The Effects of Complex Exercise Program on Postural Change, Gait and Balance Ability in Elementary School Students with Forward Head Posture - Case Study (전방머리자세를 가진 초등학생에게 복합운동프로그램이 자세변화와 보행, 균형능력에 미치는 영향: 사례연구)

  • Background: The purpose of this case study was to investigate into the effect of complex exercise program on the postural change, gait and balance ability in elementary school students with forward head posture. Methods: Four patients with forward head posture were recruited. They were evaluated pre-treatment, and after 6weeks, using neck disability index (NDI), numeric pain rating scale (NPRS), balance ability, foot pressure (fore foot/rear foot peak pressure ratio, F/R ratio), gait ability (cadence, toe out angle, stance phase). Results: First, the angle of forward head posture (craniovertebral angle; CVA and cranialrotation angle; CRA) was decreased in all subjects. The NPRS and NDI were decreased in all subjects. Also, The cadence, toe out angle and F/R ratio were increased in all subjects. The stance phase of gait cycle was positively change in all subjects. Lastly, the static balance ability improved in all subjects. Conclusion: According to the results above, the complex exercise program for students with forward head posture can help improve the postural change, gait and balance ability. Also, the complex exercise program was able to select interventions depending on the patient's condition and the desired goal.

The Effectiveness of Korean Medical Rehabilitation Treatment after Musculoskeletal Surgery: A Retrospective Observational Study (근골격계 수술 후 한방재활치료의 효과: 후향적 관찰연구)

  • Objectives This study aimed to evaluate the effectiveness of postoperative Korean rehabilitation treatment for patients with musculoskeletal disorders. Methods The medical records of patients undergoing Korean medical rehabilitation after a musculoskeletal surgery, from January 1, 2016, to December 31, 2019, were analyzed retrospectively. The effect of treatment was evaluated by using a numeric rating scale (NRS) and questionnaire for each surgical site. When the same patient was re-admitted after discharge, it was treated as the same case if it was within 7 days from the discharge date; otherwise, it was treated separately. If the period of hospitalization was within 7 days or the chief complaint was not related to musculoskeletal surgeries, the case was excluded. Results In total, 480 cases were included in this study. Acupuncture (99.8%), moxibustion (95.6%), cupping (97.5%), Chuna manual therapy (81.5%), Korean medical physiotherapy (61.0%), Daoyin exercise therapy (26.3%), and herbal medicine (87.7%) were used as the Korean medical rehabilitation treatments. A significant improvement was noted when comparing the NRS and questionnaire results for each surgical site at the time of admission and discharge. Conclusions Korean medical rehabilitation can be effectively used for patients who have undergone musculoskeletal surgeries, to relieve pain and enable returning to daily activities. However, further research with a high level of evidence is necessary to support this finding.

  • The purpose of this study was to survey a perception of frequency & importance level of job performance, from 249 paramedics who were working at fire station. The job related activities of 4 duties, 18 tasks and 145 task elements were checked by 4 rating scale. The data were analyzed using a SPSS program for descriptive statistics. The results of the study were as follows ; 1. Some of the most frequent tasks were medical tx, managing people & organization, trauma care, pt assessment, general coping skill 2. Some of the most frequent task elements were preparation of written reports, operation of pt lifting supplies, transportation of patients on stretchers, administration of cervical collar and utilization of scoop stretcher. 3. Some of the highest level of importance in task were resuscitation of circulation, surgical tx, safety & infection control, environmental emergency care, trauma care. 4. Some of the highest level of importance in task elements were administration of cervical collar, adult CPR, infant CPR, child CPR, and AED. 5. The highest level of task elements in perception of frequency & importance were administration of cervical collar, infection control after pt care, utilization of long back board, disinfection of ambulance after ride a long, care of chest pain pt, care of unconscious pt, tx of asthma. 6. A difference between frequent & importance score were due to lack of supplies(41%), structural problems(30%) and medical control system(16%), lack of skills(10%), Suggestion; 1. This paper would be more reliable and confirm through wilder range of survey. 2. It would be necessary of more depth survey through dacom study from paramedic. 3. Development of field oriented protocol & curriculum that based on task elements which have high score of both frequency & importance level is required.

The Use of Hook Plate on the Management of Unstable Neer II Lateral End Fracture of The Clavicle (불안정한 Neer II형 원위쇄골 골절의 치료에서 Hook 금속판의 이용)

  • Purpose: Unstable Neer type II lateral end fracture of clavicle may be required operation. The purpose of this study is the effectiveness of the use of Hook plate in the management of unstable Neer type II fractures. We preliminary reported the results of Unstable Neer type II lateral end fracture of clavicle using by Hook plate. Material and Methods: From May 1998 to May 2002, we operated 6 cases unstable Neer type II lateral end fracture of clavicle with Hook plate by one surgeon. We checked radiologic evaluation and disappearance of pain per 1 week, and evaluated improvement of range of motion per 2 week. And we followed up at each interval 3 months,6 months, 12 months and after that per 6months interval, at that each time we checked stress radiogram of shoulder and functional evaluation. The shoulder function was evaluated using Modified Shoulder Rating Scale (MSRC) for Distal Clavicle Fracture and UCLA score. Average follow up was on 37.2 (12∼57) months. Results: All 6 patients were regained satisfactory function. Average MSRC for distal clavicle fracture was 17.3 (15∼20) and average UCLA score was 33.2 (31∼35) at last follow up. Conclusion: There are many advantage of the use of Hook plate on management of unstable Neer II distal clavicle fracture in spite of several disadvantage. There have not yet been reported in our country. So we obtained good to excellent clinical result in surgical treatment of 6 cases on type II displaced lateral end fracture of the clavicle. But we think that more cases will be review and longer follow up will be needed in the future.

Clinical Effect Of Low Level Laser Therapy In The Treatment Of Dentin Hypersensitivity Following Periodontal Surgery (치주수술 후 상아질 지각과민증에 대한 저수준 레이저 처치의 임상적 효과)

  • Root surface exposure due to gingival recession after periodontal surgery, dentin exposure after root planing elicit pain response when exposed to mechanical, heat, chemical or osmotic stimulation. Especially, patients treated with periodontal surgery, show high frequency and there have been reports showing the 1 out of 7 patients have dentin hypersensitivity. There have been many studies on the clinical effects of various materials on the treatment of dentin hypersensitivity. but, none could provide absolute clinical efficacy. In this study, 45 teeth from 30 patients, who had had periodontal surgery and showed dentin hypersensitivity after surgery were chosen for the experimental group and they were illuminated with laser, 15teeth were chosen for the control group and they were not exposed to laser. After this dentin hypersensitivity was elicited by tactile, compressed air, cold water and then, the degree was evaluated using NRS(Numerical Rating Scale). And during LLLT(Low Level Laser Therapy) semiconductor laser using Gallium - Arsenide as a diode was illuminated for 180 seconds at a frequency of 7(500Hz). This therapy was done 10 times, and each time the changes in dentin hypersensitivity was evaluated using NRS. The results were as follows : 1. After treat with LLLT on dentin hypersensitivity due to periodontal surgery, 22.2% showed total loss of dentin hypersensitivity, 60.0% showed loss of tactile dentin hypersensitivity, 48.8% showed loss of compressed air dentin hypersensitivity, 22.2% showed loss of cold water dentin hypersensitivity. 2. As a result of clinical evaluation of dentin hypersensitivity using NRS, there was significant increase in improvement of dentin hypersensitivity in the experimental group compare to the control group(P<0.05). And there was almost no natural loss of dentin hypersensitivity in the control group. 3. In comparison of the stages of evaluation, there was significant difference in between experimental and control group. after the second visit(P<0.05), and the difference increased with each visit.

Effects of Korean Medicine Treatment on Medial Collateral Ligament Tear with Meniscal Tears : Report of 3 Cases (반월상 연골판 손상을 동반한 내측 측부인대 부분파열 환자의 한방 치료 효과: 증례보고)

  • Objectives : The purpose of this report was to introduce 3 cases of patients diagnosed with both medial collateral ligament tear and meniscal tear that showed clinical improvement after Korean medicine treatment. Methods : These patients received Korean herbal medicine, pharmacopuncture treatment, and acupuncture treatment. We assessed these cases by using the Numeric Rating Scale (NRS), Western Ontario and McMaster Universities'Arthritis Index (WOMAC), and the European Quality of Life-5 Dimension Index (EQ-5D). Results : After Korean medicine treatment, most of the NRS, WOMAC, and EQ-5D scores showed improvement on every discharge date of these 3 cases. The NRS and WOMAC scores of all cases showed a decrease. The EQ-5D of all cases showed an increase. Conclusions : Through this research, we concluded that Korean medicine treatment can be helpful in relieving pain and improving the quality of life and function of knee joint movement in patients with medial collateral ligament tear and meniscal tears.

Tumor-reducing effect of SB injection in a non-small cell lung cancer patient: A case report (비소세포폐암 환자의 종양 축소에 대한 SB 주사 효과 증례)

  • Objectives : To study the effect of SB injection on tumor size in an advanced non-small cell lung cancer patient. Methods : A patient was clinically diagnosed as advanced non-small cell lung cancer (Stage IIIa). Four cycles of intravenous SB injection were conducted. Each cycle lasted 4 days. The content of 7vials SB was injected every day. To compare the tumor size before treatment and after four cycles of SB injection, chest computed tomography (CT) was performed. Results : Follow-up CT images showed that the tumor size was reduced. In admission, size of the tumor $6.7{\times}8.5{\times}9.5cm$ on the left lower lobe of lung. After SB injection, size of the tumor $5.6{\times}6.8{\times}8.4cm$ by Chest CT. The patient's symptoms such as cough, sputum were improving until four cycles of SB injection. Numerical rating scale (NRS) showed improvement of Chest pain from point 3 to point 0. Conclusions : This case study suggests that intravenous SB injection may have significant effects of anti-tumor for non-small cell lung cancer.

Effect on One Time Application of Functional Cerebrospinal Therapy (FCST): A Retrospective Clinical Survey (기능적뇌척주요법의 단회 치료 효과에 대한 후향적 관찰 연구)

  • Objectives: The aim of this study was to review main symptoms of outpatient in Lee young Jun clinics who developed the functional cerebrospinal therapy and to investigate the therapeutic effects on one time application of functional cerebrospinal therapy (FCST). Methods: Patients who visited at least twice at an oriental medical clinic and treated with FCST were included. They were asked to write 216 numeric rating scale (NRS)-based questionnaire about degree of various symptoms at each visit. Prevalence and degree decrease of all reported symptoms were analyzed. Results: Total of 744 outpatients were included. There were fourteen symptoms of which more than half of all patients commonly complained. All symptoms were significantly reduced after one administration of FCST. About four to seven percent of patients reported at least five-point reduction on NRS in each symptom after FCST. Conclusions: FCST might have potential immediate effect on not only TMJ related symptoms, but also diverse kinds of pain, psychological symptoms, and fatigue-related symptoms.

  • Objectives : The purpose of this study was to evaluate the effect of Oriental medical treatment in patients with acute lumbar Herniated Intervertebral Disc(HIVD). Methods : This clinical study was carried out on 44 patients with acute lumbar HIVD, who had been admitted from Oct. 2011 to Nov. 2011. All of 44 patients were treated with acupuncture, Chuna treatment and herbal medicine during the whole admission period. Verbal numerical rating scale(VNRS) and Oswestry disability index(ODI) were used to evaluated the effectiveness of the Oriental medical treatment. Result : Siginificant improvement of the symptoms was seen when evaluated with VNRS and ODI. Conclusions : These results suggest that in the case of low back pain and lower limb numbness caused by acute HIVD, conservative treatments can be considered as one of the options of treating the symptoms beside surgical way.

The Results of Rotator Cuff Disease Treated by Arthroscopic Subacromial Decompression (회전근개 질환에서 시행한 관절경적 견봉하 감압술의 결과)

  • We reviewed the results of arthroscopic surgery in patients with rotator cuff disease. Arthroscopic subacromial decompression(ASD) was performed on 22 patients with rotator cuff disease who had not responded to nonoperative measures. In the patients who had a complete tear of the rotator cuff(four of ten Neer's stage III patients), mini-open repair also was performed. Results were determined by questioning patients about their satisfaction with the outcome of surgery and by functional assessment of the shoulder with the parameters of pain, function, and range of motion according to the UCLA shoulder rating scale. The average follow-up was 21 months. The results were sixteen(72%) excellent, three(14%) good, two(9%) fair, and one(5%) poor. The following variables were analyzed to assess their influence on final outcome; duration of preoperative symptoms, Neer's stage(extent of damage to the cuff), type of acromion. Satisfactory results were achieved in thirteen of fourteen patients(93%) who had duration of preoperative symptoms below one year, and in four of six(67%) above 2 years. And satisfactory results were achieved in eleven of twelve patients(92%) who had Neer's stage II and in eight of ten(80%) stage III and achieved in six of seven patients(86%) who had Bigliani's acromion type I, in nine of eleven(82%) type II, and in four of four(100%) type III. There are multiple factors that may influence the recovery after ASD or the eventual outcome. However, although there is still a controversy about the pathogenesis of rotator cuff disease, We found that arthroscopic subacromial decompression and mini-open repair in patients with rotator cuff disease were well enough documented to be considered a standard treatment.
