children and adolescents. Materials and Methods: Hand-wrist radiographs of 80 subjects (40 boys and 40 girls, 7-15 years of age) were collected. The clinical efficacy was evaluated by comparing the bone ages that were determined using the system with those from the reference standard produced by 2 oral and maxillofacial radiologists. Comparisons were conducted using the paired t-test and simple regression analysis. Results: The bone ages estimated with this bone age assessment system were not significantly different from those obtained with the reference standard (P>0.05) and satisfied the equivalence criterion of 0.6 years within the 95% confidence interval (-0.07 to 0.22), demonstrating excellent performance of the system. Similarly, in the comparisons of gender subgroups, no significant difference in bone age between the values produced by the system and the reference standard was observed (P>0.05 for both boys and girls). The determination coefficients obtained via regression analysis were 0.962, 0.945, and 0.952 for boys, girls, and overall, respectively (P=0.000); hence, the radiologist-determined bone ages and the system-determined bone ages were strongly correlated. Conclusion: This TW3-based system can be effectively used for bone age assessment based on hand-wrist radiographs of Korean children and adolescents.

Comparison of Muscle Strength for Women with Osteoarthritis after 8-week Tai-Chi Exercise and Aquatic Exercise (수중운동과 타이치운동 후 여성 골관절염 환자의 근력변화)

  • Arthritis is one of the most common chronic degenerative joint disease in elderly. Osteoarthritis is a widespread, slowly developing disease, with a high prevalence increasing with age in women. The large joints mostly involved by the disease are the knees. But there are no treatments available that cure the underlying process of osteoarthritis diseases. Physical exercise helps in increasing cartilage nutrition and remodeling, increases the synovial blood flow, decrease swelling, and improves muscle strength. Thus, exercise has been suggested as an important nursing strategy in osteoarthritis. Purpose: The purpose of this study were to compare muscle strength between Tai-Chi exercise and aquatic exercise for women with knee osteoarthritis. Methods: A quasi-experimental study with pretest and posttest measures was used. The study subjects were those who had been enrolled in a community health center, and agreed to participate in the study for eight weeks, signed the consent form, and obtained the physicians approval. The study dropout rates were 13.2% with the final study subjects of 17 on Tai-Chi exercise, 16 on aquatic exercise program. The collected data were analyzed using SPSS for Window (version 12.0). Independent sample t-test and paired t-test was performed to compare of muscle strength for women with osteoarthritis after 8-week Tai-Chi exercise and aquatic Exercise. Results: The homogeneity tests of demographic characteristics and study variables at the pretest data revealed no significant differences between two groups. After 8-week Tai-Chi and aquatic exercise, there was significant result in pre-post test comparison on muscle strength on Tai-Chi group, but no significant in aquatic group. There were no significant differences of knee extensor (p=.078), and hand grip(p=.118) in group comparisons on muscle strengths. But there were significant differences of knee flexor(p=.024). Conclusion: Tai-chi exercise was effective in improving knee flexor. So, it seems that Tai-chi exercise may be more suitable for aquatic exercise in osteoarthritis exercise programs. Further studies with other comparisons in physical and psycho social outcomes are necessary to confirm the more effects of exercise.

The effects of Cox distraction manipulation on functional assessment measures and disc herniation index in patients with L4-5 herniated disc (칵스 신연교정이 L4-5 추간판 탈출증 환자의 기능적 평가측정과 추간판탈출지수에 미치는 효과)

  • The purpose of the present study was to determine the effect of a 4 week course of Cox distraction manipulation (CDM) combined with therapeutic modalities on the treatment of patients with L4-5 herniated nucleus pulposus (HNP). A total of 15 patients with L4-5 HNP (mean age, 37.76 years; age range 20-50years) participated in the study. A 4 week course of CDM combined with therapeutic modalities was delivered to the patients for 6 days per week for the first two weeks, and three times per week for two additional weeks. The entire treatment consisted of 18 visits over 4 week period. Comparisons of changes in the muscle strengthening (MS), straight leg raise (SLR), and oswestry disability index (ODI) at pre-intervention, after two weeks treatment sessions, and at discharge (after 18 treatment sessions) were analyzed. Comparisons of changes in the disc herniation index (DHI) at pre-intervention and at discharge were analyzed using the paired t-test. There were significant improvements in the outcome measures of MS Ibs, SLR test, and ODI score after 2 weeks and 4 weeks sessions of CDM combined with therapeutic modalities as compared with the pre-intervention. However, no significant different pre-test and post-test DHI. CDM combined with therapeutic modalities appears to be a safe and efficacious, noninvasive treatment modality for patients with L4-5 HNP.

Effect of Support Surface and Visual Condition on Static Balance (안정 및 불안정 지지면에서 시각 조건이 정적 균형에 미치는 영향)

  • The purpose of this study was to investigate the effect of visual conditions on the static balance of stable and unstable surfaces. Thirty healthy adults participated in this study (13 men, 17 women). Visual conditions were set as vertical, horizontal, and diagonal. It was performed on the pad to measure the balance on the unstable support surface. Using the balance measuring instrument, the General Stability Index, Weight Distribution Index, and Weight Distribution was measured. Paired t-tests were used for ground-based comparisons and one-way ANOVAs were used for comparisons within the same ground. The general stability index showed a significant difference according to the horizontal and vertical line directions on the stable support surface(p<.05). According to the comparison between the grounds, the weight distribution index on the unstable support surface showed a significant difference(p<.05). Weight distribution showed significant differences between the left and right eyes on the support surface with the stable and the unstable horizontal visual condition(p<.05). In static balance training, vertical and diagonal visual direction conditions are predicted to be helpful for training.

Effect of Direction of Gliding in Tibiofibular Joint on Angle of Active Ankle Dorsiflexion (정강종아리 관절의 활주 방향이 발목관절의 능동적 발등굽힘 각에 미치는 영향)

  • PURPOSE: This study was to investigate the effect of direction of gliding in the tibiofibular (TF) joint on angle of active ankle dorsiflexion (AADF). METHODS: Fifteen subjects with no medical history of lower extremities were recruited in this study. The digital dynamometer was used to maintain the same gliding force in each condition. The angle of AADF was measured by using the electronic goniometer. Subjects were instructed to perform the AADF in three trials during the each gliding in the TF joint by the examiner. The conditions were no gliding and four directions of gliding in the proximal (anterior-superior: A-S vs posterior-inferior: P-I) and distal (posterior-superior: P-S vs. anterior-inferior: A-I) TF joint. A repeated measured ANOVA was used to compare angle of the AADF in each TF joint. The paired-sample t tests with Bonferroni correction were used in order to Post hoc pair-wise comparisons. The significant level was set at 0.016 (0.05/3). RESULTS: In distal TF joint, the angle of AADF in the A-I direction of gliding was significantly lower than those in no gliding and P-S direction of gliding (p<0.01). In proximal TF joint, the angle of AADF was significantly lower than those in no gliding and A-S direction of gliding (p<0.01). Although there was no significant differences, angle of AADF were largest in the P-S direction of gliding among four conditions. CONCLUSION: These findings suggest that gliding technique of TF joint would be required to improve the angle of AADF in subjects with limitation of ankle dorsiflexion.

The Effect of Elastic Taping on the Quadriceps Femoris to the Gait Cycle (대퇴사두근에 대한 탄력테이핑 적용이 보행주기에 미치는 효과)

  • Background: The purpose of present study was to investigate the effect of quadriceps femoris taping in normal gait using 3D motion capture technique. Method: Twenty healthy volunteers, have no musculoskeletal problems, were recruited as subjects for this study. In experimental group, 10 males (mean age : 23.6${\pm}$.40) and 10 females (mean age : 21.3${\pm}$.36) were included. Methods of therapeutic taping that taping of patellar inferior and medial gliding and quadriceps. To obtain the dynamic data, we captured the motion of subject attached markers without taping during repeated gaits five times or more in 7 m Capture volume of gait analysis center. The result was obtained as a mean value in three times. After taping on quadriceps femoris, the same procedure was carried out. Statistical analyses were performed using statistical software packagess SPSS WIN 12.0(SPSS, Chicago, IL, USA). Differences were tested for statistical significance using paired t-test, independent t-test, chi-squared test for comparisons between the male and female. Results: The date of 20 subjects who carried out the whole experimental course were statistically analyzed. 1. gait velocity was showed that female group had more significantly increased than male group(p<.05). 2. step length was showed that female group had more significantly increased than male group(p<.05). 3. step width was showed that female group had more significantly increased than male group(p<.05). Conclusion: Elastic taping on quadriceps femoris promoted cadence, gait velocity, step length and step width in normal subject group.

The Effect of Treadmill and Body Weight Support Treadmill Training on Balance and Gait Ability in Hemiplegia Patients (편마비 환자의 트레드밀과 체중지지의 트레드밀 훈련이 균형능력 및 보행능력에 미치는 영향)

  • Purpose: The purpose of this study is to investigate and to verification of changes that the effect of treadmill and body weight support treadmill training on balance and gait ability for sub-acute stroke patients during 4 week. Method: 16 subjects who was diagnosed stroke were divided into 2 groups(8-treadmill training group, 8-body weight support treadmill group) by randomized control trial. Both training programs were consisted with 40 minuted, 5 times a week for 4 weeks and after training programs. Analysis: We analysed effects and changes on balance and gait ability. Analyses were performed using PASW ver. 18.0 and results were reported as mean ${\pm}$ standard deviation (S.D.). To investigate within group comparisons and to verification on effects of exercise, we did paired t test and repeated measured ANOVA test. Significance was set at p<.05. Result: Both training programs showed positive changes in Limit of Stability but significant results which is area of hemiside, area of intact side, area of posterior, total area were reported in body weight support treadmill training group. Changes of Berg Balance Scale was significantly increase and it had significant correlation between groups. Changes of 6 Minutes Walking Test was significant increase in both groups but there was no significant changes on Foot print and Romberg test. Conclusion: After considering all factors, both training programs showed effect on improvement of balance and gait ability in sub-acute stroke patients, but body weight support treadmill training group had better improvement in dynamic balance than treadmill training group. For the increase of balance and gait ability in sub-acute stroke patients, we need to continues study on difference of treadmill and body weight support treadmill and then we will give stroke patients a better satisfaction if we develop and provide a rehabilitation program for improvement of balance and gait ability.

Comparisons of Diaphragm Movement and Pulmonary Function Between Normal Children and Children With Cerebral Palsy (정상 아동과 뇌성마비 아동의 감소된 횡격막 움직임 및 호흡 기능의 비교)

  • Background: Research efforts to improve the pulmonary function of children with cerebral palsy (CP) need to focus on their decreased diaphragmatic ability compared to normal children. Real-time ultrasonography is appropriate for demonstrating diaphragmatic mechanisms. Objects: This study aimed to compare diaphragm movement, pulmonary function, and pulmonary strength between normal children and children with CP by using ultrasonography M-mode. The correlation between general characteristics, diaphragm movement, pulmonary function, and pulmonary strength was also studied. Methods: The subjects of this study were 25 normal and 25 CP children between five and 14 years of age. Diaphragm movement was measured using real-time ultrasonography during quiet and deep breathing. Pulmonary function (such as forced expiratory volume in one second; FEV1 and peak expiratory flow; PEF) and pulmonary strength (such as maximum inspiratory pressure; MIP and maximum expiratory pressure; MEP) were measured. A paired t-test and Spearman's Rho test, with a significance level of .05, were used for statistical analysis. Results: The between-group comparison revealed that normal children had significantly greater diaphragm movement, FEV1, PEF, MIP, and MEP (p<.05) than CP children. The results showed that general characteristics were significantly related to FEV1, PEF, MIP, and MEP (p<.05). Conclusion: In clinical settings, clinicians need to concern decreased diaphragm movement, pulmonary function, and pulmonary strength in CP group compared to normal children.

Implementation of Health Behavior Education Concerning Liver Flukes among Village Health Volunteers in an Epidemic Area of Thailand

  • Background: Liver fluke infection is associated with cholangiocarcinoma; the bile duct cancer found frequently in the northeast and north of Thailand. Prevention and control particularly requires health education and behavior change. Objective: This study aimed to improve health behavior among village health volunteers (VHV) regarding liver fluke exposure in an epidemic area. Materials and Methods: A quasi-experimental study was performed during July 2015 to January 2016 in Sang Kha district of Surin province, Thailand. A total of 67 VHVs underwent a health education program (HEP) and data were collected on knowledge, attitude, and practice (KAP) before and after participation for HEP 3 months with a pre-designed questionnaire. The Students paired T-test was used for comparisons of mean KAP levels before/after the intervention. Results: The results revealed that knowledge (P-value=0.004), attitude (P-value=0.004), and practice level (P-value=0.000) were significantly improved after participation in the HBP. Attitude was significantly associated with knowledge (r=0.266, p<0.05), and practice (r=0.348, p<0.01). Conclusions: The implementation of health education among VHVs is feasible and increases their KAP. This improvement should have potential in liver fluke prevention and control in local communities in rural Thailand.

Effect of Aromatherapy on Skin Xerosis and Pruritus in Patients Undergoing Maintenance Hemodialysis (향 요법이 혈액투석 환자의 피부건조와 소양증에 미치는 효과)

  • This study was designed to investigate the effect of aromatherapy on skin xerosis and pruritus in patients undergoing maintenance hemodialysis. Twenty one subjects of this study were selected from St. Paul's Hosipital in Seoul. All the subjects were received the mineral oil massage at the arm without fistular three timesper week for 4 weeks. After 2 weeks period of wash-out, the subjects were received the aromatherapy of lavender and tea tree essence oil in the same way. This study was carried out from March 20 to June 13, 1998. Pruritus score, skin pH and stratum corneum hydration were measured before and after each treatment. But, biochemical parameters were measured before the treatment of the mineral oil massage, after the treatment of the mineral oil massage and the aromatherapy. Data of this study were analyzed by paired t-test, repeated measures ANOVA, Bonferroni multiple comparisons and Pearson correlation coefficient. The results were as follows ; 1. Pruritus score was significantly decreased after the aromatherapy, while no significant change after the treatment of the mineral oil massage. Therefore, there was a significant difference in the priritus score between the two treatments. 2. Stratum corneum hydration was significantly increased after the aromatherapy, while no significant change after the treatment of the mineral oil massage. Therefore, there was a significant difference in the stratum corneum hydration between the two treatments. 3. Skin pH was significantly increased after the treatment of the mineral oil massage, while no significant change after the aromatherapy. Therefore, there was no significant difference in the skin pH between the two treatments.4. After the aromatherapy, the serum calcium was significantly increased. Whereas the serum parathyroid hormone intact was significantly decreased compared with the treatment of the mineral oil massage. But the level of the serum Ca and PTH-intact were within the normal range. 5. Stratum corneum hydration was decreased corresponding to the duration of hemodialysis, while pruritus score and skin pH showed no change corresponding to the duration of hemodialysis and the age of the subjects. The correlation of pruritus score on skin pH, stratum corneum hydration and biochemical parameters was not significant. In conclusion, this findings indicate that aromatherapy may be effective in decreasing skin xerosis and pruritus score in uremic pruritus patients undergoing maintenance hemodialysis.
